1.A Case of Central Diabetes Insipidus Associated with Brachycephaly.

Brachycephaly is a kind of craniosynostosis. Because of premature closure of the coronal suture, the skull is shorter in the anteroposterior diameter but is widened with a high vault and the occiput and forehead are flattened. Diabetes insipidus had been reported in oxycephaly. We have experienced a case of central diabetes insipidus associated with brachycephaly. A brief review of related literatures is included in this report.

2.Epidemiology of Patients with Superficial Fungal Diseases and Their Family Members in Korea.

BACKGROUND: Superficial fungal disease is one of the most common human infectious diseases. A recent clinical survey reported that more than half of patients with superficial fungal diseases might be infected at home. OBJECTIVE: The aim of this study is to investigate the correlation of the fungal infection between the patients and their family members. METHODS: Dermatologists of 71 general hospitals were involved and evaluated 7,323 patients and 1,037 their family members who were confirmed microscopically to have superficial fungal diseases. RESULTS: According to the survey results, 46.1% of the patients answered they might have been infected at home and 33.6% of the patients remembered there were other fungus-infected patients among their family members at the same time. The isolated organisms were Trichophyton rubrum (1686 cases, 80.9%), Trichophyton mentagrophytes (269 cases, 12.9%), Candida albicans (112 cases, 5.4%), Microsporum canis (12 cases, 0.6%), Trichophyton tonsurans (3 cases, 0.1%), Epidermophyton floccosum (2 cases, 0.1%). On the basis of the fungus culture, it was proven that 85.4% of the patients and their family members had the same kind of microorganism. CONCLUSION: As a result, we could conclude that there is a high possibility of transmitting the fungal infection from one family member to another.

3.Achilles Project: An Epidemiology Survey to Assess the Seasonal Incidence of Foot Diseases and Patient Compliance.

BACKGROUND: Dermatological diseases affecting the feet, e.g. fungal infections and nail infections, are often not fully perceived as real medical problems even when they experience great discomfort, including pain. These misperception often prevents from seeking timely treatment, which can result in very severe conditions. This survey, so called "ACILLES PROJECT" was a "foot screening" project that would be focused on all parts of the body below the Achilles heel: foot, toe and toenails. OBJECTIVE: This study was undertaken for both medical doctors and patients to raise the awareness of foot diseases and additional insights into disease that could affect foot. And also, in seasonal variation, we tried to compare the epidemiological data and to look over the relationship of dermatophytosis in other parts with fungal infections in foot and to analyze the compliance and efficacy of antifungal agents in the treatment of patients who were diagnosed as dermatophytosis in their feet in the treatment of antifungal agents. METHODS: Achilles survey was conducted for one week from 7th July to 13th July, 1997 and 16th March to 17th April, 1998 co-worked by the Korean Society for Medical Mycology and Janssen Korea. In 1997, 19,298 patients, who visited dermatology and urology, and 353 investigators participated in this survey. And Achilles 98, in 1998, was composed of two parts, Unit I and Unit II. Unit I survey was a foot disease consult program and Unit II was a patient compliance survey 29,922 patients, 391 investigators and 16,383 patients, 350 investigators were involved in Unit I and Unit II respectively. RESULTS: Among total 29,922 patients who responded to the survey, 57.7% of them had more than one foot disease. There was no big difference comparing with the number (67%) in the survey of last summer. And it was observed that fungal infection was the most dominant disease (79.8%) regardless of seasons (83.7% in July, 1997). Furthermore, there were no significant differences in the prevalence of other foot diseases for each season (eczema 11.1%, corns 5.1%, warts 2.7%, psoriasis 2.1%). Oral antifungal agents were prescribed to the patients who were diagnosed as having fungal infections in their feet in order to survey drug compliance by filling out the patients diary. In compliance, only 24% of patients in itraconazole pulse therapy for 28 days and 16% of patients in itraconazole continuous therapy for 28 days compiled with their dosage guideline exactly. It is observed that the longer the duration of drug therapy is, the worse the patients compliance is. CONCLUSION: According to the epidemiological survey in 1997 and 1998, the prevalence of foot disease was not significantly different seasonally, and it was reported that the awareness of and the intention for the treatment of foot diseases were not high enough based on the compliance data of patients.

4.Primary Cardiac Angiosarcoma: A case report and rewiew of literture.

We report a case of primary cardiac angiosarcoma in a 35 year-old woman. She presented with dyspnea, facial edema and neck vein distension during 1 month. The oval round large mass(8x4 cm) filling the right atrium infiltrated into the entire thickness of the lateral wall and extended to the vena cava and ventricle. The interatrial septum and pericardium seemed to be intact. Despite the surgical excision and adjuvant chemo-and radiotherapy, the patient died 8 months later due to tumor recurrence. The histologic findings of tumor varied from benign looking capillary proliferation mimicking granulation tissue to sarcoma composed of spindle cells. But anastomosing vascular channels lined by anaplastic polygonal cells and intracytoplasmic lumen containing red blood cells were characteristic findings. These tumor cells showed positive reaction to endothelial cell antigen and factor VIII-related antigen. The intercellular junctional complex and vascular channels formed by cytoplasmic process of tumor cells were identified by the ultrastructural study.

5.Microcystic Adnexal Carcinoma: Report of two cases.

