We reported a case of Wilson's disease, which was histologically confirmed by liver biopsy in a 15 year-old boy and made a comprehensive analysis on the sum of 22 cases reported in Korean literature. Although Wilson's disease is still rare, it should be included in the differential diagnosis of chronic liver diseases particularly in children of 5 to 15 years old. For the clinical diagnosis of Wilson's disease, changes in serum ceruloplasmin, plasma copper and urine copper values are most important as well as Kayser-Fleischer ring and family hestory. The serum level of GOT, when elevated, were always higher than GPT. This atypical transaminase profile may be a clue for diagnosis of hepatic diseases by a metabolic derangement. Among various histologic changes of the liver in Wilson's disease, what have diagnostic importance are anisonucleosis, nuclear glycogenosis and Mallory body, all of which may appear in any stages of the disease. In 3 cases only neurologic symptoms such as dysarthria and athetosis were found. Hemolytic anemia was another rare complication.

This report deals with three cases of accessory hepatic nodules incidentally noted during operation. Two of them were found on the serosal surface of the gallbladder with no connection to the main body of the liver. The other case was in the greater omentum. All three cases were small oval shaped, measuring less than 1 cm in maximum dimension and were composed of histologically normal hepatic tissue and seemed to receive blood supply from the adjacent tissue through the capsular blood vessels. Presence of the fairly well retained intralobular mesenchymal component may reflect that accessory hepatic nodules develop after conjugation of hepatic diverticulum and septum transversum.

The enhancing potential of anatoxin a (AFB1) and D-galactosamine (DGA) on development of preneoplastic glutathione S-transferase placental form positive (GST-P+) hepatic foci was examined using an in vivo mid-term assay system based on two-stage concept of hepatocarci-nogenesis. Rats were initially given a single dose (200 mg/kg) of diethylnitrosamine (DEN) intraperi-toneally, and thereafter. with an interval of 2 weeks, AFBl at a graded concentration (0.06, 0.012, 0.0024, 0.00048, and 0.000096 mg/kg i.g.) and DGA (100 mg/kg i.p.) were administered for 6 weeks and then sacrificed. All rats were subjected to a two-thirds partial hepatectomy to induce a potent growth stimulus to DEN-altered hepatocytes at the week 3. The modifying potential was scored by comparing the number and the area (mm2) per cm2 of GST-P+ foci in the liver with those of the corresponding control group given DEN alone. AFBl (at a graded concentration between 96 ng/kg and 60 microgram/kg) exerted a strong promoting effect oil induction of GST-P+ foci with both the number and the area. The logarithmic dose of AFBl and the potency to promote hepatocarcinogenesis were in dose-dependent relationship. DGA, a known necrogenic chemical to cause periportal necrosis and stimulate hepatocellular proliferation. also revealed the increase in the area of GST-P+ foci. although its enhancing potentia1 was 1ess profound than that of AFBl. The results suggest that DGA is also a useful proliferative stimulus m improve the medium-termdetection of unknown carcinogens.

The etiologic impacts in primary hepatocellular carcinoma among Koreans seem different from those in other countries with its high incidence and close association of hepatitis B virus infection and liver cirrhosis. A series of 35 lobectomy specimens of hepatocellular carcinoma (HCC) was examined by means of electron microscopy to elucidate the general ultrastructural characteristics and to understand the morphogenesis of various histological growth patterns and cytologic features of HCC. 1) General cytological details of HCC were similar to those of non-neoplastic hepatocytes, but characterized by scantiness of subcellular organelles. Degree of cellular differentiation was not correlated with ultrastructural features of HCC. 2) Acinar pattern of HCC seemed to develop by either dilatation of central bile canaliculus or central cystic degeneration of microtrabecular growth, and clear cell group of HCC was expressed in abundance of glycogen particles and lipid droplets. 3) Intranuclear inclusions of HCC proved to be cytoplasmic herniations of tumor cells, and intracytoplasmic tubular arrays appeared to originate from the endoplasmic reticulum. 4) Hyaline globules seen in HCC corresponded to clumps of microfilamentous structures similar or identical to Mallory's hyalin.

