3.Radiological analysis of pulmonlary sequestration

Pulmonary sequestration , defined as loss of connection of the lung tissue with bronchial tree and withpulmonary artery receiving its artery supply from the systemic artery, is very rare cngenital pulmonary disease.It presents as two types, intralobular or extralobular differentiated by its location. 6 cases of pulmonarysequestrations hve been diagnosed in Seoul National University Hospital. 5 cases were confirmed pathologicallyafter lobectomy. 1 case was diagnosed with cardiac cineangiogram only. The results are as follows; 1. 4 cases hadclinical histories of pulmonary sepsis. 2 cases had cardiac problems without suspicion of the lung problem. 2. 4cases are male and 2 cases are female. 3. Ages of the patients are distributed from 2 months of age to 15 years ofage. 4. 4 cases are angiographically confirmed preoperatively. 2 cases had no suspicion of pulmonary sequestrationpreoperatively. 5. 3 cases are associated wtih cardiovascular anomalies such as right sided aortic arch, pulmonarystenosis, complicated heart disease. 6. 5 cases- operatively proven-have intralobar types with cystic change. 7.CT was performed in one case who shows left lower lobe area mass with air bronchogram.

4.Usefulness of Triple Marker Testing for Prenatal Diagnosis.

BACKGROUND: Our purpose was to assess the utility of prenatal triple-marker (alpha- fetoprotein (AFP), beta-human chorionic gonadotropin (hCG) and unconjugated estriol (uE3) testing for chromosomal abnormalities in women with Down syndrome screen-positive results. METHODS: Total 1,082 women between 15 and 21 weeks' gestation received second trimester Down syndrome risk evaluation by triple marker testing. AFP, beta-hCG and uE3 were measured by Coat-A-Count(R) IRMA (Diagnostic Products Corporation, LA, USA), The risk for Down syndrome was calculated using a commercially available software program (AFP Expert; Benetech Medical System, Toronto, Canada) by use of a Down syndrome risk cutoff value(1:270 at midtrimester). Karyotypes were reviewed for 32 (54.2%) of these patients who received prenatal chromosome analysis. RESULTS: Fifty nine (5.5%) patients of the 1,082 women screened were identified as positive. Two chromosome abnormalities (47,XYY and 46,XX, int (9) ) were found in the 32 patients who underwent prenatal chromosome analysis (6.3%). Any cases on the abnormal serum tests torn out not to be associated with trisomy 21. CONCLUSIONS: Although triple marker screen appears to be an effective method detecting chromosome abnormalities there is a high false positive rate. Therefore, new screening test that reduce false positive rate is need to be introduced.

7.Computed tomographic findings of the pancreatitis

Computed body tomography has become useful in the diagnosis of pancreatic disease. It was found to be are liable, often specific, and noninvasive method for detecting pancreatitis and extra-pancreatic extension of the pathology. Of eight hundred and seventy-two cases studied for abdominal pathology with EMI-CT 5005 whole body scanner form Oct. 1977 to August 1980, 21 cases were confirmed to be pancreatitis clinically or operatively. The authors reviewed the CT findings of the above cases and the results were as follows; 1. Among twenty-one cases, the acute pancreatitis was 12 cases and the chronic pancreatitis was 9 cases. The sex ratio as 17 males to females. 2. In acute pancreatitis, diffuse enlargement of pancreas (11/12), focal enlargement (1/12), loss of peripancreatic fat plane (9/12), thickening of anterior of pararenal fascia (6/12), and smooth margin ofpancreas (5/12) were observed. 3. In chronic pancreatitis, parenchymal atrophy (7/9), normal size (2/9), loss of peripancreatic fat plane (3/9), thickening of anterior pararenal fascia (1/9), calcification (1/9), smooth margin (2/9), and serrated margin (6/9) were observed. 4. The complications were associated with 7 cases of acute pancreatitis and 1 case of chronic pancreatitis; pseudocyst (6), abscess (2), and fat necrosis (3), The sites of the pseudocyst were lesser sac (2), anterior pararenal space (2), posterior pararenal space (1), subhepatic region (1), greater omentum (1), and intrapancreatic region (2). All of them were associated with acute pancreatitis except one in chronic pancreatitis.
