Objective To compare the effects of ouabain and digoxin on both the systolic blood pressure (SBP) and sodium pump α-subunit expression in myocardium of rats. Methods Normal Sprague-Dawley (SD) rats were injected with ouabain, digoxin or normal saline (NS) everyday and indirect SBP was recorded once a week. Six weeks later, all the rats were killed and sodium pump α1-, α2-, and α3-subunit were detected in the left ventricular myocardium with RT-PCR method and immunohistochemical assay at mRNA and protein levels. Results At the end of six weeks SBP of rats infused with ouabain increased significantly (132.6±9.0 vs 115.7±8.2 mm Hg, P<0.01), while no difference of SBP was found between digoxin group and NS group. The effects of ouabain and digoxin on sodium pump α-subunit isoform expression in myocardium were also different: both ouabain and digoxin stimulated expression of α3-isoform whereas α2-isoform unchanged at both mRNA and protein levels. α1-isoform was decreased in ouabain group and α1-isoform unchanged in digoxin group at both mRNA and protein levels. Conclusion It is suggested that both ouabain and digoxin could regulate sodium pump α-subunit isoform expression, which might be related to the physiological roles of endogenous ouabain and might be responsible for the difference between the pharmacological and toxicological effects of ouabain and digoxin, including their effects on blood pressure.

7.The collateral ligaments: normal and pathologic appearances on MRI

Objective To evaluate the MR findings of normal and abnormal collateral ligament. Methods The MR findings of 56 normal and 36 surgically confirmed injured collateral ligaments were retrospectively reviewed and compared. Results The normal tibial and fibular collateral ligaments were hypointense string on both T 1 and T 2 coronal MR images, with an average length of 6.8 cm for tibial collateral ligament and 5.7 cm for fibular collateral ligament. 36 collateral ligaments were injured, including 18 tibial and 12 fibular collateral ligaments. Three cases (6 ligaments) had both tibial and fibularcollateral ligaments injuries. The injury of collateral ligament was classified as: Grade I (18 ligaments), Grade Ⅱ (11 ligaments), and Grade Ⅲ (7 ligaments). The complications of anterior cruciate ligament sprains (13 cases), posterior cruciate ligament sprains (9 cases), meniscal tears (12 cases), bony contusion and fracture (7 cases), and joint effusion (20 cases) were found in 25 patients(78%). Conclusion Coronal MR imaging can clearly demonstrate the structure of the knee collateral ligaments, accurately diagnose and classify the ligament injury, and correctly detect the complications. MRI should be used as a routine examining method for knee trauma.
8.Clinicopathological study on Lewy body disease

Objective To investigate the pathological features of Lewy body disease. Methods An autopsy case of an 80 year old Chinese woman suffering from Lewy body disease was studied The patient had progressive dementia and myotonia for three years and was suspected with Alzheimer dementia clinically Brain tissue was analyzed neuropathologically by using HE staining and specific staining for nervous system, immunohistochemical staining for ubiquitin and by using ? synuclein and electronic microscopy. Results Neuropathological examination revealed the most characteristic features as follows: (1) The number of melanin pigmented neurons were markedly decreased in the substantia nigra and locus ceruleus, and some typical brain stem type Lewy bodies in the residual neurons with moderate gliosis were found (2) There appeared numerous cortical type Lewy bodies in deep layers of the cerebral cortex particularly in the cingulate gyrus, hippocampus, parahippocampal gyrus, transentorhinal contex, amygdala and insula (3) There were selective spongiform changes and many Lewy related neurites in positive ubiquitin immunohistochemical staining on the CA2/3 sectors, parahippocampal gyrus and transentorhinal cortex (4) A strong positive ? synuclein immunoreactivity was found in all Lewy bodies and Lewy related neurites (5) Alzheimer disease related changes such as senile plaques and neurofibrillary tangles were scarcely found only in hippocampus and parahippocampal gyrus. Conclusion Lewy body disease is a new group of degenerative disease of the central nervous system ? synuclein may play an important role in the degeneration of neurons and their neurites may be a major component of Lewy bodies in Lewy body disease It is considered that the present case should be a pure type of diffuse Lewy body disease
9.Detecting overexpression of P-glycoprotein-multidrug resistance gene in epileptic patient's peripheral blood lymphocytes

Objective To explore the relationship between the overexpression of P glycoprotein (P GP) multidrug resistance (MDR) gene in the epileptic patient's peripheral blood lymphocytes and the drug resistance of the intractable epilepsy Methods This is a prospective, observational study. First, the overexpression of P GP MDR in 85 epileptic patients' (M 39, F46,overage age 24 years) peripheral blood lymphocytes were investigated by using immunocytochemistry (ICT) or flow cytometer (FCE). Then, patients were given a single or a combining of two antiepilepstic drugs which they did not received before (including carbamazepine, valproic acid, phenobarbitone,phenytoin), when the single drug was not effective in observing the clinical efficacy Results In FCE study group, results showed that the 11 patients with overexpression of P GP MDR in the epileptic patient's peripheral blood lymphocytes became tolerant to antiepileptic drugs. Eighteen out of 32 patients without overexpression of P GP MDR are effective. In the ICT study group, it is effective that there are only 2 out of the 22 patients with overexpression of P GP MDR. Seven of 12 patients without overexpression of P GP MDR are effective Conclusion It is suggested that the overexpression of MDR in the intractable epileptic patient's peripheral blood lymphocytes might be a significant drug resistance marker. The sensibility of ICT method should be better than the FCE,though the latter may be more accurate.
10.Investigation on the relationship between endoscopic and histologic mucosal changes in chronic gastritis

Objective To study the relationship between endoscopic and histologic mucosal changes in chronic gastritis.Methods All 403 patients with chronic gastritis were examined by endoscopy and two samples of gastric mucosa were examined by the same pathologist.Results (1)Gastroscopic results:there were 380 cases of chronic superficial gastritis(CSG),38 cases of chronic atrophic gastritis(CAG) including 15 cases of coexistence of CSG and CAG.(2)Pathologic results:365 cases of CSG;49 cases of CAG including 11 cases of coexistence of CSG and CAG.Among 403 cases,108 cases with intestinal metaplasia(IM),59 cases with mucosal erosion.39 cases had active chronic gastritis(ACG).(3)For CSG,comparing with pathology,endoscopic results had 95.3% of sensitivity and 91.6% of positive predictive value.In contrast their specific rate and negative predictive value were 15.8% and 26.6%,respectively;for CAG,endoscopy had 96.0% of specific rate and 93.2% of negative predictive value for pathologic diagnosis.Its sensitivity and positive predictive value were 46.2% and 63.2% respectively.(4)Comparing with pathologic results,endoscopic features had less sensitivity(50%),but some had a higher positive predictive value for pathologic CSG,CAG,and IM.(5)CAG was most likely associated with type Ⅱ IM(85.2%).Although most type Ⅰ IM was presented in CSG(81.0%).Conclusion Any single endoscopic finding was nonspecific for pathologic diagnosis;although some charactristical endoscopic findings had a higher positive predictive value. In other words,a single pathologic change may associated with various endoscopic findings, while a single later finding only presented one or two pathologic changes.
