1.AN EXPERIMENTAL STUDY ON PATENCY RATE OF MICROVASCULAR ANASTOMOSIS USING FIBRIN GLUE.

Microvascular anastomosis has become an essential technique in reconstructive surgery. A lot of techniques have been developed to improve patency rate and reduce operation time. But interrupted suture technique is still most widely used because of limited indication and inconvenience of other methods. A comparative study was undertaken to evaluate new microvasular anastomosis technique using fibrin glue. In this report 40 femoral arteries of Sprague-Dawley rats were anastomosed by utilizing four stay sutures which were placed 90 degrees apart and the intervals covered with fibrin glue, and by conventional eight suture method served as control. The patency rate(immediate postoperative, postoperative two weeks), time needed for vascular anastomosis, and microscopic evaluation were compared to those of conventional microvascular suture technique. Postoperative patency rate was 100% and 85% by fibrin glue technique compared to 100% and 90% by conventional technique at immediate postoperative and postoperative two weeks. Less time consumed with fibrin glue technique by 16 minutes compared to conventional technique by 21 minutes. Microscopically reendothelization was complete with smooth and less injured inner lining and also less inflammatory response by fibrin glue technique compared to conventional technique.

2.Behaviour of mast cells in mice in the course of Entamoeba histolytica infection by strains.

The present report deals with the behaviour of mast cells in mice in the course of Entamoeba histolytica infection by the strains. Mice weighing about l6 gm were used for three experimental groups; control, sham infection and experimental. The experimental group was infected with Entamoeba histolytica trophozoites directly into cecum by laparotomy. Strains isolated from three hepatic amoebic abscess cases were used. Mesenteric samples from the region of terminal ileum were fixed in methyl alcohol and stained with Pugh's solution. The ulcers in cecum were examined. Changes in number and structure of mesenteric mast cells and blood eosinophils were as follows. The number of mast cell in mesenteric tissues of the infected group increased from first day of the infection and persisted up to 34th day of the observation period. Degranulation and disruption of mast cells increased in the infected group compared with groups of the sham operation and the contro1, but showed no difference by the strains of Entamoeba histolytica. The blood eosinophilia was observed in the infected group and persisted until the observation period. Ulcers in cecum were found in all the infected groups. The results above indicate that mast cells are keenly related with the course of Entamoeba histolytica infection.

3.Multiple Myeloma Associated with Adenocarcinoma of the Stomach: report of a case.

A case of multiple myeloma associated with adenocarcinoma of the stomach was review. A 59-year, old Korean man had been abmitted to St. Mary's Hospital in January 1984, with chief complaint of posterior neck pain, and intermittent headache for one year prior to amission. Osteo lytic punched-out lesions were noted on skull and other skeletal x-ray films. Bone marrow aspirates revealed diffuse infiltration of mature and immature plasma cells. Laboratory findings revealed anemia and Bence-Jones proteinuria. Immunoelectrophoresis revealed findings consistent with IgA-lamda type multiple myloma. Alkylating agents and steroids were tried with some clinical improvements. In August 1985, the patient revisited outpatient clinic with chief complaints of epigastric pain and neck mass. Endoscopic biopsy and excision biopsy of the cervical lymph node were performed. By microscopic examination, adenocarcinoma of the stomach and metastasis of multiple myeoma to the supraclavicular lymph nodes were confirmed. Subtotal gastrectomy was performed. By gross and microscopic revealed metastasis to the regional lymph nodes.

4.Statistical Evaluation of the Disabled Patients Consulted to the Department of Dermatology During Their Admission.

A statistical evaluation was made on 248 patients who were consulted to the Department of Dermatology during the disabled patients admission in Korea Veterans Hospital from July 1, 1980 to June 30, 1982. The resuIts were as followings: l. Among the 248 disabled patients, the most common patients, who constituted 68. 8% of the total, were amputee of lower extremity (25.2%), patient of posttraumatic osteoarthritis (23. 2/) and paraplegia (20. 4%). 2. In age distribution, the most frequent age groups were 50 to 59 years, 42, 4, and the lowest age groups were 60 to 69 years, 4. 0g. 3. The 6 most common dermatoses were Eczema (36. 8%), Fungal infection (33. 6%), Drug eruption (8. 0%), Acne vulgaris (6. 4%), Urticaria and Scabies (3. 2% each). 4. In seasonal distribution, the highest incidence (32. %) occurred on winter, and the lowest incidence (14. 0%) on Spring. Eczema was prevalent during Autumn and Winter season and Fungal infection during Summer season. -countinue-

5.A Case of Apocrine Hidrocystoma Occurring in the Female Genitalia.

