2.Calcifying Synovial Sarcoma.

A case of calcifying synovial sarcoma, occuring in the deep muscle fascia of the left thigh is reported. The presence of extensive calcification in synovial sarcoma is a favorable sign for prognosis. The patient was a 31-year-old female. The tumor mass had been present for 3 years, accompanying local tenderness. X-ray revealed a soft tissue tumor with central calcification, which was located between the adductor magnus and brevis of the left thigh. The tumor size was 7 x 5.5 cm. There was no connection with the knee joint or the femur. Grossly, the tumor was a relatively well circumscribed hard tumor with massive calcification. Microscopically, the tumor was composed of predominantly spindle cells with accompanying hyalinization, numerous spherical concretions and ossification. The epithelial component was not clearly noted. Mitotic figures were rarely noted in the densely cellular area. Immunohistochemical staining for EMA, S-100, vimentin, and carcinoembryonic antigen was negative while vimentin and cytokeratin were weakly and focally positive. EM study revealed multiple desmosome-like calcification intercellular junctions with a slit-like lumen and an incomplete basal lamina, which suggest that tumor show these cells were undergoing epithelia] differentiation. Above evidence suggest that this tumor is a synovial sarcoma associated with a large area of calcification, a so called calcifying synovial sarcoma. No recurrence was noted in one and half years of follow up.

4.Lithium-induced inhibition of Na-K ATPase and Ca ATPase activities in rat brain synaptosome.

To explore the action mechanism of lithium in the brain, the author investigated the effects of lithium on Na-K ATPase and Ca ATPase in rat brain synaptosomes prepared from forebrains by the method of Booth and Clark. The activities of Na-K ATPase and Ca ATPase were assayed by the level of inorganic phosphate liberated from the hydrolysis of ATP. Lithium at the optimum therapeutic concentration of 1 mM decreased the activity of Na-K ATPase from the control value of 19.08 +/- 0.29 to 18.27 +/- 0.10 micromoles Pi/mg protein/h and also reduced the activity of Ca ATPase from 6.38 +/- 0.12 to 5.64 +/- 0.12 micromoles Pi/mg protein/h. The decreased activity of Na-K ATPase will decrease the rate of Ca2+ efflux, probably via an Na-Ca exchange mechanism and will increase the rate of Ca2+ entry by the depolarization of nerve terminals. The reduced activity of Ca ATPase will result in the decreased efflux of Ca2+. As a Conclusion, it can be speculated that lithium elevates the intrasynaptosomal Ca2+ concentration via inhibition of the activities of Na-K ATPase and Ca ATPase, and this increased [Ca2+]i will cause the release of neurotransmitters and neurological effects of lithium.

6.Pathologic Correlations of Various Indications for Large Loop Excision of Transformation Zone(LLETZ) in Patients with Cervical Lesion.

OBJECTIVE: This study was performed to evaluate the effects of mutiple indications and age on the likelihood of finding cervical lesion in large loop excision of transformation zone(LLETZ). METHOD: We reviewed the medical records of 471 evaluable LLETZ of cervix for cervical neoplasia at the Yonsei University College of Medicine, from April 1996 to August l998. The indications for the LLETZ were grouped as being for treatment(biopsy-proven disease)(indication A), significant discrepancy among cytologic and histologic results(indication B), unsatisfactory colposcopic finding(transformation zone not fully visualized)(indication C), possible microinva- sion(indication D), and combinations of the above. RESULTS: The prevalence rates of cervical lesion in performed LLETZ for indication A alone, for indication B alone, for a combination of indication A and C, for a combination B and C, and for indication D alone were 85.7%, 56.5%, 82.3%, 42.6%, 95%, respectively. The data were then reanalyzed to determine the likelihood of finding high-grade disease on LLETZ specimen. Overall, those with preoperative high-grade cytologic or histologic characteristics were much more likely to have high grade disease(78.1%) than were those with preoperative low-grade cytologic or histologic characteristics(27.2%)(p<0.001). The influence of age on the likelihood of identifying diasease on the LLETZ is undertermined. CONCLUSION: Neither age nor the preoperative grade of disease are good discriminators of the likelihood that disease will be found on a LLETZ specimen, However, patients with preoperative high-grade disease are much more likely than those with preoperative low-grade disease to have high-grade dysplasia or invasive cancer on LLETZ.

7.The Clinical Significance of Absence of Umbilical Artery End-Diastolic Flow in Severe Pre-Eclampsia and Eclampsia.

To determine the perinatal mortality and morbidity of fetuses with absent end-diastolic velocities (AEDV) of the umbilical artery in severe pre-eclampsia and eclampsia, the outcome of 5 fetuses with AEDV was compared with that of 35 fetuses with positive end-diastolic velocities (PEDV). The study population comprised 38 cases of severe pre-eclampsia and 2 cases of eclampsia with structurally normal singletons, who had had umbilical artery Doppler velocimetry weekly from admission to delivery. The Doppler velocimetry result was not used for the clinical management. Perinatal death and neonatal morbidity from both groups were further examined in gestational age category to control the influence of preterm births. The incidence of AEDV of the umbilical artery Doppler velocimetry in severe pre-eclampsia and eclampsia was 12.5% (5/40). The AEDV group had a significantly higher incidence than the PEDV group in terms of ceasarean section due to fetal distress (60% : 17%), Apgar score < 7 at 5 minutes (60% : 14%), perinatal death (25% : 0%) and assisted mechanical ventilation (67% : 9%) both at 32-36 weeks. Time intervals from the detection of AEDV to delivery of live neonates varied from the day to 15 days. In conclusion, AEDV in the umbilical artery might be of clinical value in routine surveillance of pregnancies complicated by severe pre-eclampsia and eclampsia, and predict hypoxic fetal condition which needs operative interventions before or during labor and mechanical ventilation after birth.

