1.Total Knee Arthroplasty using TC IV

The proper distribution of stress & a low rate of wear has become major concern in recent design consideration of total knee prosthesis. The TC IV knee system has kept the original anatomical design of the total condylar knee and improved and refined it. And it has some characteristics: deep patellar groove of the femoral component, 5 degree posterior tilted tibial UHMWPE articulating surface with increased potential range of motion, and beaded porous coating applied to the bone-component interface. This study reports the consecutive 61 arthroplasties in 42 patients. Follow up period was average three years and six months. There were 29 osteoarthritis, 29 rheumatoid arthritis, and three infection sequelae. In all cases, we used TC IV prosthesis, bone-cement for prosthesis fixation and sacrificed PCL. All patients were assessed using HSS knee score and American knee society clinical rating system, and roentgenographic evaluation and scoring system. 1. The range of motion was significantly increased from average 87.8 degrees preoperatively to average 122.9 degrees postoperatively. 2. The HSS knee score increased from average 47.7 points preoperatively to average 90.9 points postoperatively, and the functional score of AKS was average 85.6 and knee score was 93.4 postoperatively. 3. The flexion contracture was decreased from average 29.3 degrees preoperatively to average 3.9 degrees postoperatively. 4. In radiological review, the tibiofemoral angle changed from average varus 1.5 degrees preoperatively to average valgus 6.3 degrees postoperatively, and the joint line changed average 9.3mm preoperatively to average 13.7mm postoperatively, and nine knees(14%) had a radiolucency with a width of 2mm or more beneath one or two tibial zone. 5. Complications included two superficial infections and each case of transient peroneal palsy and patellar component wear in the metal backing type. 6. In summary, although the follow-up period was short, functional score of TC IV is no less than that of the other prosthesis with PCL retention.

2.Enhancing mediastinal tumors: CT evaluation.

CT scans of 21 patients(intrathoracic goiter=7. Castleman disease=6, pulmonary carcinoid tumor=3, parathyroid adenoma=1, thyroid carcinoma=1, paraganglioma=1, benign pleural mesothelioma=1, sclerosing hemangioma=1) with mediastinal tumors that are known to be enhanced with intravenous injection of contrast media, were studied retrospectively to investigate the enhancing capabilities f those tumors and to describe their CT findings. The degree of enhancement was estimated by visual inspection with a grading system. All but one case of cystic parathyroid adenoma showed enhancement on post-contrast scan. The degree of enhancement was not helpful in differentiating these tumors. Characteristic location and pattern of tumor extension were found in cases of intrathoracic goiter, parathyroid adenoma, thyroid carcinoma and paraganglioma. Calcifications were found in intrathoracic goiter(5/7). Castleman disease(2/6). Pulmonary carcinoid(1/3), thyroid carcinoma. Necrotic low-attenuation areas were in intrathoracic goiter(7/7), parathyroid adenoma, thyroid carcinoma and paraganglioma. In conclusion, mediastinal tumors that are known to be enhanced in the literature were enhanced with rare exception, and if we consider the predilection site of those tumors, the scope of differential diagnosis can reasonably be narrowed.

3.Identification of Vibrio vulnificus in Pusan and Southern Sea of Korea in 1996 using API 20E Kit.

The halophilic bacterium, Vibrio vulnificus, causes acute fulminating wound infections and septicemia in human. Especially the septicemia shows high mortality above 50%. In Korea, septicemia by V. vulnificus was reported at westem and southern coast in every year. Here, we try to isolate this V. vulnipcus at Kyoung-nam area and coast of Pusan during 1996. Purposed sites were Dadaepo, Songjung, Chungsapo and Mipo of Pusan and Kijang, Ilkuang, Juksoung, Dongam, Waljun and Chilam of southern sea. Total 40 strains of V. vulnipcus were isolated from sea samples. Biochemical characteristics of isolated V. vulnificus were almost same with reference strain V. vulnificus ATCC 27562 on Farmer's tests and on API 20E kit test. V. vulnificus isolates in 1996, fermented cellobiose and salicin but arabinose. and had resistance to 7% sodium chloride.

