2.Predicting Variables of Violent Behavior in Schizophrenic Patients.

OBJECTIVES: This study aims to investigate the variables predicting violent behavior in schizophrenic patients. METHODS: Subjects were 65 schizophrenic patients diagnosed according to DSM-IV. Schizophrenic symptom and the insight of patients were evaluated with the Postive and Negative Syndrome Scale (PANSS) and the Scale of Unawareness of Mental Disorder (SUMD). Violent behaviors were evaluated with the Modified Overt Aggression Scale (MOAS). The State Trait Anger Expression Inventory (STAXI) and the Symptom Checklist-90-Reversion (SCL-90-R) self-reported measures were used for the evaluation of anger state & trait, and general psychopathology. Statistic method used in analized variables were stepwise multiple regression and discriminant analysis as well as t-test. RESULTS: Anger out style, hostility and trait anger variables were extracted as the significant predictors in stepwise multiple regression (37%). Emotion and attitude factors related to anger were more powerful predictor than symptom factors. These three variables could discriminate between violent group and nonviolent group in 72.4%. CONCLUSION: Violent behavior of schizophrenic patients could be predicted more accurately by stable factors such as attitude and emotion related to anger than by symptom factors.

3.A Case of Paraneoplastic Syndrome Associated with Pancreatic Cancer Presenting as Rheumatoid Arthritis.

Paraneoplastic cancer polyarthritis is a very rare manifestation of musculoskeletal disorders associated with several solid and hematologic malignancies. We describe a 71-year-old woman who presented initially with an abrupt onset of rheumatoid arthritis-like symmetrical polyarthritis. The patient was diagnosed with pancreatic cancer with multiple hepatic metastases approximately six months later. Her symptoms of polyarthritis improved after the introduction of medication including non-steroidal anti-inflammatory drugs and disease modifying anti-rheumatic drugs. This case suggests that a hidden malignancy should always be considered in elderly patients, who present with an abrupt onset of symmetric polyarthritis for the first time in their life.

4.A Case of Rheumatoid Arthritis Associated with Turner's Syndrome.

Turner's syndrome (TS) is characterized by short stature and gonadal dysgenesis. It is often associated with systemic manifestations, such as cardiovascular, gastrointestinal, and musculoskeletal disorders. Although very rare, it is possible for TS to accompany autoimmune disease, including thyroid disease, inflammatory bowel diseases, diabetes mellitus, psoriatic arthritis, and juvenile rheumatoid arthritis. A 39-year-old woman was referred for symmetric polyarthritis of her hands and feet. She had been diagnosed with Turner's syndrome with 46,XO,-X,+fragment before the age of 22 years and had developed autoimmune hypothyroidism treated with thyroid hormone replacement. At the time of first visit, she had polyarthralgia with morning stiffness for more than 3 months. The musculoskeletal examination revealed symmetrical polyarthritis affecting the metacarpophalangeal, proximal interphalangeal, and metatarsophalangeal joints, fulfilling the ACR 1987 revised criteria for rheumatoid arthritis (RA). Here, we present an unusual case of RA associated with TS. It is important to pay meticulous attention to patients with TS so that inflammatory arthritis is not neglected and the diagnosis is not delayed.

5.Implementation of a Computerized Hospital Equipment Utilization Review System.

A computerized hospital equipment utilization review system has been developed and implemented in a tertiary care utilization hospital in order to provide real-time equipment utilization with income information to be used in budgeting and strategic planning. Among various hospital fixed and clinical equipment, 283 (purchase price over US$15,000 per unit) were selected initially for utilization review. The application was written in Power Builder which was then loaded on Pentium client or on-line service. The data base management and statistical transactions were done by Oracle v 7.3.4 on NUMA Q2000 server. The utilization and associated income information of operating tables and other clinical equipment were automatically collected from physician order information in the hospital information system. Those information on other equipment which (lid not have direct utility information linked to physician order entry in the hospital information system were manually entered by users .Various utilization reports can be generated by users through computer menus; reports include name of equipment, using department, purchase price and year, frequency of use for inpatients and outpatients separately, income generated by such usage, and etc. The out put can also be generated by department, by instrument. by any interval as selected. The automated hospital equipment utilization review system has been developed and used since March, 2000 and is proven to he an essential tool for the hospital budget planners and managers. The outcome has been increasingly satisfactory.

6.Maxillary Soft Tissue and Cortical Bone Thickness for Mini-implant Placement.

