2.Effect of ovarian on FSH release and subunit synthesis in rat anterior pituitary cells in culture.

OBJECTIVE: to determine whether ovarian steroids directly regulate FSH release and B subunit synthesis at the pituitary level METHODS: In vitro study. After applying ovarian steroids in cultured rat anterior pituitary cells, we evaluate the amount of relesed FSH and FSH B subunit using dot-blot hybridization. RESULTS: Estradiol alone or GnRH-induced did not modulate FSH release and B ubunit synthesis. Although progesterone alone did not alter FSH release, GnRH-induced FSH release was significantly augmented by the treatment of progesterone. Progesterone alone stimulated FSH B mRNA level. Significant increase in FSH B mRNA level was also observed by the combined treatment of progesterone and GnRH. The effects of progesterone on GnRH-induced FSH release was significantly reduced by the treatment of progesterone antagonist, RU 486. The treatment with progesterone antagonist, RU 486 also abolished progesterone-induced FSH B subunit mRNA biosynthesis. CONCLUSION: From these results, it is, therefore, concluded that estradiol does not seem to be a major regulator for FSH synthesis but progesterone may exert its action at the pituitary level for the synthesis and release of FSH.

3.Efficacy and safety of budesonide turbuhaler in Korean asthmatic patients.

A controlled study was carried out in 50 patients with perennial bronchial asthma to assess the efficacy and safety of budesonide turbuhaler. Subjects have suffered from cough, wheezing, dyspnea and chest tightness and showed either 15% of reversibility in FEV after bronchodilator inhalation or airway hyperresponsiveness to methacholine(PC20 < or = 25mg/ml.) Patients were randomized to treatment with budesonide turbuhaler or terbutaline turbuhaler for 8 weeks after 2 weeks of run-in period. Budesonide turbuhaler was effective for cough, wheezing, dyspnea and chest tightness. It improved peak expiratory flow rate and FEV1. Budesonide turbuhaler was tolerated well and the laboratory tests showed no abnormality. It is suggested that budesonide turbuhaler is effective and safe in the management of bronchial asthma.

5.Clinical consideration between the type of pneumonia and cold agglutinin titer, and mycoplasma antibody titer caused by mycoplasma pneumonia in children.

The authors analysed 261 cases of mycoplasmal pneumonia in children who were admitted to the pediatric department of Seoul Adventist Hospital between July 1986 and June 1991. The result obtained were as follows; 1) Yearly distribution of the cases showed high in 1987 and 1991, but no significant difference was noted in seasonally or monthly distribution. 2) The peak incidence of age was between 2 and 6 years of age, and the sex ratio of boy to girl was 1.1:1. 3) pulmonary manifestations were asthma in 21 cases (8.0%), atelectasis in 5 cases (1.9%), emphysema in 3 cases (1.2%), pleural effusion in 16 cases (6.1%), otitis media in 4 cases (1.2%) and sinusitis in 9 cases (3.4%). 4) Extrapulmonary complication were hepatitis in 53 cases (20.3%), skin rash in 9 cases (2.4%), proteinuria in 6 cases (2.3%), and hematuria in 4 cases (1.5%). 5) On the chest X-ray examination, the most common type of pneumonia was interstitial pneumonia (94 cases, 39.8%), and unilateral involvement was common (85.6%), and the most common involvement was right lower lobe (83 cases, 41.4%). 6) The relationship between the type of pneumonia and cold agglutinini titer, and between the type of pneumonia and Mycoplasma antibody titer were not found (P>0.05). 7) In the 16 cases of pleural effusion, The type of pneumonia was lobar, lobular (9 cases), bronchopneumonia (7 cases), and interstitial type was not present. The site of pneumonia was left (6 cases), right (9 cases), and 1 case showed bilatrality. The extent of pleural effusion was mild (10 cases), moderate (3 cases), and severe (3 cases). The result of pleural fluid exam in severe cases was all exudate. 8) The mean duration of admission was most common from 6 to 10 days. the mean duration of admission by the type of pneumonia was lobar, lobar (10.71+/-3.40), interstitial (8.78+/-2.14), and bronchopneumonia (8.83+/-2.47). 9) Both mycoplasma antibody test and cold agglutinin test were carried out in 185 cases and sensitivity of mycoplasma antibody test was 55.1%.

8.Production, characterization, and clinical application of perchloric acid soluble M. tuberculosis antigen(TB-I).

The immune response to M. tuberculosis engages T-cell medicated reactions which determine the degree of resistance and also the clinical pattern of disease. Although antibodies produced by the infected host have yet no proven protection, they are appilicable for alternative diagnostic tests in tuberculosis. Preparation of purified antigens from Mycobacteria with specific antigenic determinants would improve serological diagnosis in tuberculosis. The antigen was prepared by extraction of M. tuberculosis with perchloric acid which was found to be major bands of 71, 42, 38 and 10 kD by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. Thirteen sera from patients with active tuberculosis showed positive reactions to TB-P by ELISA, but the reaction to perchloric acid extract antigen is weaker than to PPD. The IgG subclass profiles to TB-P were IgG1, IgG2 and IgG4. By means of western blotting, the antibodies in the tuberculous patients showed the reaction with antigens of lower molecular weight such as 14, 12, 11 and 10 kD. These results suggested that the perchloric acid soluble antigen of M. tuberculosis might be more related with cell-mediated immune reactions rather than humoral immune reactions.

9.A case of occupational asthma induced by latexin a hospital personnel.

IgE-mediated sensitization to natural rubber latex can induce immediate hypersensitivity reactions ranging from contact urticaria to life threatening anaphylaxis. Recent reports suggest that asthma is also relatively frequent manifestation of latex allergy. In this case report, lat,ex induced asthma is described in an operat.ing room nurse regularly exposed t,o latex gloves. Her latex sensitivity was detected by skin prick testing. Specific bronchial provocation test with latex extract showed an early asthmatic reaction. Her symptoms had been much improved after avoidance. Occupational asthma induced by latex may be not uncommon among health care workers.

10.Fordyce's Spots on the Penile Shaft: An Unusual Localization.

Fordyce's spots are ectopically located sebaceous glands, and these are clinically characterized by multiple pinhead- sized whitish or yellowish papules. Although the condition is not generally considered to be pathological, there are some problematic cases that have significant cosmetic concerns. These spots are usually located at different sites inside the oral cavity, and mainly at the upper lip and buccal mucosa. However, they occasionally occur on the areola, glans penis and labia minor. We herein report on a case of Fordyce's spots on the penile shaft because this condition should be considered in the differential diagnosis of lichen nitidus.
