1.Effects of Preinduction Atropine on the Hemodynamic Response to Induction with Fentanyl and Vecuronium for Coronary Artery Bypass Grafting.

BACKGROUND: Induction of anesthesia with a high dose of fentanyl and vecuronium decreases the heart rate and blood pressure. This study was designed to evaluate the effect of preinduction atropine on these hemodynamic changes in patients undergoing coronary artery bypass graft surgery (CABG). METHODS: Forty-one patients who underwent CABG were randomly divided into two groups. After insertion of a radial artery cannula and a Swan-Ganz catheter, normal saline 1 ml (control group, n = 20) or atropine 0.5 mg (atropine group, n = 21) was injected intravenously 1 min before the induction of anesthesia. Anesthesia was induced with a first dose of fentanyl (5-8 microgram/kg) and vecuronium (0.12 mg/kg) and a second dose of fentanyl (5-10 microgram/kg). The patient was then intubated. Hemodynamic variables were measured before the induction of anesthesia, 1 min after the administration of each drug during the induction of anesthesia and 5, 10, and 30 min after the intubation. RESULTS: There was no significant differences between the two groups in terms of demographic data except that the number of patients with diabetes mellitus was greater in the control group than in the atropine group. The number of patients treated for hypotension or bradycardia during the induction of anesthesia was greater in the control group than in the atropine group, but this was not statistically significant. Heart rates significantly decreased in the control group but were maintained in the atropine group without any significant tachycardia. Blood pressure significantly decreased in both groups. CONCLUSIONS: Intravenous injection of atropine before anesthetic induction in patients undergoing CABG attenuates the decrease in heart rate resulting from anesthetic induction with high dose fentanyl and vecuronium. However, it didn't prevent the decrease in blood pressure nor did it reduce the incidence of treatment for hypotension.

2.Comparison of the Nutrition Knowledge, food Habits and Life Styles of Obese Children and Normal Children in Elementary School in Kyeong-gi Province.

The nutrition knowledge and food attitude of obese children were assessed and compared with their normal children counterparts to provide information for nutrition education programs. One hundred forty six obese children and 92 normal children from the fourth to sixth grade in Bucheon and Anyang elementary schools of Kyeong-gi Province were selected for the study. A multiple choice questionnaire, including 12 items of general nutrition knowledge and 13 items of weight control, were used to test the nutrition knowledge of children. The results of this study are as follows : 1) 57.5% of male and 40.9% of female obese children had experience with weight control, but the rate of correct answers on the nutrition knowledge test ranged from 53% to 58%, which does not show any significant difference between the two groups. The rate of correct answers on items conrerning 'general nutrition knowledge' and 'nutrition knowledge for weight control' did not show any significant difference among the two groups. 2) Obese male students showed a higher rate of correct answers(59.2%) on items of 'the proper food selection for obese children', compared with their counterparts and also obese female students showed a higher rate of correct answers(88.6%) on 'physical exercise', compared with their counterparts. 3) The rate of correct answers on nutrition knowledge of 'basic food groups', 'the functions of iron and vitamins', 'the concept of energy', and 'the principle of weight change' were lower than 40%. 4) The obese children frequently did not have breakfast, overate during lunch in many cases, and had low recognition on knowledge that was necessary to weight control. 5) Only 30.9% and 22.7% of male and female obese children replied that they would participate in weight control programs. However, 53.6% of males and 68.2% of females did not show any concern in participating the programs. From these results, it was suggested that it is necessary to motivate the obese to participate in weight-control program. When developing nutrition education programs for the obese, the contents of the questions that showed a low correct answering rates should be emphasized.

3.Change of Serum IGF-l level in Knee Osteoarthritis.

BACKGROUND: To examine serum Insulin-like growth factor-I(IGF-I) level in patients with knee osteoarthritis(OA) and to determine the relationship between the serum level of IGF-I with pain intensity and severity of OA knee. METHODS: Serum IGF-I levels measured by standard radioimmunoassay techniques were obtained from 30 knee OA patients and 30 normal controls matched by age and gender. Pain intensity was measured by pain rating scale(PRS) and visual analogue scale (VAS). Kellgren grade also used for assessing the severity of knee OA. RESULTS: No difference was found in serum IGF-I levels between the cases and controls(cases: 279+/-128 ng/dL, controls: 282+/-113 ng/dl). There was no correlation between pain intensity and serum IGF-I levels. There is no significant association between IGF-I level and Kellgren grade. CONCLUSION: Serum IGF-I level does not decrease in knee OA. The serum IGF-I levels can not indicate pain intensity and severity of knee OA.

We report a rescently experienced case with atypical radiologic manifestation of solid and papillary epithelial neoplasm of the pancreas in a 37 years old female patient. The tumor had heavy calcification on its capsule wall and septa. Instead of the usual encapsulated lesion with partly solid and partly cystic-hemorrhagic components, the lesion consisted of numerous fine cavitations containing air without fluid component except focal abscess fluid.

5.The Effect of Daunorubicin on Experimental Proliferative Vitreoretinopathy.

