2.High-dose Vitamin C therapy in Methemoglobinemia.

Methylene blue is the first choice antidote for management of methemoglobinemia, however, some patients are refractory to methylene blue and in most cases, methylene blue cannot be available instantly in Korean emergency departments because of import suspension. A 69-year-old woman visited our emergency department for tachypnea and cyanosis after ingesting 30 tablets of dapsone. Because methylene blue was not available, we intravenously administrated 10 g of vitamin C for symptomatic methemoglobinemia. Repeated i.v. administrations of 10 g of vitamin C in patient without preexisting renal insufficiency successfully treated dapsone-induced methemoglobinemia without causing renal complications. Thus, we recommend that if methylene blue is unavailable or methemoglobinemia is refractory to methylene blue, repeated administrations of 10 g of vitamin C may be considered for the treatment of methemoglobinemia in patients without renal insufficiency.

3.A Study the Procurement of HLA Class I Typing Trays Using Gushed Out Blood During Placental Delivery.

BACKGROUND: Microlymphocytotoxicity test is most widely used for HLA Class l typing but almost all laboratories depend on imported HLA Class 1 typing trays. Matching criteria for the selection of HLA- matched platelets to treat platelet refractoriness is not as strict as for bone marrow transplantation. Therefore, with the acquisition of various antisera against high frequency HLA antigens, self-made HLA typing trays can be used for HLA typing of HLA-matched platelet donors. METHODS: 140 samples obtained during placental delivery were tested for the presence of HLA antibodies against a well-characterized panel of 90 cells. Specificity of HLA antisera were determined by evaluating the correlation coefficient r of the 2 x 2 table, x2 test. Antisera strength was evaluated by the strength index. RESULTS: HLA antibodies were detected in 25 samples by primary screening and 23 samples also showed a positive reaction in secondary screening(16%). Among 23 samples, 1 1 antisera were of reagent grade quality and 7 were monospecific antisera. DISCUSSION: Imported HLA typing trays can be replaced by harvesting HLA antisera against HLA antigens which are relatively common in Koreans through continuous HLA antibody screening using gushed out blood during placental delivery. 4.Factors Affecting Health Practice of Primary School Students: Based on Health Promotion Model.

OBJECTIVES: The purpose of this study was to examine the factors related to children's health behavior. METHODS: A self-administered questionnaire survey was carried out for 431 (boys; 227, girls; 204) children, 6th grade students, in Taegu from December 14 to 18 in 1998. This study employed health promotion model as a hypothetical model. Collected data were analysed through the chi-square test, ANOVA, and path analysis. RESULTS: By univariate analysis, in case of boys, health practice was related to perceived self efficacy, perceived health status, perceived benefits of health-promoting behaviors, and perceived barriers, and in girls, health practice was related to perceived self efficacy, perceived benefits of health-promoting behaviors, perceived barriers, and cues to action. By path analysis, in case of boys, the better economic status, the younger mothers' age, the higher score of family cohesion and adaptability, healthier, the more perceived benefits, and the less perceived barriers were, the more health behaviors were practiced. Girls did the more health practice, in case of living with parents only, the higher score of family cohesion and adaptability, the more perceived self-efficacy, the less perceived barriers, and the more cues to action. Family cohesion had the most important effect on health practice of primary school students. CONCLUSIONS: In order to promote health behavior of primary school students, a good family environment as well as health education might be very important. That is, we have to try together in home and as well as in school.

5.Comparison of Detection Methods and Culture Media for Isolation of Helicobacter pylori from Gastric Biopsy Specimens.

BACKGROUND: Helicobacter pylori(H. pylori) has been implicated in the pathogenesis of chronic active gastritis and peptic ulcer disease. All patients with ulcers who are infected with H. pylori receive antimicrobial therapy. Therefore diagnosis of H. pylori infection is imperative for the treatment gastritis or ulcer patients. We evaluated the four diagnostic methods and three culture media for the isolation of H. pylori. MATERIALS AND METHODS: Rapid urease test(CLO test), modified Gram stain, culture, and nested polymerase chain reaction (PCR) were performed with 108 gastroscopic biopsy specimens from patients with peptic ulcer or chronic gastritis. Among them 40 specimens were inoculated onto each of 5% sheep blood agar, e99 Yolk emulsion (EYE) agar, and 7% horse blood agar containing antibiotics. RESULTS: The positive rates were the highest by the PCR(72%), 67% by modified Gram stain, 64% by CLO test, and 57% by culture. Among the three media the horse blood agar (selective medial) gave the highest isolation rate (48%), followed by sheep blood agar (45%), and EYE agar (38%). CONCLUSION: Though PCR was the most sensitive method for the detection H. pylori modified Gram stain was sensitive enough, simple, rapid, and economical as the routine diagnostic method of H. pylori. For the culture of H. pylori combination of sheep blood agar as nonselective media and horse blood agar as selective media would show the highest isolation rate.

