1.Immunohistochemical Study of the Expression of p53, Pan-ras, c-erbB-2 and PCNA in N-Nitrosomorpholine(NNM)-Induced Hepatocellular Carcinoma of Rats.

The focus of this study was o aialyze the morphologic expression of p53, Pan-ras, c-erbB-2, and PCNA in preneoplastic and neoplastic liver lesions induced with NNM of rats. The development of hepatocellular tumors was investigated by histology and electron microscope in 65 Splague-Dawley rats administered with NNM in drinking water at low dose(5 mg/100 ml) and high dose(20 mg/100 ml). Three types of hepatocytic degeneration glycogenotic, eosinophilic and basophilic changes were followed by the appearance of hepatocellullar carcinoma. Hepatocellular carcinoma was increased in number and size according to NNM dosage and to duration of exposure. The histological classifications of hepatocelular carcinoma wer trabecular type, which was which was the most common, large eosinophilic, small cell, adenocarcinomatous and clear cell type. The expression of p53, Pan-ras, c-erbB-2 PCNA was examined by immunohistochemical stains. Eosinophilic degeneration revealed mild positivity at 18-26 weeks for expression of all oncogenic proteins studied and PCNA, whereas precancerous lesions showed variable expression from negative to moderate positivity on PCNA. Hepatocellular carcinoma lesions showed strong positivity for all stains and increased intensity during experimental period. These may indicate that chemical carcinogen produce hepatic eosinophilic degeneration and preCancerOus lesions by genetic mutation, resulting in hepatocellular carcinoma.

2.Malignant Hyperthermia During General Anesthesia: A case report.

Malignant hyperthermia is defined as a potentially fatal hypermetabolic syndrome characterized by hyperpyrexia, skeletal muscle rigidity, tachycardia, respiratory and metabolic acidosis, cyanosis etc. Any inhalation anesthetic agent or any skeletal muscle relaxant can trigger this acute catastrophic reaction. This case is presented of a 37 year old female patient in whom total gastrectomy was planed to perform under oxygen-nitrous oxide-isoflurane anesthesia with induction by thiopental sodium and succinylcholine. When administer of succinylcholine to induction, the jaw was very tight and the mouth was impossible to open. We retried with vecuronium and the jaw was slightly resistant to opening, but intubation was successfully performed. After induction, hyperpyrexia, tachycardia, increased end-tidal carbon dioxide developed. Anesthesia was terminated and vigorous emergency treatment was attempted. The patient was treated successfully with early detection and intensive care. According to decrease of temperature and normalization of arterial blood gas, the procedure continued with nontriggering agent, fentanyl. The etiologic factors, clinical features, treatment and preventive measures of malignant hyperthermia are discussed.

3.Hair Zinc and Lead : Relationship to Nutrient Intake and Height and Body Weight in Korean Preschool Children.

The objective was to evaluate the factors influencing on the growth of children aged 3 - 6 years by mini dietary assessment and anthropometric indices in 556 children and by analyzing mineral contents of hair from 273 children by inductively coupled plasma mass spectrometry (ICP-MS). The mean concentration of zinc (Zn) in hair was 68.9 +/- 31.1 ppm and the zinc level of 91% children was less than 60% of the reference value of 180 ppm. The mean content of lead (Pb) in hair was 1.76 +/- 1.20 ppm, but only 67% of 273 children was within the reference value of 2 ppm and the lead level of 33% of children was 2 - 4 times higher than the reference value. The level of Zn was negatively correlated with Pb content (r = -0.305, p < 0.01). Hair Zn content, regardless of age, was significantly positively correlated with height (r = 0.214) and body weight (r = 0.159). The height of 95.3% children was belonged to the normal range of 90 - 109.9% of standard value and the body weight of 49.8% children was in the range of 90 - 109.9% of standard value. However, 8.1% children was overweighted (110 - 119.9% of standard) and 9.4% children was obese (120 - 149.9% of standard) and 0.9% children was severe obese (> 150%) which resulted in total 10.3% obese. Contrary to our expectation 32.8% children was underweight (70 - 89.9% of standard) and 36% of the underweight children showed the low content of zinc (10 - 49 ppm) in hair. There was no significant correlation between dietary intake of Zn and hair Zn content. Hair Zn content was low even though children consumed greater than 75% of their requirement for zinc. Regardless of dietary intake of Zn, hair Zn level was rather negatively correlated to the content of Pb in hair. 38.8% children had an unbalanced diet which resulted in lower intake of calorie, Ca, Zn, vitamin B2 compared with those consuming variety of foods. Therefore, it would be needed that new approach for nutrition education to improve eating habit having the unbalanced diet in preschool children. It might be also suggested to develop new food products to complement the nutrients of Zn, Ca, Fe as snacks or dietary supplement for Korean preschool children.

4.A Comparative Study of the Toxicity Between carboplatin and Cisplatin in VBP Combination Chemotherapy.

