1.Rapid determination of four herbicides in urine with ultra-high performance liquid chromatography-triple quadrupole tandem mass spectrometry

Objective:
To establish a ultra-high performance liquid chromatography-tandem triple quadrupole mass spectrometry method for rapid simultaneous determination of quinclorac, acetochlor, butachlor and metolachlor in urine.
Methods:
Urine samples were diluted 10 times, prepared into the mixed standard solution, and subjected to gradient elution on the ACQUITY UPLC BEH C18 column (100 mm×2.1 mm, 1.7 μm) with 0.1% formic acid and acetonitrile as the mobile phase. The quinclorac, acetochlor, metolachlor and butachlor levels were determined using electrospray ionization-positive ultra-high performance liquid chromatography-tandem triple quadrupole mass spectrometry with the multiple reaction monitoring mode.
Results:
Four herbicides were effectively separated on the ACQUITY UPLC BEH C18 column (100 mm× 2.1 mm, 1.7 μm), and good linear relationships were observed for quinclorac, acetochlor and butachlor at 1 to 25 μg/L and for metolachlor at 0.2 to 25 μg/L, with all linear correlation coefficients of >0.999. The detection limts of quinclorac, acetochlor, butachlor and metolachlor were 0.10, 0.10, 0.20 and 0.01 μg/L, respectively. The recovery rates of quinclorac, acetochlor and butachlor were 107.42%, 93.94% and 90.27% from urine samples at a spiked level of 5 µg/L, with relative standard deviations of 4.82%, 3.84% and 6.76%, and the recovery rate of metolachlor was 89.51% at a spiked level of 0.5 µg/L, with a relative standard deviation of 8.98%.
Conclusion
The chromatography and mass spectrometry conditions are optimized in this ultra-high performance liquid chromatography-tandem mass spectrometry, which is effective for rapid simultaneous determination of quinclorac, acetochlor, metolachlor and butachlor in urine samples.
2.First aid protocols for severe pelvic fractures

Objective To discuss diagnostic classification and emergency treatment protocols for complex pelvic fractures. Methods The first aid management of 23 cases of major pelvic fractures were analyzed retrospectively. Results 22 of the patients survived, but one died of hemorrhagic shock.Conclusions Correct classification of severe pelvic fractures is of guidance value to the first aid management. Cooperation among multiple departments, comprehensive application of various surgical methods, early stabilization of hemodynamics, rapid repair of injured organs, stabilization of pelvic ring with external fixator are effective techniques to enhance the survival rate of patients with severe pelvic fractures.

There is a unclear understanding whether occupatioal therapy is needed by patients whohave injuries or diseases.113 cases of patients with different diagnoses were interviewed in this study. The measurement tool used was Canadian Occupational Performance Measure (COPM).Statistical anal-ysis showed that all of patients had identified and prioritized problems with occupational performancesecondary to injuries or diseases. Occupational therapy intervention was required to solve the problemsin different occupational performance areas. COPM is an useful measurment tool in the model of patient-centered occupational therapy and is well worth recommending in clinical use.
5.Marriage intention among HIV-infected men who have sex with men