We present here a case of gastric leiomyoblastoma did not expressed desmin and neuron specific enolase, and was thought to be of unknown derivation.

8.Roentgenographic findings in acute appendicitis

Appendicitis is one of the most common condition causing an acute abdomen in young adult population. Two-hundreds and fifty-nine cases of acute appendicitis provened by surgery and pathology during recent 2 years in Han-ll Hospital were studied and analyzed by preoperative plain abdominal X-ray. One case of barium enema was also done. The results were as follows; 1. Male fo female sex ration was 1:1. 2. In age distribution, the most commonage group was from two to four decade. (75.3%) 3. The most common clinical symptom was classical abdominal pain as86.5%. 4. The positive radiographic findings were obtained 75.7% of all cases. 5. Common radiographic findings as follows; 1) Reflex ileus on RLQ (73.7%) 2) Obliteration of Rt. psoas line (41.3%) 3) Scoliosis of lumbar spine(21.2%) 6. The most common site of appendix was retrocecal in position. (84.2%) 7. One case of appendicolith and one case of appendiceal diverticulum were demonstrated.

9.Relationship of Restless Legs Syndrome Symptom, Sleep Disturbance and Depression in Middle-aged Women.

PURPOSE: The purpose of this study was to investigate relationships between the symptoms of Restless Legs Syndrome (RLS), sleep disturbance and depression among middle-aged women. METHODS: The study took a descriptive approach. A self-reported questionnaire was provided to women aged between 45 and 65 living in community area. The data were analyzed via SPSS program (version 25). RESULTS: 47.8% of the respondents showed moderate to severe RLS symptoms and the severity of RLS symptoms was 10.72. Depression score was 13.12 and sleep disturbance score was 4.97. There was a significant correlation between RLS symptoms and sleep disturbance (r=.423, p < .001), and RLS symptoms had a positive impact on sleep disturbance. There was also a significant correlation between RLS symptoms and depression (r=.494, p < .001) and RLS symptoms also had a positive impact on depression. CONCLUSION: Women with RLS symptoms had more severe sleep disturbance, depression and vasomotor syndrome than those without RLS symptoms. If RLS symptoms among middle-aged women is simply diagnosed as a menopausal symptom, this misdiagnosis can lead to sleep disorder and depression. Therefore, further empirical researches should be done to estimate the prevalence of RLS symptoms and to launch a program to detect RLS at the early stage.

10.Relationships between Symptom of Restless Legs Syndrome, Depression, Somatic Symptom and Activities of Daily Living among the Elderly

PURPOSE:
The purpose of this study was to investigate relationships between the symptom of restless legs syndrome (RLS), somatic symptom, depression, and activities of daily living (ADL) among the elderly.
METHODS:
A descriptive approach was taken in this study. Data were collected from May 23 to July 5, 2019 and 140 elderly people replied to the questionnaire. The data were analyzed by descriptive statistics, t-test, One-way ANOVA, Pearson's correlation coefficient, and χ² test.
RESULTS:
48.6% of the respondents had RLS symptom and the severity of RLS symptom was 9.57±11.55. RLS symptom had a positive correlation with depression (r=.538, p<.001), with somatic symptom (r=.528, p<.001), with ADL (r=.308, p<.001), and with IADL (r=.360, p<.001). Depression had the most significant correlation with RLS symptoms. There were significant differences in depression, somatization, ADL, and IADL according to the RLS severity.
CONCLUSION
The results of this study showed that many elders experienced mild to severe RLS symptom. Also, the elderly with RLS symptom had more severe depression, somatic symptom, ADL, and IADL than those without RLS symptom in this study. Depression, somatic symptom, and ADL could be deteriorated if the RLS symptom is not diagnosed early.
