4.Comparison of Propofol-N2O and Propofol-fentanyl Anesthesia for Outpatient Surgery.

BACKGROUND: The choice of anesthetic agents and adjuvants during outpatient surgery is of critical importance. Propofol is widely used for the induction and maintenance of outpatient anesthesia. Because propofol lacks analgesic properties, very high concentrations may be required when propofol is used as the sole anesthetic drug. Propofol is used with adjuvants such as nitrous oxide or opioid. This study was designed to evaluate the intraoperative hemodynamic response, recovery characteristics and side effects of propofol-fentanyl anesthesia compared with propofol-N2O anesthesia for outpatient surgery. METHODS: Twenty six healthy and unpremedicated patients scheduled for outpatient surgery were randomly allocated to receive either propofol-N2O anesthesia (N-group) or propofol-fentanyl anesthsia (F-group). The patients in N-group were ventilated with nitrous oxide 60~70% in oxygen and the patients in F-group were ventilated with oxygen 40% in nitrogen via laryngeal mask airway (LMA). RESULTS: There was no significant difference in blood pressure during anesthesia, recovery time and side effects between two group. There was significant decrease of heart rate in F-group. CONCLUSION: We concluded that nitrous oxide and fentanyl are reasonable adjuvants of propofol anesthesia in outpatient anesthesia.

5.A retrospective study on related factors affecting the survival rate of dental implants.

PURPOSE: The aim of this retrospective study is to analyze the relationship between local factors and survival rate of dental implant which had been installed and restored in Seoul Veterans Hospital dental center for past 10 years. And when the relationship is found out, it could be helpful to predict the prognosis of dental implants. MATERIALS AND METHODS: A retrospective study of patients receiving root-shaped screw-type dental implants placed from January 2000 to December 2009 was conducted. 6385 implants were placed in 3755 patients. The following data were collected from the dental records and radiographs: patient's age, gender, implant type and surface, length, diameter, location of implant placement, bone quality, prosthesis type. The correlations between these data and survival rate were analyzed. Statistical analysis was performed with the use of Kaplan-Meier analysis, Chi-square test and odds ratio. RESULTS: In all, 6385 implants were placed in 3755 patients (3120 male, 635 female; mean age 65 +/- 10.58 years). 108 implants failed and the cumulative survival rate was 96.33%. There were significant differences in age, implant type and surface, length, location and prosthesis type (P<.05). No significant differences were found in relation to the following factors: gender, diameter and bone quality (P>.05). CONCLUSION: Related factors such as age, implant type, length, location and prosthesis type had a significant effect on the implant survival.

6.A Case of Lichen Aureus.

A case report of a 60 year-old man with lichen aureus of leg is presented. Clinically the solitary golden to purple colored patch consists of lichenoid papules, plsque, and mild petechiae around the lesion, and the histology reveals a dense and bandlike infiltrate hugging thepidermis. It consists of lymphocytes, histiocytes, eosinophils, and many extravasated erythrocytes and hemosiderin in dermis, but a few red cells and increased melanin pigment can also be seen in the epidermis. The eruption is mildly itchy and has been present 6 months.

Trisomy 9p syndrome was first described by Rethore et al in 1970 and about 100 cases have been reported since. The phenotypic spectrum of this syndrome is characterized by craniofacial malformation, facial deformity, skeletal and dermatoglyphic anomalies with variable degrees of mental retardation. We experienced a case of partial trisomy 9 syndrome in a 15-month-old female who had multiple congenital anomalies of frontal bossing, oblique antimongoloid palpebral fissures, enophthalmos, hypertelorism, globular prominent nose, down-turned mouth, prominent low-set ears, simian creases of both hands, clinodactyly and single crease of 5th finger, congenital dislocation of both knees and mental retardation. In cytogenetic studies using G banding technique and fluorescent in situ hybridization(FISH), she presented with an extra derivative chromosome No. 9. The karyotype of the patient was confirmed as 47,XX,+der (9),t (6:9) (q27;q21.2) mat. We report the case with the review of the associated literatures.
