1.Effects of nitric oxide synthase inhibitors on osteoclast-like cell formation.

Orthodontic tooth movement in response to orthodontic force results from actions of osteoclasts and osteoblasts in the cell level. Convincing evidence has now been provided to support the view that osteoclasts are derived from mononuclear cells that originate in the bone marrow or other hematopoietic organs and they migrate to the bones via vascular routes. Nitric oxide(NO), which accounts for the biological properties of endothelium-derived relaxmg factor(EDRF), is the endogenous stimulator of soluble guanylate cylase. The discovery of the formation of nitric oxide(NO) from L-arginine in mammalian tissues and its biologioal roles has, in the last 7 years, thrown new light onto many areas of research. Data from experiments in vitro showed that N-metyl-L-arginine(L-NMA) and L-nitro-L-arginine(L-NAME) are competitive inhibitors of nitric oxide synthase. This study suggest that the multinucleated cells in our culture have characteristics of osteoclasts and that the potential bone cell activity of nitric oxide in vitro may be mediated in part by stimulation of marrow mononuclear cells to form osteoclast-like cells. Bone marrow cells were obtained from tibia of 19-days old chick embryo. After sacrifice, tibia was quickly dissected and the bone were then split to expose the medullary bone. The cells were attached for 4 hours and the nonadherent cells were collected. Marrow cells were cultured in 96-well plate in medium 199. To examine the number of TRAP-positive multinucleated cells(MNCs), 10(-8) M Vit-D3 and various concentration of L-NMA and L-NAME were added at the beginning of cultures and with each medium change. After 7 days of culture, tartrate-resistant acid phosphatase(TRAP) staining was performed for microscopic evaluation. Cells having more than three nuclei per cell were counted as MNCs. The observed results were as follows; 1. 1,25-dihydroxyvitamine D3 stimulated the osteoclast-like multinucleated cells in cultures of chick embryo bone marrow. 2. Nitric oxide synthase inhibitors(NOSI ; N-NMA, N-NAME) stimulated the osteoclast-like cells in cultures of chick embry bone marrow. 3. 1,25-dihydroxyvltamine D3 and nitric oxide synthase inhibitors did not appear to have additive effect on the generation of TRAP-positive MNCs. These results suggest that nitric oxide synthase inhibitors may stimulate the osteoclast-like multinucleated cell formation and fusion in cultures of chick bone marrow.

2.Biochemical characterization of embryonic chick calvarial cells.

Chicken calvarial bone is known to contain various cell types, but their exact composition is unknown. By characterizing the chicken calvarial bone biochemically, it can be used to study biochemical, histochemical actions of bone cells in general. Calvaria of I8-day-old white leg horn embryo was aseptically dissected and bone cell populations were isolated by sequential enzymatic digestion. Histochemical study for osteoclast-like bone cell population was performed with tartrate resistant acid phosphatase(TRAP) stain and for osteoblast-like bone cell population, alkaline phosphatase(ALP) stain was performed. Biochemical study for osteoblast-like bone cell population was performed using alkaline phosphatasetal.P) assay. Following conclusions were obtained from this study. 1. TRAP positive multi and mononuclear cells were mostly observed in group I and II, indicating that osteoclast-like bone cell population is mostly found in these groups. 2. All the cultured groups showed almost equal ALP activities and were positive for ALP stain, indicating that osteoblast-like bone cell population is evenly dispersed in all culture groups. 3. Experimental group treated with I,25(OH)2D3 showed increase in ALP activity in contrast to the control group, confirming previous studies that I,25(OH)2D3 increases ALP activities in vitro bone cultures. 4. Results from von Kassa's stain indicated that in vitro bone formation had occured after 3 weeks of culture with beta glycero phosphate.

3.A FEM analysis for initial stress on the upper canine by orthodontic force of intrusion arch wire activation.

The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA point.

4.Effects of sodium nitroprusside on the formation and activation of the osteoclast in culture.

