3.A Case of Piriform Sinus Fistula Complicated with Suppurative Thyroiditis Treated with Chemocauterization Using Trichloroacetic Acid.

A 23 months-old girl visited the hospital because of fever and left neck mass. She was diagnosed as acute suppurative thyroiditis with piriform sinus fistula. Thyroid sonography showed perithyroidal abscess formation and thyroid scan showed decreased uptake of Tc-99m pertechnate of both thyroid glands. Magnetic resonance imaging of neck showed abscess cavity extending to the swollen left thyroid gland. And there was tiny fistula between thyroid and piriform sinus in the barium esophagogram. Streptococcus gordonii was isolated on needle aspiration culture. We report a case of piriform sinus fistula of the neck complicated with suppurative thyroiditis. The fistula was treated with chemocauterization using trichloroacetic acid.

4.Fungal Ball in Sinus: Multi-Center Study in Daegu.

BACKGROUND AND OBJECTIVES: Fungal sinusitis has been reported increasingly in immunocompetent patients and the fungus ball is the most frequent and best recognized form of fungal sinusitis. Fungus ball is usually unresponsive to appropriate medical treatment and surgery is the treatment of choice. The aim of this study is to evaluate the incidence, clinical symptoms and signs, and CT findings, the proportion of the patients with fungus ball. MATERIALS AND METHODS: In retrospective study, we reviewed medical records and CT findings of 255 patients with fungus ball confirmed by histopathologically among 6,217 patients who had undergone endoscopic sinus surgery between 1995 and 2004 at five medical centers in Daegu. RESULTS: The symptoms of fungus ball were nasal obstruction (64.4%), rhinorrhea (56.7%), PND (49.2%), cheek pain (26.4%) and foul odor (25.1%). Fungus ball was mostly encountered in only one maxillary sinus (76.4%) of an otherwise healthy person and preoperative computerized tomography (CT) revealed mottled calcific densities within the involved sinus in 55.2% of cases. The success rate of fungus ball surgery by endoscopy was 98.4%. CONCLUSION: This is the retrospective study to evaluate the prevalence of fungus ball (nearly 5%) among the endoscopic sinus surgery. To make a diagnosis of fungus ball, a high index of suspicion to unilateral symptomatic chronic sinusitis, often painful, unresponsive to appropriate medical treatment is necessary and endoscopic sinus surgery is and should remain the mainstay of treatment.

5.Pseudoaneurysm of the popliteal artery mimicking tumorous condition.

Diagnosing pseudoaneurysms of the popliteal artery is usually straightforward in physical examinations and imaging findings. However, when a pseudoaneurysm shows a soft tissue mass with adjacent osseous change, it can mimic a bone tumor or a soft tissue sarcoma. We present a case of a 65-year-old man who had a pseudoaneurysm of the popliteal artery showing soft tissue mass and insinuating into the intramedullary cavity of the tibia. This presented case emphasizes the importance of considering pseudoaneurysms in the differential diagnosis of an apparent soft tissue mass with pressure erosion in adjacent bone.

6.Inflammatory Skin Response to Ultraviolet Radiation: Ear Swelling Response in C57BL Mouse.

This study was undertaken to investigate the change of ear swelling response (ESR) of the mice according to repetitive ultraviolet radiation. 100mJ/crn of UVB was irradiated daily for 4 weeks, and a total of 25 C57BL mice were used as subjects. The results were as follows : 1. After 1 day, 2 days and 3 days irradiation of UVB, ESR reached 21.3+/-1.5, 22.3+/-1.1 and 24.3+/-1.6 respectively with increasing tendency, but there was no statistical significance (Wilcoxon rank sum test p>0.05). 2. ESR reached at maximum, 78.0+/-1.8, after 1 week(p<0.01), thereafter ESR decreased gradually with reparative process. 3. Inspite of continued irradiation, ESR decreased gradually after 2 weeks. After 2 weeks, 3 weeks and 4 weeks irradiation, ESR decreased to 51.6+/-11.8, 45.1+/- 3.3 and 42.8+/-6.8 respectively, but, there were still increment of ESR (p<0.01).

