2.A Case of Tubular Apocrine Adenoma.

Tubular apocrine adenoma(TAA) is a rare benign neoplasm usually found on the scalp. It has been designated as an apocrine histogenesis on the basis of its ultrastructural characteristics, enzyme, and immunohistochemical phenotype. Histopathologically the neoplasm consists of tubules or cysts, which show signs of apocrine secretion and sometimes needs to be differentiated from papillary eccrine adenoma. We report a typical case of TAA on nostril which shows differentiation toward apocrine in nature.

3.A Study on the Correlations between Social Support, Dependence of Activities of Daily Living and depression in Patients with parkinson's disease.

The purpose of this study was to investigate the correlations between social support, dependence of activities of daily living and depression among Parkinson's disease patients. Subjects were 122 patients with Parkinson's disease. Neurologic wards inpatients or neurology clinic out patients department in a tertiary-affiliated hospital in Seoul. Data were collected from April 1996 to March 1997. The results of this study were summarized as follows: 1. The lower the level of dependence of activities of daily living, the lower the level of depression. 2. The higher the level of perceived family support, the lower the level of depression. 3. The higher the level of perceived health personnel support, the lower the level of depression. 4. The rate of depression evaluated by Zung depression scale was 37.4%(mild to moderate:30.1%, moderate to severe:4.1%, severe:3.2%). 5. Socio-demographic characteristics sex, education, occupation, and the charge person of treatment cost, significantly influenced the level of depression. 6. There was no significant difference in level of depression, dependence of activities of daily living, stage of disease between user and nonuser of L-dopa. Suggestions were made for further studies: 1. To develop nursing intervention for decreasing the dependence of activities daily living is needed. 2. To identify the change pattern of depression by time.

5.A Case of Ki - 1 Positive Lymphomatoid Papulosis.

Lymphomatoid papulosis, first described by Macaulay in 1558 is a chronic disease characterized by the recurrent self-healing papulonecrotic or nodular skin lision, showing histological features of malignant lymphoma. The clinical course is variable, with rogression to disseminated malignant lymphoma in up to 10-20% of patients. Ki-1 appears to the a representative marker for large cells ir lymphomatoid papulosis and these cells can be detected in l-1 lymphorna, Hodgkins diseasen other disorders with aetivated T cells. However, positive Ki-1 staining is useful for supporting a diagnosis of lymphomatoid papulosis, under the appropriate clinical and histopathologic findings. We report a case of positive lymphomatoid papulosis which is characterized by the typical findings of clinical, histojpathological and immunohistochemica sizdies.

6.Morphologic Characterization of Polycystic Kidney in inv Transgenic Mouse.

The aim of this study was to characterize the morphology of a polycystic kidney which was found in 100% of the transgenic mice homozygous for inv mutation and to gain insight into the pathogenesis of inherited polycystic kidney disease during the pre- and postnatal periods. The fetal and postnatal kidneys from the homozygous and heterozygous transgenic mice were examined by the light, transmission and scanning electron microscopes, image analyzer, and an immunohistochemistry utilizing the antibodies specific for each segment of the renal tubules (Tetragonolobus purpureas, Arachis hypogaea, Tamm-Horsfall protein, AE1/AE3, EMA, vimentin, Phaseolus vulgaris) was performed to determine the site of origin of renal cysts. Two developmental phases of a cystic disease were identified. The first phase, seen in fetal kidneys, was characterized by dilatation mainly of the proximal tubules and a few distal tubules. The later phase, in postnatal period, was characterized by progressive enlargement of the kidneys due to mainly cystic change of the collecting ducts, which distorted the normal architecture of both cortex and medulla and almost completely replaced the renal parenchyma. The cystic dilatation involved all segments of the nephron and the collecting duct as well as the Bowman's spaces of glomeruli. The epithelial cell hyperplasia was found as a micropolyp formation within the renal cysts and an increase in PCNA positive cells. These findings suggest that a cyst is not simply a ballooning of a renal tubule and the stretching of cells, formerly thought to be due to an altered compliance of an abnormal basement membrane, but indeed the result of increased numbers of tubular epithelial cells.

8.The Relationship of Ki-67, Proliferating Cell Nuclear Antigen, AgNORs and p53 Protein Expression in Astrocytoma.

The grading of astrocytoma has traditionally relied on histological assessment, but there are some differences in their parameters, application, and reproducibility. Recently, numerous studies have attempted to correlate biological aggresiveness with tumor proliferation index using new immunohistochemical methods. The purpose of this study is to correlate the histopathological grades of astrocytoma with the expression of Ki-67, PCNA, p53 protein and AgNORs. The paraffin sections of 41 consecutive cases of astrocytomas were examined. Histologically the tumors were graded as three groups under the St. Anne-Mayo system and showed 14 cases in grade II, 15 cases in grade III and 12 cases in grade IV (glioblastoma multiforme). As a result, the Ki-67 labelling index and p53 protein expression tended to increase with increasing grade of malignancy. But the univariate analysis showed that there was no significant difference between the tumor grades (p>0.05). The PCNA labelling index and number of AgNORs revealed striking differences between the grade II and grade III astrocytomas (p<0.05). We concluded that the PCNA labelling index and AgNORs counting are useful markers for differentiation between grade II and III astrocytomas.

10.Beta Dosimetry in Intraperitoneal Administration of 166Ho-chitosan Complex.

Intraperitoneal adminstration of radioisotopes is suggested to treat the metastatic ovarian cancer in the pertioneal cavity. Administering beta-emitting radioisotopes into the pertioneal cavity allows the maximum energy delivery to the cancerous cells of the pertioneal wall surface while sparing the normal cells located in deep site of the peritoneal wall. In this study, dose estimates of the peritoneal wall are provided to be used for prescribing the amount of 166Ho-chitosan complex administered. The 166Ho-chitosan complex diffused in the peritoneal fluid may attach to the peritoneal wall surface. The attachment fraction of 166Ho-chitosan complex to the peritoneal wall surface is obtained by simulating the ascites with Fischer rats. Both volume source in the peritoneal fluid and the surface source over the peritoneal wall surface are counted for the contribution to the peritoneal wall dose. The Monte Carlo code EGS4 is used to simulate the energy transfer of the beta particles emitted from 166Ho. A plane geometrical model of semi-infinite volume describes the peritoneal cavity and peritoneal wall. A semi-infinite plane of 10 micrometer in thickness at every 1 mm of depth in the peritoneal wall is taken as the target in dose estimation. Greater han 98 percents of attachment fraction has been observed from the experiments with Fischer rats. Given 1.3 microcurie/cm2 and 2.4 microcurie/ml of uniform activity density, absorbed dose is 123 Gy, 8.59 Gy, 3.00 Gy, 1.03 Gy, and 327 Gy at 0 mm, 1 mm, 2 mm, 3 mm, and 4 mm in depth to the peritoneal wall, respectively.
