1.Brain Atrophy and its C.T.Criteria.

Since opening of the Yeungnam University Hospital in March 1983 till the end of 1986, total 156 cases of brain atrophy and encephalopathy (control) diagnosed at the neurology department were investigated, adopting C.T. criteria of brain atrophy. Among the 156 only 77 cases had available C.T. films. Of the 77,35 cases were normal (control) and 42 had brain atrophy. The following results are obtained: 1. A new C.T. criteria of brain atrophy and its degree were devised. (Table 8.9) 2. As the age increased, number of brain atrophy cases increased. 3. Degree of EEG abnormalities were not parallel with the degree of brain atrophy. 4. The most frequent EEG abnormalities with or without brain atrophy were moderate abnormalities. 5. The EEG abnormalities were improved with treatment in most of the cases. 6. The group with suspected organic brain lesions had 69% cases of brain atrophy, whereas the group with only headache or dizzines had 57% of no atrophy. 7. There exists a clinical entity of "Cryptogenic Brain Atrophy". (Park) 8. Recent literatures of Brain Atrophy were reviewed.

2.C.T.Findings and Prognosis of 25 Cerebellar Infarction Cases.

Cerebellar infarction (CbI) is rare but because of its location it can give vital influence on the neighbouring structures. Therefore, it is important to make correct diagnosis and know general clinical pictures including prognosis. We studied 25 cases of CbI about their C.T. findings and clinical pictures, and the following results were obtained. 1. Location of the CbI was in inferior cerebellum 20 cases, superior 4 cases. Prognosis was good in 17 inferior and 2 superior cases. 2. Size of the CbI was variable, but fatal cases showed more than 5-6cm in diameter. 3. Deformity of the 4th. Ventricle and slight hydrocephalus were seen in 12 among 20 acute cases. 2 cases expired. 4. Among the total 25 cases 20 cases showed clinically complete cure or improvement, 2 cases worse, 1 cases undetermined, and 2-cases expired.

3.The effects of Broad Spectrum Antibiotics and Endotoxin to the Carbon Tetrachloride-induced Liver Injury.

This study was performed to investigate the effect of endotoxin to the CCl4-induced liver injury. Twelve Sprague-Dawley rats were intraperitoneally injected 1.6 g/kg CCl4 as control group. Another 24 rats were orally administrated 300 mg/kg of neomycin at 16 and 3 hours prior to CCl4 injection as experimental group. Twelve among them were intraperitoneally infected 1.0 mg/kg of endotoxin(E-Coli, 0111:B4, No L-2630, lipopolysaccharide, Sigma, USA) and CCl4 simultaneously for offsetting neomycin effect. The rats were sacrificed at 1, 4, 10, and 24 hours after CCl4 injection. The liver tissues from all experimental groups were observed by light and electron microscopy. The results obtained were summarized as follows: In the CCl4 only group, the hepatocytes revealed sweling of ER and mitochondria with many lipid droplet in the cytoplasm. Focal cellular necrosis was seen at the later phase. The Kupffer cells were activated and showed many cytoplasmic processes, secondary lysosomes, and vaculoles. The endothelial cells were edematous. Several neutrophils, platelets, and microthrombi were scattered in the sinusoid. In the neomycin-CCl4-endotoxin administrated group, both hepatocytic destruction and intrasinusoidal microthrombi formation were more pronounced. In the neomycin pretreated group, the hepatocytes revealed mild cellular destruction without necrosis. There is no intrasinusoidal microthrombi. According to these results, it would be concluded that the small dosage of gastrointestinal tract-derived endotoxin affects to the liver injury caused by CCl4. The synergistic effects of CCl4 and gastrointestinal tract-derived endotoxin which can not be detoxified by damaged Kupffer cells, may be more important in the pathogenesis of CCl4-induced liver injury.

4.103 Cases of Young Adults with Stroke: The Causes and Prognosis.

Stroke in young adults are relatively uncommon. To investigate the causes, clinical findings and prognosis of cerebral infarction (CI), intracerebral hemorrhage (ICH), and subarachnoid hemorrhage (SAH) in young patients, we reviewed the medical records of 103 young patients(age: 15-45 years) who had been admitted to Yeungnam University'Hospital from Jan. 1986 to Dec. 1990. The proportion ol young adults was 6.1% in all cerebrovascular disease patients in our hospital during the same period. The mean age was 36.38 +/- 7.27 years. About two third of the patients were older than 35 years old. The ratio of male to female patients was 1.39 to 1. The percentage of patients with ICH was 45.6%, CI, 37.9%, and SAH, 16.5%. The causes of cerebral infarction were atherosclerosis (38.4%), cardiogenic cause (28.2%), nonatherosclero tic vasculopathy (10.3%), and hematologic abnormality (7.7%) in order of frequency, and the most involoved site was the carotid territories (79.4%). The major causes of intracerebral hemorrhage were hypertension (59.6%) and arteriovenous malformation (8.5%). The locations of the hematoma on brain CT scan were the basal ganglia (42 : 5%), lobe (34%), thalamus (8.5%), pons (6.4%) and ventricle (4.3%) in that order of frequency. Subarachnoid hemorrhage was mainly due to ruptured aneurysm (76.4%). Undetermined causes noted in 15 cases (14.6%). The overall mortality of this group of patients was 23.3%. Most of the patients (83.3%) with fatal outcome died within the first 2 weeks. The degree of neurologic sequelae among survivors was mild (45.6%) to moderate (22.3%).

5.Myositis ossificans traumatica IN MASSETER MUSCLE.

