2.Epicardiac fat lump in the left atrioventricular groove: CT and MR findings.

Fat can be prominet around the aortic root and within the right atrioventricular (AV) groove (1). At times, the fat can be globular, suggesting a mass especially on echocardiography. Computed tomography (CT) and masnetic resonance (MR) images are well suited for characterization of fat in and around the heart (1, 2). This report illustrates CT and MR findings of a very unusual case of prominent fat lump in the left AV groove.

3.The effect of azelaic acid and retinoic acid on epidermal melanocytes in UVB-irradiated black mice.

Increased melanin pigmentation following ultraviolet irradiat.ion is due to increasing tyrosinase activity and multiplicatian of functioning melanocytes. After UV-irradiation, the size of melanocytes increases, and melanocyte dendrites elongatc, and branch. In this experiment, we induced the activation of melanocyts in the epidermis of C57BL black mice by ultraviolet-B(UVB) irradiation and observcd ihe effect of azelaic acid and retinoic acid on the UVB activated epidermal melanocytes. Sixty C57BL black mice were irradiated by UVB 100mJ/cm daily for 10 days, and then azeiaic acid and retinoic acid were topically applied daily for 7 weeks. For the estimation of morphologic change of epidermal melanocytes, light microscopic observation with split DOPA stain was performed at the end of the 1st, 3rd, 5th and 7th week of topical application. The results are summerized as follows : 1. The number, size and circumference of DOPA-positive epidermal melanocytes were significantly decreased in 20% azelaic acid applied group and 30% azelaic acid and 0.05% retinoic acid applied group. 2. In 20% azelaic;i.cid and 0.05% retinoic acid applied group, the number, size and circumference of DOPA-positive epidermal melanocytes were nore significantly decreased than in 20% azelaic acid applied group. In summary, the present study suggets that azelaic acid act as a depigmenting agent on epidermal melanocyte; and such depigmenting effect of azelaic acid was increased by addition of retinoic acid.

4.A Case of Apocrine Hidrocystoma Occurring in the Female Genitalia.

5.A Case of Trichilemmoma on the Scalp.

Tricilemmoma is a benign, solitary tumor that may be clinically misdiagnosed as basal cell carcinoma or verruca. It affects frequently the face of elderly rnen. We present herein a case of trichilemrnoma in a 70-year-old Korean wornan who had a slowly growing tumor on the scalp for 50 years. The histopathologic findings revealed lobular acanthosis of glycogeri rich clear cells and palisade of columnar cells resting on prominent basement membrane.

7.Computed Tomography(CT) in Head Trauma Patients with Alert Mental Status : How Important are the Clinical Symptoms.

STUDY OBJECTIVES: To identify the value of symptoms in head trauma patients with alert mental status requiring a head CT scan. METHODS: A retrospective study was performed at an emergency center over 8 months period. Patients(age > or = 7) were enrolled if they presented with a Glasgow Coma Scale score of 15 and underwent head CT after head trauma. The symptoms and signs were recorded in the entry forms prior to head CT The symptoms found in the patients with normal CT were compared with those in abnormal CT showing intracranial injuries. RESULTS: Among the 168 patients, traumatic intracranial abnormality was identified in 34(20.2%) by CT. Skull fracture was idendified in 8(4.8%) in normal CT, 17(10.1%) in abnormal CT group by simple X-ray and CT. In the normal CT group without skull fracture, 65(52.0%) had headache, 61(48.8%) had loss of consciousness( LOC), 43(34.4%) had vomiting, 24(19.2%) had nausea, 7(5.6%) had dizziness. In the abnormal CT group without skull fracture, 14(77.8%) had heacache, 9(50.0%) had LOC, 9(50.0%) had vomiting, 2(11.1%) had nausea, 2(11.1%) had dizziness. The patients with headache were significantly more common in the abnormal CT group. CONCLUSION: A head CT is required in alert head trauma patients with any symptoms and signs of intracranial injuries, especially with headache.

9.Percutaneous biliary drainage

Percutaneous biliary drainage is an effective new nonsurgical method for combined external/internal catheterdecompression of obstructed extrahepatic bile ducts. Molnar and Stockum, and more recently others, have reportedsucdess utilizing this approach to establish biliary drainage in patients with obstructive jaundice. Percutaneousbiliary drainage was performed in 9 patients with obstructive jaundice. Internal drainage was accomplished in 4patients (44.4%), and external drainage in 5 patients (55.6%). Major complications occurred in 2 patients; bileleakage, one; sepsis, one. Minor complications occurred in 7 patients; mild or high fever, 5 cases; transienthemobilia, 2 cases. The catheter management problems encoutered including pain, catheter dislodgement ormigration, lumen occulusion, and external bile leakage.

10.Ectopic paragonimiasis on colon wall and mesocolon of the descending colon

Paragonimiasis is prevalent in Far East and a kind of endemic Korean disases. The primary site of humanparagonimiasis is the lung, but hte ectopic infection of lung fluke has been reported by many authors. Weexperienced one case of abdominal paragonimiasis in a 44 year old male with a complaint of left lowerquadrantpain. Physical examination, barium enema, and other data suggested the possibility of tumor originating from thewall of descending colon. (intramural tumor). Postoperative specimen taken from the solid tumorous lesion showedparasitic granuloma, characteristic of Paragonimus Westermani. Here, we report a case of very peculiar ectopicparagonimaisis involved descending colon wall simulating neoplastic tumor, and also review the literaturesbriefly.