Objective:
To investigate the marriage intention and its influencing factors among HIV-infected men who have sex with men (MSM), so as to provide the reference for formulating high-risk behavior intervention strategies and reducing the risk of HIV transmission among this population.
Methods:
Unmarried HIV-infected MSM were sampled from newly reported HIV/AIDS patients in Gongshu District of Hangzhou City from 2020 to 2022. Participants' sociodemographic information, family status and marriage intention were collected using questionnaire surveys. Factors affecting marriage intention were identified using a multivariable logistic regression model.
Results:
A total of 430 questionnaires were allocated and 400 valid questionnaires were recovered, with an effective recovery rate of 93.02%. The respondents had a mean age of (28.90±4.67) years, and included 271 cases with registered residence outside Hangzhou City (67.75%). There were 82 cases indicating that they would probably or definitely get married, accounting for 20.50%, including 46 cases (56.10%) aged 30 years and older, 67 cases (81.71%) engaged in commercial services/freelance work/worker and 53 cases (64.63%) being the only children. Multivariable logistic regression analysis showed that unmarried HIV-infected MSM who were aged 30 years and older (OR=2.751, 95%CI: 1.230-6.154), had a monthly income of <10 000 Yuan (5 000 to <10 000 Yuan, OR=7.010, 95%CI: 2.049-23.981; <5 000 Yuan, OR=5.229, 95%CI: 1.803-15.165), had a high school education and below (OR=5.935, 95%CI: 1.775-19.844), were the only children (OR=3.407, 95%CI: 1.468-7.910), came from families with economic conditions above the average level (OR=8.906, 95%CI: 2.070-38.308), did not disclose their sexual orientation (OR=3.298, 95%CI: 1.118-9.727), held a positive attitude towards government policies (OR=70.544, 95%CI: 14.583-341.237), had no homosexual intercourse in the past six months (OR=3.583, 95%CI: 1.278-10.044), and had not informed their sexual partners of their HIV status (OR=2.910, 95%CI: 1.228-6.897) were more willing to get married.
Conclusion
A certain proportion of unmarried HIV-infected MSM in Gongshu District are willing to get married, and their marriage intention is affected by age, educational level, family economic conditions, the only children or not, disclosing sexual orientation or not, having homosexual intercourse in the past six months or not, informing sexual partners of HIV status or not, and the attitude towards government policies.
6.Metabolic syndrome among employees in petrochemical enterprises

Objective:
To understand the prevalence and its influencing factors of metabolic syndrome (MS) among employees in petrochemical enterprises, so as to provide insights into the prevention and control of MS among employees in petrochemical enterprises.
Methods:
The employees in petrochemical enterprises who underwent health examinations at the Fourth Affiliated Hospital of Gansu University of Traditional Chinese Medicine from May 2021 to December 2022 were selected as the survey subjects. Demographic information, lifestyle behaviors and occupational exposure were collected using questionnaires, and the blood biochemical indicators were measured through laboratory testing. Factors affecting MS were identified using a multivariable logistic regression model.
Results:
A total of 2 479 individuals were included, with a mean age of (44.84±7.87) years. There were 1 684 males (67.93%) and 795 females (32.07%). There were 905 cases of MS, with a detection rate of 36.51%. Multivariable logistic regression analysis showed that gender (male, OR=2.246, 95%CI: 1.353-3.728), age (≥40 years, OR=3.523, 95%CI: 2.003-6.194), noise exposure (OR=1.894, 95%CI: 1.272-2.821), smoking index (>0~200 cigarette-years, OR=1.907, 95%CI: 1.155-3.149; >200 cigarette-years, OR=2.257, 95%CI: 1.320-3.859), hyperuricemia (OR=3.013, 95%CI: 1.852-4.900) and γ-glutamyltransferase (abnormal, OR=2.691, 95%CI: 1.589-4.559) were the influencing factors of MS among employees in petrochemical enterprises.
Conclusion
The risk of MS occurrence among employees in petrochemical enterprises is related to gender, age, noise exposure, smoking index, hyperuricemia and γ-glutamyltransferase level.
7.Correlation of serum interleukin-6 and interleukin-18 levels to carotid artery intimal-medial thickness in patients with acute cerebral infarction

Objective To investigate the correlation of serum interleukin-6 (IL-6) and IL-18levels to carotid artery intimal-medial thickness (IMT) in patients with acute cerebral infarction (ACI).Methods In 64 patients with ACI and 30 healthy volunteers, serum IL-6 and IL-18 levels were detectedusing radioimmunoassay and enzyme-linked immunosorbent assay (ELISA), respectively. Carotid arteryIMT were measured in the two groups using color Doppler ultrasound, and the ACI patients were dividedinto two groups with carotid IMT greater or lower than 1.0 mm. Results The ACI patients and thehealthy volunteers showed significant differences in serum IL-6 (P<0.05) and IL-18 levels (P<0.05),which were also significantly different between ACI patients with carotid IMT ≥ 1.0 mm and those withIMT<1.0 mm (P<0.05). The ACI patients with IMT<1.0 mm had comparable serum IL-6 and IL-18 levelsto those in the healthy volunteers. In patients with carotid IMT ≥ 1.0 mm, the serum IL-18 level waspositively correlated to IMT (r=0.549, P=0.001), but the IL-6 level was not related to IMT (P>0.05).Conclusion The serum levels of IL-6 and IL-18 are elevated in ACI patients with increased carotid IMT.
8.Sphincter of Oddi Manometry: Reproducibility of Measurements and Effect of Sphincterotomy in the EPISOD Study.

