1.Relationships among Attitudes toward Menstruation, Peri-menstrual Symptoms, and Coping in Adolescent Girls.

BACKGROUND: This study aimed to identify the attitudes of adolescent girls towards menstruation, peri-menstrual symptoms, and coping. METHODS: A convenience sample of 521 adolescent girls was obtained from two middle schools and two high schools in M city. Data were collected through questionnaires about menstrual attitudes, menstrual symptoms, and coping. Data was analyzed by descriptive statistics, t-test, ANOVA, Pearson correlation coefficients, and multiple regression using SPSS/WIN 17.0 program. RESULTS: Subjects showed both negative and positive attitudes toward menstruation. There were significant correlation between attitudes toward menstruation and peri-menstrual symptoms. Subjects having a positive attitude toward menstruation needed a nursing mediator to provide relief for menstrual symptoms during the peri-menstrual period. Attitudes toward menstruation explained 11.0% variance of menstrual symptoms, and factor 4, in particular, (event whose onset can be predicted and anticipated) correlated strongest with menstrual symptoms and explained 21.5% variance of menstrual symptoms. CONCLUSIONS: Our results show that evidence-based strategies for positive attitudes toward menstruation, menstrual symptom, and coping should be included in a comprehensive approach to a nursing intervention to help adolescent girls.

3.Improvement of Pregnancy Rate by the Selection of Early Cleavage Embryos to 2-cell Stage in Human IVF.

OBJECTIVE: Evaluation of embryos using early cleavage to 2-cell stage has been proposed, but a critical time-point for selecting embryos is unclear. The aim of the present study is to provide a guideline including critical time-point in the selection of early cleaving embryo for the reduction of multiple pregnancies as well as the increase of pregnancy rate in human IVF. METHODS: This prospective study was performed in 116 cycles from 85 patients who underwent conventional IVF or ICSI at the infertility clinic of Good Moonhwa Hospital from January 2002 to December 2003. Early cleavage (EC) of embryos to 2-cell stage was assessed at 25 h and 27 h postinsemination/microinjection. Embryos that had early cleaved at each time point were designated as EC-1 and EC-2, respectively, while others were designated as non-early cleavage (NEC). RESULTS: At least one early cleavage embryo was observed in 54 (46.6%) for the EC-1 and 84 (72.4%) for the EC-2 of the 116 cycles assessed. Clinical pregnancy rates (PR) were significantly higher in the EC-1 group (66.7%) compared to the EC-2 group (53.6%) or the NEC group (31.2%) (p<0.05). Significant improvement of the pregnancy rate was found when at least two or more embryos were early cleaved at 25 h postinsemination or when the proportion of early cleavage embryo at 25 h postinsemination was higher than 20% (p<0.05). CONCLUSIONS: The critical time-point for the selection of early cleavage embryos with high implantation potential is more effective in 25 h postinsemination/microinjection compared to 27 h. The proportion as well as number of early cleavage embryos is also an important factor for the prediction of pregnancy outcome and the chance of multiple pregnancies. These results demonstrated that the evaluation of early cleavage embryos to 2-cell stage is an easy, simple, and objective method for the selection of good quality embryos suitable for embryo transfer.

5.A Case Combined Germ Cell Tumor in Testis.

Primary germ cell tumor of the testis is rare, which occupies 1 to 2% of all reported malignant male neoplasms. Combined primary germ cell tumor of the testis composed of embryonal carcinoma and seminoma is more rare tumor. The authors experienced a case of 50-year old male who presented with painless enlargement of right testicle. He has had radical orchiectomy and diagnosed by pathologically as combined germ cell tumor of testis. The testicle is measuring 180 gm in weight and 9×6×5 cm in dimension, and almostly replaced by tumor mass. Grossly the tumor is rubbery solid smooth tumor mass, with variegated cut surface with geographically outlined diffuse necrotic area. Histologically the tumor is composed of two components of tumor, which are solid growth pattern of large round to polyhedral cells with clear or granular cytoplasm and distinct cell border, and anastomosing glandular and papillary arrangement of anaplastic epithelial cells. The former corresponds to seminoma, and the latter to embryonal carcinoma. Each tumor lobule is separated by abundant fibrous stroma.

6.c-erbB-2 Oncoprotein Overexpression in Breast Cancer.

c-erbB-2 oncogene is a normal cellular proto-oncogene coding transmembrane glycoprotein structurally similar to the epidermal growth factor receptor. Amplification of this oncogene in a variety of human adenocarcinomas has been reported and is particularly well documented in breast carcinoma. It has been suggested that amplification of this oncogene is indicative of poor prognosis and is valuable only second to the lymph node status. Using immunohistochemical staining for the c-erbB-2 protein, overexpression of this protein was analysed in 228 primary breast cancer specimens and the frequency of overexpression and the relationship between overexpression and the other established prognostic variables are evaluated. Ninty three cases out of 228 cases(40.8%) show postive oncoprotein overexpression and using the chi-squared test for a trend, a significant correlation was found between c-erbB-2 protein staining and the histological grade, lymph node status, and estrogen receptor status(P<0.05). No significant association was found between staining and the patient's age and tumor size. Most of the tumors with histological types known to have good prognosis showed negative expression. Above findings strongly suggest that expression of c-erbB-2 oncogene is another independent indicator of poor prognosis in breast carcinoma.