Due to the great deal of effort that has gone into the study of osteoclastic differentiation and activation over the last few decades, the mechanisms of these two events have been discovered gradually. Nitric oxide(NO-), which is produced from arginine by a nitric oxide synthase, opened up a new area of biological research. Recently, it has been reported that NO- is produced by osteoblasts stimulated by lipopolysaccharide and several other cytokines. In this study, the effect of sodium nitroprusside(SNP), a donor of nitric oxide(NO-), on osteoclast-like cell formation and on mature osteoclast function was examined. To determine the mechanism of the inhibitory effects of SNP decreased not only the basal 45Ca release but also thee bone resorption induced by PTH and l,25-dihydroxyvitamin D3 (I ,25[OH]2D3). The inhibitory effect of SNP on bone resorption induced by PTH appeared 2 days after treatment, whereas SNP effect on inhibiting bone resorption induced by l,25[OH]2D3 appeared at the third days. When chicken and rat osteoclasts were cultured on dentin slices, treatment of 300micronM SNP resulted in a significant decrease in dentin resorption by osteoclasts in terms of total resorption area and average individual area. We also examined the effect of SNP on formation of osteoclast-like cells that is TRAP-positive multinucleated cells from chicken and rat bone marrow cells in the presence or absence of 10(-8)M l,25[OH]2D3. The addition of 300 micronM SNP inhibited the formation of TRAP-positive multinucleated cells. The present data suggest that SNP, possibly as a NO- donor, inhibits the osteoclastic differentiation and osteoclastic activity.

5.A study on the comparisons between dental calcification and skeletal maturity.

This study was conducted on 342 patients(male 157, female 185)aged 8 to 15 years old, who visited Dankook University Dental Hospital. Pre-orthodontic treatment orthopantomograms were used to assess the dental calcification stages of mandibular 3rd molar, 2nd molar, 2nd premolar and 1st premolar by 8 stages.(by Demirjian) Hand-wrist radiogrms were used to evaluate the skeletal maturity in 11 stages.(by Fishman) Following results were obtained after investigating the correlationship between dental calcification and skeletal maturity. 1. Chronologic age showed high correlation to dental calcification and skeletal maturity. 2. Dental calcification and skeletal maturity showed high correlation and no statistical difference was observed between male and female. 3. SMI stages 1 to 4 showed high statiscal significance to mandibular 2nd molar, 2nd premolar and 1st premolar. SMI stages 5 to 8 showed high stastical significance to mandibular 2nd molar, 2nd premolar. SMI stages 9 to 11 showed high statistical significance to mandibular 3rd molar.

6.A study on the characteristics of craniofacial skeleton in anterior open-bite.

This study was performed to verify the craniofacial skeletal characteristics in anterior openbite group, in contrast to normal occlusion group, and also to find out, vertical factors which shows, correlate to the amount of anterior openbite. 21 individuals, including 10 males and 11 females, without orthodontic treatment history and anterior - posterior skeletal malrelationships, were selected and analyzed basic morphology and vertical factors, using standard lateral cephalogram. The obtained results as follows: 1. Comparison of anterior open-bite group with normal occlusion group using Moyers analysis. a) In basis morphologic analysis, Ba-SE-Me was lesser and Ba-SE-Mn. P. and Mn.P/A-B were greater in both sexes of anterior openbite group than in normal occlusion group. b) In angle measurements of vertical analysis, PMV/Pal P. was lesser in male anterior open-bite group and PMV/Occ. P. and PMV/Mn. P. were greater in both sexes of anterior openbite group than in normal occlusion group. c) In height ratio of vertical analysis, ATFH/PTFH and ALFH/ATFH were greater in both sexes of anterior openbite group than in normal occlusion group. 2. The amount of anterior open bite was correlated with PMV/Occ. P. and PMV/Mn. P.

7.Soft-tissue thickness of South Korean adults with normal facial profiles.

OBJECTIVE: To standardize the facial soft-tissue characteristics of South Korean adults according to gender by measuring the soft-tissue thickness of young men and women with normal facial profiles by using three-dimensional (3D) reconstructed models. METHODS: Computed tomographic images of 22 men aged 20 - 27 years and 18 women aged 20 - 26 years with normal facial profiles were obtained. The hard and soft tissues were three-dimensionally reconstructed by using Mimics software. The soft-tissue thickness was measured from the underlying bony surface at bilateral (frontal eminence, supraorbital, suborbital, inferior malar, lateral orbit, zygomatic arch, supraglenoid, gonion, supraM2, occlusal line, and subM2) and midline (supraglabella, glabella, nasion, rhinion, mid-philtrum, supradentale, infradentale, supramentale, mental eminence, and menton) landmarks. RESULTS: The men showed significantly thicker soft tissue at the supraglabella, nasion, rhinion, mid-philtrum, supradentale, and supraglenoid points. In the women, the soft tissue was significantly thicker at the lateral orbit, inferior malar, and gonion points. CONCLUSIONS: The soft-tissue thickness in different facial areas varies according to gender. Orthodontists should use a different therapeutic approach for each gender.

