BACKGROUND: Current studies on multimodal strategy for N2 non-small cell lung cancer are being high interest have drawn much attention. N2 lung cancer, however, is composed of is divided into several subgroups with that have different prognoses. The prognostic factors still ramain controversial. METHODS: Between January 1990 and June 1999, 180 patients with N2 lung cancer who underwent surgical resection were investigated, excluding 10 of these for surgical mortality. All patients underwent mediastinal lymph node dissection. 20 clinicopathologic factors were investigated by univariable and multivariable analyses to identify significant prognostic factors among resected N2 disease. RESULTS: The overall 5-year survival rate was 20.6%. Multivariable analyses among overall patients revealed 3 significant prognostic factors:Age, Histologic type, Vascular invasion. Based on the result, 49 patients with both age more than 60 and pathologic Non-squamous cell showed a 5-year survival of 5.0%, whereas 37 patients with neither of the factors showed a 5-year survival of 56.6%(p<0.001). And 12 patients with both vascular invasion and pathologic Non-squamous cell showed a 5-year survival of 11.9%, whereas 67 patients with neither of the factors showed a 5-year survival of 33.6%(p=0.01). CONCLUSION: The prognosis of surgically resected N2 disease varies according to the 2 significant prognosis factors. Tumor size may be an additional influencing factor in the prognosis of N2 disease.

An aberrant right subclavian artery, the most common congenital anomaly of the aortc arch, is rarely symptomatic during the infancy, if an anomalous origin of a right common carotid artery is also associateda varient of innomiate artery compression syndrome. We experienced a case of an aberrent right subclavian artery associated with an anomalous origin of the right common carotid artery in a female newborn, who showed severe respiratory distress soon after birth. The diagnosis was confirmed by aortogram and operative findings. the ligature and section of the aberrent right subclavian artery resulted in improvement of respiratory distress. A brief review of the related literature is also presented.

We present two cases of acute febrile neutrophilic dermatosis developed in women aged 56 and 52. One of the two patients deveIoped the lesions during longterm antituberculosis chemotherapy for her advanced pulmonary tuberculosis, The other case developed the lesions, initially at the site of acupuncture done for relief of her arthralgia, followed by the involvement of the other sites. The etiology of acute febrile neutrophilic dermatosis is not clear, however, hypersensitivity and association with systemic diseases such as upper respiratory infection, malignancies, ulcerative colitis and pyoderma gangrenosum are postulated.

Chronic illness requiring attention and management during a long period of time puts great burden onto patients, their family and society. For patients with chronic illnesses, providing social support is the most important, and the fundamental support comes from their spouses. Amount and quality of support from spouses seems to differentiated according to the sex of patients. Female patients tend to believe that their spouses are not very supportive. Therefore, the researchers assessed the burden of husbands of female arthritis patients to discover the factors that result in greater burden. Also, they developed a theoretical model of husbands' care for their wives through a qualitative research into husbands' experience. Method 1: The study material was 650 female arthritis patients registered in an arthritis clinic. The questionnaire about the disease experience of female arthritis patients and the burden of husbands were sent. Returned questionnaires numbered 210(32.3%) and 27 were excluded because of inadequate answers. The remaining 183 questionnaires were analyzed. The mean age of the patients was 51 years and the mean age of spouses was 55 years. The mean marital period was 28 years. The average duration since diagnosis was 9.1 years. Education level was varied from primary school to graduate school, and average income/month was 1,517,300 won. Method 2: Initial questionnaire studies on the burden of husbands were performed. Among 183 responding husbands, 23 consented to participate for a qualitative research. Data was obtained by direct and telephone interviews. The mean age of participants was 58 years, and the educational level and socioeconomic status also varied. Result: 1. Husbands' burden: The average burden was 57.68 with a range of 6-96. 2. Burden and general characteristics: The husband's burden correlated with the age of the patients, numbers in the family, therapy methods, patient's level of discomfort, patient's disease severity, patient's level of dependence and the husband's understanding of the level of severity. 3. Linear correlation analysis on burden: The husbands' burden is explained in 22.5% by husband's recognition of level of severity and husbands' age. 4. There were four patterns of the burden on husbands: both objectve burden and subjective burden were high(pattern I), both of objectve burden and subjective burden were low(pattern II), objective burden was high but subjective burden was low(pattern III), objective burden was low but subjective burden was high(pattern IV). The pattern was correlated with the family income, educational level of the patients and their husbands, therapy methods, patient's level of discomfort, patient's disease severity, patient's level of dependence and husband's understanding of level of severity. 5. The core category of the caring experience of the husbands with arthritis patients was "companionship". The causal factor was the patients' experience due to symptoms : physical disfigurement, pain, immobility, limitation of house chores, and limitation of social activities. Contextural factors are husbands' identification of housework and husbands' concern about the disease. The mediating factors are economic problems, fear of aging, feeling of limitation and family support. The strategy for interaction is mind control and how to solve emotional stress. The "companionship" resulted from caring activities, participation of household activities, helping patients' to coping with emotional experience. 6. Companionship is established through the process of entering intervention, and caring state of mind. Entering intervention is the phase of participation of therapy and involvement of houseworks. The caring phase consists of decision on therapy, providing therapy, providing direct care, and taking over the household role of wife. Through caring phase, the changing phase set a stage in which husbands consolidate the relationship with their wives, and are reminded of the meaning of marriage. As a result, in changing phase, husbands' companionship is enhanced. In conclusion, nursing care of chronic illnesses should include a family member especially the spouse. All information on disease shoud be provided to patients and whole family member. Strong support should also be provided to overcome difficulties in taking over role of other sex. Then the quality of life of patients and families will be much improved.

