3.Cryopathic Hemolytic Anemia: A case report.

The cryopathic hemolytic anemia is an autoimmune hemolytic anemia induced by cold antibody. Authors report a case of cryopathic hemolytic anemia confirmed by immunohematologic examination. A 22-year-old man was admitted at Kyung Hee Medical Center in March, 1994, with a past history of Kleinerfelter's syndrome and complained jaundice as well as dizziness. Direct antiglobulin test using polyvalent and anti-C3d monovalent antisera was positive, but was negative against anti-IgG, anti-IgA, and anti-IgM monovalent antisera. Cold agglutinin titer was as high as 1:1024 at 4 degrees C and anti-I specificity was confirmed by using cord bloods. The patient was not improved despite transfusion of washed red cells and administration of prednisolone, and on 16th day of hospitalization he manifested sudden episode of headache and loss of conscioussness, soon followed by death.

4.A FEM analysis for initial stress on the upper canine by orthodontic force of intrusion arch wire activation.

The purpose of this study was to find the distribution and measurement of compressive and tensile stress when intrusi- on arch wire is forced engage with upper canine and to analysis stress at each section through FEM. And we compare compressive and tensile ratio at each section. The results were as follows. 1. At FA point and cemento-enamel junction of upper canine, compressive and tensile force ratio is about the same. 2. At apex, compressive force is the four times as tensile force. ; In intrusion, we show root resorption at apex. 3. At Cemento-enamel junction, the compressive and tensile force show the maximun value except FA point.

Peripartum cardiomyopathy is defined as a syndrome of cardiac failure occurring in tbe latter part of pregnancy or in the puerperium, without obvious cause and without prior evidence of heart disease. Despite its infrequency, this clinical problem may have devastating consequences upon maternal and fetal outcome. It is important to recognize the association of cardiac failure and pregnancy as a separate syndrome so that, once identified, peripartum cardiomyopathy can be treated promptly and aggressively. We experienced two cases of peripartum cardiomyopathy, and report these cases with a brief review of the literatures.

9.Hepatoid Adenocarcinoma of Stomach: A case report.

A case of primary gastric carcinoma with patterns resembling those of hepatocellular carcinoma(HCC) is reproted. This patient who represented high serum alpha-fetoprotein(AFP) underwent a subtotal gastrectomy for stomach cancer and lobectomy of liver metastasis. The hepatoid component of the tumor was characterized by solid nests of large polygonal cells arranged in cords and trabeculae, with central nuclei, prominent nucleoli, and abundant eosinophilic cytoplasm; single-nuclear giant cells were frequently noted. These tumor cells of stomach stained positive immunohistochemically for AFP, ACT(alpha-1-antitrypsin), AACT(alpha-1-antichy-motrypsin). Thus, this hepatoid gastric adenocarcinoma tumor cells demonstrated both morphologic and immunohistochemical features of partial differentiation to the HCC. Careful histologic examination in conjunction with the immunohistochemical demonstration of AFP can provide a useful contribution to the diagnosis of this rare histological type of gastric carcinoma And the presence of metastatic hepatoid adenocarcinoma from the stomach should be considered in a patient with hepatic tumor mimicking HCC. Because of frequent venous and lymphatic invasion, as well as liver metastasis, prognosis appears to be poor. The authors reviewed 45 cases of hepatoid adenocarcinoma of stomach reported in the literature, also.

10.Effect of different application modalities of EDTA on dentinal tubule opening: a SEM study.

Root conditioning has been introduced to dissolve the smear layer, and to aid in detoxification following root planing. It produces surface demineralization, resulting to exposure of dentin collagen fibrils and opening of dentinal tubules. The purpose of the present study was to examine the possibility of obtaining root conditioning effect by using ethylenediamine tetraacetic acid(EDTA) solution instead of low pH etchants such as citric acid and tetracycline hydrochloride(Tc-HCl). Total 48 root specimens(4x5x2mm) were prepared from the mesial and distal root of 24 periodontitisaffected human mandibular molars after scaling and root planing. The specimens were treated with EDTA(17%, neutral pH, Pulpdent?, Pulpdent Co., USA) for 1 min., 2 min., and 3 min., and tetracycline hydrochloride(100mg/ml) for 5 min. using the rubbing technique and the placement technique without rubbing. The dentinal tubules of the specimens were examined under a scanning electron microscopy(JEOL, JSM- 840A, Japan). On the microphotographs taken at x3,000 magnification, the number of identifiable dentinal tubules, opened dentinal tubules, and fully opened dentinal tubules(over 0.9micrometer in diameter) per the unit area(1,067?m2) were counted. And the rate of opened dentinal tubule and the rate of fully opened dentinal tubule were calculated. The difference of those rates among all groups was statistically analyzed by ANOVA and Dunkan test. The results were as follows; 1. In the specimens applied with EDTA, the rate of opened dentinal tubule was not significantly different between each group regardless of application technique and application time(p>0.1). 2. In the specimens applied with EDTA, the rate of fully opened dentinal tubule had tendency to be increased in proportion to application time regardless of application technique. 3. In the specimens applied with EDTA within 2 min., the rate of fully opened dentinal tubule failed to show significant difference between the two techniques(p>0.1). But, in the specimens applied for 3 min., the rate of fully opened dentinal tubule was significantly higher in the specimens applied by the rubbing technique than those by the placement technique(p<0.001). 4. In the specimens applied with Tc-HCl for 5 min., both the rate of opened dentinal tubule(p<0.01) and the rate of fully opened dentinal tubule (p<0.001) were induced significantly higher by the rubbing technique than the placement technique. 5. The rate of fully opened dentinal tubule was significantly higher in both the specimens rubbed for 3 min. with EDTA and the specimens rubbed for 5 min. with Tc-HCl than other groups(p<0.001). But there was no significant difference between the two(p>0.1). The results demonstrate that 17% EDTA solution can replace low pH etchants for root conditioning, and the rubbing technique for at least 3 min. is recommendable.