8.Silent Aortic Regurgitation.

Aortic regurgitation is a common valvular heart disease, usually the result of rheumatic fever, or syphilis, and rarely of congenital origin. It is frequently associated with other valvular heart disease, especially mitral valve disease. It can be diagnosed by the presence of pulse pressure widening, a Corrigan pulse, and an early decreascendo diastolic murmur at the left sternal border between the second and third intercostal spaces. After the clinical application of cineaortography in the diagnosis of valvular disease, Segal et al (1964) first reported rheumatic aortic regurgitation without an audible murmur in patients having mitral valve disease. The importance of discovering aortic reguritation in patients with predominent mitral disease has begun to be appreciated recently, especially as commisurotomies for the relief of mitral stenosis are performed more frequently. Nowadays eventhough the severity of aortic regurgitation is often not evident preoperatively, aortic regurgitation can become very evident when mitral stenosis is relieved. This study was comprised of seventeen patients with silent aortic regurgitation which was confirmed by cineaortography at Severance Hospital from January, 1970 to August, 1976. 1. Of the seventeen patients, 12 patients were associated with mitral stenosis, 4 with mitral steno-insufficiency, and 1 with mitral insufficiency. 2. Silent aortic regurgitation was suggested from the accompanying clinical features such as chest pain, apical heaving, and left ventficular hypertrophy pattern on both roentgenogram of the chest and electrocardiogram. 3. The severity of the aortic regurgitation was mild to moderate; 7 of the 17 patients being grade I, and 10 patients being grade II on cineaortogram.

9.A Case of Hematometrocolpos due to Complete Septate Uterus and Obstructed Hemivagina associated with Ipsilateral Renal Agenesis.

We report a case of a 13-year-old girl who was noted to have combined malformation of complete septate uterus, obstructed hemivagina and ipsilateral renal agenesis with complaints of dysmenorrhea. Hematometrocolpos was developed in the result of retained menstrual blood since menarche. This rare congenital anomaly is supposed to be derived from disorders of mesonephric and M llerian developement in embryogenesis of female genitouinary tract. An accurate preoperative diagnosis of this syndrome is critical because of the difference in therapeutic strategy and prognosis. Simple and definite treatment of the condition is excision of the obstructed vaginal septum with prompt exit of retained blood. We present here a interesting case of M llerian anomaly in attempt to alert gynecologist to the possible occurrence of such a malformation with a brief review of concerned literatures.

10.A study of the variances in pre-and post-treatment dental arch shapes in extraction and non-extraction cases.

This study was carried out in order to find out the amount of tooth movement, the changes arch size and the changes in arch morphology following orthodontic treatment and to provide a guideline for to predict post-treatment arch morphology. The sample group for this study consists of 15 males and 22 females, totalling in 37 persons, who received orthodontic treatment at Orthodontic Department of DanKook Univ. Dental Hospital. They are classified into Extraction Class I treatment group (E I), Non-extraction Class I treatment group (N I), and Non-extraction Class III treatment group (N III), according to their pre-treatment malocclusion state and methods of treatment. Following conclusions and averaged dental arch form for each group were obtained by cephalometric linear measurements and dental arch measurements using pre- and post-treatment lateral cephalograms and plaster study models. 1. Intercanine width were reduced in max. of both EI and NI during the period of treatment. 2. Intermolar width were reduced in max. of EI and increased in max. of NI. Therefore although there was no difference between these two groups before the treatment, intermolar width of the max. of NI was wider than that of EI after the treatment. 3. PMV-incisor distance and PMV-canine distance were decreased in both max. and mand. of EI and that of NI, during the period of treatment. PMV-molar distance was decreased in both max. and mand. of NI and in mand. of NIII. 4. Items that showed stability during the treatment were: max. & mand. PMV-molar distance, mand. intercanine and intermolar width in EI; mand. intercanine and interrnolar width in NI; mand. & max. PMV-incisor distance, PMV-canine distance, max. PMV-molar distance and max. & mand. intercanine and intermolar width in NIII. 5. The differences in averaged canine and molar variances to post-treatment dental arch form were present only in EI and in NI. There was no variance between maxilla and mandible in each group.
