Multiple neurilerv,omas occurred i.n a 14-year-old boy with neurofibromatosis and right eye blindness. His father succumbed to neurofibromatosis at the age of 42. Histologically-proved neurilemornas and neurofibromas were noted on the face, neck, trunk and extremities, but no cafe au-Iait spots ohserved. There were nevus anemicus, hairy nevus and pigmented moles. The associated features in this patient includc muscular atrophies, weakness of the left hand, parest,hesia of the medial side of left forcarm and Dupuytrens contracture of left fourth finger.

3.Pachyonychia Congenita.

A case of pachyonychia congcnita occurring in a 8 year-old boy is described. A few weeks after delivery, his mother noticed thickening of all the finger and toe nails of her baby. Family history was denied. In addition to characteristic features of pachyonychia congenita of all the nails, follicular keratotic papules were noted on the knees, elbows, buttocks and legs, and also tender thick calluses, on the soles with palnioplantar hyperhidrosis. There were three cornu cutaneurn on the popliteal and left antecubital fossae. Other associated fea.tures include a painful tense bulla at the left fourth finger, irnpetiginous patches on the anterior thighs, hoarseness and leukokeratosis linguae, sparseness of the scalp hairs, and granulosis rubra nasi accompanied by facial hyperhidrosis.

6.Effects of On-Campus and Off-Campus Smartphone Overdependence Prevention Programs Among University Students

Purpose:
The purpose of this article is to evaluate effects of self-determination theory-based on-campus and off-campus prevention programs on smartphone overdependence among university students.
Methods:
This was a pre-posttest quasi-experimental study with a nonequivalent control group (CG). Seventy-eight students were recruited as participants. They were allowed to choose either an experiment group (EG) or a CG. On-campus smartphone overdependence prevention program was provided to participants in experimental group 1 (EG1), while on-campus program combined with off-campus prevention camp was provided to those in experimental group 2 (EG2). Instruments used in this study included a smartphone overdependence self-diagnosis scale, a basic psychological needs scale, and a self-regulation ability scale. Data collection was performed at baseline, immediately after intervention, at 1 month and 3 months after intervention. Data were analyzed using mixed analysis of covariance. Focus group interview was performed for qualitative evaluation.
Results:
After the intervention, smartphone overdependence and basic psychological needs exhibited significant interactions between group and time. Smartphone overdependence scores decreased in EG1 and EG2 but increased in CG (F = 4.56, p = .001). Basic psychological needs improved in EG1 and EG2 but deteriorated in CG (F = 5.04, p = .009). Focus group interviews revealed that participants strived to control their smartphone usage through individual efforts and by interacting with new friends in college even after completing the program.
Conclusion
In this study, on-campus only program and combined intervention of on- and off-campus programs were both effective in maintaining and managing smartphone use. However, participants perceived that the off-campus program provided an opportunity to apply the theory learned in on-campus to the real world.
7.Quality of Sleep and Its Influencing Factors in the Institutionalized Elderly.

PURPOSE: The purpose of this study was to identify the quality of sleep and its influencing factors on the institutionalized elderly. METHODS: The respondents were 145 institutionalized elderly in Daegu and a structured questionnaire was used for data collection, which was conducted from February 10 to 28, 2007. The questionnaire was the Yi(2004) quality of sleep scale. All the information was collected through face-to-face interviews, using the questionnaire. The data were analyzed by SPSS/WIN 12.0. RESULTS: The results of this research were as follows: First, the mean for the quality of sleep in the institutionalized elderly was 83.92. Second, in the subscales of the quality of sleep scale, the mean for daytime dysfunction was 40.35; for restoration after sleep it was 10.43; for difficulty in falling asleep it was 11.00; for difficulty in getting up it was 8.88; for satisfaction with sleep it was 8.62; for difficulty in maintaining sleep it was 4.75. Third, the factors influencing the quality of sleep were economic status, perceived health status, and pain. CONCLUSION: Based upon the findings above, this study provides useful information that could assist in improving the quality of sleep among institutionalized elderly, and indicates that nursing intervention is needed.

