2.Ossification of the Coracoacromial Ligament in Subacromial Impingement Syndrome: A Case Report.

Here, a case of a 59-year-old man with rotator cuff tear and impingement syndrome caused by an ossified coracoacromial ligament is presented. Ossification of the coracoacromial ligaments can occur because of degenerative changes due to trauma or repeated stress, which can lead to impingement syndrome. Therefore, when coracoacromial ligament ossification is present, rotator cuff damage due to impingement syndrome should be considered. Here, we conducted arthroscopic subacromial decompression, removal of the ossified coracoacromial ligament, and supraspinatus and subscapularis tendon repairs. We achieved satisfactory surgical outcomes without relapse; therefore, we report this case with a literature review.

3.Comparison study of AAS and ISE method in the lithium analysis of serum and urine.

In the method for lithium (Li) analysis, flame emission photometry and atomic absorption spectrophotometry (AAS) have been used most frequently. In addition, lithium can be analyzed by ion-selective electrode (ISE) or fluorscence polarization immunoassay. We evaluated the comparison between AAS method based on the principle of absorption of light at 670.8 nm by Li and ISE method based on the principle of voliage difference generated by Li in contact with lithium ionophore. We compared with those obtained by AAS (AA/AE Spectrophotometer 551, Instrumentation Laboratory Co.) and ISE (SYNCHRON EL-ISE, Beckman Co.) in the serum and urine of 6 pati,,:nts- and evaluated time-related changes of serum lithium concentration after dosing in both methods. The results are summarized-as follows: 1. In within-run precision study for lithium concentration, coefficient variations (CVs, %) ranged from 1.34 to 2.17 for AAS and from 0.34 to 0.85 for ISE method. In between-run precision study for lithium concentration, CVs ranged from 1.23 to 1.72 for AAS and from 0.61 to 1.38 for ISE method. 2. The correlation study between AAS and ISE method resulted in Y= 0.946X+ 0.137 (N = 32, r= 0. 933, X= AAS, Y= ISE) for serum lithium and Y= 1. 092X+ 0. 977 (N= 28, r= 0.943, X= AAS, Y= ISE) for urine lithium. 3. Time-related changes of serum lithium concentration in both AAS and ISE method resulted in peak serum levels about 2 hours after dosing and then rapidly decreased after the peak serum level and finally arrived at nearly initial levels about 9 hours after dosing. 4. The reference range of serum lithium was found as undetectable level for both AAS and ISE method and the reference range of urine lithium to the urine creatinine was 0 0.00014 mmol/mg(mean 0.00002 mmol/mg) for AAS method.

4.Fraxetin Induces Heme Oxygenase-1 Expression by Activation of Akt/Nrf2 or AMP-activated Protein Kinase α/Nrf2 Pathway in HaCaT Cells.

BACKGROUND: Fraxetin (7,8-dihydroxy-6-methoxy coumarin), a coumarin derivative, has been reported to possess antioxidative, anti-inflammatory and neuroprotective effects. A number of recent observations suggest that the induction of heme oxygenase-1 (HO-1) inhibits inflammation and tumorigenesis. In the present study, we determined the effect of fraxetin on HO-1 expression in HaCaT human keratinocytes and investigated its underlying molecular mechanisms. METHODS: Reverse transcriptase-PCR and Western blot analysis were performed to detect HO-1 mRNA and protein expression, respectively. Cell viability was measured by the MTS test. The induction of intracellular reactive oxygen species (ROS) by fraxetin was evaluated by 2′,7′-dichlorofluorescin diacetate staining. RESULTS: Fraxetin upregulated mRNA and protein expression of HO-1. Incubation with fraxetin induced the localization of nuclear factor-erythroid-2-related factor-2 (Nrf2) in the nucleus and increased the antioxidant response element-reporter gene activity. Fraxetin also induced the phosphorylation of Akt and AMP-activated protein kinase (AMPK)α and diminished the expression of phosphatase and tensin homolog, a negative regulator of Akt. Pharmacological inhibition of Akt and AMPKα abrogated fraxetin-induced expression of HO-1 and nuclear localization of Nrf2. Furthermore, fraxetin generated ROS in a concentration-dependent manner. CONCLUSIONS: Fraxetin induces HO-1 expression through activation of Akt/Nrf2 or AMPKα/Nrf2 pathway in HaCaT cells.

6.Upper Urinary Tract Gross Hematuria: Clinical Diagnosis and Disease Distribution.

