Each eyelid contains two to three rows of lashes with a total of 100 to 150 lashes. The eyelashes form a first strong link in the protective chain of the eyelids, The partial loss of eyelashes is namely encountered by dermatologists. We describe a 31-year-old man with partial alopecia on the left upper eyelid due to chronic rubbing. We would like to call it "friction alopecia".

5.Mid Limbal Incision vs Scleral Pocket Incision in Cataract Surgery.

Recent cataract surgery has a trend to use clear corneal incision that does not require conjunctival peritomy and has fewer complications such as hyphema and ciliary body damage etc. Clear corneal incision also provides less corneal astigmatism and thereby better naked visual acuity. However the side effects of clear corneal incision such as includes wound leakage and associated endophthalmitis, endothelisal cell damage, irregular corneal astigmatism and instability of wounds have been reported continuously. Thus, we have introduced the 3.5mm sized mid limbal incision technique which may compensate the above complications and have rapid visual rehabilitation. Postoperative corneal astigmatism and uncorrected visual acuity of patients with clear corneal incision were compared with the results of scleral pocket incision. The corneal astigmatic changes of both group showed trivial against- the-rule change(0.44-0.12D) in early post operative period. Mid limbal incision group presented a little highter against-the-rule change at post operative 1 wee, but no significiant diffeence was noted sice postoperative 1 month. Uncorrected visual acuity showed no difference between two groups. From these results, we conclude mid limbal incision is an effective procedure that can compensate the disadvantage of both clear corenal incision and scleral pocket incision.

6.The Additive of Latanoprost 0.005% in Patients with Maximum Tolerated Medical Therapy Regimen.

Latanoprost 0.005% which is a currently available ocular hypotensive agent has been known to produce an additional reduction of intraocular pressure when used in combination with beta adrenergic bloker. The aim of this study was to evaluate the additive effect of this drug in patients with maximum tolerated medical therapy regimen. 48 eyes of 32 patients were categorized into 2 groups as to their type of glaucomas(primary open abgle glaucoma(POAG) vs chronic angle closure glauoma(CACG), and pilocarpine use. POAG group showed 22.3, 24.8, 22.7, 21.0, 21.4% and CACG group showed 17.6, 17.1, 15.7, 17.5, 17.8% of ocular hypotensive effect at post treatment 2 weeks, 4 weeks, 3 months, 6 months, 9 to 12 months, respectively and there was no satistically significant difference between two groups. Pilocarpine using group showed 25.5, 30.5, 26.4, 23.4, 17.8% and non-using group showed 20.6, 18.9, 19.0, 18.7, 25.1% of ocular hypotensive effect at each follow up period. Conclusively, latanoprost 0.005% appeared to be a very effective drug in reducing intraocular pressure in patients with maximum tolerated medical therapy regimen without regard to their type of glaucomas, and use of parasympathomimetics.

7.Axial Length Change after Encircling Scleral Buckling Procedures.

Pars plana vitrectomy with encircling scleral buckling was performed for 30 eyes of 30 patients with complicated retinal disorders, and the changes of axial length, refractive power, and corneal curvature were analyzed. Significant axial length elongations were observed after encircling procedures. Mean elongation was 0.93+/-0.48mm after 3 months and 0.91+/-0.54mm after 6 months. These spherical equivalent of refraction showed myopic shift of -2.26+/-1.51 diopters(D) after 3 months and -2.19+/-1.39D after 6 months. These changes seem to be less prominent in the eyes with preoperative axial length of 24.50mm or longer(0.68+/-0.46mm after 3 months, 0.60+/-0.49mm after 6 months) than in the eyes with axial length of 24.49mm or shorter(1.18+/-0.35mm 1.23+/-0.38mm). Slight increase in astigmatism was observed, 0.80+/-1.35D after 3 months and 0.64+/-1.01D after 6 months. The mean keratometric readings showed no significant changes, 0.00+/-0.95D after 3 months and 0.26+/-0.95D after 6 months. The result of this study may give an important information when combined procedure of encircling scleral buckling and IOL implantation be planned.

8.The effects of a single exposure of UVB on morphologic changes and proliforation in cultured normal melanocytes.

