1.Postoperative Radiation Therapy in High-risk Breast Cancer.

PURPOSE: To assess the locoregional recurrence rate, survival rate and prognostic factors after modified radical mastectomy and postoperative adjuvant radiation therapy with or with chemotherapy in high-risk breast cancer patients. METHODS: Between 1984-1995, 48 patients underwent postoperative irradiation to the regional lymphatics and chest wall due to large tumor size (> or = 5 cm) or small tumor size (<5 cm) with axillary lymph node involvement after modified radical mastectomy. The median age of the patients was 47 years (range, 31-79 years). The clinical tumor size was <2 cm in 1 patient, 2-5 cm in 15 patients, and >5 cm in 32 patients. Thirty two patients had positive axillary lymph nodes. Forty two patients were irradiated to the chest wall and regional lymph node and 6 patients were irradiated in the chest wall only. Radiation dose to the chest wall and regional lymph node was 5040 cGy/28 fraction. The median follow-up time was 61 months. RESULTS: Locoregional recurrence rate was 8% and distant metastatic rate was 14%. The actuarial overall survival rate and disease-free survival rate was 63% and 62% at 5 years, respectively. The median survival time was 67 months. Five-year overall survival rate by the stage is 70% in IIB and 58% in IIIA. The significant prognostic factor for survival on multivariate analysis was the stage. CONCLUSION: Postoperative adjuvant radiation therapy in high-risk breast cancer can reduce the locoregional recurrence rate and increase the survival time by combined chemotherapy. The significant prognostic factor for survival rate was the stage.

2.A Case of Sacrococcygeal Chordoma Diagnosed by Fine Needle Aspiration Biopsy Cytology.

A case of sacrococcygeal chordoma diagnosed by fine needle aspiration is presented. This is a case of a 54-year-old woman who came with coccygeal pain of 5-6 months duration. Aspiration biopsy cytology revealed many nests of cells having abundant bubbly cytoplasm and round to oval variably sized nuclei. The cells had indistinct cytoplasmic borders and many of the cells had cytoplasmic vacuoles. The nuclei had thin regular nuclear membranes, finely granular chromatin and one or two small nucleoli. The cells were generally monotonous, but focally pleomorphic with giant cell formation. Mitotic figures were scanty. The backgroud of the aspirate contained abundant mucinous materal. These findings were typical of those of recorded chordoma cases and the diagnosis was confirmed by a following open biopsy. The patient received 4,000 rads of neutron radiotherapy and has been well till March '88.

3.Fine Needle Aspiration Cytology of Chondrosarcoma.

Fine needle aspiration cytologic findings of four cases of chondrosarcoma were described. The cases consisted of one primary scapular tumor, two recurrent shoulder masses, and right upper quadrant mass which developed after an A-K amputation for an unknown tumor. The aspirates characteristically revealed cell-rich smears containing clusters and isolated cells having abundant cyanophilic cytoplasm and round to oval or elongated vesicular nuclei. The cytoplasm was occasionally foamy. The nuclei were usually small but prominent. Nuclear atypism and pleomorphism were frequently associated. The last case showed epithelioid sheets of polygonal cells, possibly representing chondroblasts, and a well differentiated chondroid element. The fine needle aspiration could be a good diagnostic tool for primary, recurrent, and metastatic chondrosarcomas.

6.Urinary calculi in traumatic spinal cord injury

The evaluation of urinary calculi was done in 84 patients of paraplegia and quadriplegia due to traumatic spinal cord injury. These patients were diagnosed by KUB and intravenous urography at National Veterans Hospital during 6 years from Jan. 1975 to Dec. 1980. The results were as follows; 1. Overall incidence of urinary calculi was 38.1 %; Incidence of renal calculi was 8.3%, ureteral calculi 4.8%, and urinary bladder claculi 32.1%. 2.Relation of neurological level and incidence of urinary calculi were as follows; Cervical injury in 34.8% upper thoracic injury in 40.0%, lower thoracic injury in 45.0%, and lumbar injury in 36.5%. 3. Laterality was not toplay a role information of urinary claculi. 4. The urinary calculi were developed 62.5% during the first 36 months following spinal cord injury. 5. The recurrence of urinary calculi was 40.6%; True recurrence was 15.6% and pseudo recurrence was 25.05.

7.A Concept Analysis of Well-Being: In the Lives of Korean Elderly.

Well-being has been considered to be an important nursing concept. However, there is no agreement among nurse researchers as to a conceptual definition of well-being and a similar concept to well -being. Clarification of the concept, well-being in nursing, is necessary to promote communication among nurses and to efficiently apply nursing intervention. This research was designed to analyze the concept and to make a definition of well-being in the lives of Korean Elderly. The study used the concept analysis approach developed by Walker and Avant(1995), and interviews. Firstly, well-being was defined as the balance and harmony of the internal and external environments and as the pursuit of improvement. Attributes of well-being were defined as follows: 1) homeostasis 2) harmony 3) fulfilment 4) pursuit of improvement And empirical referents were followed as: 1) health status, degree of activities of daily living, 2) degree of happiness, degree of satisfaction, 3) self-efficacy, self-esteem, 4) support, interpersonal relationships, social-leisure activity, 5) economic status, 6) degree of belief, 7) space and facilities Consequently, this study has identified the definition and attributes of "well -being" and has located empirical referents through concept analysis and interviews.

