1.Analysis and Problems of Urolgic Counseling by PC Communication.

Recently with the development in computer technology and its communication system, many changes have come to the medical field. For example, Picture Archiving and Communication System(PACS) has been used in the medicine experimentally. But this system needs high cost and instrument, so this system is not used in personal generally. Our hospital established the medical forum for health and medical counseling in Computer communication network called HITEL. We analysed the questions about urologic problem especially from Sep, 1994 to Jan, 1997. Most of questions are related to external genitalia and sexually transmitted diseases and most of users were in their 2nd, 3rd and 4th decades and male-predominant. In conclusion, medical counseling using PC communication is an effective method especially in urologic field.

2.The Relationship between Health Value Cognition, Health Promotion Behavior and Health Examination Results Among Transit Corporation's Workers.

OBJECTIVES: This study was performed to compare health value cognition and health promotion behavior compliance between healthy group and not being healthy group, and to evaluate the difference of health promotion behaviors compliance between high group and low group in health value cognition. METHODS: The subjects of this study were 565 workers, 233 healthy workers, 172 observation cases and 160 disease cases, selected from 4,919 transit coporations workers. RESULTS: In health promoting behavior compliance, not being healthy group showed lower level than healthy group in exercise and showed higher level than healthy group in interpersonal support significantly. A worker who replied on health as the most value in life was consisted in 37.3% of healthy group and 36. 1% of not being healthy group, Health value cognition according to general characteristics showed no significant difference except only variable of age in healthy group. Health promoting behavior compliance according to cognition of health value showed in healthy group that a high level group of health value cognition was more significant difference than a low level group in self actualization and health responsibility of health promoting behavior and didn't show difference significantly in not being healthy group. CONCLUSIONS: In conclusion, the relationship between health value cognition, health promotion behavior compliance and periodic health examination results showed weakly. Therefore, in order to develop and apply spontaneous health promotion program, it was considered that should emphasize compliance than cognition.

3.Semi - quantitative Comparison of Terminal Restriction Fragment Length and Telomerase in Breast Cancer for Biotherapy.

PURPOSE: We determined the clinical significance of telomerase activity and telomere length in breast cancer patients and also developed the measuring system of telomerase activity change with RNAse A pre-treatment. MATERIALS AND METHODS: We measured the telomerase activity in 71 breast cancer tissues and paired normal tissues with TRAP (Telomeric Repeat Amplification Protocol) assay. Telomerase activity was calculated by computer-assisted densitometry compared to telomerase activity of the 293 control cell line. To develop the measuring system of telomerase activity modulation, we measured the telomerase activity after the treatment with RNAse A, 150microgram/ml, which inhibited 70% of telomerase activity compared to control in the 293 control cell line. In 59 paired tissues with telomerase activity, terminal restriction fragment (TRFs) length were measured using Southern blotting. RESULTS: Sixty-three out of 71 cancer tissues showed telomerase activity (88.7%), while no telomerase activity was detected in their paired normal tissues. Telomerase activity was correlated to the node metastasis (p=0.02) and stage (p=0.005), but not to the tumor size or the hormonal receptor status. TRFs were neither specific to tumor tissues nor related to any of the clinical parameters. However, changes of TRFs of the tumor tissues from their paired normal tissues were correlated to the telomerase activities. Also the patients with different TRFs between cancer and normal tissues were in more advanced stage. After pre-treatment with the 150microgram/ml of RNAse A, telomerase activity in the tumor tissues showed variable inhibition. Relative inhibition, the ratio of inhibited telomerase activity in each tumor tissue compared to the inhibition of 293 control cell line, was proportional to the telomerase activity. CONCLUSION: In breast cancer, telomerase activity was specific to the tumor tissues and correlated to tumor progression. A combination of telomerase activity and TRFs changes can be used as a guidline in detecting a better candidate for telomerase inhibition. Semi-quantitative assay with RI system can be used in evaluating the changes of telomerase activity after treatment with a new telomerase inhibitor with TRAP assay.

4.Comparison of Doppler Waveform Index in Benign and Malignant Ovarian Tumor.

Doppler sonography has been introduced to evaluate uterine and ovarian diseases. It has been reported to be used to distinguish benign from malignant adnexal masses. The purpose of this study was to assess the blood flow characteristics of adnexal masses and to determine the optimal cut-off points for the Doppler indices obtained, resistance index and pulsatility index, to best differentiate benign from malignant lesions. We performed pulsed Doppler ultrasound in seventy-eight adnexal masses between February 1989 and June 1994. The pulsatility and resistance index were calculated from the waveforms generated from blood flow within the ovary. There were 51 benign and 27 malignant histologically proved ovarian lesions. All women have been operated on. The results were as follows ; Benign tumors and cysts had a significantly higher pulsatility index(mean, 1.23+/-0.74; range, 4.46-3.36) and resistance index(mean, 0.64+/-0.19; range 0.34-1.43) than did malignant tumors(pulsatility index : mean, 0.87+/-0.49; range, 0.31-2.27; resistance index : mean, 0.50+/-17; range, 0.23-0.8). However, some overlap in individual values for benign and malignant lesions was found. In conclusion, our data suggest that high pulsatility and resistance indices may indicate benign adnexal masses ; however, considerable overlap in pulsatility and resistance indices between benign and malignant lesions were noted, and further work will be needed before the validity of these factors is proved.

7.Full-mouth rehabilitation with pressed ceramic technique using provisional restorations.

With the introduction of dental implants, restoration of missing teeth with conventional fixed or removable partial dentures is being replaced with implants. Especially, with young patients, not only longevity but also esthetic factors need to be considered. Implant restorations provide long-term success functionally but, esthetic complications such as, marginal exposure due to gingival recession, loss of the papilla and dark color of metal abutments may occur. Recently, zirconia restorations with CAD/CAM technology provide functional, biocompatible and esthetic restorations possible. All-ceramic restorations using the pressed ceramic technique show better fracture toughness values than those of the conventional porcelain veneering technique. Pressed ceramic technique creates the veneer design in wax and the lost wax technique is used to create the restoration. The final contour of the restoration may be controlled during wax-up. A 22-year old female patient was restored with dental implants and zirconia restorations using the pressed ceramic technique presenting short-term but optimistic prognosis.