In humans, the major stimulus for cutaneous pigmentation is ultraviolet radiation. Little is known about the mechanism underlying this response, in part, because of the complexity of the interactions involving the whole epidermis. The present stucy was undertake to evaluate the effects of a single exposure of UVB on cultured normal melanocytes. Melanocytes were exposed to UVB from 5.1 mJ/cm to 203 mJ/cm. The results were as follows : 1. The main morphologic changes in UVB-exposed groups w re larger sized cells, more blunted dendrites, and shorter dendrites than in the control group. These cells increased sized according to the increased doses of VVB, but above 101.5 mJ/cm, the melanocytes shrunk and were destroyed. 2. From 20.3 mJ/cm of UVB, the proliferation of melanocyte was decreased, Especially, there was statistical!y significant difference above 50.8 mJ/cm (p<0.05, p<0.01). 3. The antiproliferativo effect increased with the passage of tirie after UVB exposure. So, cell count could not be done in 101.5 mJ/cm and 203 mJ/cm on the third day, and in 50.8 mJ/cm, 101.5 m J/cm and 203 mJ/cm on the seventh day. 4. Statistically the melanin content per well was significantl dicreased to 11-28% of each control group with dose above 50.8 mJ/cm (p<0.05, p<0.01). The melanin content per cell was increased to 107-128% of each control group when doses were below 20.3 mJ/cm and decreased to 49-79% of each control group when above 0.8 mJ/cm on the third day, but there was no statistically significant difference. In summary, when melarocytes were exposed to UVB, morphclogic changes progressed to cell differentiation. The results also suggested that a low or dose of UVB has an antiproliferative arid mild melanogenic effect, and a higher dose of UVB has a direct cytotoxic effect.

9.Clinical Comparison of Three Viscoealsitcs: Healon, healon GV, Provisc.

Viscoelastics which are used in intraocular surgery sell many kinds as to its unique properties. We intended to valuate the clinical usefulness of three viscoelastics, Healon, Healon GV and Provisc. We performed phacoemulsificatioin and posterior chamber intraocular lens implantation in 26, 28, 29 eyes with each viscoelastics and measured the corneal thickness at preoperatively, postoperative 1 day, 1 week, 1 month, 2 months, 5 months and endothelial cell count at preoperatively, postoperative 2 months. We also analysed the anterior chamber maintenance and easiness of injection and removal during operation, In Healon used group, Preoperative corneal thickness was 549.7+/-24.3micrometer, in Healon GV group, 550.1+/-28.7micrometer, in Provisc group, 543.1+/-39.0micrometer and which were changed into 552.1+/-39.0micrometer, 533.1+/-21.1micrometer, 522.2+/-32.0micrometer at postoperative 5 months respectively, but there was no statistically significant difference between preoperative and postoperative status in all three groups. There was a tendency of slight reduction of endothelial cell count at postoperative 2 month in three groups, but it wasn`t statistically significant, too. Healon GV and Provisc were better in anterior chamber maintenance but injection into anterior chamber and removal by irrigation and aspiration were more easy in Healon used group. So, we suggest all three viscoelastics has hood endothelial protective effect and can be used freely as to their unique properties and the operative situation.

10.A Clinical Study of Spinal Tuberculosis

The incidence of spinal tuberculosis has been decreased because of the development of anti-tuberculosis drugs, the use of radical surgical procedures and the improvement in the nations general hygiene. The anterior fusion is the most frequently used surgical measure in the treatment of spinal tuberculosis. One-hundred-eighty cases of spinal tuberculosis were treated by surgery during the 5 years from 1976 to 1980 at Presbyterian Medical Center with the following results: 1. The age distribution showed the peak incidence to be in the third decade. 2. The predilection site was the lumbar area and the percentage was 43.3. 3. In 17.2% of the cases, anti-tuberculosis drug was ingested before surgery. 4. Paraplegia was present in 16.1% of spinal tuberculosis. 5. In paraplegia, most of the location of the involved bodies was in the thoracic and the thoracolumbar vertebrae and the percentage was 79.3. 6. In paraplegia, complete recovery was 82.8%, and incomplete recovery was 10.3%. 7. In 90% of the patients, anterior fusion was done and the union rate was 91%. 8. Kyphosis was the most common in late complication.
