1.Polysaccharides isolated from Phellinus gilvus enhances dermal wound healing in streptozotocin-induced diabetic rats.

Dermal wound healing is a complex process that involved inflammation leading to re-epithelialization, granulation tissue, and tissue remodeling. Previous studies from our laboratory have shown that polysaccharides isolated from fungus, Phellinus gilvus (PG) have various anti-inflammatory activities. In present study, we have assessed the effect of polysaccharides from PG on the dermal wound healing of polysaccharides from PG in streptozotocin-induced diabetic rat model. Six of 6-mm circular wounds were created with biopsy punch on the 4th day after induction of diabetes. After 24 hours, each test substance was applied to the wound twice a day for next 5 days. Circular wounds treated with PG showed significantly reduced wound contraction and complete reepithelialization, as compared to wounds of non-treated (p < 0.05). These results show that polysaccharides isolated from PG enhanced wound repair in diabetic impaired healing, and could be developed as a wound healing agent in such clinical settings.

2.A Case of kerion Celsi in An adult Caused by Trichophyton rubrum.

Kerion celsi caused by Trichophyton(T.) rubrum is rare endemically and there has been no report of it in Korean literatures. We report a case of karion celsi caused by T. rubrum in a 71-year-old male, who showed anindurated, swollen, boggy mass exuding pus on the right parietal scalp for about 7 weeks. The colonies and microconidia were identified as T. rubrum by fungus culture in potato dextrose agar. After administration of itraconazole for 11 weeks, the lesion was healed and the regrowing of new hairs was observed.

3.Congenital Posterior Urethral Valves.

A clinical observation was made on 5 cases of congenital posterior urethral valves admitted recently to the Department of Urology, Busan National University Hospital. The results were following: 1. Age distribution showed; 1 case (20%) under 6 months, 2 cases (40%) between 6 and 12 months, 2 cases (40%) over 6 years old. 2. The symptoms of urinary dribbling, weak stream, frequency and difficult urination were observed in all 5 cases. Abdominal mass, abdominal pain, fever, vomiting and failure to thrive were observed in 4 cases (80%) . Enuresis was observed in 2 cases (40%) . Hypertension and gross hematuria were observed in 1 case (20%) . 3. IVP revealed bilateral hydronephrosis and hydroureter in all cases. Vesicoureteral reflux was observed in 3 cases (60%) , in which 2 cases were unilateral and the other 1 case was bilateral and bladder diverticulum appeared in 3 cases (60%). Voiding cystourethrogram was performed on 4 cases, and in all of these 4 cases (100%) dilatation of posterior urethra and cut-off at the membranous level and a thin stream in the bulb and 2 cases (50%) bladder neck contracture due to congenital posterior urethral valves had developed. 4. Endoscopic examination was performed on 5 cases, in which 3 cases (60%) were young's type I, and remaining 2 cases 140%) were Young's type III. 5. All 5 cases were treated by transurethral electrofulguration of the valves and one case of them was performed bilateral ureteroneocystostomy 3 years after valve ablation but in the case with marked bilateral hydronephrosis and vesicoureteral reflux a cystostomy was also performed. 6. The follow up study was performed in 4 cases with the follow-up period from five months to 36 months and mean duration of 2 years. All 4 cases had excellent results with much improved urinary stream and Renal condition after operation, but in 2 cases intermittent incontinence occurred for 3 years and 1 year respectively. In all 4 cases much diminished hydronephrosis and urethral dilation on IVP were observed. The posterior urethral dilatation became nearly normalized and abnormal findings of bladder improved on voiding cystourethrogram, but in 2 cases with bladder diverticulum 1 showed nonspecific interval changes during 3 years and the other 5 months. In 2 cases with vesicoureteral reflux, 1 showed loss of vesicoureteral reflux spontaneously 1 month after the operation and the other showed persistent vesicoureteral reflux for 1 year, which was observed to be improving.

8.Apoptosis and Expression of bcl-2, p53, and Ki-67 in Mycosis Fungoides.

