1.A radiologic findings of periappendiceal abscess : comparison of simple abdomen, B.E., and ultrasonography

With simple abdomen, double contrast colon study, and ultrasongraphy, authors retrospectively analyzedradilogic findings of 46 cases which were proved periappendiceal abscess, due to perforation of appendix, byhistology or follow-up ultrasnongraphy from May in 1983 to July in 1985 at Yeung-Nam University Hospital. Theresuls obtained were as follows: 1. Of 46 cases, 24 cases were males and 22 cases females, with the results ofabove statistics, periappendiceal abscess was not influenced by sex. 2. Periappendiceal abscess was mostfrequently demonstrated in forties. 3. Comparison of ultrasonography and B.E. Pseudorenal pattern(Ultrasonography)- well defined defect (B.E.) Cystic pattern(Ultrasonography)- serrated defect (B.E.) 4. Missed 2 cases inultrasonography were detected in B.E., i.e. sensitivity of B.E. is higher than that of ultrasonography. 5.Differences of ultrasonography and B.E.

2.An Analysis of Articles for International Marriage Immigrant Women Related to Health.

OBJECTIVES: This study was for analyzing the research about international marriage immigrant women and a trial to find the right direction for future research. METHODS: Sixty articles published from June, 2004 to June, 2009 were reviewed and analyzed according to the general characteristics, major of author, and theme of health domains. RESULTS: Most of them were master's thesis(71.7%) and journals(21.7%) and doctoral dissertation(6.7%) have been published mostly after thesis. Among 83.3% for quantitative research, descriptive(33.3%) and descriptive correlation(41.7%) methods were the most used and there were some qualitative researches(16.7%). The most frequently used data gathering method was questionnaire(81.7%) and the next was interview(16.7%). The major rates of the author were 61.7% for social welfare and 2.1% for nursing. The investigated variables in social health domain were adaptation(28.3%), and communication(1.7%). In psychological health domain, marriage satisfaction(16.7%), life satisfaction(11.7%), and depression(10.0%) were most researched. Utilization of medical center(5.0%) and health promotion behavior(1.7%) were investigated in physical health domain. CONCLUSIONS: Above this, most articles were researched about the adaptation of international marriage immigrant women. But the life in foreign countries can cause physical and psychosocial unhealthy conditions, so many-sided health related researches are supposed to be conducted for adaptation and prevention health problems of international marriage immigrant women.

4.US Characterization of Soft Tissue Hemangiomas of Extremities: Correlation with MR Signal Intensity.

PURPOSE: To evaluate the US findings of soft tissue hemangiomas in extremities and to correlate the echo-pattern with MR signal characteristics of hemangiomas. MATERIALS AND METHODS: We retrospectively studied forty-two patients either with pathologically proven cases or with characteristic features of hemangiomas on MRI, US and plain film. Hemangiomas were divided into two types according to the locations;subcutaneous and intramuscular. Analytic points on US and MR findings were gross morphology of the mass, internal echo-pattern or signal characteristics. We correlated the echo-pattern with MR signal characteristics in hemangiomas. RESULTS: Subcutaneous hemangiomas revealed homogeneously anechoic mass on US, which were well correlated with MR signal characteristics' homogeneous low singnal intensity(SI) on Tl-weighted image(WI) and high SI on T2-WI. Intramuscular hemangiomas showed heterogeneously mixed echoic masses on US. An anechoic component on US corresponded to dilated vascular channels on MRI. The signal intensity of intramuscular hemangioma was less than that of subcutaneous fat on T1-WI and greater than that of fat on T2-WI. Both types of hemangiomas had tubular or lace-like appearance with interspersed fibrofatty septa. CONCLUSION: The echo-patterns in US were well associated with MR signal characteristics on MRI. Regarding US and MR features, intramuscular hemangiomas were different to subcutaneous hemangiomas.

5.Pain Reduction Effects of Continuous Fentanyl Infusion to Intensive Care Neonates.

PURPOSE: To determine whether pain control using fentanyl would effectively reduce behavioral distress, physiologic and hormonal responses to postoperative pain and mechanical ventilation in neonates. METHODS: Neonates who were admitted to the NICU(neonate intensive care unit) of Kyungpook University Hospital, requiring surgery or mechanical ventilation were evaluated. Twelve neonates undergoing operation were randomised into two groups. After operation, one group received fentanyl and the other was given saline. Behavioral distress using postoperative comfort scores, heart rate, blood pressure and blood glucose were evaluated before and after operation. Cortisol concentration and beta endorphin were measured before and at the end of operation and at 60 minutes after fentanyl infusion. The values were compared between the two groups. In eight neonates on mechanical ventilation, behavioral distress using pain scores, heart rate, blood pressure, oxygen saturation and concentrations of cortisol and beta endorphin were measured before and 60 minutes after fentanyl infusion. RESULTS: In neonates undergoing surgery, fentanyl infusion diminished the elevation of post operation heart rate and blood glucose(P<0.05) and induced the improvement of postoperation comfort scores(P<0.05). Cortisol levels were significantly lower in neonates receiving fentanyl(P< 0.01) at 60min after infusion. In neonates on mechanical ventilaion, fentanyl infusion diminished the elevation of systolic blood pressure and heart rate on endotracheal suction(P<0.05). At 60 minutes after infusion, cortisol levels were markedly lower in neonates receiving fentanyl(P<0.05). CONCLUSION: Neonates feel pain during postoperation and on mechanical ventilation. As fentanyl therapy appears to be effective in controlling the pain, anaesthetic agents should be given to all neonates undergoing painful procedures or under painful situations.