Cisplatin has been widely used as chemotherapeutic agent for the treatment of cervical cancer, ovarian cancer, head and neck cancer and squamous cell carcinoma of lung. But cisplatin is highly toxic with nephrotoxicity, gastrointestinal toxicity, myelosuppression and neurotoxicity. The second generation drug of platinum compound, carboplatin was developed in 1980s to reduce side effects. Carboplatin has low nephrotoxicity but its major toxic effect is thrombocytopenia, In this study, the side effects of cisplatin and carboplatin were evaluated on 37 patients of cervical cancer in 169 chemotherpy cycles who were recieved combined VBP chemotherpeutic regimen consisting of cisplatin or carboplatin. Nephrotoxicity of grade 2 or over were 16% in cisplatin group and 1% in carboplatin group. Granulocytopenia of grade 2 or over were 34% in cisplatin group and 10% in carboplatin group. Thrombocytopenia of grade 2 or over were 7% in cisplatin group and 21% in carboplatin group. Gastrointestinal toxicity of grade 2 or over were 11% in cisplatin group and 0% in carboplatin group. This clinical study demonstrated that cisplatin has more toxic effects than carboplatin except thrombocytopenia.

5.Effect of Foot Reflexomassage on Constipation-affected College Women.

PURPOSE: This study was to determine the effects of foot reflexomassage on constipated college women for the treatment of constipation. METHOD: The researcher sampled nursing students attending a college of nursing science in Gyeonggi-Do, and checked if they were affected by constipation in reference to the Rome II criteria & CAS. They were randomly divided into an experimental group(15 subjects) and a control group(15 subjects). Foot reflexomassage was applied to the experimental group 10 times in 2 weeks : each time lasted 40 minutes. The frequency of defecation and severity of constipation by CAS were measured. The collected data was analyzed using the SPSS 11.5 program. RESULT: The CAS score of the experimental group was significantly lower after foot reflexomassage than those of the control group (p=.000). CONCLUSION: Foot reflexomassage can be considered an effective alternative treatment for constipated young women.

7.Study on Smoking and Sexual Satisfaction of Male Adults.

PURPOSE: This study aimed to compare differences on sexual satisfaction between non-smokers and smokers of Korean adult males. METHOD: The subjects of this study were 304 current smokers and 331 current non-smokers(stop smokers, never smokers) (58.0%). Because of the nature of the study, the participants were recruited through a home page advertisement of a brand newspaper company in Korea, and the survey was done through Internet system. The tool to measure participant's sexual satisfaction was adopted from the Arizona Sexual Experience Scale-Male. RESULT: Participant's age ranged from 20 to 49, mostly in their 30s-40s(96.1%). They were smoking average of 15 cigarettes per day. There were no significant difference on sexual satisfaction between current smokers and non- smokers(t=-.55, p>.05). However, there were significant difference on sexual satisfaction between ever smokers(n=481) and never smokers(n=154) (t=2.10, p<.05). There were significant differences an erectile and orgasm satisfaction according to number of cigarettes they smoked per day. 54.5% smokers who said that they have difficulty in keeping erectile state considered on smoking cessation within 30 days whereas 24.2% in those who said that keeping erectile state was easy. CONCLUSION: Informing about the high risk of sexual dysfunction with cigarette smoking should become a important part of smoking cessation education and for smoking adolescents.

8.Nursing Education between South and North Korea through Verbal Evidence from Defecting North Korean Medical Personnels.

Recently, there has been an increasing interchange between South Korea and North Korea. Accordingly, there has been active research to understand the society and culture of North Korea, it has been attempted to have comparative study about nursing education to increase understanding between South and North Korea. In the current educational system, 12 years of education is required for entering a nursing college or university in South Korea, but there are only 10 years for entering nursing college in North Korea. After finishing undergraduate studies one can enter graduate school for a masters degree and or a doctoral degree, but there is a longitudinal relation to medical education in North Korea. Regarding the number of nursing educational institutions, there are 50 BSN programs & 61 Diploma programs in South Korea and 11 Diploma programs in North Korea. In regards to curriculum, South Korea has diverse subjects for general education for freshmen, then is subjects to basic specialities sophomore year, and speciality subject and clinical practices from junior year corresponding to the student's intentions. North Korea has minor subjects for general education and basic specialities in freshmen, speciality subjects sophomore year, speciality subjects and clinical practice in the junior year that may not correspond with the student's intentions. The most outstanding difference in the curriculum is North Korea has various subjects for oriental medicine with clinical application. North Korea also does not teach computer science and English is at a very low level. In clinical practice, South Korea has various settings for clinical practice including community health institutions under the nursing professor or clinical instructor. However, North Korea has limited settings for clinical practice (general hospitals) under a doctor's instruction. Also both South and North Korea have a similar licensing system. Therefore, there must be many more studies regarding North Korea, especially in nursing and nursing education in order to decrease differences and confusion between the Koreas and to prepare for a future unification.

9.Effect of Recombinant Human Erythropoietin in the Anemia of Prematurity : a Pilot Study.

The recent availability of recombinant human erythropoietin has opened new perspectives in the management of a variety of anemias. Clinical trials have been initiated in several countries using different approaches and methodology. We randomly assigned twelve premature infants(gestational age < 32 week) at high risk of requiring erythrocyte transfusion for anemia of prematurity with either subcutaneous recombinant human erythropoietin or a placebo. Treatment with rHuEPO was initiated at a dose of 100 units/kg day for 3 days a week. All patients were given supplemental oral iron therapy at a dose of 3 mg/kg per day, as tolerated and oral vitamin E at a dose of 25 units per day. Treated and control babies did not differ with respect to weight, hematocrit, overall mean reticulocyte count or rate of growth respectively. However, reticulocyte counts increased earlier in patients given rHuEPO. We conclude that rHuEPO administration is safe and feasible at the dose studied.
