1.Pleomorphic Xanthoastrocytoma: A case report.

The Pleomorphic xanthoastrocytoma(PXA) is considered as a special subgroup of gliomas because of its distinctive characteristics: onset in young subject; predilection for the temopral or parietal lobe and a superficial location; frequent appearance as a yellow encapsulated mass with a grossly visible tumor-associated cyst; marked histological pleomorphism; little or no mitosis and no necrosis; presence of a rich reticulin network; and demonstrable GFAP in many of the fusiform and giant cells; most importantly, the relatively favorable prognosis despite plemorphism and bizzare giant cells in the microscopic picture. The objective of this report is to add one more case of pleomorphic xanthoastrocytoma to the medical literature.
2.The Effects of Indomethacin of the Rat Gastric Mucosa.

Indomethacin is used widely in clinics nowadays and the side effect of ulceration is well known. This experiment was performed to Study the morphological and histochemical changes on gastric mucosa after indomethacin treatment. The microscopic finding of the mucosa was observed following oral administration of 10 mg/kg indomethacin in alcohol as solvent. The histological changes were observed from 6 hours after administration of indomethacin and the maxima1 injury was found at 24 hours. Structural changes of injury included hemorrhage, epithelial desquamation and inflammatory cell infiltration. From the 3 day specimens, regeneration signs had started and in the 6 day specimens almost complete recovery of the mucosal epithelium was noted. The histochemical changes of the mucus were also observed from the 6 hr specimens. As far as neutral glycoprotein was concerned, the decrease was most significant in the 3 day o1d group, and besides, they showed minimal reaction to PAS stain. For acidic mucus, the decrease was significant in the 24 hr group and the 3 day group showed minimal reaction to Alcian blue stain. It was noted that these changes of the mucus had recovered 6 days after the administration of indomethacin.

3.Effect of Atenolol(Tenormin(R)) on Blood Pressure and Plasma Renin Activity in Essential Hypertension.

The effect of Atenolol on the blood pressure were studied in 31 cases of essential hypertension and on the effect of plasma renin activity in 8 cases. There were 8 cases of male with age 49.5(42-70) and 23 cases of female with age 49(35-71) years. 27 cases of 31 cases were untreated hypertensive patients and 4 cases were refractory to hypotensive drugs of diuretics and vasodilators. The Atenolol 50mg once-daily was given to all patients orally for 3 weeks. The blood pressure, heart rate, ECG and symptoms were checked in one week interval. In 8 cases, the plasma renin activity was measured by Dainabot Kit before and after one week medication. The results were as follows: 1. In 27 cases of untreated hypertension, the control blood pressure was 182.9/11.4mmHg in average. The blood pressure decreased in average by 18.3/11mmHg in one week, 23.7/15.9mmHg in two week and 21.5/10.5mmHg in three week. The heart rate also decreased by 7.4/min. in one week, 14.9/min. in two week and 7.8/min. in three week. These data showed considerable reduction of blood pressure and heart rate with 50mg Atenolol once-daily. 2. The basal plasma renin activity after one week medication was reduced by 46% in 8 cases and this data showed considerable reduction regardless control level of basal plasma renin activity. 3. There were considerable blood pressure reduction by adding Atenolol 50mg once-daily to diuretics and vasodilator to which patient's blood pressures were refractory. 4. There was no orthostatic hypotension or serious side effect by Atenolol medication.

5.The Comparison Study of teh Effect of Ambroxol on Prevention of Infantile Respiratory Distress Syndrome in Preterm Delivery.

This study was conducted to evaluate the effect of ambroxol on preventing the infantile respiratory distress syndrome (IRDS) in preterm birth at the Dept. of Obstetrics and Gynecology of Taegu Catholic Medical Center during the period from Jan. 1996 to Dec. 1996. Total of 68 cases were evaluated including 16 ambroxol group and 52 control group. The result were as follows : 1. In the comparison of preventing IRDS, there was 0 case of IRDS in ambroxol group and 7 cases of IRDS in control group (13.46 %). There was a significant difference between two groups (p<0.05). 2. The side effects of ambroxol after administration were nausea in 5 cases, headache in 3 cases, and chest discomfort in 4 cases, but these were not serious and self controlled. 3. There was no significant difference in neonatal morbidity between two groups (p > 0.05).

6.Three Cases of Membranous Reticular Inflammatory Polyp of the Colon.