7.Mini-open Rotator Cuff Repair Using Anterolateral Approach.

PURPOSE: To introduce mini-open rotator cuff repair using the anterolateral approach and evaluate its clinical outcomes and effectiveness. MATERIALS AND METHODS: 59 consecutive cases of rotator cuff tearing which were treated with mini-open repair utilizing the anterolateral approach were evaluated. The population comprised 39 men and 20 women, with an average age of 56.6 years. An average follow-up time period was 26 months. Clinical outcomes were analyzed based on VAS, ADL, and ASES scores. RESULTS: The average respective VAS, ADL, and ASES scores improved from 7.04, 12.37, and 35.32 preoperatively to 1.02, 27.20, and 90.08 postoperatively (p=0.000). There were 41 excellent, 11 good, 2 fair, and 5 poor results. There were satisfactory results in 52 cases (88.1%). There were no statistically significant differences between the final ASES scores and age, sex, duration of symptoms, tear size, and preoperative stiffness (p>0.05). CONCLUSION: Mini-open rotator cuff repair using the anterolateral approach effective in providing better visualization.

8.A Case of Cholesterol Granuloma of the Sphenoid Sinus.

Cholesterol granuloma is usually described in association with middle ear diseases, but has also been reported to be found in the paranasal sinuses and orbit. It is currently thought that impaired drainage, hemorrhage and inadequate ventilation play significant roles in its pathogenesis. Histopathologic examination of cholesterol granuloma shows abundant foreign body giant cells, cholesterol crystal clefts, histiocytes surrounded by a dense fibrous capsule. We report our endoscopic surgical experience in one female patient with a large cholesterol granuloma of the sphenoid sinus presenting bilateral visual involvement and brain invasion.

9.Functional Changes of Interleukin-4 and Interferon-gamma of Peripheral Blood and Nasal Mucosa after Immunotherapy in Patients with Perennial Allergic Rhinitis.

BACKGROUND AND OBJECTIVES: Immunologic changes of immunotherapy can include reduced allergenstimulated mononuclear cells prolifer-ation and lymphokine production and the generation of allergic-specfic suppressor T cells. The changes and the relationships of the mononuclear cells between the peripheral blood and the nasal mucosa are still unclear during immunotherapy. The purpose of this study was to evaluate the functional changes of the mononuclear cells in peripheral blood and the nasal mucosa following immunotherapy. MATERIALS AND METHODS: We collected the mononuclear cells from the peripheral blood and the nasal mucosa in 25 patients who had received immunotherapy and 10 patients without any treatment. All of the patients were sensitive to house dust mites. And 5 healthy subjects were used as a control. The mononuclear cells were incubated with antigen or phytohemaglutinin (PHA) for 48 hours. And productions of Interleukin-4 and Interferon-gamma were measured in supernatants by ELISA. On the peripheral mononuclear cells stimulated by antigens, the production of Inter-leukin-4 was dominant in the non-treated allergic group and there were no differences of the productions in the group of immunotherapy and healthy control group. Similar findings of Interleukin-4 production were seen on mononuclear cells from the nasal mucosa. And there were no differences of Interferon-gamma production in the nontreated and control group. However the immunotherapy group showed more Interferon-gamma production than those 2 groups in mononuclear cells from both sites. CONCLUSION: These findings suggests that immunotherapy can change the function of mononuclear cells from the peripheral blood as well as the nasal mucosa.

10.Endoscopic Surgery for Intraorbital Abscess: A Report of One Case.

Intraorbital abscess is one of the serious complications of the paranasal sinusitis and an inadequate treatment for this condition may result in irreversible damages of the optic nerve and retina. Conventionally, surgical treatment of intraorbital abscess has been performed by external ethmoidectomy approach. More recenty, the endoscopic drainage technique has been reported. The authors experienced a case of intraorbital abscess in a 16 year-old male patient for whom we applied the endoscopic drainage technique under the general anesthesia. After the partial removal of the lamina papyracea, we incised the periorbita and then induced drainage of intraorbital abscess by applying external pressure to the eye ball. The surgery was satisfactory and there was no evidence of complications.
