2.Symptoms of Hwabyung.

OBJECTIVES: This study was to investigate the characteristic symptoms of socalled hwabyung, a syndrome related to anger. METHODS: One hundred and fifteen normal healthy volunteer and 279 patients with anxiety disorders, somatoform disorders, major depressive disorder without psychotic features, and dysthymic disorder completed a self-rating questionnaire which include the Korean version of SCL-90, and selected Korean culture-related symptoms, and the severity of hwabyung. The data was statistically analyzed with t-test, chi2-test, Spearman correlation test and cluster analysis. RESULTS: The symptoms of hwabyung group were severer than in non-hwabyung group, and hwabyung was characterized by the symptoms of chest stifling/oppression, impulse to go-out, a pushing-up sensation in the chest, weeping, mortification, heat sensation, sighing, depersonalization, many dreams, hate, shame, paranoid feeling, easily being frightened, blurred vision, fearfulness, destructive impulsiveness, absent mindedness, cold sensation, pleading, irritability, intolerance to the heat and facial flushing as well as typical neurotic symptoms such as anxiety states, depression and obsessive-compulsiveness. CONCLUSION: These results suggest that an anger disorder can be conceptualized.

3.Effects of Atypical Antipsychotics on Serum Prolactin and Testosterone Levels in Schizophrenic Patients.

OBJECTIVES: The dopamine-blocking effects and the associated side effects(amenorrhea, lactation, sexual dysfunct of classical antipsychotics in schizophrenic patients have been studied for a long time. The purpose of this study to find out these effects of new antipsychotics(risperidone, olanzapine) in schizophrenic patients treated with clinical relevant doses. METHOD: Plasma levels of both prolactin and testosterone were measured in 91 schizophrenic patients(28 taking haloperidol, 4-20mg/day ; 31 taking risperidone, 2-6mg/day/ 32 taking olanzapine, 5-20mg/day). RESULTS: In male schizophrenic patients, the prolactin levels of risperidone group(76.44+/-38.85ng/ml) and haloperidol group(60.26+/-20.74ng/ml) had no significant difference, but were significantly higher than that of olanzapine(26.90+/-5.36ng/ml). In female, the prolactin level of olanzapine group(36.66+/-17.55) was significantly lower than those of risperidone(121.7+/-.33) and haloperidol group(161.66+/-37.53). And prolactin level of risperdone group was lower than that of haloperidol group. While the testosterone plasma level of risperidone, haloperidol and olanzapine in both male and female schizophrenic patients had no significant difference. CONCLUSIONS: At doses known to be effective in popular clinical setting, prolactin level in patients taking risperidone we higher than that of haloperidol, while olanzapine showed no significant difference in terms of prolactin plasma level haloperidol. New antipsychotics may not influence the testosterone plasma level.

4.Association between Apolipoprotein E Genotype and Treatment Response in Chronic Hepatitis C.

BACKGROUND: Hepatitis C virus (HCV) causes a chronic infection, resulting in progressive liver damage. Recent studies have described the protective effect of the apolipoprotein E (ApoE) genotype on liver damage in cases of HCV infection. Their findings were explained by the influence of the ApoE genotype on HCV pathology, which seems to be integrally linked to the process of HCV uptake into hepatocytes. We investigated whether specific ApoE genotypes were associated with the different clinical aspects of HCV infection in patients with chronic HCV. METHODS: From the whole blood of 196 chronic HCV hepatitis patients, the ApoE genotypes were determined by an allele-specific polymerase chain reaction. Several markers, including liver enzymes, platelet counts and HCV viral loads, as well as the radiologic findings, were investigated. In order to estimate the treatment outcome, the sustained virologic response (SVR), early virologic response (EVR) and end-of-treatment response (ETR) were determined according to the HCV viral loads. RESULTS: Based on genotyping, 15.8% (n=31) of the patients had the ApoE E4 allele (E2/E4, E3/E4, E4/E4), while 84.2% (n=165) were missing the ApoE E4 allele (E2/E2, E2/E3, E3/E3). Several clinical results of the E4-positive group, including liver enzymes, albumin, platelet counts, HCV viral loads and hepatic coarseness were not significantly different from those of E4-negative group. There were no differences in the SVR, EVR and ETR between patients with the ApoE E4 allele and those without the ApoE E4 allele. CONCLUSION: There was no significant effect of the ApoE genotype on the clinical aspects of HCV infection and the anti-viral response, including SVR, EVR and ETR, in chronic HCV hepatitis patients.

Intraabdominal abscess usually causes distress with fever, leukocytosis, pain and toxicity. Diagnosis of intraabdominal abscess is occasionally difficult and it has high morbidity. However radiologic method, such as ultrasonography, CT scan, or RI scan are helpful to early detection of intraabdominal abscess. Among these methods, ultrasonography is a noninvasive technique and performed without discomfort to patient. And also differential diagnosis between cystic and solid lesion is very easy and sequential ultrasonography in same patient is valuable for the evaluation of treatment effect. We analyzed the ultrasonic features of 48 cases with intraabdominal abscesses and the results are as follows; 1. In total 48 cases, the intraabdominal abscesses were 30 cases, the retroperitoneal abscesses, 5 cases, and the visceral abscesses, 13 cases. 2. The causes of the intraabdominal abscesses were perforating appendicitis (25 cases), postoperative complications (5 cases), pyogenic and amebic hepatic abscesses (13 cases), and the other (5 cases). 3. Round or oval shaped lesions were 26 cases (54%), irregular shape, 18 cases (38%), and multiple abscess formation in 4 cases (8%). 4. The size of the lesions were between 5 and 10cm in diameter in 54% of total 48 cases, and the most frequent feature of the echo pattern of the lesions was cystic with or without internal echogenicity (69%).

9.A Case of Recurrent Expansile Cyst Formation after Gamma Knife Radiosurgery for a Cerebral Arteriovenous Malformation: Role of the Residual Nidus: Case Report.

The authors report a case of recurrent expansile cyst formation after gamma knife radiosurgery(GKRS) for a cerebral arteriovenous malformation where the residual nidus might play an important role. A 28-year-old male patient underwent gamma knife radiosurgery for a left parietal lobe arteriovenous malformation which had manifested with generalized tonic-clonic seizure in November 1992. In March 1999, 76 months after GKRS, the patient presented with headache, dysphasia and right hemiparesis and the computed tomography(CT) and magnetic resonance(MR) image revealed a huge cystic lesion with enhancing nodule at the anterior wall of the cyst. Cerebral angiography showed a small residual nidus. Aspiration and drainage of the cystic fluid via a burr hole was performed and the symptoms improved. One year later, in March 2000, the symptoms recurred, and CT and MR image revealed a new huge cystic lesion anterior to the enhancing nodule. Surgical removal of the residual nidus was performed, and there is no recurrence of the cyst for two years till now.
