2.A Case of Pregnancy Using Recombinant Follicle Stimulating Hormone and Gonadotropin Releasing Hormone Antagonist.

OBJECTIVE: To report the pregnancy which was made by in vitro fertilization using recombinant follicle stimulating hormone and gonadotropin releasing hormone antagonist. MATERIAL AND METHOD: Case report. RESULTS: Six oocytes were retrieved and all were fertilized by intracytoplasmic sperm injection. Six embryos were transferred and the pregnancy was confirmed. CONCLUSION: It is envisaged that the availability of recombinant gonadotropins and gonadotropin releasing hormone antagonists will ultimately lead to shorter, cheaper and safer treatments, using reduced dosages.

3.Nonbronchial Systemic Feeding Arteries in Patients with Hemoptysis: Predictive Factors at Radiography.

PURPOSE: To determine the radiographic findings which predict the presence of nonbronchial systemic feeding arteries (NBFAs) in patients undergoing embolotherapy to control hemoptysis. MATERIALS AND METHODS: In 48 patients (39 men and 9 women; mean age, 51 years) who underwent embolotherapeutic procedures for controlling hemoptysis, selective angiography was performed at the intercostal, subclavian and bronchial arteries in 65 hemithoraces (right 11, left 20, bilateral 17). Underlying diseases were tuberculosis (n=34, including three patients with aspergilloma), bronchiectasis (n=11), paragonimiasis(n=2) and metastatic cancer (n=1). The presence of NBFA at angiography was correlated with radiographic findings including pleural thickening, parenchymal distortion, and the location of lung lesions. RESULTS: NBFAs were found in 34 (77%) of 44 hemithoraces with pleural thickening, and in six (29%) of 21 without pleural thickening; the sensitivity and specificity of prediction were 85% and 60%, respectively. NBFAs were observed with greater frequency as the thickness of the pleura increased, and the extent of pleural thickening correlated less with the presence of NBFA than did thickness. NBFAs were found in 35 (78%) of 45 hemithoraces with parenchymal distortion, and in five (25%) of 20 without distortion (p<0.001). In addition, the distribution of the underlying disease in the upper lung zone showed close correlation with the presence of NBFAs (p<0.05). CONCLUSION: In patients with hemoptysis, the pleural thickening revealed by radiography has a high sensitivity and a relatively low specificity for predicting the presence of NBFA, and patients with parenchymal distortion and upper lung lesions have a high incidence of NBFA.

The authors retrospectively reviewed the high resolution computed tomographic scans of 19 patients who had hematogenous or lymphangitic metastatic lung lesions. In all patients, the histologic diagnosis for primary cancers and the radiographic manifestations of pulmonary metastasis were established. Certain characteristic findings of metastatic lung tumors on HRCT scans were evident: uneven thickening of bronchovascular bundles, multiple small nodules, thickening of interlobular septum, and the presence of polygonal lines. The nodules are more predominent in hematogenous metastasis. The prevalent site of nodules is cortical portion of lung.

7.Embryonal Rhabdomyosarcoma of the Biliary Tree: A Case Report.

Rhabdomyosarcoma are reportedly the most common soft tissue sarcoma occuring in childhood, but the biliary tree is a rare site of origin for this tumor. Recently we experienced a case of embryonal rhabdomyosarcoma of the biliary tree in a 30-month-old child. UItrasonography showed hypoechoic mass filling the dilated left. intrahepatic & extrahepatic bile ducts, and CT showed hypodense mass with heterogeneous enhancement after contrast infusion. Intraoperative cholangiography showed filling defects within the dilated left. intrahepatic & extrahepatic bile ducts. Postoperative MRI showed residual mass within the left. intrahepatic duct which was hypointense on T1WI and hyperintense on T2WI.

9.A Diagnostic Study on the Development of a Health Promotion Program for Vinyl House Farmers.

PURPOSE: The purpose of this study was to investigate influencing factors of health problems which the vinyl house farmers had experienced. METHOD: Based on PRECEDE model, this study was done with 110 respondents(50 to 69 years old), working in vinyl house and living in the remote area where the primary health post located. RESULTS: Quality of life was low in economical status, emotional status, and physical function. Health problems were founded as pain and chronic fatigue that were caused by physical symptoms, weakened physical strength, and increased blood pressure and cholesterol. Behavioral and environmental factors were related with lack of exercise, irregular eating habit, curtailed sleep, overwork, wrong working postures, stress, and non-fulfillment of safety standard during spraying pesticide. Self efficacy was low in excercise practice, working way, and stress management. CONCLUSION: The Health Promotion Program for vinyl house farmers should be developed to improve health promoting behavior and self efficacy, to reduce stress, and to strengthen physical function.

10.Distribution of Serotonin-2 Receptor in the Superior Collicullus of the Adult Rat: A Radioarutographic Study.

The superior colliculus is a laminated structure that consisted with superficial and deep layers in the roof of midbrain and the center of visual information and sensorimotor intergration. The deep layers of the mammalian superior colliculus are concerned with generation of eye movements. Serotonin is a neurotransmitter of the central nervous system and its binding sites can be classified into serotonin-1 receptor, serotonin-2 receptor and serotonin-3 receptor according to their pharmacological characteristics. The serotonin-2 receptors in the human brain presents in the motor cortex(area 4 of Brodmann) which role for the control of ocular motor activity and in the primary and secondary visual areas(areas 17 and 18 of Brodmann). Some of them presents in the limbic system role for the control of emotive activity, memory and other limbic functions. We performed this experiment to identify the anatomical distribution of the serotonergic nerve terminals in the superior colliculus of the rat by microscopicautography. It has been demonstrated that the serotonin-2 receptors of the superior colliculus are concerned with generation of saccade eye movements.