We had experienced a case of traumatic myositis ossificans arising in right masseter muscle, inferior to zygomatic arch occurred on 25 year old male. He had some trauma on the site one year ago and visited with complaint of mouth opening limitation and swelling. Palpable mass with facial disfigurement was noted. and several ovoid radiopaque masses were revealed on C.T. examination. On histopathologic examination, multiple bone trabeculae with osteoblasts in its periphery was noted in connective tissues and invaded to neighbouring muscles, but any chondroid components were not revealed.

6.Study on the changes of nerve conduction with wrist fixation in carpal tunnel syndrome.

The author studied 20 healthy adults (20 hands) as a control and 30 patients (40 hands) with carpal tunnel syndrome to evaluate the clinical usefulness of measuring nerve conduction velocity after wrist flexion in diagnosis of carpal tunnel syndrome. The median nerve conduction velocity over wrist to finger segment was measured before and after wrist flexion for 1, 2 and 5 minutes, using belly-tendon method for motor nerve distal latency (MNDL) and antidromic method for sensory nerve conduction velocity (SNCV). The results were as follows: 1. In control group, MNDL increased in 1 hand and SNCV decreased in 2 hands after wrist flexion. In patient group, MNDL increased in 2 hands and SNCV decreased in 3 hands after wrist flexion. 2. In both control and patient group, there were no significant changes in mean values of SNCV and MNDL between before and after wrist flexion. 3. Phalen's wrist flexion test was positive in 5 percent of control and 60 percent of patient group. 4. Tinel's sign was present in 10 percent of control and 33 percent of patient group.

7.A Day Care Model for Rehabilitation of Chronic Psychotic Patients.

OBJECTIVES: The purpost of this study was to present general system,operation,and program of a day care model managed by a psychiatrist,to evaluate the results of performing this model,and thus to developa day care model applied to Korean situation appropriately for promoting maintenance and rehabilitation of chronic psychotic patients. METHODS: We performed this day care model(hereinafter called as this model) from March 1997 to February 1998. The subjects were composed of 23 psychotic patients. PANSS, Quality of Life Scale,Life Satisfaction Self-Rating Scale were used before beginning this model-and after three months of performing this model. Program Helpfulness Scale,13 Therapeutic Factors Scale were used at one month of day care and after three months of day care. RESULTS: 1) This model helped maintenance and rehabilitation of chronic psychotic patients through reducing their symptoms and increasing quality of life. 2) This model used the various group therapy, especially creative art therapy and activeity therapy, played an important fole,for recovering Physical,psychological,and social functions. 3) Important therapeutic factors at early and late period of this model were identification with therapists,guidance of therapists,and the corrective recatitulation of the primary family group. Important therapeutic factors at early period were group cohesiveness and altruism, and at late period instillation of hope. 4) This model was useful for integration the multidisciplinary therapeutic team and the various therapeutic methods. CONCLUSION: This model would be applied to day care in this country effectively and efficiently for maintenance and rehabilitation of chronic psychotic patients.

9.The Results of Autologous Chondrocyte Implantation with a Concomitant Injury.

PURPOSE: To evaluate the results of autologous chondrocyte implantation with a concomitant injury. MATERIALS AND METHODS: Sixty-seven chondral defects (39 cases, 36 patients), which were treated with autologous chondrocyte implantation, were analyzed with a minimum follow-up of 2 years. The cases were divided into the following five groups: 4 cases of a single chondral defect (group I), 3 cases of multiple chondral defects (group II), 5 cases of osteochondritis dissecans (group III), 9 cases of a single chondral defect with a concomitant injury (group IV), and 18 cases of multiple chondral defects with a concomitant injury (group V). The clinical outcomes were evaluated using the International Knee Documentation Committee (IKDC) subjective and objective scores and the International Cartilage Repair Society (ICRS) functional evaluation system. Arthroscopic examinations were performed on 12 cases and 21 chondral defects. RESULTS: The mean IKDC subjective score was 39.8 preoperatively, which improved to 64.1 postoperatively, and the IKDC objective score was C in 54% and D in 46%, which improved to A in 74%, B in 23% and C in 3%. The ICRS functional evaluation system was III in 82% of cases, and IV in 18% preoperatively, which improved to I in 15% and II in 85%, postoperatively. At the arthroscopic evaluation, the results of the ICRS system were I in 9%, II in 67%, and III in 24%. There was no significant difference between the single chondral defect and chondral defects with a concomitant injury. CONCLUSION: In the treatment of the multiple chondral defects with a concomitant injury, autologous chondrocyte implantation produced similar excellent clinical results to those of a single chondral defect.

10.A Study on the Development of Degenerative Osteoarthritis after Arthroscopic Total Menisectomy.

PURPOSE: This study compared the clinical and radiological incidence of osteoarthritis after a total meniscectomy. MATERIALS AND METHODS: Seventy eight patients, who underwent a total meniscectomy, were evaluated after a minimum follow up of five years. The operations were a medial meniscectomy (group I) in 16 cases, a lateral meniscectomy (group II) in 17 cases, a discoid meniscus (group III) in 29 cases and medial meniscectomy with an anterior cruciate ligament reconstruction (group IV) in 16 cases. The development of degenerative osteoarthritis was analyzed using the Kaplan-Meyer survivorship. RESULTS: At postoperative 5 years and 7 years, degenerative osteoarthritis developed in 17% and 36% of patients, respectively. The incidence ofdegenerative osteoarthritis at postoperative 5 years and 7 years in groups I, II, III and IV was 9% and 18%, 14% and 29%, 25% and 46%, and 28% and 55%, respectively. The difference was statistically significant. CONCLUSION: The meniscus deficient knee joint which varies according to the patterns of a meniscal injury, had a higher incidence of degenerative osteoarthritis. Careful attention should be paid to the treatment of meniscal tears.