7.Childhood Experiences, Paternal Attachment and Paternal Role of Primiparous Spouses.

PURPOSE: This study aims to provide foundational data for preparing for educational programs on the roleof a father that are designed to help primiparous spouses carry out the paternal role successfully. METHODS: This study was conducted in two OB/GYN clinics and a study sample of 277 primiparous spouses who satisfied the selection criteria was selected. RESULTS: Of the correlation among the childhood experiences, paternal attachment and role of a father of the subjects, the childhood experiences had a positive correlation with the paternal-infant attachment and the role of a father. The paternal-infant attachment and the role of a father also had a positive correlation. CONCLUSION: An infant is influenced primarily by his or her parents, who will have immense influences on the child's growth and development. Since the paternal-infant attachment is closely related to the role of a father, thedevelopment of positivepaternal-infant attachment must be preceded in order for a primiparous spouse to carry out the role of a father successfully. Therefore, the development of nursing intervention programs is needed to foster the proper understandingof the roleof a father, accommodate the practical needs of primiparous spouses and nurture the paternal-infant attachment for the promotion of realistic, successful role of a father.

8.The Effect of Music Therapy on Reduction of Anxiety before Gastroscopy.

PURPOSE: This study examined the effect of music therapy on the pre-exam anxiety of gastroscopy clients. METHOD: The research period is from June 17 2002 to August 30 2002, and the subjects were 60 individuals, 30 for the experimental and 30 for the control group among adult clients. The data were analyzed by using the SPSS program. RESULT: The results of this study were as follows: The score of the experimental group was lower than the score of the control group for the situational anxiety (t=-12.56, p=0.000) and systolic blood pressure (t=-3.97, p=0.000) before the examination was supported. The vital sign was partially supported. There was no significant difference(t=-1.05, p=0.297) in the diastolic blood pressure (t=-1.05, p=0.297) and pulse rate(t=-1.30, p=0.199) before the examination of the experimental group and lower than the score of the control group. CONCLUSION: Music therapy could be useful in the nursing practice, and be utilized as a way of nursing intervention to reduce anxiety of the patients in specific clinical situations. Finally, it is recommended that further studies be conducted on this therapy by utilizing different musical selections.

9.Relationships among Marital Satisfaction, Spousal Support and Practice of Taekyo in Pregnant Women.

PURPOSE: The purpose of this study was to investigate the relationships among marital satisfaction, spousal support and practice of Taekyo among pregnant women. METHODS: This study was conducted by the use of questionnaire surveys with two hundred and eighty-three pregnant women at several hospitals located in Gyeonggi province and Daejeon city from October 2008 to April 2009. Contents of questionnaire included marital satisfaction, spousal support and practice of Taekyo. Descriptive statistics, t-test, ANOVA, Pearson correlation, and stepwise multiple regression were conducted for data analysis using SPSS/WIN 17.0 program. RESULTS: Degree of marital satisfaction, spousal support, and practice of Taekyo showed the high ranking among pregnant women. There was a significant difference in order of fetus, need for Taekyo, and time in Taekyo. There was a positive relationship between marital satisfaction, spousal support, and practice of Taekyo. The significant factors influencing the practice of Taekyo was connected with marital satisfaction, order of fetus and time in Taekyo. The total explanatory range was 29.3%. CONCLUSION: There is a need for multidimensional strategy in improving marital satisfaction. This study can be used as a foundation for maternal nursing education program and can be developed for practice of Taekyo.

10.Job Description of the Nurses Who Work in Operating Room Using DACUM Technique.

PURPOSE: Developing a curriculum(DACUM) is a method of analyzing job focused competency, which is obtained from the data of an expert belonging to a certain career. In this study the DACUM method was used to analyze the jobs of operating room nurses. METHOD: Through the DACUM workshop which was arranged by two DACUM facillitators, a definition of the role of operating room nurses was developed and then duties and tasks of operating room nurses' were identified. For the workshop, a DACUM committee with 10 operating room nurses was organized. Finally, the duties and tasks which were identified were validated by 422 nurses for importance, difficulty, and frequency. RESULTS: Thirteen duties and 105 tasks were identified on the DACUM chart, where importance, difficulty, and frequency of tasks were represented by alphabet letters A, B, and C as higher degree of importance. The determinant coefficient(DC) showed that the most important duty was assisting with operations(DC=6.61), and the least, managing operating materials(DC=4.22). For tasks, the most important ones were assisting in orthostatic surgery(DC=7.60), and assisting in thoracic surgery(DC=7.38), and the least important making gauze ball(DC=2.39), and saving of operation site((DC=3.27). CONCLUSION: The results suggest a need to develop an education program using the DACUM chart as a basis for the development and as a clinical career ladder and for curriculum of operating room nursing.