The midpalatal suture area and maxillary interdental area are suitable site for the placement of orthodontic mini-implant. The purpose of this study was to provide a guideline to indicate the best location for mini-implant placement as it relates to the thickness of soft tissue and cortical bone. Fifteen maxilla from 15 cadavers were cut in midsagittal plane and buccopalatal plane to measure the thickness of soft tissue and cortical bone of midpalatal and maxillary posterior interdental areas. Sectioned samples were scanned and the thickness was measured. The thickness of soft tissue and cortical bone were measured at 6 points from the interdental papilla with 5-mm intervals in the mid-sagittal section. And, the thickness of soft tissue and cortical bone were also measured at 5 points from the alveolar crest with 1-mm intervals in the buccopalatal section. The mean and standard deviation of the measurement were calculated. Soft tissue thickness at the midpalatal suture area was 1.46 mm at 15 mm from the interdental papilla and remained uniformly thick posterior to this point, and steeply increased at 35 mm area posteriorly. Cortical bone thickness were greatest (2.13 mm) at 20 mm from the interdental papilla and remained uniformly thick posterior to this point, and decreased at 30 mm area posteriorly. Palatal soft tissues thickness in all groups was thinnest at the 1 mm from the alveolar crest and gradually increased from alveolar crest to apical portion. Cortical bone thickness in all groups was thickest at the 1 mm from the alveolar crest and slightly decreased from alveolar crest to apical portion. Buccal soft tissue thickness in all groups was thickest at the 1 mm from the alveolar crest and gradually decreased from alveolar crest to apical portion. Cortical bone thickness in all groups was thinnest at the 1 mm from the alveolar crest and slightly increased from alveolar crest to apical portion. Soft tissue thicknesses were greater on the palatal side than on the buccal side. Cortical bone thicknesses of the buccal side were thicker than the palatal side. These results provide anatomical data of soft tissue and cortical bone thickness to assist in the determination of safe location for the mini-implant placement in the midpalatal and maxillary interdental areas.

7.The Effects of Sleep Apnea and Variables on Cognitive Function and the Mediating Effect of Depression.

OBJECTIVES: This study aimed to analyze causality among sleep apnea, depression and cognitive function in patients with obstructive sleep apnea. METHODS: We reviewed the medical records of 105 patients with sleep apnea and snoring who underwent overnight polysomnography (PSG). We analyzed various biological data, sleep variables (sleep duration and percentage) and respiratory variables [arousal index (AI), periodic leg movement index (PLM index), snoring Index (SI), mean SpO2, minimum SpO2, apnea-hypopnea index (AHI), and respiratory disturbance index (RDI)]. We also analyzed various data by sleep, cognition, and mood related scales: Pittsburgh sleep quality index (PSQI), Epworth sleepiness scale (ESS), snoring index by scale (SIS), Montreal Cognitive Assessment-Korean (Moca-K), Mini-mental State Examination-Korean (MMSE-K), clinical dementia rating (CDR), and Beck Depression Inventory (BDI). We analyzed causation among sleep, and respiratory, mood, and cognition related scales in obstructive sleep apnea patients. We analyzed the mediating effects of depression on sleep apnea patient cognition. RESULTS: As Duration N1 increased and Total sleep time (TST) decreased, MOCA-K showed negative causality (p < 0.01). As BDI and supine RDI increased, causality was negatively related to MOCA-K (p < 0.01). As PSQI (p < 0.001) and SIS (p < 0.01) increased and as MMSE-K (p < 0.01) decreased, causality was positively related to BDI. BDI was found to mediate the effect of age on MOCA-K in patients with obstructive sleep apnea. CONCLUSION: Duration N1, total sleep time, BDI, and supine RDI were associated with cognitive function in obstructive sleep apnea patients. Depression measured by BDI partially mediated cognitive decline in obstructive sleep apnea patients.

Brucellosis is an endemic zoonotic disease, particularly in the Middle East and Mediterranean regions, and can involve many organs and tissues. Recently, the incidence of human brucellosis has increased rapidly in Korea. Brucellosis may often appear as other infections and asymptomatic conditions because of its range of clinical manifestations. Therefore, its diagnosis is frequently missed or delayed. Early diagnosis of brucellar spondylitis is important for reducing the number and intensity of complications. Therefore, it should be included in a differential diagnosis of back pain with an unknown origin. Brucellosis is normally transmitted to humans by direct contact with infected animals or by the ingestion of unpasteurized dairy products. We report an unusual case of brucellar spondylitis complicated by acupuncture.

9.Increased Thrombogenicity in Chronic Renal Failure in a Rat Model Induced by 5/6 Ablation/Infarction.

PURPOSE: Abnormalities in hemostasis and coagulation have been suggested in chronic renal failure (CRF). In this study, we compared processes of thrombus formation between rats with CRF and those with normal kidney function. MATERIALS AND METHODS: CRF was induced by 5/6 ablation/infarction of the kidneys in Sprague-Dawley rats, and surviving rats after 4 weeks were used. Ferric chloride (FeCl3)-induced thrombosis in the carotid artery was induced to assess thrombus formation. Whole blood clot formation was evaluated using rotational thromboelastometry (ROTEM). Platelet aggregation was assessed with impedance platelet aggregometry. RESULTS: FeCl3-induced thrombus formation was initiated faster in the CRF group than in the control group (13.2±1.1 sec vs. 17.8±1.0 sec, p=0.027). On histological examination, the maximal diameters of thrombi were larger in the CRF group than in the control group (394.2±201.1 µm vs. 114.0±145.1 µm, p=0.039). In extrinsic pathway ROTEM, the CRF group showed faster clot initiation (clotting time, 59.0±7.3 sec vs. 72.8±5.0 sec, p=0.032) and increased clot growth kinetics (α angle, 84.8±0.2° vs. 82.0±0.6°, p=0.008), compared to the control group. Maximal platelet aggregation rate was higher in the CRF group than in the control group (58.2±0.2% vs. 44.6±1.2%, p=0.006). CONCLUSION: Our study demonstrated that thrombogenicity is increased in rats with CRF. An activated extrinsic coagulation pathway may play an important role in increasing thrombogenicity in CRF.