6.Hurthle Cell Tumor of the Thyroid Gland in an Infant: A case report.

Hurthle cell tumors are an infrequent neoplasm of the thyroid gland in adults. Hurthle cell tumors represent 4. 5% to 10% of all primary thyroid epithelial neoplasms in the foreign literature. It has been known as Hurthle cell tumor since Ewing was the first to use this term in 1928. Tumor occurring in an infant was described by Symmers(1941) and Morrow(1945). The authors experienced a case of congenital Hurthle cell tumor of the thyroid gland in a 2 months old boy. He was admitted to the pediatric surgical department because of a growing mass in the neck since birth. Ultrasonogram showed a huge lobulated homogenous solid mass with medium level echogenicity in the region of the thyroid gland. Subtotal thyroidectomy of right lobe and total thyroidectomy of left lobe were done. The specimen measures 2x3x1.5 cm and 7x3x3 cm, respectively. It was an encapsulated, yellow gray firm and solid mass. The cut surface was smooth, glistening and homogenous. Microscopicully, the tumor was composed of tightly packed regular follicles lined by polyhedral, cuboidal, large cells with a granular acidophilic cytoplasms. The nuclei are vesicular, usually only a little bit larger than those of normal thyroid cells.

7.Influence of Self-Esteem and Family-Support on Powerlessness of Hospitalized Elderly Patients with Chronic Disease.

PURPOSE: The purpose of this study was to identify an influence of self-esteem and family- support on powerlessness of hospitalized elderly patients with chronic disease. METHOD: The subjects were 151 hospitalized elderly patients, age over 60, with chronic disease and admitted for at least 1 week. The data were collected by individual interview using a tructured questionnaire during the period from July 10th to August l0th, 2003 from three general hospitals in Busan. The data were analyzed by descriptive statistics, t-test, ANOVA, Pearson's correlation, hierarchical multiple regression. RESULT: The level of self-esteem, family support, and powerlessness was 38.00, 38.26, and 38.38, respectively. There was a significant positive correlation between self-esteem and family support and a negative correlation between self-esteem and powerlessness and between family support and powerlessness. Self-esteem and family support were each significant predictor of powerlessness. CONCLUSION: This study showed the hospitalized elderly patients need greater family-support and higher self-esteem to relieve the level of powerlessness. I suggest to study for replication in a larger sample size and considering the lengths of hospitalization for generalization of this study and to develop individual intervention programs for increasing family support and self esteem and testify their effects on the relief of powerlessness of the elderly.

8.Relationship between Fatigue, Sleep Disturbance, and Gestational Stress among Pregnant Women in the Late Stages.

PURPOSE: The objective of this study was to investigate the relationship between fatigue, sleep disturbance, and gestational stress in women during late stage of pregnancy. METHODS: This study was conducted with 113 healthy pregnant women at gestational age greater than 27 weeks who were registered at community health centers and received prenatal care. A structured questionnaire regarding fatigue, sleep disturbance, and gestational stress was used. The data was analyzed using a t-test, an ANOVA, and Pearson correlation coefficients. RESULTS: The subjects with unplanned pregnancies and irregular exercise patterns showed a higher level of fatigue than those with planned pregnancies and regular exercise patterns. Pregnant women with caffeine intake manifested higher levels of gestational stress and sleep disturbance than those without. The levels of sleep disturbance and gestational stress increased as the fatigue levels increased. The fatigue levels increased with increased levels of sleep disturbance. CONCLUSION: Planned pregnancy, regular exercise patterns, and caffeine intake were related with fatigue, sleep disturbance, and gestational stress in women during late stages of pregnancy. Fatigue, sleep disturbance, and gestational stress had close associations to each other. In the future, such results should guide development of nursing intervention programs for women in late stages of pregnancy.

10.Dermatophagoides Pteronyssinus Specific IgG-subclass Antibodies in Allergic Children.

PURPOSE: Although immunoglobulin E play a major role in immediate hypersensitivity reactions, some reaginic activity is associated with IgG. Part of the reaginic activity is associated with IgG1, IgG4. In contrast with this reaginic activity of IgG4, allergen-specific IgG4 antibody is allergen neutralizing antibody and contribute to the benificial effect of immunotherapy. The role of specific IgG antibody in allergic disorder is uncertain. The Dermatophagoides pteronyssinus is well known as the major allergen triggering allergic disorder. For determining the distribution of specific IgE and IgG subclass antibodies and their role in allergic disorders. Mehtods : Specific IgE and IgG subclasses to Dermatophagoides pteronyssinus (Dp) were measured in 62 allergic children (Group I;Bronchial asthma, Group II; Bronchial asthma +Allergic rhintis) aged 8-13 year. All of them had positive skin test to Dp and every data were compared with thet of 42 normal control. RESULTS: 1) Dp specific IgE antibody titer was significantly higher in Group I, II, III than in control group (p< .01). 2) pecific IgG, IgG1, IgG2, IgG4 antibodies were higher in all three allergic groups than in control group (p< .01). 3) Specific IgG, IgG1, IgG2, IgG3, IgG4 antibodies were higher in patient with asthma and allergic thinitis than asthma or allergic rhnitis. 4) The linear correlation between specific IgE and IgG, IgG1 or IgG4 was not significant (p< 0.05). CONCLUSIONS: Specific IgE, IgG and IgG subclass antibodies were increased in allergic disorders than normal control. The level of specific IgG and IgG subclass antibodies were higher as the clinical symptoms were more severe.