6.A Study on Effects of Balance Taping Therapy applied on the patients with Arthritis caused upon the Pain in Legs and Obstacle in Daily Activity.

PURPOSE: This study is to examine the effect of balance taping therapy applied on the old people who suffer from arthritis upon the pain in legs and obstacle in daily activity. Methods: The research period was from Dec. 2003 to Feb. 2004, and among the old female people who visited one of the welfare halls in Seoul downtown and 4 assembly hall for old people who aged over 60 years. 30 of the experimental group and 33 of control group were optionally collected. RESULTS: The pain degree in legs and the uneasiness degree of the obstacle in the daily life of the experimental group who were treated by the balance taping was decreased than those of the control group(p=0.015, p=0.000). CONCLUSIONS: Based upon the above result, it was confirmed that the balance taping therapy has influence in decreasing the degree of the uneasiness in the leg pain and daily activity obstacle of the old people who suffer from the arthritis, and therefore it is thought that this therapy can be usefully applied for one of the nursing methods for the old patients, and besides the repeated future researches are needed.

7.Urethral caruncle occurring in a young girl: a case report.

Urethral caruncles are the most common benign tumors in the female urethra, and are usually found in the posterior lip of the urethral meatus of post-menopausal women. It is very rare in young girls. Its etiology is unknown. We add a case of urethral caruncle occurring in a 2 year and 5 month-old girl's mid-urethra. We believe that further accumulation of unusual cases may be helpful in discovering the pathogenesis.

8.Esophagectomy for Benign Esophageal Disease.

BACKGROUND: Benign esophageal diseases are less common indications for an esophagectomy. The indications for an esophagectomy have not been clearly defined for benign esophageal diseases. The purpose of this study was to defermine whether an esophagectomy should be performed for benign esophageal disease and the indications for an esophagectomy. METHODS: The postoperative morbidity and mortality were examined in 11 patients who had received esophagectomy and esophageal reconstruction for benign esophageal diseases between 1981 and 1996 in this hospital. The indications were identified. RESULTS: The indications included corrosive stricture (6 cases), recurrent achalasia (2 cases), accidental esophageal perforation (1 case), a esophageal diverticulum with stricture (1 case) and suspicion of malignancy (1 case). All of the patients with recurrent achalasia had undergone at least one prior esophageal operation, and all of the patients with stricture had had at least one esophageal dilation previouly. There was no postoperative mortality after the esohagectomies. Postoperative complications developed in 7 patients. Early complications were anastomotic leakage (3 cases), bronchopneumonia (1 case) and DIC & ARDS (1 case). Later complications were intestinal obstruction (1 case), anastomotic bleeding (1 case) and anastomotic stenosis (4 cases) which needed one or two dilation procedures to improve the symptoms. CONCLUSION: Although an esophagectomy for benign esophageal diseases resulted in high morbidity, there were no mortalities. Thus, an esophagectomy should be considered for benign conditions of the esophagus because of corrosive cancer and the end-stage of motility disorders.

9.A Case of Esophageal Candidiasis in a Healthy Child.

Esophageal candidiasis is an opportunistic infection, often reported in patients who have acquired immune deficiency syndrome (AIDS), a neoplastic disease, or undergoing protracted antibiotic therapy. Impaired cell mediated immunity was often considered as the major predisposing factor in patients of esophageal mucosal colonization of Candida spp. However, it is increasingly reported that the occurrence of esophageal candidiasis with no underlying disease or immune suppression. We have experienced a case of esophageal candidiasis in a 15-year-old girl who was immunologically normal and have no underlying disease and whose main symptoms were epigastric and retrosternal pain with dysphagia. This case suggests the possibilities of candidal infections in children without predisposing factors such as immune compromised conditions, so it will be needed to differentiate the esophageal candidiasis among healthy children with symptoms of odynophagia and dysphagia.

10.Spontaneously Resolved Neonatal Hemochromatosis.

Neonatal hemochromatosis is a very rare disorder with an unclear etiology with an extremely poor outcome. We report a case of female newborn who presented with direct hyperbilirubinemia without any evidence of congenital infection, immune related hemolysis or exogenous iron uptake. Diagnostic studies revealed highly elevated ferritin level. T2 weighted image of abdominal Magnetic Resonance Imaging (MRI) showed decreased signal intensity of entire left lobe and a part of right lobe of liver with normal spleen. Liver biopsy showed a large amount of hemosiderin in the hepatocytes and Kupffer cells. All the biochemical and excretory liver functions normalized with conservative managements.