Inflammatory polyposis of the colon is rare portion of the colon polyp entity. We experienced three patieats who suffered from colitis of unknown'etiology and discovered membranous retkular with/ without filiform polyp in the distal sigmoid colon of the them. Case 1, 56-year-old female, combined with basaloid cercinoma of the rectal canal that confirmed by pathology after abdominoperineal resection. So we obtained specimen of inflammtory polyp end tried to evaluate the mechanism of it. We found healed ulcer under the mucosal bridge. Case 2. 55-year-old male, had been suffered from irritable bowel syndrome. Prior to visit outpatient clinic, he complained tenesmus with diarrhea for a week. Inflammatory polyp was discovered by colonoacopy but his symptoms relieved by tranqulizers only. Case 3. 57-year-old male, has been a good health. He undertook colonoscopy for routine check thst revealed membranous reticular with filiform inflammatory polyp from distal sigmoid colon to hepatic flexure. Such shape of inflammatory polyp was not documented in Korea, so we reported three cases of it with review of the literature.

9.Pulmonary Adenocarcinoma of Fetal Type: Report of a case.

Adenocarcionma of fetal type is in lung is a newly recognized malignant tumor sharing morphologic features with the epithelial component of the pulmonary blastoma devoid of sarcomatous component. We present a case of adenocarcinoma of fetal type in a 28-year-old female, consisting of numerous branching tubules or glands and morula-like epithelial complexes. Histologically, the tubules and glands were composed of glycogne-rich nonciliated epithelial cells showing in part argyrophilia. Some of tubular and morula-like epithelial cells revealed immunoreactivity for neuron-specific enolase. We report this case with a review of literatures with special references on the histogenisis. This report is the pathologically confirmed second case of the pulmonary adenocarcinoma of fetal type in Korea, following the report of Cho and Lee, 1990.

10.Clinical Observation of Neonatal Sepsis according to Onset of Disease.

We have experienced 113 cases of neonatal sepsis comfirmed by clinical manifestations and blood cultures from Jan. 1988 to Dec. 1992 at the Neonatal Intensive Care Unit of Ulsan Dong-Kang Hospital and observed the incidence, predisposing perinatal factors, clinical manifestations, associated illnesses, laboratory findings, isolated microorganisms, antibiotics sensitivity test and mortality rate of neonatal sepsis according to onset of disease. The result were as follows: 1) The incidence of neonatal sepsis was 1.39% and male to female ration was 1.38:1. The incidence and sex difference between early onset and late onset disease were not significant. 2) Neonatal sepsis was more prevalent in premature infants (2.47%) than in fullterm infants (1.28%) and nore prevalent in low birth weight infants(3.01%) than in normal birth weight infants (1.25%). In premature infants, neonatal sepsis was more prevalent in early onset (63.2%) than in late onset diease (36.8%). In low birth weight infants, neonatal sepsis was more prevalent in early onset (64.8%) than in late onset dieases (35.7%P). 3) Predisposing perinatal factors, such as meconium staining, birth asphyxia, difficult delivery, premature rupture of membrane, maternal infection, toxemia and postpartum bleeding were slightly frequent in early onset disease. 4) Among the clinical manifestations, jaundice, respiratory symptoms, pallor, lethargy, poor feeding and hepatosplenonegaly were slightly frequent in early onset disease, but temperature instability and gastrointestinal symptoms were slightly frequent in late onset disease. 5) Among the associated illness, pneumonia, disseminated intravascular coagulopathy, amnionitis, hyaline membrane disease and osteomyelits were more common in early onset disease, but gastroenteritis, urinary tract infection, necrotizing enterocolitis, wound infection and meningitis were mors common in late onset disease. 6) The difference of laboratory findings between early onset and late onset disease was not significant. 7) Causative organisms were gram positive organisms in 87 cases(77.0%), gram negative organisms in 22 cases (18.6%) and mixed infections in 5 cases (4.4%). Among them, coagulase negative staphylococcus was the most common one and staphylococcus aureus was the second. The incidence of infections caused by coagulase negative staphylococcus and staphylococcus aureus, between early onset and late onset disease, was not significantly different. Streptococcal infection was more prevalent in early onset disease, especially all group B streptococcus caused early onset disease. 8) Gram positive organisms ware sensitive to Cephalothin (92.9%), Chloramphenicol (90.0%) and Ceftriaxone (88.9%). Gram negative organisms were sensitive to Amikacin (91.3%) and Colistin (82.6%). The difference of antibiotics sensitivity for organisms causing early onset and late onset diease were not significant. Gram negative organisms causing early onset disease were resistant to gentamicin and terramycin, but those organisms causing late onset disease were more sensitive to gentamicin (88.9%) and tobramycin (77.8%). 9) The mortality rate was 7.96%. It was higher in gram negative infections (23.8%) than in gram positive infections (4.6%). No significant difference of mortality rate between early onset and late onset disease was found.