BACKGROUND: Mycosis fungoides(MF) is a form of cutaneous T cell lymphoma with clonal differentiation of helpr' T cell. It has a patch, plaque, and tumor stage. But pathogenetic factors controlling the development and progression of MF are still unclear. Apoptosis plays a major role in developmental biology and homeostasis. The bcl-2 oncogene prolongs ce11 life by inhibiting apoptosis. The mutant pS3 gene induces apoptosis indirectly. Ki-67 antigen is the cell proliferation marker. Recently, it has been shown that the relationships among them are important in the tumorigenesis of the various tumors. OBJECTIVE: The aim of this study was to examine the expression of these genes and apoptotic rate and clarify the relationship among them in the development and progression of MF. METHODS: The eighteen specimens from 8 patients with MF and 10 specimens from benign lymphocytic infiltrating diseases including 5 lichen planus, 3 lupus erythematosus, and 2 contact dermatitis were included. We performed immunoperoxidase staining(LSAB technique) using monoclonal antibodies including bc1-2, p~53, and Ki-67(MIB1). We used ApoptaqTM(Oncor) in situ labelling kit for detecting apoptotic cell.

9.A Case of Woringer Kolopp Disease Treated with Topical PUVA.

A 26-year-old Korean man had an erythematous scaly plaque on his right thigh for fifteen years. He was diagnosed with chronic eczema and treated with topical steroid, but no clinical improvement was obtained for 15 years. Laboratory studies including complete blood cell count, urinalysis, liver and renal function test, and peripheral blood smear were normal. A biopsy specimen showed hyperkeratosis, acanthosis, and mononuclear cellular infiltration in the epidermis. These cells had a perinuclear halo and showed a Pautrier's microabscess like configuration. In the dermis, there was a band like infiltration of inflammatory cells. Infiltrates of the epidermis were negative for leukocyte common antigen(CD45) and positive for Pan T cell(CD45RO). He was treated with topical PUVA twice a week for twelve weeks. Clinical and pathological improvements were obtained. We propose PUVA may be a useful therapeutic modality to treat Woringer Kolopp disease.

10.A study on osteoblast-like cell responses to surface-modified titanium.

STATEMENT OF PROBLEM: The success of implants depends on intimate and direct contact of implant material on bone tissue and on functional relationship with soft tissue contact. Creation and maintenance of osseointegration depend on the understanding of the tissue's healing, repairing, and remodeling capacity and these capacities rely on cellular behavior. Altering the surface properties can modify cellular responses such as cell adhesion, cell motility, bone deposition. Therefore, various implant surface treatment methods are being developed for the improved bone cell responses. PURPOSE: The purpose of this study was to evaluate the responses of osteoblast-like cells to surface- modified titanium. MATERIALS AND METHODS: The experiment was composed of four groups. Group 1 represented the electropolished surface. Group 2 surfaces were machined surface. Group 3 and Group 4 were anodized surfaces. Group 3 had low roughness and Group 4 had high roughness. Physicochemical properties and microstructures of the d iscs were examined and the responses of osteoblast-like cells to the discs were investigated. The microtopography was observed by SEM. The roughness was measured by three-dimension roughness measuring system. The microstructure was analyzed by XRD, AES. To evaluate cell responses to modified titanium surfaces, osteoblasts isolated from calvaria of neonatal rat were cultured. Cell count, morphology, total protein measurement and alkaline phosphatase activities of the cultures were examined. RESULTS AND CONCLUSION: The results were as follows 1. The four groups showed specific microtopography respectively. Anodized group showed grain structure with micropores. 2. Surface roughness values were, from the lowest to the highest, electropolished group, machined group, low roughness anodized group, and high roughness anodized group. 3. Highly roughened anodized group was found to have increased surface oxide thickness and surface crystallinity. 4. The morphology of cells, flattened or spherical, were different from ach other. In the electropolished group and machined group, the cells were almost flattened. In two anodized groups, some cells were spherical and other cells were flattened. And the 14 day culture cells of all of the groups were nearly flattened due to confluency. 5. The number of attached cells was highest in low roughness anodized group. And the machined group had significantly lower cell count than any other groups(P<.05). 6. Total protein contents showed no difference among groups. 7. The level of alkaline phosphatase activities was higher in the anodized groups than electropolished and machined groups(P<.05).