A basic problem in evaluating upper urinary tract gross hematuria is the large number of diagnostic possibilities. The causation may be suspected after the initial history and physical examination, and can be confirmed with appropriate studies. We have retrospectively studied of 60 patients who confirmed upper urinary tract hematuria through the cystoscopy in period from Oct. 1971 to Dec. 1987. We have performed relatively precise clinical studies such as urinalysis, urine culture, urine cytology, blood clotting disorders, intravenous pyelography, renal arteriography and others and then we have obtained following results. 1. Disease distributions Calculous disease 12 cases (20.0%) Malignant disease 11 cases (18.3%) Vascular disease 7 cases (11.7%) Glomerulonephropathy 4 cases ( 6.6%) Unexplained hematuria 2l cases (35.0%) etc. 2. Cystoscopy, intravenous pyelography and renal arteriography are valuable essential procedures for diagnosis of upper urinary tract bleeding. 3. Cystoscopy, intravenous pyelography and angiography will bring the percentage of patients with a clear diagnosis up to about 70 per cent, especially excretory urography and cystoscopy must always be done.

8.Clinical Results of Piezoelectric Shock Wave Lithotripsy for Treatment of Patients with Urolithiasis.

Extracorporeal shock wave lithotripsy(ESWL) is becoming treatment of choice for most upper tract calculi, and especially EDAP made piezoelectric E. S. W. L. can treat kidney, upper ureter and lower ureter stones. We present the results of 245 consecutive treatments performed between May, 1987 and February, 1988 with piezoeletric shock wave lithotriptor. 1. The oldest patient was age of 78 and the youngest patient was age of 15 and there was no contraindication due to other systemic disease. 2. No anesthesia were required and only narcotics were given for pain that induced from shock wave, but one, 18 years old female with renal stone was treated with ketamine. 3. Response rate revealed that complete response was 174 cases (71%), partial response was 63 cases (25.7%) and non-response was 8 cases (3.3%). 4. In complete response cases, average requirement of shock wave storages were noted that renal stone was 323, upper ureter stone was 562 and lower ureter stone was 377 and then the shock wave storage was increase depend upon the location of the stone, such as kidney, lower ureter stone and upper ureter stone in priority. 5. Staghorn calculi, multiple renal stones, caliceal diverticular stones, renal stones with migration into the ureter while treatment, impact ureter stones, ureter stones with larger than 2.5 cm and obesity were poor response factors. 6. We adopted push up (9 cases), double J stent (6 cases), stone basket (6 cases), URS.(2 cases) and nephrostomy (1 cases) as auxiliary procedure. 7. Chemical composition of treated stones were that calcium oxalate and phosphate mixed stone8 were 50.6%, calcium oxalates 33.5%, struvites 10.5%, uric acid 3.9% and calcium phosphate 1.3%. 8. We experienced 11 cases (4.4%) of complications: 5 cases of stone street, 3 cases of post ESWL flank pain, 1 case of high fever, 1 case of sepsis and 1 case of perirencal hematoma. 9. As a result, we suggest that we should consider stone size, location, situation of patient and history of stone surgery for good effectiveness from treatment with E.S.W.L.

9.Amelobastic Fibrosarcoma of the Mandible: A case report.

Ameloblastic fibrosarcoma is an extremely rare variety of odontogenic tumor. It has not previously been reported in Korea. The tumor is composed of benign odontogenic epithelium with a mesenchymal part which exhibits the histologic features of fibrosarcoma. We have reported a case of amloblastic fibrosarcoma of the mandible in a 26-year-old man with swelling of right mandible for 2 weeks. The tumor showed yellowish ill-demarcated ulcerating mass involving right premolar and molar area. Light microscopy revealed irregularly arranged strands and islands of odontogenic epithelium surrounded by abundant mesenchymal tissue with the feature of fibrosarcoma. The fibrosarcoma cells were strong positive on immunostain for vimentin and ameloblastic cells were weakly positive for cytokeratin. S-100 and CEA were negative in both epithelial and sarcoma cells. The sarcoma cells were corresponding to fibroblasts on the electron microscopy with abundancy of RER and mitochondria and covering of basal lamina. Two types of virus like particles were distributed in the cytoplasm and nuclei of sarcoma cells. We treated the patient with surgery and chemotherapy. The recovery was uneventful and the prognosis is under observation.

10.Squamous Cell Carcinoma Arising from Mature Cystic Teratoma of the Ovary: A report of three cases .

Malignant transformation develops in a little less than 2% of mature cystic teratomas. A wide variety of malignant tumors may arise within benign mature cystic teratomas, and the most common of these is squamous cell carcinoma, which account for 75~85%. In general, the tumors are in an advanced stage and the prognosis is poor as most patients die within a year. However, when the tumor is confined to the ovary, they have a good prognosis and the 5-year survival rate is 63~83%. We experienced three cases of squamous cell carcinoma arising in mature cystic teratoma. Two of the carcinomas occurred in postmenopausal women: 58-(case 1) and 66-(case 2) year-old, and were confined to the ovaries. They were alive 37 months and 18 months after the operation, respectively. The third case was a 45-year-old premenopausal woman who had an extraovarian extension of the tumor and early recurrence within two months. Histologically, cases 1 and 3 were conventional well to moderately differentiated squamous cell carcinomas and case 2 showed a well-differentiated squamous cell carcinoma with exuberant proliferating trichilemmal tumor-like areas.
