1.A Study on the Parents of Adult with the Mental Disabilities: Comparison between Parents of the Mentally Ill and Parents of the Mentally Retarded.

OBJECTIVE: As social welfare practice for the disabled has been changed institution -based to community- based care, burden of primary care takers to the mentally disabled(generally, their parents) was increased. This study aims to compare the parents of the mentally ill and the parents of the mentally retarded regarding social support, care burden, level of permanency planning, and type of residential planning and to finding out service needs of the each group. METHOD: For the purpose of the study, 93 parents of the adult with the chronic mental illness and 99 parents of the adult with mental retardation conducted a survey which included social functioning level of the mentally disabled, care burden, social support, level of permanency planning, type of residential planing and service needs. RESULTS: Even though little differences of social functioning level was found, significant differences existed between the two groups regarding care burden and social support. Parents of the mentally ill were perceiving more burden and less social support than parents of the mentally retarded. And they were having pooer permanency planning than parents of the mentally retarded. However little difference was found between the groups regarding type of residential planning. Fifty-one percent of the parents were having a plan for institution and 28% of them were having a plan for living with offsprings without mental disabilities as residential planning. The most needed services of all subjects were institution and community living facility. Secondly, it was income security, such as disability allowances, financial support to caretaker, and so on. CONCLUSION: Parents of the mentally ill are perceiving more burden than parents of the mentally retarded by reason of the fluctuating course, aggressive symptoms, unpredictable prognosis of mental illness. Therefore this study concluded that policy for mental health which secure various types of community living facilities was needed. At the same time, income security was strongly needed for social integration of the mentally disabled. In the mental health professions, it is needed to have more active interest of meeting needs of the parents.

2.Nevus Sebaceous with Special Reference on Its Aging Effect.

A histopathological study was performed on nevus sebaceus to observe its aging effect based on 75 cases of neuvs sebaceus those were collected during the past 10 years from three university hospitals in Seoul. The results are as follows: 1) Clinical findings The incidence was most frequent in the teenage group. The 75 cases consisted of 41 males and 34 females with a sex ratio of 1.2:1. Most of cases developed in the head and neck areas with 62.7% on the scalp and 29.3% on the face. 2) Histopathologic findings. The epidermal changes such as acanthosis(40%), papillomatosis(73%), hypergranulosis(44%) were most remarkable in the second decade and gradually decreased with aging. The apparent proliferation of sebaceous gland was observed in 73% and it was most prominent in the second decade. Apocrine glands were absent before the first decade but apparently increased after then. Proliferation of eccrine gland was not significant in all the age groups. Mild increase of immature hair follicles were noted in 49% of our cases with gradually decreasing tendency in the older age. The dermal inflammatory infiltrates were noted from the 2nd decade(28%) and thereafter gradually increased. Associated neoplasms were one apocrine adenoma, one sebaceous adenoma, two trichilemmomas and two arteriovenous hemangiomas. The majority of tumors occured in the third decade. Therefore, it is observed that neuvs sebaceous undergoes dynamic histopathologic changes according to the age of patient and later develop various secondary neoplastic changes. The pathogenesis of the nevus sebaceus is suggested to be closely related with developmental anomalies of primitive hair germ units in fetal stage.

3.Relationship of Nutritional Status at the Time of Admission to Length of Hospital Stay ( LOS ) and Mortality : A Prospective Study Based on Computerized Nutrition Screening.

This study was prospectively conducted to investigate any relationship of nutritional status at the time of admission to length of hospital stay and mortality. All patients admitted to the Asan Medical Center between October 13 and November 12, 1997 who met the study criteria were included in the study. Patients were classified as Not-at-risk, At-risk I or At-risk II based on the levels of serum albumin and total lymphocyte count in a computerized nutrition screening program. Sixty three percent of the patients were classified as Not-at-risk Group, 29% as At-risk Group I and 8% as At-risk Group II. Significant correlation was observed between nutritional status and LOS (P<0.01) as well as mortality rate (P<0.05). The more the patient had the nutritional risk factors, the longer the LOS and the higher the mortality rate were. Further studies have to be done in order to demonstrate cost-effectiveness of medical therapy for the malnourished hospitalized patients.