8.An I/D Polymorphism in Angiotensin-Converting Enzyme Gene in Myocardial Infarction.

BACKGROUND: The angiotensin-converting enzyme(ACE) plays an important role in cardiovascular disease by production of angiotensin and degradation of bradykinin. Cloning of ACE gene revealed an insertion/deletion(I/D) polymorphism according to the presence/absence of a 287 base pair fragment in the 16th intron of ACE gene, and the ACE polymophism was associated with ACE activity. The genotype DD was identified as a risk factor for myocardial infarction in several studies. We analyzed the ACE I/D polymorphism in 62 patients with myocardial infarction and 67 normal subjects. METHODS: Genomic DNA from peripheral blood was amplified by polymerase chain reaction and characterized by three ACE genotypes; two insertion alleles(genotype II), two deletion alleles(genotype DD) and heterogenous alleles(genotype ID). ACE activity was determined by spectrophotometric method utilizing the synthetic substrate. RESULTS: There was no significant difference in ACE polymorphism between patients and normal subjects. But, the frequency of genotype DD was significantly increased in the low-risk group of patients compared with the high-risk group. The multi-vessel disease was more strongly associated with genotype DD, but there was no statistical significance. The ACE activity was strongly associated with ACE polymorphism with the activity being highest in genotype DD. There was no significant difference between patients and control subjects of the same genotype. CONCLUSION: There was no significant difference in ACE polymorphism between patients and normal subjects. The frequencies for genotype II, ID, DD were 0.328, 0.537, 0.134, respectively in normal subjects. There was high frequency of genotype II compared with Caucasians. A deletion polymorphism(genotype DD) may increase the risk for myocardial infarction in lowrisk group, and the serum ACE activity was correlated with three genotypes.

9.Porokeratosis: Clinical Studies of 14 Cases.

Fourteen cases of porokeratosis were studied clinically. There were four clinical types consisting of classical plaque type(Mibelli), superficial disseminated eruptive form of Respighi (SDE type) and linear type, each in 3 cases, and disseminated superficial actinic porokeratosis (DSAP) in 5 cases. The Mibelli and linear type of the disease appeared in the early teens but other two showed late onset of second to third decade. All cases of SDE type occurred in males and most cases of Mibelli and DSAP type in females. Five cases of DSAP were apparently associated with sun-exposure. The disease process of case 6 (SDE type) seemed to be related to metastatic adenocarcinoma of the brain. The family occurrence of porokeratosis was found in 2 cases of SDE typc and 3 of five cases of DSAP type. They were transrnitted as an autosomal dominant trait. The linear type oppears to be distinct variety from the early appearance of lesions in linear fashion over the lower extremity. The histological features were essentially the same including the typical cornoid lamella, but the features in types other than classical plaque were often minimal, No effective mode of therapy has been found.

10.Effects of an Exercise Program on Body Composition, Cardiopulmonary Function, and Physical Fitness for Obese Children.

PURPOSE: The study was done to investigate the effects of an exercise program on body composition, cardiopulmonary function and physical fitness in obese children. METHODS: Thirty nine children in grades 4, 5, and 6 in elementary school were employed: experimental group(20) and control group(19). The subjects in the experimental group participated in an exercise program for 8weeks, thirty five minutes per session, four times per week. This data was analyzed by ANCOVA and paired t-test. RESULT: The Height(F=2.615, p=0.027) of the experimental group was significantly different from the control group. Also, in the experimental group, there were significant differences of height(t=4.79, p=0.000), cardiac rate(t=-6.461, p=0.000), body mass index(t=-2.13 p=0.046), muscle strength(t=3.48, p=0.003), and muscle endurance(t=3.20, p=0.005). Otherwise, in the control group, there were significant differences of weight(t=2.27, p=0.036), waist-hip ratio(t=3.16, p=0.005), and muscle endurance(t=-2.38, p=0.030). CONCLUSION: Jumping rope improved the body composition, cardiopulmonary function, and physical fitness for obese children.