A 43 year-old Korean male has had multiple, grouped, asymptomatic, yellowish brown flat papules and nodules on the knees, elbows, buttocks, palms and soles for 3 years. Small yeIlowish, discrete papules first appeared on the right palm and left elbow, and gradually increased in size and number and spread to knees, buttocks and soIes. Physical examination revealed normal except for the skin lesions and no familial occurrence was noted. Gross finding of the serum was turbid, more or less creamy and paper electrophoresis revealed marked increase of prebetalipoprotein, increase of betalipoprotein and alphalipoprotein and absence of chylomicron, suggesting type III or type IV hyperlipoproteinemia. Serem choleterol was 200mg% and fasting blood sugar was 115.0mg%. with normal glucose tolerance test. Histopathology showed many aggregates of foam cells on H-E stain and many lipid droplets in the dermis on Sudan red stain. Based on the clinical and histopathological findings and lipoprotein analysis by the paper electrophoresis, the patient was diagnosed as type IV hyperlipoproteinemia with xanthoma tuberosum.

Traumatic dislocation of the knee joint occurs very infrequently, but is one of the true emergencies in the orthopedic field. It is a serious injury, associated with extensive soft tissue demage and the danger of neurological and vascular involvement. There is a lot of theories about the mechanism of injury, the treatment and the incidence of complications. Generally they accept that in irreducible cases operative reduction is essential, but there is no uniformity of opinion on the treatment of the uncomplicated cases. This paper is based on 7 traumatic knee dislocations treated at Kang Nam Sacred Heart Hospital, Hallym College during the period from January 1980 till December 1984. The results of the study are as follows: 1. The patients are mostly injured due to the traffic accident and show even age distribution from twenties to fifties. 2. 5 cases out of 7, reduced at our hospital, were all anteriorly dislocated. 3. The posterior capsule and the posterior cruciate ligament were injured in all cases. 4. There were no significant differences between primary repair of soft tissues and the late reconstruction.

OBJECTIVES: This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition. METHODS: The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers. RESULTS: In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group. CONCLUSIONS: In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

A total of 14 children underwent 29 treatments with EDAP-LT01 extracorporeal shock wave lithotripsy for urinary tract stones between February, 1987 and ugust, 1988. The results were obtained as follows : 1. The sex ratio was about A 2:1, 9 (64.3%) in male and 5 (35.7%) in female patients. The average patient age was 7.1 years with a range of 2 to 16 years. 2. Treated stone locations were kidney in 8 children (57.1% ) ; calyx in 6 (42.8% ) and renal pelvis in 2 (14.3% ) and ureter in 6 (42.9% ) ; upper ureter in 2 (14.3% ) and lower ureter in 4 (28.6 %). There were multiple renal stone in 4 (28.6% ) and bilateral renal stone in 1 (7.1%), the average stone size was 1.6cm with a range of 0.7 to 3.1cm. 3. The average numbers of treatment were 1.8 sessions and the average treatment time was 38.7 minutes. The average storage required was 107 in one session. 4. Of the 11 patients whose age was less than 10 years, 10 (71.4% ) required caudal anesthesia. Three patients whose age was more than 10 years treated without any type of anesthesia. 5. The total average success rate of treatment-ended 11 patients was 81.8% in stones of 0.7-3.0cm. The cause of failed ESWL were stone component in 1 and impacted stone in 1. 6. All or the patients were not required the pre and post-ESWL additional manipulation. ESWL complications were not required admission and surgical management. Therefore, we confirmed that EDAP-LT01 ESWL is a safe, effective and easily applicable technique for treatment of urinary stones in children.
