1.Effect of the non-Medical Services on Attitude to Hospital Utilization.

Effects of various factors related to the process of social action of hospital services on the selecting a hospital were investigated by using of questionnaires answered by 1,319 patients in a certain University Hospital, in Chungchongnam-Do, from November, 1986 to November, 1987. The results summarized were as follows ; 1. Each of the examined factors, that is, sincerity of an attendant physician in medical treatment and explaining the condition of a disease, confidence to doctor and nurse, doctor's and nurse's response to calling by patient, kindness of the hospital staffs and food handlers, sanitariness of hospital, cleanness of clothes and bed sheets, hospital foods, and rest surroundings, are not solely decisive to attitude of hospital utilization, but the factors effects compositely on hospital utilization. 2. Hospital services of the social action process are more effective to hospital utilization in cases of the hospitalization caused by the traffic facility than for the medical skill and facilities.

2.Prevalence of intestinal parasites in ROKA solidiers.

A survey of intestinal parasites was performed during the period from April 1970 to December 1971 in ROKA (Republic of Korea Army) soldiers. Samples were collected from 591 Army troops(enlisted men), 1,755 recruits at completion of 6 weeks basic training, 304 recruits during basic training and 245 stool specimens of students of ROKA Nursing School at Taegu area. Each of the specimens was examined one time or twice under the microscope, one for protozoan cysts stained with iodine solution and the other for helminth eggs. Findings were summerized as follows: A positive rate of intestinal parasites was 88.0 percent by Formalin-Ether concentration method and 89.2 percent by Kato's method. Of the 541 soldiers examined by Formalin-Ether concentration method, the infection rate of A. lumbricoides was 28.1 percent, T. trichiurus 79.3 percent, hookworm 14.2 percent, T. orientalis 30.7 percent, C. sinensis 7.8 percent and M. yokogawai 0.6 percent. Of the 1,532 soldiers examined by Kato's method A. lumbricoides showed positive rate of 45.8 percent, T. trichiurus 78.3 percent, T. orientalis 1 4.5 percent, hookworm 7.5 percent, C. sinensis 6.6 percent and Taenia sp. 0.6 percent. The incidence of E. vermicularis out of 822 samples was 19.8 percent from the anal swabs. E. histolytica was recovered in 4.2 percent among 541 samples, E. coli 6.1 percent and also infected G. lamblia, E. nana, I. butschlii and Chilomastix mesnili. Most of the infected cases revealed single or double infections.

3.The Effect of the Prevention Program of Needle-Stick Injury on Needle-Stick Knowledge, Compliance and Incidence.

BACKGROUND: The purpose of this study was to develop and apply a prevention program for needle-stick injuries among health care workers who have a high occupational risk of such injuries, and to analyze the effect of the program on the subjects' knowledge regarding such injuries and compliance with protocols, as well as the incidence of needle-stick injuries among subjects. METHODS: The subjects of this study were nurses (RNs) and nurse's aides (NAs) working at a university hospital in Daegu. The data were collected by conducting surveys and determining the incidence of needle-stick injuries during a 1-year period before the application of the program. The program consisted of preventive education on needle-stick injury, the use of standardized boxes to measure blood sugar test, and the use of intravenous catheters with safety needles. Six months after the intervention, surveys were conducted to analyze the incidence of needle-stick injuries. The data on knowledge and compliance related to needle-stick injuries before and after program application were analyzed using the paired t-test, and data on the incidence of injuries were analyzed using the chi-square test. RESULTS: The scores on knowledge (RN, P<0.001; NA, P=0.007) and compliance (RN, P<0.001; NA, P=0.038) were significantly higher after the intervention than before. However, the number of reports on the incidence of needle-stick injuries before and after the intervention were not significantly different (RN, P=0.691; NA, P=0.079). CONCLUSION: The prevention program for needle-stick injuries was effective in improving the knowledge and compliance related to needle-stick injuries among RNs and NAs, but the number of reports on the incidence of injuries showed no difference. Therefore, the prevention program must be reevaluated and further developed after continuously monitoring and analyzing it in a hospital setting in order to decrease the incidence of needle-stick injuries.

4.Allergic contact dermatitis due to 8-methoxypsoralen(8-MOP).

Allergic contact dermatitis due to 8-MOP is a rarely known si(ie effect of this widely used drug. Other known adverse reactions due to 8-MOP such as the oallergic dermatitis as well as some isolated cases of exanthema, papular eruptions, and astloma like symptoms are also sporadically reported. A 52-year-old man with vitiligo developed erythema to the UVA exposed 0.3% Oxoralen cream applied area. Prior to this episode, the patient had history of generalized burns after systernic PUVA therapy in 1983. Even after this experience, the patient had few more episodes of erythema at the site of 0.3%. Oxoralen cream application. We performed patch test and photopatch tests with Scandinavian series, 0.3% Oxoraler or am (as is), and diluted 8-MOP, 5-MOP, TMP solution. The result showed positive reactivity to 6-methylcoumarin, 8-MOP, as well as to 0.3% Oxoralen cream. The size of erythema was same in both irradiated areas which indicates an allergic contact dermatitis rather than photoallergic dermatitis or phototoxic dermatitis.