8.Factors Affecting Pulmonary Function in the Elderly Ex-Smokers.

BACKGROUND: The elderly ex-smokers are not free from the effect of previously exposed smoking, and have a tendency to participate in health promoting behavior eagerly. This study was designed to evaluate factors that affect pulmonary function in the elderly ex-smokers. METHODS: The ex-smokers with normal chest radiological findings who visited Seoul National University Hospital Health Promotion Center were analyzed. The effect of various factors including age, height, smoking amount, and duration of smoking cessation on pulmonary function indices were analyzed with bivariate correlation analysis. Multiple regression analysis was performed using factors indentified significant in bivariate correlation tests. RESULTS: Multiple regression analyses were done for the all age group and the elderly group. Factors significantly affecting Forced vital capacity(FVC) were height,age and duration of smoking cessation for both grroups. Smoking amount was not significant. Forced expiratory volume in 1 second(FEV1) was significantly influenced by height, age, duration of smoking cessation, and smoking amount for both group. In the all age group, peak expiratory flow(PEF) was siginificantly influenced by age, regular exercise, and duration of smoking cessation. In the elderly group, PEF was significantly affected by height, age, regular exercise, and duration of smoking cessation. The factors significantly affectiong FEV1/FVC were age, smoking amount in the all age group, and regular exercise, smoking amount in the elderly group. CONCLUSION: Smoking amount was a significant factor decreasing FEV1 in the elderly ex-smoker. Duration of smoking cessation was significant in normalization of pulmonary function indices in both the all age group and the elderly group. It is inferred that smoking cessation is beneficial in reversing the adver sely affected pulmonary function indices even in the elderly.

9.Biochemical characteristics of human periodontal ligament cells in vitro.

To find out the differences between periodontal ligament cells (PDL cells) and gingival fibroblast cells (GFB cells), alkaline phosphatase, a marker enzyme for osteoblast, was used to measure the activities and 45CaCl2 isotope was used to find out cellular and release of 45Ca, a requisite for bone formation,. PDL cells and GFB cells from 1 to 5 passages were also measured in alkaline phosphatase activity assay. By the use of above methods, followings were concluded that the PDL cells and the GFB cells have characteristics that are different from each other. In that PDL cells showed large amount of calcium uptake and large amount of calcium release in initial stage, they seem to possess characteristics which are similar to osteoblast-like cells. 1. The PDL cells, in contrast to the gingival fibroblast, showed exceedingly high alkaline phosphatase activity which was highest at the second passage, decreasing thereon. But gingival fibroblasts cells showed no distinct differences in alkaline phosphatase activity as the passage were elapsed. 2. For both PDL cells and GF cells, the 45Ca uptake was greatest at 2 hours period. The PDL cells showed higher measuring than GFB cells through out the whole time period. 3. Whereas the GFB cells showed slow increase of 45Ca release as time relapsed, the PDL cells showed rapid increase of 45Ca release.

10.Fibrous dysplasia of bone

Fibrous dysplasia of bone is a skeletal developmental anomaly of unknown etiology characterized by single ormultiple areas of fibrous tissue replacement of medullary cavity of one or more bones. The disease may belocalized to single bone (monostotic form), of may affect multiple bone(polyostotic form). Eighteen cases offibrous dysplasia diagnosed by roentgenologic or histologic assessments at Chosum University Hospital ChonnamUniversity hospital and kwangju Christian Hospital during recent ten years were analyzed clinically andradiologically. The results were as follows; 1. 16 cases of them had monostotic involvement, and 2 cases showedpolyostotic disease, but none of our series presented Albright's syndrome. 2. The male to female ratio in thisseries was 10:8, but then 2 polyostotic forms of them were femals. In age distribution, peak incidence at the timeof diagnosis was in the age group of second decade (10 cases). 3. Maxilla (6 cases ) and femur (4 cases) werefrequently involved sites in patients with monostotic lesion, whereas polyostotic lesions diffusely affectedskull, pelvis, ribs and limb bones. 4. The clinical symptoms according to the extent and site of disease were veryvariable, which were localized painless or painful swelling, nasal obstruction, deformity of face or extremity andincidentally during routine roentgen study. 5. The chemical abnormality of blood serum was moderate degree ofelevated serum alkaline phosphatase in only one patients with monostotic lesion. 6. The main radiologic findingsof fibrous dysplasia were relatively well circumscribed single or multiloculated cystlike appearance, boneexpansion, cortical thinning and/or erosion, bony deformity and pathologic fracture, but especially in maxilla,dense homogenous area with expanding lesion was observed in our series.