The clincopathologic features of microcystic adnexal carcinoma are presented. Microcystic adnexal carcinoma is a recently described, very rare neoplasm characterized by a locally aggressive growth pattern, and as far as we know, only two reports on the microcystic adnexal carcinoma have been published in the Korean literature. Recently we experienced two cases of microcystic adnexal carcimoma occurred in a 32-year-old male and 27-year-old female patient. They had 2.5 x 2.0 x 2.0 cm and 1.2 x 0.9 x 0.9 cm sized, slowly growing mass in the glabella and below the lower lip, respectively. The immunohistochemical staining for CEA antigen stains the glandular structures but not pilar structures. So we thinked that this tumor would be originated from primitive adnexal cells differentiating into both pilar and eccrine structures.

6.Histopathologic Re-evaluation of Thymoma with Immunonhistochemical Study for bcl-2 and MIC-2 Protein.

We reviewed 86 thymic epithelial tumors and reclassified them according to the Kirchner and Muller- Hermelink classification. They were subtyped as medullary, mixed, predominantly cortical (organoid), cortical, well differentiated thymic carcinoma, and poorly differentiated thymic carcinoma. The frequency of each subtype was determined and histologic findings were related to stage and myasthenia gravis. Immunohistochemical stains for bcl-2 protein as a marker for medullary thymocytes and MIC-2 protein as a marker for cortical thymocytes were performed in each case. The stages and association of myasthenia gravis was significantly different in each subtypes. The results of this study demonstrate that this histogenetic classification is clinically applicable. The bcl-2 protein was specifically demonstrated in lymphocytes within areas of medullary differentiation and MIC-2 protein in cortical differentiation. The expression of bcl-2 and MIC-2 proteins lend histogenetic support for this new classification of thymoma. Bcl-2 protein is strongly expressed in tumor epithelial cells of every case of poorly differentiated thymic carcinoma whereas the other types of thymic epithelial tumors do not show epithelial expression of this protein. The strong expression of bcl-2 protein in tumor epithelium may be considered as a predictor of aggressive behavior in thymic epithelial tumors.

8.Inhibitory Effects of Structurally Different Neuromuscular Blockers on the Serotonin Type 3 Receptor Expressed in Xenopus Oocytes.

BACKGROUND: The serotonin type 3 receptors are diffusely distributed in both the central and the peripheral nervous system. Physiological and pathophysiological processes thought to be mediated by this receptor include nausea and vomiting, peripheral nociception and central antinociception, conditioned aversion response to drugs, anxiety, and cognition. Because of the structural similarity between the nicotinic acetylcholine receptor and the 5HT3 receptor, we investigated the effects of clinically used neuromuscular blockers on the 5HT3 receptor function related with PONV. METHODS: A cDNA clone encoding the full length murine 5HT3a receptor was subcloned into an oocyte expression vector and 50 ng of cRNA transcribed in vitro injected per oocyte. After 24 72 h incubation, oocytes were placed into a recording chamber continuously perfused with frog Ringer's solution and electrophysiological recordings were obtained by the two electrode voltage clamp technique. Serotonin with or without the various drugs were bath applied by a computer controlled solenoid valve. Peak currents induced by the drug applications were measured and dose responses were obtained. RESULTS: The 5HT3 receptor expression in Xenopus oocyte was identified by the pharmacologic tools. Serotonin induced rapid inward currents, and thus was showed dose-dependent: KD = 2.5 micrometer, Hill coefficiency = 2.09. Inhibition by the neuromuscular blockers showed dose-dependence and their inhibitory potency on 5HT3 receptor (IC50) was in order of d-tubocurarine (0.046 micrometer) > vecuronium (16.32 micrometer) > gallamine (1,169 micrometer). CONCLUSIONS: There was a different inhibitory effect of nicotinic cholinergic antagonists, clinically used neuromuscular blockers, on the 5HT3 receptor and a judicious selection of them might contribute to reducing the incidence of PONV clinically.

10.Analysis on Production Processes between Conventional and Commissary School Foodservice Systems in Kyunggi-do.

This study was designed to analyze and compare production management practices and labor productivity between conventional and commissary school foodservices and 46 dietitians of commissary school foodservices in Kyunggi-do. The response rates were 89.7% and 91.3%, respectively. The number of meals served was ranged from less than 100 to over 1,900 in conventional school foodservices and from 200 to 1,600 in commissary foodservices. Thirty three conventional foodservices(42.3%) produced less than 300 meals per day. Numbers of satellite school per central kitchen were ranged from 1 to 5 schools ; fifty percent of commissary foodservices have contained 3 satellite schools. Meals for satellite schools were transported between 11:00 a.m.and 12:00 a.m ; transportation time was from 10 to 90 minutes. Waiting time before service in satellite schools was between 10 minutes and 80 minutes. Part time employees supported by parents were hired in 37.3% of conventional and 50% of commissary school foodservices. Voluntary workers were supplied for 64.9% of conventional and 52.4% of commissary school foodservices. Labor productivity was calculated from work schedule. Labor productivity(labor minutes per meal)was lowest in foodservices with 101-4-- meals(8.48 min)was lower than that of foodservices with above 401 meals ; no significant differences were found among 401-700(6.02 min), 701-1,100(4.01 min), 1,101-1,500(3.41 min), and 1,501-1,900(3.15 min)meals in conventional foodservices. Labor minutes per meal of foodservices which served less than 400 meals(6.90 min) per day was significantly lower than those of foodservices which served 401-1,900 meal(3.41-4.92 min) in commissary foodservices(p<0.05)