8.A Structural Model for Depression in Middle-aged Women.

The purpose of this study was to develope and test the structural model for depression in middle-aged women. A hypothetical model was constructed on the basis of previous studies and a review of literatures. The conceptual framework was built around eight constructs. Exogenous variables included in this model were volunteer activity, health status & economic status. Endogenous variables were meaning of life, self-esteem, perceived life stress & depression. Empirical data for testing the hypothetical model was collected using a self-report questionnaire from 216 middle-aged women in Pusan City. The Data was collected from May to June, 1999. Reliability of the seven instruments tested with Cronbach's alpha was ranged from .86-.94. For the data analysis, SPSS 7.5 WIN Program and LISREL 8.12 WIN Program were used for descriptive statistics and covariance structural analysis. The results of covariance structure analysis were as follows: 1. The hypothetical model showed a good fit with the empirical data. [chi2/df =2.87(p=.72), GFI=1.00, AGFI=.98, RMSR=.025, NFI=.99, standardized residuals (-1.44~1.44) ]. 2. To heighten for the parsimony and fitness of the model, a modified model was constructed by deleting according to the criteria of statistical significance and meaning. 3. The modified model also showed a good fit with the data. [chi2 =5.26(01=7,p=.63), GFI=.99, AGFl=.97, RMSR=.025, NFI=.99, standardized reslduals (-1.50~1.45) ] Results of the testing of the hypothesis were as follows : 1. Self-esteem(beta42=-.48, t=-5.64) had a negative and perceived life stress(beta43 =.20, t=3.21)had a positive direct effect on depression. Meaning of life had a negative direct(beta41=-.17, t=-2.19) and a negative indirect effect through self- esteem on depression. Volunteer activity had a negative indirect effect through meaning of life, meaning of life and self-esteem on depression. Health status had a negative direct and a negative indirect effect through meaning of life, meaning of life and self-esteem on depression. Economic status had a negative indirect effect through perceived life stress on depression. Self-esteem was the most significant variable. 2. Volunteer activity(gamma11=.43, t=6.78) and health status(gamma12=.35, t=4.88) had positive direct effect on meaning of life. 3. Meaning of life(beta21=.50, t=6.53) had a positive direct effect on self-esteem. Volunteer activity had a positive indirect effect through meaning of life on self-esteem. Health status had a positive direct(gamma22=.18, t=2.23) and a indirect effect through meaning of life on self-esteem. Meaning of life was the most significant variable.

The purpose of this study is setting proper portion by consumed size of food and food preference and then reducing plate wasts in school lunch. The survey was conducted from Oct. 21 to Nov. 22, 1996 through the practical measurement and questionnaires. The subjects were 43 students of 1 elementary school. The results of this survey was as follows : 1. Average consumption size of cooked rice was 115g. Average consumption size of one course dishes was 209g. The soup & pot stewes showed lower consumption rate than the other kind of foods. Average consumption size of fresh & boiled salad was 23.4g Average consumption size of stir fries was 55g. Average consumption size of steams & stewes was 42g. Most grill & fried foods showed high consumption rate compared to most of the other foods. Average consumption size of kimchies was 10.6g. The most of given desserts except apple were all consumed. 2. There was significant correlation between food preference and consumption rate for total food items. 3. Response of satisfaction about serving size of most of given foods except fresh & boiled salads, soup & pot stewes and kimchies were "moderate". 4. The purpose of this study was to evaluate the preference trend of frequently served meals in the elementary school lunch program. Therefore, this evaluation was surveyed on frequently served meals in the elementary school lunch program in Inchon. Also, this study was surveyed on food preference using questionnaire including frequently served food items. Statistical analysis of data was completed using SAS program. The results of this survey was as follows: 1. Freqrenctly served fooods were 56 food items including 5 boiled rice, 3 one course dishes, 10 soup & pot stewes, 10 fresh & boiled salads, 8 stirred fries, 7 stewes, 6 grill & fries, 3 kimchies and 4 desserts. 2. Students liked barley bab more than the other kinds of cooked rice. One course dishes were preferred most by the subjects. Among soup & pot stewes, fermented soybean paste stew showed lower preference than the other kind of stewes. Among fresh & boiled salads, fruits salad was the most favore, whereas, root of bellflower salad was the most hated. Among side-dishes by different preparation method, stir fries, stewes, grill & fries, kimchies, dessert were prefered most by the subjects. There were significant difference between male and female students for most given foods except soup & pot stewes. Female students showed higher preference for most given foods except fresh & boiled salads, kimchies, dessert than male students.
