1.MR Classification of Tear Types and Popliteous Muscle Attachments in Lateral Disccid Menisci.

PURPOSE: To determine the types of meniscal tears and type of popliteal muscle attachment in lateral disccid menisci. MATERIALS AND METHODS: MR images from 40 patients with arthroscopically proven disccid meniscal tears were reviewed. The type of capsular attachment of the popliteal muscle in these patients was compared with 100 normal controls. Fourteen men and 26 women were included in this study ranging in age from 8 to 52 yea rs. RESULTS: Disccid mensical tears were divided into five types. Arthroscopic cor- relation revealed that thirty-six cases(90%) of disccid meniscal tears were cor- rectly diagnosed using MR imaging. Peripheral tear wasthe commonest tear type occurring in 13 patients. In 9 of these the meniscal fragment was displaced. Hori- zontal tears occurred in 5, longitudinal tears in 4 and transverse tears in 2 patients. Multiple tears were present in 16 patients with the commonest combi- nation of peripheral and horizontal. Popliteal capsular attachments of disccid mensical tears were more prominent than in the normal control group. CONCLUSION: The types of tears in disccid menisci were different from normal meniscal tears. Peripheral detachment with displacement of torn segments was a very frequent finding in disccid mensical tears. Prominent capsular attachment of popliteal muscles is probably an associated finding in disccid menisci.

2.The Clinical Studies on Congestive Heart Failure.

The author analyzed 231 patients of congestive heart failure which was confirmed by the clinical symptoms and the laboratory findings among the patients admitted to Ewha Womans University Hospital between the begining of 1974 and the end of 1978. The result were as follow; 1. In sex distribution, the female was predominant. Male to female ratio is 1:2.45. The most common incidence by age group was sixth decade. 2. Regarding the underlying disease, there were hypertensive heart disease(27.3%), post-partum heart disease(19.5%), rheumatic heart disease(16.0%) and arteriosclerotic heart disease(15.6%) in order. 3. The precipitating factors were observed in 77.5% of total cases and the order of frequency was pregnancy and delivery(26.4%), infection(19.1%), arrhythmia(12.6%) and physical exertion(12.1%). 4. On admission, patients usually complained of dyspnea, cough, right upper quadrant pain and the physical examination revealed moist rales, hepatomegaly etc. 5. The laboratory findings were anemia(19.0%), increased E.S.R.(33.8%) and leukocytosis (34.2%). 6. The blood chemistry showed increased level of creatinine(24.5%), S.G.O.T. (20.9%), B.U.N.(20.0%) and decreased albumin(15.5%), increased S.G.P.T.(15.0%) in order. 7. The X-ray films of chest on admission revealed increased vascular marking and pulmonary congestion(85.7%), cardiomegaly(80.0%) and pleural effusion(21.4) etc. 8. The E.C.G. findings showed(205 cases among 231 cases) LVH 56.6%, atrial fibrillation 26.3% etc. 9. The echocardiography perfomed in 59 cases among 231 cases gave valvular heart disease (28.9%), atrial fibrillation (11.9%) arteriosclerotic heart disease(8.5%), ventricular hypertrophy (6.8%) and atrial enlargement(5.1%) in order of frequency.

4.MR Imaging of the Coronary Ligaments of the Knee Joint: Cadaveric and Clinical Study.

PURPOSE: Our objective was to identify a coronary ligament of the knee joint on MR images. MATERIALS AND METHODS: We retrospectively evaluated 50 MR knee studies. Using MR imaging, the thickness and length of the anteroinferior portion of the capsule were measured and compared at the medial and lateral compartments. We also carefully dissected 5 fixed cadaveric knees and took photographs. RESULTS: The thickened synovial membrane(coronary ligament) of the anterior horn of the lateral meniscus appeared as a band of low signal intensity on MR images. On anatomic dissection of the knee joints, the inferior synovial attachment of the lateral meniscus was more redundant than the medial side. CONCLUSION: Anatomic correlation revealed that the coronary ligament contributed to the band-like structure at the anteroinferior aspect of the lateral meniscus.

5.Clinical Reviews on 28 Patients with Ulcerative Colitis.