A survey of intestinal parasites was performed during the period from April 1970 to December 1971 in ROKA (Republic of Korea Army) soldiers. Samples were collected from 591 Army troops(enlisted men), 1,755 recruits at completion of 6 weeks basic training, 304 recruits during basic training and 245 stool specimens of students of ROKA Nursing School at Taegu area. Each of the specimens was examined one time or twice under the microscope, one for protozoan cysts stained with iodine solution and the other for helminth eggs. Findings were summerized as follows: A positive rate of intestinal parasites was 88.0 percent by Formalin-Ether concentration method and 89.2 percent by Kato's method. Of the 541 soldiers examined by Formalin-Ether concentration method, the infection rate of A. lumbricoides was 28.1 percent, T. trichiurus 79.3 percent, hookworm 14.2 percent, T. orientalis 30.7 percent, C. sinensis 7.8 percent and M. yokogawai 0.6 percent. Of the 1,532 soldiers examined by Kato's method A. lumbricoides showed positive rate of 45.8 percent, T. trichiurus 78.3 percent, T. orientalis 1 4.5 percent, hookworm 7.5 percent, C. sinensis 6.6 percent and Taenia sp. 0.6 percent. The incidence of E. vermicularis out of 822 samples was 19.8 percent from the anal swabs. E. histolytica was recovered in 4.2 percent among 541 samples, E. coli 6.1 percent and also infected G. lamblia, E. nana, I. butschlii and Chilomastix mesnili. Most of the infected cases revealed single or double infections.

Benign epiepsy of childhood with centrotemporal spikes (BECCT) is an electroclinical syndrome characterized by noctural seizure that remit spontaneoully before adulthood, and belong to idiopathic age and location related epilepsies. We reviewed the medical records to analyse the seizure the seizure pattern, and also inspect the EEG recording to identify topography of the epileptiform discharge of 24 patient who met the following criteria: 1) presence of nocturnal seizure or partial seizure confined to the face, 2) normal intecual and development, 3) normal background EEG feature, 4) monomormhic spikes or sharp wave that increase in the frequency during sleep if those state were achieved. The result as follow: 1) The age of seizure onset were 3 to 11 years old (7.08 1.89) and male to female ratio was 1, 4:1. 2) Past history of birth trauma, CNS infection and other diseases involving CNS, were not found. There was past history of febrile convulsoin 33.3% of patient and epilepsy in 8.3%. One of paient's sister had childhood abscence epilepsy and never a nocturnal partial seizure, also and had centrotemporal spikes in EEG. 3) In 91.7% of Seizures occurred during sleep, especially short after sleep onset and the symptomatology of seizure was shown 66.7% of oropharyngeal sign, such as hypersalivation and gargle sound, and 54.2% of hemifacial involvement, 33.3% of speech arrest, 25.5% of upper limb involement, 18.3% of lower limb involvement, and 45.8% of general seizure. 4) In 91.5% of patients had on location of epileptiform discharge that located central (37.5%) midtemporal (33.3%), parietal and frontal. We could observe tangental dipole in 16.7% of patients.

Linear porokeratosis is a rare variant of porokeratosis and usually occurs in childhood. A 37-year-old female is presented with linear porokeratosis on the posterior aspeet of the right lower extremity exhibiting the classical histopathologic criteria of the disease. Treatment with an oral etretinate resulted in a remission of the lesions.

Carbon monoxide is colorless, oolorless, tasteless and non-irritating gas produced by the incomplete combustion of carbonaceous rnaterial. It combines with hemoglobin and displaces oxygen because the affinity of hemoglobin for carbon monoxide is two hundred times greater than oxygen. Symptoma and signs of carbon monoxide poisoning include headache, nausea, vomiting, dizziness, collapse, unconsciousness, blindness, convulsion, coma and skin lesions. Recently we have observed seven patients with carbon monoxide poisoning who expressed cutaneous syrnptoms. In this work we investigated the pathogenesis of cutaneous manifestations of carbon monoxide poisoning through clinical, histologic and electronmicroscopic study. The results are summarized as follows: 1. Mental states of the patients were comatose in two, Semicomatcse in two, stuporous in two, and drowsy in one patient. In routine laboratory tests, we observed elevated blood sugar in six, elevated sorum creatinine phosphokinase in four and abnormal findings in urinalysis in all patients. 2. Cutaneous lesions were vesicobullae, plaque or swelling, erythema, gangrene and 'ulceration in order of frequency and located in the dependent areas in six caies. 3. Histopathologically, the sites of the bullae were subepidermal in four cases and intraepidermal in. one case and there was one case with ulceration. 4. A case of "Follicular Occlusion Triad" associated with acne conglobata, hidradenitis suppurativa and perifolliculitis capitis abscedens et suffodiens in 40-year-old male patient is presented with brief review of literature. Widespread acne occured on the face k upper back at the age of 16. Several years later, firm & tender erythematous poorly demarcated nodules developed on the back, buttock and axilla. Also, erythematous nodules developed on temporooccipital area of the scalp. Thereafter, the skin lesions become fluctuant cystic. Some of the skin lesions healed leaving severe hypertrophic scars, and purulent materials expressed from some of persistent lesions. The patient was trested with systemic 13-cis-retinoic acid, antibiotics, corticosteroids injection and incision & drainage with considerable improvement.