BACKGROUND/AIMS: The reproducibility of sphincter of Oddi manometry (SOM) measurements and results of SOM after sphincterotomy has not been studied sufficiently. The aim of our study is to evaluate the reproducibility of SOM and completeness of sphincter ablation. METHODS: The recently published Evaluating Predictors and Interventions in sphincter of Oddi dysfunction (EPISOD) study included 214 subjects with post-cholecystectomy pain, and fit the criteria of sphincter of Oddi dysfunction type III. They were randomized into 3 arms, irrespective of manometric findings: sham (no sphincterotomy), biliary sphincterotomy, and dual (biliary and pancreatic). Thirty-eight subjects had both biliary and pancreatic manometries performed twice, at baseline and at repeat endoscopic retrograde cholangiopancreatography after 1-11 months. Sham arm was examined to assess the reproducibility of manometry, and the treatment arms to assess whether the sphincterotomies were complete (elevated pressures were normalized). RESULTS: Biliary and pancreatic measurements were reproduced in 7/14 (50%) untreated subjects. All 12 patients with initially elevated biliary pressures in biliary and dual sphincterotomy groups normalized after biliary sphincterotomy. However, 2 of 8 subjects with elevated pancreatic pressures in the dual sphincterotomy group remained abnormal after pancreatic sphincterotomy. Paradoxically, normal biliary pressures became abnormal in 1 of 15 subjects after biliary sphincterotomy, and normal pancreatic pressures became abnormal in 5 of 15 patients after biliary sphincterotomy, and in 1 of 9 after pancreatic sphincterotomy. CONCLUSIONS: Our data suggest that SOM measurements are poorly reproducible, and question whether we could adequately perform pancreatic sphincterotomy.

9.Recombinant expression of mutant human annexin V.

OBJECTIVETo mutate human annexin V gene and transform it to Pichia Pastoris for mutant human annexin V expression, so as to be purified as active annexin V with endogenous metal chelating site.
METHODSThe 5' and 3' end of native annexin V gene were mutated by specific primers. The mutant annexin V gene was inserted into pPIC9K and sequenced. The correct plasmid was linearized and transformed into Pichia Pastoris strain GS115 by electroporation. The transformants were selected from MD plates and cultured in BMGY medium and induced with methanol. The culture was centrifuged and the supernatant was analyzed by SDS-PAGE and silver staining. The binding activity of mutant human annexin V from culture supernatant was determined with phosphatidylserine exposed erythrocytes and fluorescein isothiocyanate-annexin V.
RESULTSThe 5' end of native human annexin V gene was fused with GCAGGCGGCTGCGGCCAT coding sequence and 3' end 946-948 site TGT was mutated to AGC. Pichia Pastoris transformants secreted proteins of relative molecular mass 36 000 48 h after methanol induction. The concentration of this protein that inhibited 50% of the binding of fluorescein-annexin V was 4nmol/L.
CONCLUSIONHighly-active recombinant mutant human annexin V with endogenous metal-chelating sites can be expressed in Pichia Pastoris system.

10.Identification of a lens-specific cis-acting element within the basal promoter of the human lens intrinsic membrane protein MP19 gene (LIM2).

Using overlapping and mutant oligonucleotides as probes, gel mobility assays and competition experiments identified a sequence from -47 to -32 bp upstream of the LIM2 CAP site, which a lens protein complex bound with high affinity which appeared to bind only to the "sense" strand of the double-stranded DNA molecule. This sequence consisted of a string of four guanine residues followed by seven other nucleotides (AACCTAA) and followed by another four guanines, i.e. GGGGAACCTAAGGGG, called the Hsu element. Promoter-CAT constructs containing this sequence or mutations of the sequence indicated that the Hsu element is located within the basal promoter, and is essential for expression of the LIM2 gene. The trans factors binding to the Hsu element are present throughout development, and appear to be lens-specific. Since the LIM2 gene promoter does not contain a classic TATA box, the Hsu element may serve as the site for binding the RNA polymerase complex.
