Cystic mesothelioma of the peritoneum is a rare benign neoplasm that occurs predominantly in young to middle-aged women and tends to recur locally. Pelvic viscera is the most common predilection site of cystic mesothelioma. Authors report a case of multicystic mesothelioma of the pelvic pelvic peritoneum. The pelvic ultrasonography showed a large, confluent, and thin-walled multilocular cystic mass occupying the pelvic cavity and lower abdomen. The cystic mass was flattened and elongated. Each cyst of variable size was separated by thin septa and filled with translucent fluid. Cystic mesothelioma should be included in the differential diagnosis of the cystic peritoneal masses, especially in reproductive-aged women.

Dislocations of the interphalangeal joint of the great toe that are irreducible are very rare. Invagination of the plantar plate or the sesamoid bone into the IP joint, which prevents reduction. To our knowledge, however, dislocations of the IP joint of the great toe that were irreducible because of lateral collateral ligament entrapment, not invagination of the plantar plate or the sesamoid bone, have not been reported by any English literature. We report a 29-year-old ballet dancer who sustained an irreducible dislocation of the interphalangeal joint of the great toe owing to lateral collateral ligament entrapment.

BACKGROUND: Elevated plasma homocysteine appears to be causally related to atherosclerosis by inducing both endothelial dysfunction and vascular structure alteration. Brachial-ankle pulse wave velocity, a non-invasive means of measuring atherosclerosis, reflects arterial stiffness. In this study, we investigated the association between the plasma homocysteine level and arterial stiffness in Korean adults. METHODS: The study group comprised of 405 subjects selected from examinees in the Health Promotion Center. Medical history, medication, and life style were recorded through a questionnaire and physical examination was performed on all subjects. We measured glucose tolerance index, lipid profile, inflammatory index, and plasma homocysteine level. We studied brachial-ankle pulse wave velocity (baPWV) as a measure of arterial stiffness. RESULTS: A total of 379 subjects were examined on all components. The mean age in 172 males was 54.5 +/- 8.7 years and in females was 57.2 +/- 7.0 years. The baPWV was observed to be positively correlated with age, blood pressure, hs-CRP, and homocysteine in males. In contrast, males, age, BMI, waist circumference, blood pressure, glucose tolerance index, lipid profile except for HDL cholesterol, hs-CRP, and homocysteine were positively correlated with baPWV in females. The baPWV was increased according to the level of plasma homocysteine in males. After ontrolling or risk factors of cardiovascular diseases, homocysteine was associated with increased baPWV CONCLUSION: The results indicate that elevated plasma homocysteine was associated with baPWV in middle-aged male after adjustment for the risk factors of cardiovascular diseases.

3-Deazaadenosine (DZA), one of the potent inhibitors of S-adenosylhomocysteine hydrolase, is known to possess several biological properties including an induction of apoptosis. To evaluate a possibility that DZA may be utilized for the treatment of human leukemia, we studied molecular events of cell death induced by DZA in human leukemia HL-60 and U-937 cells. DZA induced a specific cleavage of poly ADP-ribose polymerase (PARP) and an activation of the cysteine protease caspase-3/CPP32 which is known to cleave PARP. DZA-mediated nuclear DNA-fragmentation was completely blocked in the presence of a universal inhibitor of caspases (z-VAD-fmk) or the specific inhibitor of caspase-3 (z-DEVD-fmk) unlike of cycloheximide (CHX). DNA fragmentation was preceded by the lowering of c-myc mRNA in the DZA treated cells. In addition, DZA-induced apoptosis was blocked by pretreatment with adenosine transporter inhibitors such as nitrobenzylthioinosine (NBTI) and dipyridamole (DPD). Taken together, these results demonstrate that DZA-induced apoptosis initiated through an active transport of DZA into human leukemia cells, is dependent on the caspase-3-like activity without de novo synthesis of proteins and possibly involves c-myc down-regulation.

PURPOSE: This study aimed to examine the development of socializing and emotional expressions through vocalizations and joint attention (JA) behaviors in Korean-speaking children with autism spectrum disorder (ASD), compared to those with developmental delay (DD). MATERIALS AND METHODS: Video samples were collected from 28 toddlers with ASD and 18 age-matched toddlers with DD, and vocalizations were each coded in detail for the purpose of this retrospective research. In addition to some statistical analysis, Computerized Language Analysis was conducted to obtain the final results. RESULTS: Although they produced a higher number of vocalizations than the DD group, the ASD group did not engage in emotional or social interactions with their caretakers, whereas the DD group did. The children with ASD used more atypical vocalizations and socially unengaged vocalizations than the children with DD did. JA using vocalizations in the ASD group, in particular, was largely dyadic, with triadic types occurring at a significantly lower frequency than those in the DD group. CONCLUSION: Results from this study indicate the importance of assessing early vocalizations in toddlers with ASD, suggesting that some common symptoms of ASD, such as lack of typical, emotional, and social functions in early vocalizations, could be used to develop screening and intervention programs related to ASD.

