2.The Effect of Lamaze Preparation on Labor and Delivery in Primiparas.

OBJECTIVE: Currently, preparation for childbirth is becoming an increasingly popular addition to patient education. Because of this interest, it seems advisable to review the evidence on the effect of prepared childbirth. The objective of this study is to evaluate the effectiveness of Lamaze preparation on labor and delivery. METHODS: To investigate whether prepared-childbirth courses offer measurable physical advantages, we compared the labor and delivery characteristics of 80 primiparas who had taken Lamaze-training classes with 76 control patients who had not. We reviewed retrospectively obstetrical records of 156 pregnant women delivered in Ewha Womans University MokDong Hospital from January to December, 1997. RESULT: The length of the first stage of labor in the group that attended childbirth preparation classes was 5.70 hours and 6.93 hours in the comparison group. This difference was statistically significant (P= .040). The mean duration of the second stage of labor in attenders was 42.43 minutes and 41.16 minutes in the nonattenders(P= .719). Average duration of labor in attenders was 1.07 hours shorter than in nonattenders (P= .075). The cesarean section rate in attenders was 25% compared with 36.8% in nonattenders (P= .10920). CONCLUSION: No statistical significance was found in this study. However we need to be aware that the length of first stage of labor was shorter and the rate of cesarean section was lower in prepared group. Childbirth education should be integrated into prenatal health care to be readily available for all women.

3.The Effect of rhG-CSF and rhGM-CSF on Expression of Fc gamma Receptors and beta2 Integrin and Respiratory Burst Function in Human Granulocytes.

PURPOSE: Receptors for the Fc of IgG(FcvR) and a beta2 integrin molecule, CD11c/CD18 are important in clearance of microbes by granulocytes. We performed an in vitro study on the effect of recombinant human granulocyte colony-stimulating factor(rhG-CSF), or granulocyte-macrophage colony-stimulating factor(rhGM-CSF) on the expression of Fc R II, Fc R III, CD11c and CD18 and respiratory burst function of granulocytes. METHODS: Peripheral blood was collected from six healthy adults. The isolated granulocytes were stimulated with rhG-CSF, 250mg/mL, rhGM-CSF, 100ng/mL or both of them for 1, 3, 9, 24, and 48 hours. Using flow cytometry, we compared the expression of the Fc R II, Fc R III, CD11c and CD18 of granulocytes before and after stimulation. We also the studied respiratory burst of granulocytes with chemiluminescence assay. RESULTS: Fc R II and CD18 expression were not changed significantly after stimulation. Fc R III expression was decreased significantly after stimulation with rhG-CSF, rhGM-CSF, or both of them. CD11c expression was increased initially but was found to decrease significantly after 9 hours of stimulation. Granulocytes treated with rhG-CSF, rhGM-CSF, or both displayed enhanced respiratory burst. Our results suggest that exposure of granulocyte to growth factor results in granulocyte activation. CONCLUSION: We have shown that rhG-CSF and rhGM-CSF resulted in an activation of peripheral blood granulocytes immunophenotypically and functionally.

4.Schmorl's Nodes of the Lumbar Spine: Are They Simply Normal.

PURPOSE: To evaluate the accompanying abnormal findings of Schmorl's nodes(SN), a normal variants of thelumbar spine. MATERIALS AND METHODS: Seventy-five patients with one or more SN, as seen on lumbar spine MRI werestudied. Using a 1.5T MR unit, the number and location of SN, their site of the end plate, adjacent disc changesand lesion associated and not associated with SN, and accompanying associated bony spinal stenosis wereretrospectively investigated. RESULTS: Among the 75 patients, 230 SN were noted in 375 vertebral bodies; theywere relatively frequently located on the second(65, 28.2%) and third(65, 28.2%) lumbar vertebrae. The most commonend-plate site of SN was the posterior one-third portion(160;69.6%). In 450 discs of these 75 patients, 172lesions were noted;those associated with SN(76/167, 45.5%) was more common than those not thus associated (96/283,33.9%)(p <0.05), and those associated with SN were relative frequently located on intervertebral disc L2-3 orL3-4(p <0.05). Thirty-seven SN (16.1%) were associated with bony spinal stenosis. CONCLUSION: Because it isfrequently associated with disc lesions and bony spinal stenosis, SN of the lumbar spine may be a pathologiccondition rather than a normal variant.

9.A qualitative study to understand the perception and the need for preventive dental services among dental professionals

Objectives:
This study aims to understand the perceptions regarding preventive dental services among dental professionals.
Methods:
In-depth interviews were conducted with three groups of dental professionals―dentists, dental hygienists, and dental hygienists that specialized in preventive service―who were selected using a convenience sampling method.
Results:
First, there was limited awareness among dentists regarding the need for preventive care, because of the treatment-oriented philosophy and scarcity of preventive care success models. Second, dental hygienists commented that there was a lack of the person in charge and systematic process for preventive services. It would not help when they decided to change working place even though they had high anxiety working for preventive care. However, both these professionals agreed that preventive treatment is important as the core of care and that it was important to develop various improvement methods to promote preventive services.
Conclusions
To activate preventive services in dental clinics, it is essential to improve the operating system and encourage dental professionals to realize the importance of preventive services.
10.Expression of Phospholipase C Isozymes in Radiation-Induced Tissue Damage and Subsequent Regeneration of Murine Small Intestine.

Phospholipase C (PLC) isozymes play significant roles in transmembrane signal transduction. PLC- 1 is one of the key regulatory enzymes in signal transduction for cellular proliferation and differentiation. The exact mechanisms of this signal transduction of tissue damage and subsequent regeneration, however, were not clearly documented. This study was planned to determine the biological significance of PLC isozymes following irradiation in rat small intestine. Sprague-Dawley rats were irradiated to the entire body by a single dose of 8 Gy. The rats were divided into 5 groups according to the sacrifice days after irradiation. The expression of PLCs in each group was examined by the immunohistochemistry and immunoblotting. The histologic findings were observed using hematoxylin and eosin staining. The regenerative activity, which was estimated by mitotic count and proliferatin cell nuclear antigen (PCNA) immunostaining, was highest in Group III (5th day after irradiation). By the immunohistochemistry, the expression of PLC- 1 was higher in Group III and Group II (3rd day after irradiation), and was found in the regenerative zone of the mucosa. The expression of PLC- 1 was highest in Group I (1st day after irradiation) and was dominantly in the damaged surface epithelium. The immunostaining of PLC- 1 was negative in all groups. The results of the immunoblotting study was compatible to that of the immunohistochemical study. Group II and III showed positive bands for PLC- 1, and group I and II for PLC- 1. These results suggest that PLC- 1 plays a significant role in mucosal regeneration following irradiation. PLC- 1 may play a role in radiation - induced mucosal damage.