5.CT findings of the mediastinal tumors; excluding mediastinal granuloma and primary carcinoma

Computerized Tomography can make accurate diagnosis in most of the mediastinal tumors and cysts by assessingtheir location, shape and internal architecture. Authors analysed and present CT findings of 89 surgically provenmediastinal tumpors and cysts that were studied and treated in Seoul National University Hospital during recent 5years. The results are as follows; 1. The most common tumor was teratoma(25 cases). Neurogenic tumor (20 cases),thymic tumor or cyst (16 cases), lymphoma(7 cases), bronchogenic cyst (6 cases), intrathoracic goiter (6 cases),pericardial cyst(3 cases) and cystic hygroma(2 cases) were next in order of frequency. 2. The most constant findings of teratoma was thick walled cystic area(100%), while pathognomonic fat and calcified density were seenonly in 52% and 48% of cases, respectively. 22 cases were located in anterior mediastinum, 2 cases were inposterior mediastinum and a case is in middle mediastinum. 3. There were 20 cases of neurogenic tumor consiting of6 neurilemmomas, 7 ganglioneuromas, 4 neurofibromas, 1 ganglioneuroblastoma, 1 neuroblastoma and 1 malignantschwannoma. Most of them were located in posterior mediastinum with exception of 2 neurilemmomas arising from leftvagus nerve and left recurrent laryngeal nerve in middle mediastinum. Cystic change was seen in 2 cases ofneurilemmoma and in a case of ganglioneuroma. Calcification was seen in 3 cases, of neuroblastoma, aneurilemmoma,and a ganglioneuroma. 4. There were 11 cases of thymoma showing homogeneous solid mass with speckeldcalcification in 4 cases and irregular cystic change in 3 cases. 2 cases were invasive thymoma and myastheniagravis was present in 4 cases. A case of thymolipoma and a case of thymic cyst were included. 5. Lymphoma(2Hodgkin's and 4 non-Hodgkin's) appeared as lobulated, matted mass in anterior mediastinum especially inprevascular area expnading bilaterally. 6. Intrathoracic goiter appeared as slingtly high density mass within termingled calcification and cystic area in 5 cases of secondary goiter and homogeneous high densitymass(100-110 H.U. in precontrast scan) in a case of primary goiter. 7. Among the 6 bronchogenic cysts, 3 werelocated in subcarinal area, 2 were above carina and one was in left hilar area. 3 cases showed high CT number morethan 70 H.U. and others showed water density. 8. 3 cases of pericardial cyst were located in right cardiophrenicangle and all of them showed water density. 9. 2 cases of cystic hygroma were located in superior mediastinum,with extension to lower neck and all of them showed water density.

6.CT diagnosis of the fat containing mediastinal masses

Fat containing masses, except mediastinal lipomatosis, of the thorax are uncommon. In spite of uncommonoccurences, as CT can detect not only the fat content but also the presence and character of the non-fattycomponent of the mass, accurate preoperative diagnosis can usually be made in most cases of fat containingmediastinal masses. Authors report 6 cases of fat containing mediastinal masses, that were diagnosedpreoperatively by CT scan, comprising 2 cases of limpoma(combined with plexiform neurofibroma in a case), 1liposarcoma, 1 thymolipoma, and 2 cases of omental hernia through Morgagni foramen.

8.Evaluation of Computer Aided Volumetry for Simulated Small Pulmonary Nodules on Computed Tomography .

PURPOSE: To determine the accuracy of automated computer aided volumetry for simulated small pulmonary nodules at computed tomography using various types of phantoms MATERIALS AND METHODS: Three sets of synthetic nodules (small, calcified and those adjacent to vessels) were studied. The volume of the nodules in each set was already known, and using multi-slice CT, volumetric data for each nodule was acquired from the three-dimensional reconstructed image. The volume was calculated by applying three different threshold values using Rapidia(R) software (3D-Med, Seoul, Korea). RESULTS: Relative errors in the measured volume of synthetic pulmonary nodules were 17.3, 2.9, and 11.5% at -200, -400, and -600 HU, respectively, and there was good correlation between true volume and measured volume at -400 HU (r=0.96, p<0.001). For calcified nodules, relative errors in measured volume were 10.9, 5.3, and 16.5% at -200, -400, and -600 HU, respectively, and there was good correlation between true volume and measured volume at -400 HU (r=1.03, p<0.001). In cases involving synthetic nodules adjacent to vessels, relative errors were 4.6, 16.3, and 31.2 % at -200, -400, and -600 HU, respectively. There was good correlation between true volume and measured volume at -200 HU (r=1.1, p<0.001). CONCLUSION: Using computer-aided volumetry, the measured volumes of synthetic nodules correlated closely with their true volume. Measured volumes were the same at each threshold level, regardless of window setting.

9.Neurofibroma in Breast: A Case Report.

Neurofibromas are common benign tumors and can originate from any nerve tissue in the body. A solitary neurofibroma in breast parenchyma has rarely been reported, however. We report a neurofibroma originating from breast parenchyma in a 61-year old woman. On mammography, the mass appeared as a well marginated and circumscribed mass, suggesting a benign tumor, and after excisional biopsy, was pathologically proven to be a neurofibroma.

10.A case of breast sparganosis.

A 29-year-old Korean woman visited the Department of Surgery in MizMedi Hospital with a palpable itching mass on the right breast that had existed for the past 7 months. She had no history to eat either frogs or snakes, but had the history of drinking impure water. Sonography revealed a serpiginous hypoechoic tubular structure associated with partial fat necrosis in breast parenchymal layer and subcutaneous fat layer. It also revealed oval cystic lesions. At operation, an ivory white opaque ribbon-like worm that measured 16.5 cm in length and 0.5 cm in width was extracted. Anti-sparganum specific serum IgG level in the patient's serum (absorbance = 0.71), measured by ELISA, was found to be significantly higher than those of normal controls (cut off point = 0.21). Sonography and ELISA appear to be helpful to diagnose sparganosis. Breast sparganosis is rarely found throughout the world.