BACKGROUND: Recently, there have been a few reports of renal cell carcinoma (RCC) cases with anaplastic lymphoma kinase (ALK) gene fusion. In this study, we screened consecutively resected RCCs from a single institution for ALK protein expression by immunohistochemistry, and then we performed fluorescence in situ hybridization to confirm the ALK gene alteration in ALK immunohistochemistry-positive cases. METHODS: We screened 829 RCCs by ALK immunohistochemistry, and performed fluorescence in situ hybridization analysis using ALK dual-color break-apart rearrangement probe. Histological review and additional immunohistochemistry analyses were done in positive cases. RESULTS: One ALK-positive case was found. Initial diagnosis of this case was papillary RCC type 2. This comprises 0.12% of all RCCs (1/829) and 1.9% of papillary RCCs (1/53). This patient was a 44-year-old male with RCC found during routine health check-up. He was alive without evidence of disease 12 years after surgery. The tumor showed a papillary and tubular pattern, and showed positivity for CD10 (focal), epithelial membrane antigen, cytokeratin 7, pan-cytokeratin, PAX-2, and vimentin. CONCLUSIONS: We found the first RCC case with ALK gene rearrangement in Korean patients by ALK immunohistochemistry among 829 RCCs. This case showed similar histological and immunohistochemical features to those of previous adult cases with ALK rearrangement, and showed relatively good prognosis.

BACKGROUND: Due to advancements in treatment of metastatic and advanced renal cell carcinoma (RCC), it has become increasingly important to diagnose metastatic RCC and the specific subtype. In this study, we investigated the diverse histologic features of metastatic clear cell renal cell carcinoma (CCRCC) cases in comparison with corresponding primary lesions. METHODS: We identified 119 metastatic CCRCC cases from 81 corresponding primary lesions diagnosed between 1995 and 2010 and evaluated the diverse histologic and immunohistochemical features of these lesions. RESULTS: A total of 44 primary lesions (54.3%) had a non-clear cell component in addition to a typical clear cell component. Of the 119 metastatic lesions, 63 lesions (52.9%) contained a non-clear cell component, and 29 metastatic lesions were composed of a non-clear cell component only. Rhabdoid features were the most frequent non-clear cell histology among the metastatic lesions. Metastatic CCRCCs mainly showed positive CD10 and epithelial membrane antigen staining and negative cytokeratin 7 staining. CONCLUSIONS: Metastatic CCRCC commonly showed a variety of histologic features. If there is a difficulty to diagnose metastatic CCRCC due to a variety of histologic features or small biopsy specimen, histologic review of the primary lesion and immunohistochemical analysis can help determine the correct diagnosis.

The general treatment methods of chronic osteomyelitis of calcaneus with soft tissue defect was curettage of necrotic bone with antibiotics mixed bone cement insertion or cancellous bone grafting, and free or pedicled flap coverage. The muscle flap for soft tissue defect has many advantages including control of infection, but in cases of pedicled flap in calcaneus, there are some limitations because of functional loss of the donor site, limitation of pedicled length of donor muscle In these reason, free muscle flap was more preferred for pedicled muscle flap. But, in case of small sized defect which was located in center or lateral side of calcaneus, the abductor digiti minimi muscle flap can be one of the solutions. The abductor digiti minimi muscle flap has minimal functional loss of donor site and can be performed easily. There are some reports of the abductor digiti minimi muscle flap in other country, but in korea, this report is the first case report of the abductor digiti minimi muscle flap. We performed one case of abductor digiti minimi muscle flap as a treatment of chronic osteomyelitis of calcaneus and could obtain a good result.

Diffuse pulmonary nodular lesions have many causes. When they are caused by infection, the likely organisms are M. tuberculosis and various fungi. Silicosis, eosinophilic granuloma and pulmonary metastasis should be considered for differential diagnosis. Differential diagnosis needs detailed clinical history, physical examination and various laboratory tests. A case of persistent diffuse pulmonary nodular lesions which had persisted 5 years is reported. The patient was a 25 years old man with minimal pulmonary symptoms. Detailed past history and physical examination suggested thyroid tumor. Chest radiography showed numerous evenly sized well-defined nodules scattered in entire lung fields. Previous chest X-rays showed similar nodular lesions, which had lasted for 5 years. The number of nodules was slightly increased. Neck CT showed heterogenous mass in left lobe of thyroid gland and multiple lymphadenopathies along both internal jugular chains. Total thyroidectomy was performed. A case of lung metastasis which progressed slowly in papillary thyroid cancer is reported.