6.A Case of Trichilemmoma on the Scalp.

Tricilemmoma is a benign, solitary tumor that may be clinically misdiagnosed as basal cell carcinoma or verruca. It affects frequently the face of elderly rnen. We present herein a case of trichilemrnoma in a 70-year-old Korean wornan who had a slowly growing tumor on the scalp for 50 years. The histopathologic findings revealed lobular acanthosis of glycogeri rich clear cells and palisade of columnar cells resting on prominent basement membrane.

7.Gross Anatomical Typing of Hepatocellular Carcinoma: Classification of 49 lobectomized hepatocellular carcinomas.

Forty-nine lobectomized hepatocellular carcinomas(HCC) were classified according to the gross anatomical features. Because the presence of cirrhosis in the remaining liver has a good clinico-pathological implication, cases of HCC were divided into non-cirrhotic(non-LC) and cirrhotic(LC) groups. In both groups, the tumors themselves belonged to either expanding, focal spreading, spreading or mixed type. Another special type, which has been called a "diffuse type" is added in the LC group with the name of "cirrhotomimetic type" Among 49 cases, 21 belonged to the non-LC group and 28 to the LC group. Most common was expanding type(20 cases, 40.8%), which was followed by spreading(32.7%), focal spreading(16.3%), mixed(6.1%) and cirrhotomimetic(4.1%) types. Expanding type of the LC group was the single most common type(13 cases, 26.5%). The accordance rate of gross typing was 0.94. Tumor masses of the LC group showed a greater tendency of having a fibrous capsule(60.7%) and a lobulated cut surface(82.1%), in contrast to those of the non-LC group (28.6% and 42.9% respectively). The patient's age and the HBsAg seropositivity were not different between the groups and between the types. Increased serum level of AFP was particularly frequent in the spreading type(81.3%) of both groups and in the cirrhotomimetic type(100%).

8.A Clinical Study of Benign Epilepsy of Childhood with Centrotemporal Spikes.

Benign epiepsy of childhood with centrotemporal spikes (BECCT) is an electroclinical syndrome characterized by noctural seizure that remit spontaneoully before adulthood, and belong to idiopathic age and location related epilepsies. We reviewed the medical records to analyse the seizure the seizure pattern, and also inspect the EEG recording to identify topography of the epileptiform discharge of 24 patient who met the following criteria: 1) presence of nocturnal seizure or partial seizure confined to the face, 2) normal intecual and development, 3) normal background EEG feature, 4) monomormhic spikes or sharp wave that increase in the frequency during sleep if those state were achieved. The result as follow: 1) The age of seizure onset were 3 to 11 years old (7.08 1.89) and male to female ratio was 1, 4:1. 2) Past history of birth trauma, CNS infection and other diseases involving CNS, were not found. There was past history of febrile convulsoin 33.3% of patient and epilepsy in 8.3%. One of paient's sister had childhood abscence epilepsy and never a nocturnal partial seizure, also and had centrotemporal spikes in EEG. 3) In 91.7% of Seizures occurred during sleep, especially short after sleep onset and the symptomatology of seizure was shown 66.7% of oropharyngeal sign, such as hypersalivation and gargle sound, and 54.2% of hemifacial involvement, 33.3% of speech arrest, 25.5% of upper limb involement, 18.3% of lower limb involvement, and 45.8% of general seizure. 4) In 91.5% of patients had on location of epileptiform discharge that located central (37.5%) midtemporal (33.3%), parietal and frontal. We could observe tangental dipole in 16.7% of patients.

9.Epidermal Changes with Ultraviolet Irradiation and PUVA Treatment in the Black Mouse.

The pretent study was performed to study the epiderrnal changes of ultraviolet irradiation and PUVA treatment in the black mice. The following results were obtained: 1) The histologic changes of the epidermis, number of DOPA positive melanocytes and ultrastructural changes of keratinocytes due to UVB and PUVA were rnore prominent than those of UVA irradiation. 2) The histological changes after 8-MOP photosensitization closely mirnic those of UVR irradiation, and constant, finding was hyperkeratosis. 3) Early histological change after UVL irradiation & IUVA treatment was appearance of dytkeratotic cells. 4) Pro]onged exposure to UVB & PUVA resulted in transient epidermal hyperplasia, and epidermal atrophy occurred 3 weeks after PUVA treatment. 5) Early electron microscopic changes was perinuclear edema, juxtanuclear vacuoles and clumping of tonofilament.
