1.Five Cases of Microsporum Gypseum Infection in Seoul Area.

Microsporum gypseum, long considered to be of animal origin, has recently been found to be soil inhabitating fungus and abundant in soil throughout the world. M. Gypseum infections tend to increase nowadays in Korea. Nineteen cases of ringworm caused by M. gypseum were reported in Korean literature until 1980 to the best of our knowledge. We present 5 cases of M. gypseum infection confirmed by culture findings on Sabouraud agar media and microscopic examination. The first case was 4 year-old boy with solitary coin sinzed pruritic annular erythernatous plaque ontaining papules, pustules and scales on the right cheek of on month duration. The second case, 7 year-old boy, had solitary walnut sized ovoid plaque consisted with papulopustules, oozing and peripheral scaling on right cheek of one month duration. The third case, 5 year-old boy, had solitary egg sized pruritic erythematous plaque with papuIes, scales and crust on the right lower leg of three weeks duration. The fourth case, 63 year-old female, had solitary egg sized sharply demarcated erythematious scaly patch with central clearing on nose and. nasolabial area of 10 days duration. The last case was 2 year-old girl with solitary coin sized annular erythematous scaly pIaque on left lower leg of two weeks duration.

2.Epicardiac fat lump in the left atrioventricular groove: CT and MR findings.

Fat can be prominet around the aortic root and within the right atrioventricular (AV) groove (1). At times, the fat can be globular, suggesting a mass especially on echocardiography. Computed tomography (CT) and masnetic resonance (MR) images are well suited for characterization of fat in and around the heart (1, 2). This report illustrates CT and MR findings of a very unusual case of prominent fat lump in the left AV groove.

3.Mycological and Clinical Observation on Tinea Faciale.

Tinea faciale is a dermatophyte infection of glabrous skin of the face except moustache and beard areas of the adult male. Generally it is not considered to be unique entity, and is included under Tinea corporis. However, T. faciale is somewhat different from T. corporis in its clinical manifestation, age and sex distribution and composition of causative fungi. Because superficial fungal infection of the face has not been emphasized, there is a low index of suspicion and often incorrect diagnoses are made. This clinical and mycological investigation was made with 36 cases of Tinea faciale among out-patients in dermatologic clinic of St. Pauls hospital, Catholic Medical College, from October, 1980 to September, 1981. Following results were obtained: 1. The incidence of Tinea. faciale was 0.67% among 5,406 out-patients and 3.7% among 973 superficial fungal infections. 2. The ratio of male to female patient was 1: 2.6 and incidence rate among divided age groups was the highest in under 9(36.1%) and 10-19(16.7%), 20-29(16.7%) in order(Table 1). 3. Central clearing was showed in 43.5% of the patients, annular or circular features in 65.2%, scales in 69.6%, papules in 39.1%, pustules in 17.4% and 73.9% of the patients had singie lesion(Table 3). 4. Coexisting fungal infection was found in 10 patients(27.8%) among 36 cases a,nd the most common coexisting fungus was Microsporum canis (Table 6). 5. The incidence of causative fungi in descending order were as follows: Microsporum canis(43.8%), Trichophyton rubrum (25%), Trichophyton mentagrophyte(15.6%) Microsporum gypaeum(9.4%), Trichophyton interdigitale(6.2%) (Table 4).

4.A Cases of Endodermal Sinus Tumor in Sacrococcygeal Region

A endodermal sinus tumer (Teilum) is a highly malignant germ cell tumor showing a selective overgrowth of yalk sac endoderm intimately associated with the extraembryonic mesoblast. Several cases of the tumor originating from the ovaries have been reported in the literature. We present a case of the tumor with a rare origin from the sacrococcygeal region.

5.Two Cases of Solitary Leiomyoma Cutis.

Leiomyoma Cutis is an unusual benign tumor of smooth muscle derivecl either from the arrector pili muscle, from the media of blood vesseles, or from smooth muscles of scrotum, labia or nipple. Two casea of leiomyoma cutis confirmed by H&E stain and Massons stain is presented, one with lesion on the left thigh and the ather with lesion of the left ankle. The first case was a 49 year-old female with solitary pinhead sized subcutaneous firm mass with intermittent pain and tenderess for 10 months, The second case waa a 36 year-old female with painful solitary peanut sized firm nodule which had pinkish center of 1 year duration.

7.A Case of Suspected Post-Irradiation Delayed Necrotizing Leukoencephalopathy.

