1.Wallerian degeneration of brain: MRI and CT findings.

Wallerian degeneration is well known as the anterograde degeneration of axon and their accompanying myelin sheath from injury to the proximal portion of the axon or its cell body. The most common cause of wallerian degeneration is cerebral infarction. Authors experienced three patients with old hemispheric infarct with typical wallerian degeneration in the brain stem, which was demonstrated by magnetic resonance imaging (MRI) in two cases and CT in one case. This report demonstrates the wallerian degeneration in the corticospinal tract on the MRI and CT with the brief review of the literatures.

2.CT findings of early acute cerebral infarction.

The CT findings of the acute cerebral infarction are well known. However the CT findings of early stroke within 24 hours of the onset have not been sufficiently reported. The purpose of this study is to evaluate early acute cerebral infarction on CT within 24 hours after ictus. The early and accurate CT diagnosis could lead to the appropriate therapy and improved outcome of the patients. Authors retrospectively analyzed 16 patients with early acute cerebral infarction. Acute cerebral infarction was confirmed by follow-up CT in 11 patients, SPECT in 4 patients, and MRI in 1 patient. The CT findings of early acute cerebral infarction include effacement of cortical sulci or cistern (n=16, 100%), hyperattenuation of MCA (n=3), obscuration of lentiform nucleus (n=6), loss of insular ribbon (n=6) and subtle low density in hemisphere (n=5). The most frequent findings was effacement of cortical sulci in our study, and it was though to be the most important sign of early acute cerebral infarction.

4.A case report on correction of Angle's Class III malocclusion with macroglossia.

This is case report of true class III malocclusion with macroglossia is corrected by glossectomy in 13 years female patient. After orthodontic treatment, the patient is bound to glossectomy because the corrected condition is relapsed to the previous condition due to relatively enlarged tongue compared with the original dental arch. By the interpretation of the cephalogram and model analysis, it is approved that the growth pattern and direction are normal range and mandible is located anterioly to the cranium. The results are follows: 1. We could treat the true Cl III malocclusion. 2. We could prevent the relapse of the treated condition by the surgical intervention, such as partial glossectomy. 3. Sensory, speech, swallowing and so other functions after the operation have been with in normal limit without any serious complications or seguellae.

5.Temporary Complication of Gentian Violet in LASIK.

PURPOSE: To report four cases in which temporary complication developed after using Gentian Violet as a marker in LASIK procedure. METHODS: Four cases who had undergone LASIK using Gentian Violet (Viscot Surgical Marker, U.S.A.) as a marker were followed. During follow up, corneal edema and opacity developed in landmark area which appeared as a hot spot in topography. Topical Corticosteroids and antibiotics applied for 3 weeks. RESULTS: At 3 month after LASIK, the corneal opacity and edema were disappeared. CONCLUSIONS: Temporary complication may develop by gentian violet following LASIK procedure. When using highly concentrated gentian violet marking pen, great attention should be paid not to make any corneal erosion.

6.The Results in LASIK According to The Use of The Fixation Ring and The Accuracy of The Focusing Beam.

PURPOSE: In order to know the effects of inaccurate laser shot of vertical or horizontal direction, we evaluated the clinical results of LASIK according to the use of fixation ring and the accuracy of the focusing beam on the corneal stromal bed. METHODS: LASIK was performed in 73 eyes with one surgeon between April, 2001 and January, 2002 were divided into two groups: those who exceeded -6.00D were high myopia group (37 eyes), and those who did not were low myopia group (36 eyes). We retrospectively analyzed the results of the LASIK in two methods, the one is the evaluation of the vertical directional error with the accuracy of the focusing beam and the other is the horizontal error with the use of fixation ring. We measured the preoperative and postoperative uncorrected visual acuity, spherical equivalent, and the postoperative predictability. RESULTS: There was no significant statistical difference in the spherical equivalent, uncorrected visual acuity, postoperative predictability between the use and nonuse of fixation ring and good and bad accuracy of focusing beam in high myopia and low myopia (P>0.05). CONCLUSIONS: These results suggest that a little difference in the accuracy of focusing beam on the corneal stromal bed and the use of fixation ring would not affect the clinical result after LASIK.

