1.Rupture of Renal Artery in a Patient with Behçet's Disease.

The vascular involvement is seen in 8% to 24% of patients with Behçet's syndrome. Arterial lesions are less frequently observed and the involvement of renal artery is very rare. Indeed, there is only one case report of ruptured renal aneurysm due to Behc et's disease. We report a case of renal artery rupture, which was likely caused by Behçet's disease. Even though we cannot completely rule out the other causes of renal artery rupture with this patient, the rupture was likely due to vascular involvement of Behçet's disease. Dermatologists always should consider the possibility of major vessel involvement when they treat patients with Behçet's disease, even if the incidence is very low and the patient may not complain of any systemic symptoms. INTRODUCTION: Behçet's disease was first defined by Hulusi Behçet's in 1937 as a multisystemic disease with the triad of recurrent aphthous stomatitis, genital ulceration and relapsing iritis. Its etiology remains obscure. It is accepted that the pathologic process of Behçet's disease is an immunologic vasculitis. Among vascilar lesions, the venous system is the major affected site. Arterial lesions, such as aneurysms or occlusions, are rarely reported, but sometimes adversely affects the course of the disease. Aneurysm formation is known to be life-threatening because of the risk of rupture.

3.The Effects of Symptom Distress, Social Support, and Work Change on Liver Transplant Recipients.

BACKGROUND: The purpose of this study was to evaluate the factors influencing quality of life (QOL) in liver transplant recipients. METHODS: The subjects of this study were 103 liver recipients who received their follow-up more than 3 months after liver transplantation at one general hospital in Seoul. A questionnaire survey was perfomed from September 1, 2012 to September 30, 2012. QOL and factors were evaluated using SF-36, Transplant Symptom Distress Scale and Multidimensional Scale of Perceived Social Support. RESULTS: Symptom distress, work change after transplant, duration after transplant were significant factors influencing QOL (Cum R2=0.39, F=19.34, P<0.001). Symptom distress was the most important factors related QOL. There were significant differences in QOL according to general characteristics such as education (t=2.16, P=0.033), work change after transplant (t=3.67, P=0.000), duration after transplant (t=2.25, P=0.027), and economic status (t=3.08, P=0.027). CONCLUSIONS: In conclusion, symptom distress had an influence on the QOL of liver transplant recipients. Thus, it is necessary to develop interventions for symptom distress to improve the QOL of liver transplant recipients.

4.A Case of Congenital Tuberculosis woth Massive Ascites and Pleural Effusion.

We experienced a case of congenital tuberculosis infected in utero, who had been found to have massive ascites and pleural effusion on routine ultrasonography at the 39 weeks' gestation. The emergency C-section delivered a male neonate weighing 2,050 g with poor condition and in severe respiratory distress. He was the first child born of a 27-year-old woman, who was diagnosed as having tuberculous pleurisy after delivery. Detection of acidfast bacilli in gastric aspirates obtained from the newborn as well as the clinical features and maternal history confirmed the diagnosis of congenital tuberculosis. Though rare, congenital tuberculosis requires early detection and treatment because of its severity and high mortality. A brief review of the relevant literature was made.

7.Maternal Working Conditions on Adverse Pregnancy Outcomes: A Prospective Cohort Study.

OBJECTIVES: To evaluate the association between working conditions and adverse pregnancy outcomes in Korea. METHODS: We obtained data on health history, lifestyle, housework and working conditions, such as shift work, hours standing, working time, job demand, lifting at work and at home, between August and September 2000, from self-reported questionnaires. A group of 344, occupationally active, pregnant women from 51 industries were studied. Of the women studied, 328 women were further interviewed by telephone between November 2000 and September 2001. Result : Compared with daytime work, shift work increased the risk for preterm birth (an adjusted risk ratio of 2.74, 95% CI=1.02-2.62) and low birth weight (an adjusted risk ratio of 2.74, 95% CI=1.02-2.62). A significantly increased risk was found for prolonged standing, with an adjusted risk ratio of preterm births of 6.80 (95% CI=2.01-23.0). There were no significant differences in the incidence of spontaneous abortion between the occupational working conditions, with the exception of a previous history of spontaneous abortion. CONCLUSION: These findings suggest that maternal working conditions, such as shift work and prolonged standing, contribute significantly to preterm birth and low birth weight.

8.Clinical Evaluation of BacteriokillerSystem(R).

BACKGROUND: Bacteriokiller system(BKS) produces disinfectant which is generated by the mixture of active oxygen and hypochlorous acid with tap water. Previous studies showed that BKS disinfectant is highly bacteridal against clinical isolates in in vitro testings and more effective than general soap and water for the disinfecttion of contaminated handa. This study was performed to evaluate ling-trem effects of BKS as a handwasing agent in clinical settings. METHODS: Five BKS were installed for the 9-month period from June 1995 to February 1996 in 16-bed Neurosurgery Intensive Care Unit. Handwashing was frequency was observed after 1, 6, and 8 month of BKS use. Six-month incidences of nosocomial infecrion before and after BKS installation were compared to evaluate the possible effect of handwashings on nosocomial infection rates. A self-administered questionaire was used to collect data on handwashing frequency and their opinions of the BKS disinfectant at 2 and 8 months after the installation. RESULTS: Handwashing frequency of healthcare workers(HCWs) before and after patient contacts was increased from 34.1%(137/402) to 50.3%(193/384) (p<0.00001). At the same time, the 6-month nosocomial infection rate was down to 10.5%(43/411) from the pre-BKS rate of 13.0%( 51/431) and the patient-day rate was also decreased from 18.3(56/3068) to 15.1(43/2,844). Most (80.5%) of HCWs thought that BKS promote handwashing and "convenience" was the main reason for more frequent handwashing. Ninety three percent of HCWs would recommend the replacement of tne existing sinks and handwashing disinfectants with BKS. NO adverse skin reaction were reported after 8 months. Conclusions: BKS appears to promote handwashing because it is easy to use with no skin irritation and may contribute toward the prevention of nosocomial infections.

OBJECTIVE: In order to confirm the local production of total and specific IgE antibodies in the nasal polyp tissues. MATERIAL AND METHOD: We measured total IgE and house dust mite(Dermatophagoides pteronpssinus .' DP)-specific IgE antibody using enzyme-linked immunosorbent assay(ELISA) in the supernatant of nasal polyp homogenates from 72 subjects undergoing nasal polypectomy. 10.Development of a Tool to Measure the Need for Child Hospice Care in Families of Children with Cancer.

PURPOSE: The purpose of this study was to develop a tool to assess the need for child hospice care in families of children with cancer. METHOD: The research design was a methodological study. The tool was developed in 4 stages : first, preliminary items were developed based on a questionnaire about the needs for child hospice care that was given to 20 families of children with cancer; second, a panel of specialists reduced the number of preliminary items using 3 validity tests for the content; third, final items were selected from the results of a pre-test. Finally, from February to July 2004, reliability and validity were tested with a sample of 104 families who had a child with cancer. RESULTS: The final tool on the need for child hospice care consisted of 22 items and Cronbach's alpha coefficient for internal consistency was .93. Using factor analysis, 5 factors were extracted and these factors explained 69% of the total variance. CONCLUSION: The instrument, for assessing the need for child hospice care in families of children with cancer, developed in this study was identified as a tool with a high degree of reliability and validity. In this sense, this tool can be effectively utilized for implementing and improving hospice care for children with cancer.