5.The comparative study for occlusal plane between articulated cast model and cephalogram in orthogantihic surgery patients.

The common errors in preoperative treatment plan for the orthognathic surgery can be occurred during cast impression, cast mounting procedure with face-bow transfer, surgical stent fabrication, and so on. One of the most common errors exists during mounting process of the model on the articulator. Accurate mounting of dental casts to articulator should be achieved by transferring the 3- dimensional spatial relationship of the maxillary arch to an articulator. A face-bow is used for transfer this relationship to articulator, usually by relating the face-bow to a plane of reference of maxillary cast. The purpose of this study is evaluation of the accuracy of face-bow transferring of maxillary model to the articulator. The maxillary casts of thirty patients for orthognathic surgery were mounted on articulator with an face-bow instrument. The relationship of occlusal plane angle to Frankfort horizontal plane relations were compared the cephalogram with the cast-mounted articulator. As a result of this study, the significant difference between the maxillary occlusal planes angle in the cephalogram and articulator were found . The results were followed,1. The mean occlusal plane angle in cast-mounted articulator was 13.5.(SD+/-5.4).2. The mean occlusal plane angle in cephalogram was 10.4.(SD+/-4.3).3. The mean difference of occlusal plane angle between cast-mounted articulator and cephalogram was 3.3.(SD+/-4.6).According to the result, we should suggest that the occlusal plane angle to Frankfort plane in cast-mounted articulator is more steeper than that of cephalogram.And then, maxillofacial surgeon should try to get a more predictable result by suggesting the proper correction method and mounting the cast accurately.

6.Immunohistochemical Observation of Placental Form of Glutathione S-Transferase in Squamous Cell Carcinoma.

Glutathione S-Transferase (GST) is a conjugation enzyme in the metabolism of exogenous and endogenous lipophilic compounds for their excretion and detoxification. Acidic isozyme of GST, GST-Pi, has been recognized as a preneoplastic marker in the experimental hyperplastic nodules of liver in rats, and GST-Pi is abundant in the squamous cells of the skin, also. This histochemical study was carried out to evaluate the distribution and the relationship between the differentiation status of squamous cells in dysplastic or neoplastic epithelium in various organs. The human placental form of glutathione S-transferase (GST-Pi) were stained immunohistochemically with specific anti GST-Pi rabbit antibody in 23 cases of human squamous cell carcinomas. The patients consisted of 14 cases from the uterine cervix, 3 cases from the esopahgus, 3 cases from the lung and 3 cases from the larynx. The results obtained were as follows; 1. Basal cells in normal mucosa were stained negative for GST-Pi while superficial keratinocytes were stained moderately positive. Basal dysplastic cells were stained negatively or weakly positive. Carcinoma cells especially large cells either keratinizing or nonkeratinizing were stained moderately to strongly. Carcinoma cells surrounding keratin pearl were strongly reacted with GST-Pi than other carcinoma cells. 2. Differentiated cells of squamous cell carcinoma showed moderate to strong positive reaction to GST-Pi staining irrespective of its site of origin. 3. Therefore, Immunohistochemical staining pattern of GST-Pi in various squamous carcinoma cells showed similar immunohistochemical reaction to the GST-pi, which is closely correlated to the degree of differentiation, keratinigation and also suggested that squamous carcinoma cells had abundant GST-Pi related detoxifying system.

7.Attitudes of Adolescents toward Suicide: Q-Methodological Approach.

PURPOSE: The purpose of this study was to identify attitudes of adolescents toward suicide. METHODS: Q-methodology which provides a method of analyzing the subjectivity of each item was used. Thirty middle and high school students classified 37 selected statements into a normal distribution using a 9 point scale. Collected data were analyzed using the Quanl PC Program. RESULTS: Three types of attitudes toward suicide were identified. The first type (opposing suicide-moral minded) showed an attitude of opposing suicide and thinking that suicide is a sin. The second type (understanding-empathizing suicidal person) showed an attitude of understanding the situation of the adolescents who has suicidal ideation and empathizing with them. The third type (ambivalent attitude) showed an attitude of understanding the suicidal person but, at the same time, opposing suicide. CONCLUSION: Results of the study indicate that different approaches to suicide prevention programs should be developed based on the three types of suicide attitudes among adolescents.