Therapeutic irradiation may produce a variety of adverse side effects on the central and the peripheral nervous systems. These adverse effects are divided into two main groups; transient and permanent. Four major clinical syndromes have been described in relation to permanent radiation injury of the nervous system: encephalopathy, cranial neuropathy, myelopathy, and peripheral neuropathy. Among them, delayed encephalopathy represents the major hazard in CNS irradiation. The characteristic MR imaging pattern of radiation induced delayed necrotizing leukoencephalopathy is symmetric, high-signal foci on T2WI in the periventricular white matter. We report a case of suspected post-irradiation leukoencephalopathy showed progressive mental change, quadriparesis, and urinary incontinence with characteristic MR findings who had taken therapeutic irradiation 12 months ago.

8.Active solitary tuberculoma of the lung:CT and clinical findings.

To present CT and clinical features of active tuberculomas, we analyzed retrospectively CT findings of 14 tuberculomas (n=14) in 13 patients which appeared as solitary pulmonary nodules on plain radiographs and evaluated the response of tuberculomas to antituberculous chemotherapy. Nine tuberculomas (64%) were ovoid in shape and 10 (72%) showed smooth margin. Twelve(86%) tuberculomas were shown as low density lesions on unenhanced or enhanced CT scans. Calcification and cavitation were noted in three (21%) and eight (57%) tuberculomas respectively. Seven (50%) tuberculomas were accompanied by satellite nodules. Acid-fast bacilli (AFB) was positive in all tuberculomas in sputum, lavage fluid, or percutaneous transhoracic needle aspiration (PTNA). Smear and culture of lavage fluid and PTNA aspirate were superior to the detection of AFB than sputum examination. Follow-up study with antituberculous chemotherapy in 14 tuberculomas resulted in complete disappearance in three, decrease in size in seven, and no visible change in the remaining four. These observations suggest that tuberculomas are well-defined, ovoid, and low-density nodules containing calcifications and/or cavitations. Tuberculomas are relatively indolent even with threatment.

9.CYFRA 21-1, The New Marker for Lung Cancer.

BACKGROUND: Cytokeratins were subdivided into 20 different cytokeratins. It is known that cytokeratin 19 is particularly abundant in carcinoma of the lung. METHOD: A sandwich enzyme-linked immunosorbent assay called CYFRA 21-1, combining two monoclonal antibodies directed specifically aganist cytokeratin 19 was done by ES-300(Boehringer Mannheim, Germany). We investigated the diagnostic value of the new tumor marker CYFRA 21-1 in lung cancer in comparison to carcinoembryonic antigen(CEA) and neuron specific enolase(NSE) in the pilot study on 39 serum samples from lung cancer (17 squamous cell cancer, 11 adenocarcinoma, 11 small cell carcinoma) and 51 benign lung diseases and 41 normal healthy persons. RESULTS: Postulating a specificity 95% versus benign lung disease and healthy group, the diagnostic sensitivity of CYFRA 21-1 in lung cnacer independent of histologic type was 43.6% whereas CEA was 48.7% and NSE was 30.8%. The diagnostic sensitivity is increased to 69.2% by combining CYFRA 21-1 with CEA. Classifying the lung carcinomas following their histologic type, squamous cell carcinomas proved to possess the highest CYFRA 21-1 sensitivity(CYFRA 21-1 47.1%, CEA 23.5%, NSE 11.8%) and the combination with CEA 52.9% could not markedly increase the number of true-positive test results. In adenocarcinoma, CEA possessed the highest sensitivity (81.8%) whereas CYFRA 21-1 was 45.5% and NSE was 9.1%. In small cell carcinoma of lung, NSE possessed the highest sensitivity(81.8%) whereas CYFRA 21-1 was 36.4% and CEA was 54.5%. CONCLUSION: This study indicates that CYFRA 21-1 has a potential as a new marker for squamous cell lung cancer. If histologic type is unknown at the time of primary diagnosis. the combined determination of CYFRA 21-1 and CEA showed the best discrimination between benign and malignant lung disease.

10.A Case of Twin Pregnancy with Ine Fetal Demise(Turner's Syndrome).

Chromosomal abnormalities are higher in twin gestations than in the singleton population. Turner's syndrome(gonadal dysgenesis) variety may result from chromosome loss during gametogenesis in either parent or a mitotic error during one of the early cleavage divisions of the fertilized zygote. The vast majority of 45, XO conceptions result in first or second-trimester miscarriage. Fetuses with Tumer's syndrome commonly exhibit posterior nuchal cystic hygromas and generalized edema. Recently we experienced one fetal demise in twin pregnancy. The affected fetus was associated with Turner's syndrome which was diagnosed by amniocentesis and karyotyping. The fetus was associated with cystic hygroma which was antenatally diagnosed by ultrasonogram. The unaffected fetus had normal karyotype and was delivered through cesarean section without any abnormalities. we report this case with brief review of literatures.
