1.Change of Electrolytes after Transfusion of A.C.D. Stored Blood Warmed to 36 degrees C.

In the operation room of S.N.U.H., 10 patients were transfused with A,C.D. stored blood after warming to 36 degrees C by using a blood warmer. Observed changes of Na+, Cl-, K+ and. Ca++ were as follows. 1) Before warming, the electrolytes of A.C.D. stored blood were Na+ 148. 2 mEq/1, Cl-; 117. 0 mEq/1 and K+; 8. 4 mEq/l. After warming to 36C, these were significantly reduced(P <0. 05) and became 144. 6mEq/1, 112. 4mEg/1 and 68. 13mEq/1 respectively. 2) There was a tendency to increasing of K+ cencentration in proportion to storage duration. 3) In the case of calcium, pre-warmed concentration of calcium was 4. 09 mEq/1 and there was no significant change after warming(P>0.05).

3.The Comparative Study on the Prevalence of Injury/Poisoning in the Agricultural and Fishery Population and the General Population.

OBJECTIVE: We compared the prevalence of injury and poisoning in the agricultural and fishery population with that of the general population. METHODS: The national health insurance data and agricultural and fishery qualification data were used for this study. The age-adjusted standardized morbidity ratio was used to compare the prevalence of all injuries and poisonings of the agricultural and fishery population with that of the general population for the year 2002, as well as the prevalence of certain injuries and poisonings common to the agricultural and fishery population. The age-adjusted standardized morbidity ratio and 95% confidence intervals were attained by using the general population as the standard population group. RESULTS: The age-adjusted standardized morbidity ratio of total injuries and poisonings was significantly high in the agriculture and fishery population. The standardized morbidity ratio was 137.6 in the male agriculture and fishery population and 123.3 in the female agriculture and fishery population. In terms of injuries and poisonings common to the agriculture and fishery population, the age-adjusted standardized morbidity ratio was significantly high regarding the dislocations and strains of lumbar spine/pelvis, shoulder and neck, the fracture of rib/thoracic spine/sternum and pesticide poisoning. CONCLUSIONS: The overall prevalence of injury/poisoning was significantly higher in the agriculture and fishery population than in the general population. Various forms of research should be conducted on the injuries of the agriculture and fishery population in the future. In order to compare the differences in the prevalence rates of injuries according to time and region, standard definitions of injuries and occupation related injuries are required.

4.Cardiac Arrest in the Operating Room .

This is a report of cardiac arrests which occurred in the operating room of National Medical Center from Jan. 1, 1970 to Dec. 31, 1976. All kinds of operations were performed in this hospital, butlopen heart surgery was excluded from this study. During this period 22, 825 surgical operations were done and 74 cardiac arrests occurred among them. Overall incidence of cardiac arrest was 1: 308 (0. 32%) and 43% of cardiac arrests were successfully resuseitated.

5.p53 protein expression and its prognostic importance in patients with nodal non-Hodgkin's lymphoma.

To determine whether the p53 expression might be a predictor for treatment sponse and overall survival in nodal non-Hodgkin's lymphoma (NHL), we analyzed e expression of p53 in 69 NHL patients. p53 protein expression was analyzed by munohistochemistry with long-term follow up (1-148 months: median 12.2). p53 pression was noted in 23/69 (33.3%) patients. Complete response (CR) rate to stemic chemotherapy was correlated with stage (I/II) (p=0.038), but not with 3 expression (p=0.2856). Poor overall survival was associated with stage =0.0010) or IPI score (p=0.0076), but not with p53 expression (p=0.8601). From ratification analysis by stage, in stage III/IV patients, the p53 positive oup had a trend to be associated with poor overall survival than the p53 gative group. Multivariate analysis revealed that p53 positive group was sociated with less CR rate compared to the p53 negative group (p=0.046), ereas overall survival was correlated with stage (p=0.0320), not with p53 atus. p53 expression was associated with less CR rate in patients with DLBL. rther studies with large numbers of samples and homogenous group of NHL are eded to determine the prognostic value of cell cycle regulator, p53 in NHL.

6.Hysteroscopy Guided Diagnosis and Treatment: An Analysis of 8 Years Experience.

OBJECTIVE: To evaluate the diagnosis and treatment of hysteroscopy and its complications. METHODS: Reviewed 415 cases treated by hysteroscopy during 1994.9-2003.2 at Ajou University Hospital. RESULTS: Major indications were infertility (321 cases, 77.3%) and gynecologic indications such as myoma, polyps etc (94 cases, 22.7%). Hysteroscopic findings were polyp (192 cases, 46.3%), synechiae (72 cases, 17.3%), submucosal myoma (21 cases, 5.1%) and uterine anomaly (13 cases, 3.1%). Complications were 2 cases of uterine perforations and one case of bowel injury. CONCLUSION: Hysteroscopy is a safe, minimally invasive procedure with a low rate of complications.

7.A Case of Satoyoshi Syndrome Presented with Progressive Muscular Spasm and Alopecia.

Satoyoshi syndrome(generalized Komuragaeri disease) is a rare disorder of unknown cause, characterized by progressive, painful, intermittent muscle spasms and alopecia. Endocrinopathy with amenorrhea, secondary skeletal abnormalities, and diarrhea or unusual malabsorption are frequently seen. It seems that autoimmunity may play a role in its pathogenesis. We report a 13-year-old girl with characteristic manifestations of the syndrome. She was treated with intravenous gammaglobulin and Prednisolone. Painful muscle cramps were gradually improved, but the scalp condition did not change. Satoyoshi syndrome should be considered in children with unexplained muscle spasms and alopecia.

8.Three Cases of Internal Jugular Phlebectasia.

Phlebectasia is an abnormal dilatation of an isolated vein and a rare venous anomaly and is usually asymptomatic. Clinically internal jugular phlebectasia is a self limited benign condition and usually no treatment is required after initial diagnosis. So suspection of this disease and appropriate diagnostic approaches are essential to avoid unnecessary surgical intervention. We present three cases of internal jugular phlebectasia of which diagnosis was made by neck sonography and CT.

10.Early Management of Total Parenteral Nutrition Induced Hepatic Dysfunction with Cyclic Parenteral Nutrition.

PURPOSE: Liver function test abnormalities have been reported frequently in patients receiving total parenteral nutrition (TPN). In adults, it is known that liver complications decrease with the use of cyclic parenteral nutrition (CPN), especially if the shift to cycling was not too late. However, there are few studies about the effects of cycling on liver injury in children beyond the neonatal period. The aim of this study is to evaluate the effect of the early use of CPN on total parenteral nutrition induced hepatic dysfunction. METHODS: Twelve sets of CPN in 11 children (2 months to 17 years) were included in this study. Data on underlying diseases, age, length of time on TPN, macronutrient intake, complications, and biochemical parameters were collected from clinical records. All children had received CPN in the early period of persistent transaminase elevation or cholestasis complicated by previous continuous PN. The duration of infusion off-time in CPN was 2 hours in patients less than 3 months of age and 4 hours in the older children. RESULTS: All 12 cases showed elevated aminotransferase and 5 of them also showed cholestasis. Serum total bilirubin concentration was normalized in all 5 cases with median periods of 8 days (p<0.05) after initiation of CPN. ALT either decreased significantly or was normalized in all cases with median periods of 30 days (p<0.05) on CPN. The CPN was well tolerated without significant complication except for one case of hyperglycemia. CONCLUSION: The early use of cyclic parenteral nutrition had a beneficial effect in improving hepatic dysfunction complicated by TPN in children.