8.Developing a Tool to Assess Competency to Consent to Psychiatric Hospitalization (KATOC): Reliability and Validity.

OBJECTIVE: The justification of informed consent requires that a patient be provided with the information necessary for deciding treatment and able to use such information based on reasonable thinking. The clinical decision to consider anyone who has mental disorder as incompetence without objective assessment does not only encroach human rights of the persons with mental illness, but seriously prevent them from being recovered. Hence the objective assessment of competency is needed in mental health. Our study aimed to develop the Korean Tool of Competency to Consent to Psychiatric Hospitalization and to analyze the reliability and validity of this tool. METHODS: Totally 98 patients with mental illness who were hospitalized in mental hospital, participated in this study. For the subjects a questionnaire composing of 22 questions of understanding, appreciation, reasoning and expression of a choice was used. To investigate validity of this tool, MMSE-K, insight test, estimated IQ, BPRS were conducted. Its reliability and usefulness were examined with Cronbach's alpha, ICC and ROC analysis respectively and criterion related validation performed. RESULTS: As results, this tool shows that agreement between raters is relatively high and the confirmatory factor analysis for constructive validation shows that the tool is valid. Also, for criterion related validation, estimated IQ, insight and MMSE are significantly correlated to understanding, appreciation and reasoning. However competence to express a choice did not show any significant correlation with criterion variables, nor showed BPRS any significant correlation with sub-competences. CONCLUSION: Our study developed the Korean Tool of Competency to Consent to Psychiatric Admission Treatment in the Mentally Ill, verified the reliability and validity of the tool and analyzed the optimum cutoff to distinguish between competence and incompetence in sub-competences. Korean Assessment Tool of Competency to Consent to Psychiatric Hospitalization (KATOC), analyzed the reliability and validity of this tool and presented the cutoff points by subarea. As a result, the reliability and validity of satisfactory levels were verified, the ROC analysis was implemented based on the clinical assessment and the cutoff points were found in understanding, appreciation, expression of a choice and reasoning. Such findings showed that the tool developed by researchers could be very favorably used in Korea.

9.Sports injuries and the changes in physical activity, perceived health state and exercise self-efficacy according to the sports injuries of the elderly who participate in physical activities.

PURPOSE: The purpose of this study was to describe sports injuries among elderly people and to compare physical activity, perceived health status and exercise self-efficacy between elderly people who had experienced a sports injury and elderly people who hadn't. METHODS: The sample of this study were 100 elderly in Korea, 51 of whom had had a sports injury. The study used questionnaires to gather data. There were 8 demographic factors, 4 factors related to physical activity, perceived health status and exercise self-efficacy of the subjects. Data were analyzed by frequencies, chi-square test, and t-test. RESULTS: Among the sports injured elders, one occurrence of a sport injury was 86.3%. There were more outdoor than indoor sports injuries of elders (66.7%), and sports injuries of elders occurred more in the winter (82.7%). The major cause of the sports injuries was loss of balance. Ankles were frequently damaged from these injuries. The most significant change after the sports injury was reduction of physical activity (60.8%). There were significant differences between sports injured elderly and non-injured elderly for age (χ2=2.58, p=.011) and the amount of physical activity (t=1.96, p=.050), but there was no significant difference in perceived health status and exercise self-efficacy. CONCLUSION: To prevent sports injuries in the elderly means to maintain a medium level of physical activity and to improve their balance.

10.Malignant Acanthosis Nigricans Associated with Stomach Adenocarcinoma.

Malignant acanthosis nigricans is very rare disease. The disease is related with internal malignancy, especially gastrointestinal tract malignancy. Reported occurrence rate of malignant acanthosis nigricans associated internal malignancies are 64% of stomach adenocarcinoma, 27% of other organs of abdomen (liver, gall bladder, small intestine, rectum, uterus, ovary), 9% of non-abdominal organs(breast, lung). We report a case of malignant acanthosis nigricans associated with stomach adenocarcinoma from 20-year-old female and discuss review of literature on a acanthosis nigricans briefly.
