2.Intracardiac Foreign Body: A Sewing Needle in Right Ventricle of Unknown Etiology.

A 34 year-old woman was hospitalized with anterior chest pain and indigestion. Chest radiograph and computed tomogram revealed a sewing needle in the cardiac cavity. She had no histories of surgical intervention, drug abuse, or acupuncture. We removed the needle from the right ventricle under cardiopulmonary bypass.

3.A Case of Chondrodermatitis Nodularis Chronica Helicis.

A 51-year-old man had small, tender, skin colored firm nodule on the helix of the both ears for eight months. He was found to have classic features of chondrodermatitis nodularis chronica helicis, namely, tender nodule less than 1 cm in length with central crusting, localization to helix, age, men, and histologic findings. The histologic examination of the biopsy specimen showed the presence of thinned epiderrnis on center. acanthosis on the adjacent epidermis, with hyperkeratosis and parakeratosis, and cleft between epidermis and dermis. In the dermis, highly vascularized connective tissue and chronic inflarnmatory infiltrate were visible with perichondrial fibrosis. Elastic tissue stain showed degeneration of elastic fibers. After complete excision, no recurrence was obsered until now.

5.Clinical Reviews on 28 Patients with Ulcerative Colitis.

The ulcerative colitis is a relatively common disease in the European and North American countries aince Dr. Wilks has first reported the cases in 1895, But in Korea, this disease is a rare entity and only a few case have been reported. However, rescently the diagnoetic methods for ulcerative colitis are much developed we have experienced 28 cases of ulcerative colitis which were diagnosed by endoscopy, barium enema exam and biopsy. Therefore a total of 28 patient with ulcerative colitis diagnosed and treated at the department of internal medicine of HYUH from June 1979 to June 1986 was reviewed. We obtained the results as follows; 1) The sex distribution assumed a ratio of 1 to 1.33 with 12 males and 16 females. 2) The age dietribution was relatively even but the majority of cases were between 5th and 6th decade (32.1%), 3) The most prominent clinical symptoms were rectal bleeding, abdominal pain, diarrhea, fever, wight loss and vomiting. 4) Significant laboratory findins were anemia, eleveited ESR, leukocytosis, positive stool OB, electrolyte imbalance and decreased serum albumin level. 5) As to the extent of disease determined by the barium enema examination and endoscopic exam., rectum or rectosigmoid colon is involved in majority of the cases (92.8%). 6) The endoscopic examination was performed in all 28 cases and showed significant findings such as ulceration, hyperemia, mucosal friability, bleeding etcs. 7) The barium enema examination was performed in 19 cases and showed positive findings such as ulceration, luminal narrowing, bowel shortening and rigidity in 17 cases. 8) 21 of 28 patients treated by medical therapy showed relatively improved but 4 of 28 patients was exacerbated or relapsed and one has expired.

7.The Roll of Continuous ICP Monitoring for the Application of Shunt Operation in Adult Ventriculomegaly Patients.

We evaluated one month follow-up results of the 12 adult patients who showed ventriculomegaly in the aspects of symptoms. Evans ratio, periventricular low density, grades of radio-isotope cisternography, and grades of continuous intracranial pressure monitoring that were classified on the pressure wave and basal pressure level. The grades of continuous ICP monitoring were compared to the grades of radio-isotope cisternography. History, symptoms, CT findings, and radio-isotope cisternography are often helpful to decide shunt operation but sometimes continuous intracranial pressure monitoring gives invaluable information.

8.Antibiotic Concentrations of Cerebrospinal fluid in Patients with Various Amount of Extraventricular Drainage during Intraventricular Infusion.

Phamacokinetic study of the antibiotic concentrations in the cerebrospinal fluid was done with high pressure liquid chromatography of CSF samples from twelve patients who had various amount of continuous extra-ventricular drainage during antibiotic administration. Five patients with large amount(over 150cc/day) of drainage had decreased CSF antibiotic concentrations dramatically below the effective concentration within 4 hours after intraventricular antibiotic administration, but six patients with small(less than 150cc/day) drainage maintained CSF antibiotic level above effective concentration even 24 hours after administration. In conclusion, repeated intraventricular administration of antibiotic is necessary to maintain the effective drug level in the patient with CSF drainage over 150cc/day.

9.Efficacy of Intra-Aortic Balloon Pump in Postcardiotomy Cardiogenic Shock.

BACKGROUND: Intra-aortic balloon pump (IABP)is well known for its hemodynamic benefit but still has its own complications.Proper use of IABP is the best way t o obt ai n maximum benefit with low complication rate. MATERIALS AND METHOD: Twenty one(men 10,female 11) patients were included in this study among the 100 consecutive adult cardiac surgery patients in our hospital.Eighteen(85.7%)were ischemic heart disease patients.They all received IABP therapy due to postcardiotomy cardiogenic shock according to the well-known indications.Their preoperative conditions,intraoperative factors including hemodynamics, postoperative conditions and IABP-related complications were analyzed. RESULT: Nineteen patients(90.5%)were successfully weaned from IABP.There were 2 patients of operative death and the mortality rate was 9.5%.Duration of IABP use was 40.7+/-24.3 hours.There were 2 cases(9.5%)of IABP-related vascular complications that required surgical intervention. CONCLUSION: We concluded that IABP could be used effectively and safely for postcardiotomy cardiogenic shock patients with low complication rate.