7.MR findings of spinal epidural mass.

We analyzed the MR findings of spinal epidural mass in 23 patients retrospectively. MR images were performed at 1.5T unit. The lesions were confirmed as metastasis(12 cases), lymphoma (2 cases), leukemia(1 cases), multiple myeloma(2 cases), meningioma(3 cases), neurofibroma(1 case), dysraphism with lipoma(1 case) and lipomatosis(4 cases), Most MRI examinations consisted of T1, proton density and T2 weighted sagittal imaging of the spine, with additional pulse sequences or image planes as needed for clarification. The level of the spinal epidural mass was cervical spine level in 2 cases, thoracic in 15 cases, lumbar in 7 cases, and sacral in 2 cases. The location of epidural mass within the spinal canal was eccentric in 20 cases, multiple in 2 cases, and encircled in 4 cases. Paraspinal mass was seen in 11 cases. Signal intensity of epidural mass was variable. The marrow of spine revealed low signal intensity(SI) on TIWI and high SI on T2WI in 14 cases. In conclusion, MRI is a useful diagnostic tool for the evaluation of spinal epidural mass.

8.Analysis of the T Lymphocyte Subsets in the Peripheral Blood of Patients with Myasthenia Gravis.

It is known that the activated peripheral T lymphocytes are increased in patients with autoimmune disorders such as systemic lupus erythematosis, rheumatoid arthritis, multiple sclerosis and autoimmune thyroiditis, but not in the patients with myasthenia gravis on previous studies. We investigated the subsets of T cells in peripheral blood of the patients with myasthenia gravis using flow cytometric analysis. Forty-three patients of myasthenia gravis who were not on steroid or other immunosuppressants were chosen, and thirty-six age-matched healthy persons were evaluated as controls. The peripheral blood mononuclear cells from patients and controls were stained with FITC or PE-conjugated monoclonal antibodies to several surface molecules expressed on T cells within 6 hours after collection. The samples were analysed by flow cytometry within 24 hours. In patients with myasthenia gravis, the expressions of DR and CD25 molecules on the T lymphocytes were increased significantly compared to those of the control group. The expression of CD25 was increased on CD4+ T cells, but not on CD8+ T cells . The expression of DR molecule was increased on CD8+ T cells, but not on CD4+ T cells. Therefore we suggest that the activated T cells are increased in myasthenia gravis and CD25 surface markers on CD4+ T cells may be a more sensitive indicator of immune status.

9.MR imaging of spinal epinal epidural abscesses.

Spinal epidural abscesses were evaluated using Gadolinium-enhanced magnetic resonance imaging (MRI) in 19 surgically and clinically confirmed cases, including 13 with tuberculous spondylitis and 6 with pyogenic spondylitis. We observed rim enhancement patterns in 10 patients and diffuse enhancement patterns in 9 patients. The spread patterns were localized to ventral or dorsal epidural space and encircled entire epidural space. We conclude that rim enhancement indicates abscess and diffuse enhancement indicates granulation tissue and fibrosis. Tuberculous epidural abscess spreads to ventral epidural space and pyogenic epidural abscess encircles entire epidural space.

10.Magnetic resonance imaging of the pituitary adenoma: analysis of the enhancement patterns.

The magnetic resonance images (MRI) of 30 patients with surgically or biochemically confirmed pituitary adenomas (20 macroadenomas, 10 microadenomas) were retrospectively evaluated. Ten patients had hyperprolactinemia, another eight had acromegaly, another eight had nonfunctioning adenoma and four had Cashing disease. The examination were performed at a 1.5-T superconducting MR system using a multisection spin-echo technique with 3mm-thick sections and a 256×224 matrix. TI weighted sagittal and coronal images were obtained before and within 30 minutes after the administration of Gd-DTPA (0.1mmol/kg). Analysis of the MRI was focused on the signal intensity and enhamcement patterns of the pituitary adenoma before and after Gd-DTPA administration. Compared with endocrinologic diagnosis, macroadenoma showed heterogeneous enhacement in 55%, rim enhancement in 35% and homogeneous enhancement in 10%, Conclusively, the enhancement patterns of the pituitary adenoma did not correlate with the subtypes made according to hormone production.