The ulcerative colitis is a relatively common disease in the European and North American countries aince Dr. Wilks has first reported the cases in 1895, But in Korea, this disease is a rare entity and only a few case have been reported. However, rescently the diagnoetic methods for ulcerative colitis are much developed we have experienced 28 cases of ulcerative colitis which were diagnosed by endoscopy, barium enema exam and biopsy. Therefore a total of 28 patient with ulcerative colitis diagnosed and treated at the department of internal medicine of HYUH from June 1979 to June 1986 was reviewed. We obtained the results as follows; 1) The sex distribution assumed a ratio of 1 to 1.33 with 12 males and 16 females. 2) The age dietribution was relatively even but the majority of cases were between 5th and 6th decade (32.1%), 3) The most prominent clinical symptoms were rectal bleeding, abdominal pain, diarrhea, fever, wight loss and vomiting. 4) Significant laboratory findins were anemia, eleveited ESR, leukocytosis, positive stool OB, electrolyte imbalance and decreased serum albumin level. 5) As to the extent of disease determined by the barium enema examination and endoscopic exam., rectum or rectosigmoid colon is involved in majority of the cases (92.8%). 6) The endoscopic examination was performed in all 28 cases and showed significant findings such as ulceration, hyperemia, mucosal friability, bleeding etcs. 7) The barium enema examination was performed in 19 cases and showed positive findings such as ulceration, luminal narrowing, bowel shortening and rigidity in 17 cases. 8) 21 of 28 patients treated by medical therapy showed relatively improved but 4 of 28 patients was exacerbated or relapsed and one has expired.

7.Tears of Anterior Cruciate Ligament and Associated Injury in the Knee Joint: MR Imaging.

PURPOSE: The objective of the study was to evaluate the characteristic findings in tears of the anterior cruciate ligament(ACL) and associated injury at MR imaging. MATERIALS AND METHODS: We reviewed the findings of MR images and the corresponding arthroscopic results of 32 patients with ACL tears. we evaluated the signal intensity and contour of ACL surrounding bony structures, menisci and associated injury of the knee joint. RESULTS: Complete ACL tears were present in 25 patients and partial ACL tears were in 7 patients. Complete ACL tears showed heterogenously increased signal intensity with contour bulging of the ACL in 14 patients (56%) and without bulging or absence in 11 patients(44%). Most patients torn ACL with contour bulging(12/14) had bone bruise, but only one patient torn ACL without bulging contour had bone bruise. ACL with thin continuous low signal band surrounding heterogenously increased signal intensity suggests partial tear, which was seen in three patients of seven proved partial ACL tears. Combined bone injury in ACL tear were in 23 patients (73%) and most of these(22/23) were at midportion of lateral notch of femur and/or posterior portion of lateral tibial plateu. Deepening of lateral notch of femur were noted in 17 patients(53%). Associated injuries of the other ligaments of knee joint were buckling of the posterior cruciate ligament(16/32, 50%) and tears of the medial collateral ligament(11/32, 34%). Posterior horns of menisci were more frequent site of combined injury within menisci in patients with ACL tear. CONCLUSION: Acute tearing of ACL in MRI is seen as heterogenously increased signal intensity with contour bulging of ACL and combined bone bruises. Patients with torn ACL frequently have various combined injury. In patient with knee injury, these associated or ancillary findings suggest that ACL tear is present.

8.Distribution of Serotonin-2 Receptor in the Superior Collicullus of the Adult Rat: A Radioarutographic Study.

The superior colliculus is a laminated structure that consisted with superficial and deep layers in the roof of midbrain and the center of visual information and sensorimotor intergration. The deep layers of the mammalian superior colliculus are concerned with generation of eye movements. Serotonin is a neurotransmitter of the central nervous system and its binding sites can be classified into serotonin-1 receptor, serotonin-2 receptor and serotonin-3 receptor according to their pharmacological characteristics. The serotonin-2 receptors in the human brain presents in the motor cortex(area 4 of Brodmann) which role for the control of ocular motor activity and in the primary and secondary visual areas(areas 17 and 18 of Brodmann). Some of them presents in the limbic system role for the control of emotive activity, memory and other limbic functions. We performed this experiment to identify the anatomical distribution of the serotonergic nerve terminals in the superior colliculus of the rat by microscopicautography. It has been demonstrated that the serotonin-2 receptors of the superior colliculus are concerned with generation of saccade eye movements.

9.Acute Tears of the Anterior Cruciate Ligament:Analysis of the Tear Site and the Degree Using MR Imaging.

PURPOSE: To evaluate the sensitivity of MR imaging in determining tear sites and degrees in acute anterior cruciate ligament tear. MATERIALS AND METHODS: MR imagings were undertaken in 19 patients who had trauma on their knee joints. All imaging studies were performed within 2 weeks after trauma and compared with operative findings. The degrees of ligament tear were divided into complete and incomplete, and sites of tears were divided into superior middle and inferior portions. MR findings were compared with operative findings. RESULTS: There were 14 cases of complete ligament tear and 5 cases of partial ligament tear. We could diagnose correctly in all 14 cases with complete tear and in 3 of 5 cases with partial tear. The tear sites were correctly predicted in 10 of 14 cases with complete tear(71%) and 1 of 5 cases with par In complete tears, MR findings were transversely or obliquely coursed band-like high signal intensity within the ACL or abrupt Switch over to as indistinct signal intensity. In partial tears, the tear sites could not be evaluated mostly and the tear appeared as linear low signal intensity lesions in posterolateral bundles of AC/. CONCLUSIONS: MR revealed higher sensitivity in determining the degree and sites of ACL tear in complete tear as compared with partial tear.
