Korean Journal of Dermatology 1993;31(6):851-856
BACKGROUND: Squamous cell carcinoma(SCC) is one of the most conimon forms of skin cancer that arises from the surface epidermis. Although many authors have reported on the incidence and clinical features of SCC, we could not find any reports concerning the widerange of histopathologic features of a number of cases of SCC in Korean dermatologic literature. OBJECTIVE: We examined the histopathologic features of SCC. METHOD: Twenty six biopsy specimens from 26 patients with SCC were examined microscopically including the tumors themselves and surrounding epidermis and dermis on routine histologic staining. RESULTS: Grade 1 tumor was found in 15 cases, grade 2 in 8 casese, and grade 3 in 3 cases. Eighteen cases showed keratin pearls, and 11 cases showed squamous eddies. P cantholysis of tumor cells was found in all 26 cases, but glandular structure was found only in 5 cases. The epidermis overlying the tumor showed hyperkeratosis and parakeratosis in all 26 cases, follicular plug in 13 cases, and ulceration in 18 cases. The dermis beneath the tumor showed vessel dilataticn, hemorrhage, and infiltration of inflammatory cells in all 26 cases, and basophilic degeneration in 1.3 cases, fibrosis in 20 cases, lichenoid reaction in 6 cases, and necrosis in 6 cases. Rare dermal findings were abscess, foreign body reaction calcification and Russell body. CONCLUSION: The histopathologic findings of SCC were variable not inly those of the tumors themselves, but also those of the surrounding epidermis and dermis.

Korean Journal of Pathology 1992;26(4):355-359
Routine use of commercially available antisera against hepatitis B core antigen(HBcAg) has permitted a reevaluation of the histochemical distribution of the antigen in liver tissue. HBcAg, classically described almost exclusively in the nucleus, was found with a very high frequency in the cytoplasm of liver cells as well. To elucidate the biologic significance of HBcAg expression and its relation to the natural course of hepatitis B virus(HBV) infection, the patterns of activity in 33 needle liver biopsies of HBsAg carriers were analysed. A good correlation of liver HBcAg with disease activity was demonstrated. HBcAg was present in the hepatocyte nuclei(nHBcAg) or cytoplasm(cHBcAg), or in both(mixed). Pure nHBcAg was seen mainly in non-aggressive reactive liver tissue and cHBcAg was predominantly associated with chronic active hepatitis(95%). The results suggest that expression of HBcAg correlates with the liver pathology and the possibility of HBcAg to be an immunological target for T cell mediated hepatocyte damage.

Early chondromalacia patellae is initiated by malalignment in the extensor machaniam and abno-rmal patellofemoral tracking. It is well known that chondromalacia often occurs in recurrent pateIlar subluxation. In the 50 normal and 60 pathologic knees, the following five parameters were studied; 1) Q angle 2) PT ratio by Insall & Salvati method 3) AB ratio dy Blackburne & Peel method 4) sulcus angle 5) congruence angle. The following conclusions were made. l. In 50 normal asymptomatic knees, the average Q angle was 14°. The patellar length was equal to the patellar ligament length and the average congruence angle was −8°. In 40 knees with “Chondromalacia”, the Q angle was increaed (19.5°), and the average congruence angle was −2°. 3. In 20 knees with recurrent subaxation, the patellar was high riding (PT ratio, 0.89), the sulcus angle was shallow (145.1°), and the average congruence angle was +4.35°.

9.Microvascular Reconstruction of Cranial Base Defects and Midfacial Defects.

Journal of the Korean Society of Plastic and Reconstructive Surgeons 1999;26(2):215-221
Until recently, cranial base tumors or midfacial tumors were deemed unresectable due to an inability to accurately diagnose the extent of the involvement and to approach and excise the tumor safely. With refinements in CT and NMR scanning, as well as the development of craniofacial technique, reconstruction has become absolutely crucial in allowing successful resection of these tumors. Resection of these tumors may sometimes result in massive and complex extirpation defects that are not amendable to local tissue closure. In such cases, free tissue transfer was an effective alternative because it can provide a large amount of well-vascularized tissues arts reliable separation of the intracranial space from bacterial flora of the upper airway. Microvascular free tissue transfer was used in 15 patients at our center to reconstruct the cranial base and/or midfacial defects. Of these 12 were free rectus muscle flaps, 2 were free latissimus dorsi muscle flaps, 1 was a free scapular osteocutaneous flap and 1 was a free scapular fasciocutanegus flap. There were 2 cases of total flap loss. In those cases, revisions were performed using latissimus dorsi muscle free flap in one case and STSG in the other. One patient had a postoperative cerebrospinal fluid leak which was spontaneously resolved by conservative management. The large complex defects were successfully reconstructed by one-stage operation and the functional and esthetic results wee satisfactory with acceptable complication rates.