Proliferative vitreoretinopathy (PVR) is a main cause of failure in retinal reattachment surgery. There have been many studies about the inhibition of proliferative vitreoretinophthy with several drugs. Authors investigated the inhibitory effect of proliferative vitreoretinopathy and retinal toxicity with various concentration of daunorubicin after intravitreal injection into the eyes of the pigmented rabbit. 7 pigment rabbit (11eyes) were used as subjects. After lensectomy and vitrectomy, control group was injected dermal fibroblast and F-BSS, and treatment group was injected dermal fibroblast and 5, 10, 15, 30 nmol Daunorubicin. At two weeks after intravitreal injection, both group were enucleated and examined with gross finding, light--microscopy, and electronmicroscopy. In all control group, proliferative vitreoretinopathy was found, but only preretinal membrane formation was found in 5, 10 nmol Daunorubicin injected group. In 15 nmol Daunorubicin injected group, the retina structure was preserved normally. In 30 nmol Daunorubicin injected group, the retinal outer segment was degenerated in microscopic finding. These results show that Daunorubicin has a potent effect on proliferative vitreoretinopathy, especially in 15 nmol, but retinal toxicity is suspected in marethan 30 nmol.

6.Recognition status of early at-risk alcohol intake.

BACKGROUND: Alcohol has much effect on our lives as a taste food that people in almost all of the world generally have had. The effects of alcohol cause social problems as well as physical and psychological problems. Recently, interest about drinkirg increasingly rises as the rate of drinking accident increases in Korea. For this, our study examined the amount of alcohol consumption and early at-risk alcohol intake. METHODS: The prepared questionnaire was presented to subjects who were employees in Inchon . The anonymous, self-administered questionnaire was composed of questionnaire concerning alcohol consumption, early at-risk alcohol intake and doctor's role in drinking behaviour. RESULTS: Among 305 subjects, there were 189 (62%) men, 116 (38%) women whose mean age was 37. 9years. 87(28.5%)subjects were classified as high alcohol group and 218(71.5%) low alcohol group. High alcohol group was defined as early at-risk alcohol intake for men who were consuming 135g of alcohol per day, 630 per week and for women 90g per day, 270g per week. Low alcohol group was defined as early at-risk alcohol intake for men whose consumption was 90g per day, 450 per week and for women 45g per day, 270g per week . Mean alcohol consumption of drinker was 158.9g per week. 3 days per week people should not drink in order to avoid risk Although 54% of respondents thought doctors should ask about their drinkirg habits, only 16.4% of respondents reported ever being asked about alcohol intake on the subject. CONCLUSION: Early at-risk drinking subject in the study was much more than those suggested by NIH(National Institutes of Health). We conclude that primary care physicians should take more interest in screening, treating and preventing alcohol related problems and advise and educate patients to cut down or quit drinking.

7.Study of Fluorescein Pharmacokinetics.

Authority estimated the concentration of fluorescein sodium in the blood, anterior chamber and vitreous after injection of 10% fluorescein sodium, 25mg/kg, according to the lapse of time. Also intravitreal fluorescein sodium concentration was measured after intravitreal injection of 0.1 % fluorescein sodium 10 micro liter according to the lapse of time. The results were as follows. 1. In the normal rabbits, the concentration of fluorescein sodium in the anterior chamber and vitreous were 159.3 +/- 101.7 X 10(-9) gm/ml and 3.6 +/- 4.7 X 10(-9) gm/ml, respectively after 1 hour of fluorescein sodium intravenous injection. The Cv/Ca was 0.02 +/- 0.028(Cv: concentration of fluorescein in the vitreous) (Ca: concentration of fluorescein in the ant. chamber). 2. After injection of sodium fluorescein in the normal rabbit, the concentration of fluorescein in the blood showed rapid decrease in logarithmic manner in the vitreous, the concentration peaked 1 hour and 2 hours after injection which persisted several hours. 3. The concentration of fluorescein sodium in the vitreous after intravitreal injection of fluorescein decreased at logarithmic rate which disappeared 24 hours after injection.

8.Retinal Vessel Obstruction caused by Orbital Compression After Paranasal Sinus Surgery.

Various ophthalmic complications can occur after intranasal sinus surgery due to the closed anatomic relationship between orbit and paranasal sinuses. The authors experienced a patient who had complained of sudden proptosis and loss of vision in his right eye 2 days after Caldwell-Luc operation from the otolaryngologic clinic. Fundoscopic finding showed cherry-red spot, scattered retinal hemorrhage, retinal edema, arteriovenous obstruction sign. Emergent decompression surgery was done, under the impression of retinal vessel obstruction caused by orbital compression, and his visual acuity improved moderately after this procedure.

9.Effect of Occlusion Therapy and Change of VEP in Amblyopia.

VEP(Visual Evoked Potential) is being widely used as a diagnostic method in amblyopia. In order to assess the visual improvement during occlusion therapy and VEP change between pre and post-occlusion therapy in amblyopia, 32 amblyopic chi1dren ranging from 3 to 13 years old were investigated. The results were as follows: 1. Among 21 cases of visual improvement above 2 lines with Han's visual acuity chart, occlusion therapy was effective in 73.7% of strabismic amblyopia, and 87.5% of anisometropic amblyopia. The mean period of visual improvement above 2 lines was 2.2 months. 2. As compared to non-amblyopic eye, VEP amplitude of amblyopic eye was significantly reduced. 3. There was no statistic significance of P3 latency between amblyopic and non-amblyopic eyes. 4. As compared to control group, the side difference of amblyopic group was significantly increased. 5. With visual improvement above 2 lines, increase of amplitude was revealed 87.5% and decrease of side difference was revealed 87.5% in 10 cases of investigating VEP change. 6. In 7 eyes of visual improvement above 2 lines, change of P2 latency between pre-occlusion and post-occlusion therapy revealed no significance statistically.

10.The Effect of Anticancerous Drug in the Fiboblast-Mediated Collagen Matrix Contraction.

In vitro PVR(Proliferative vitreoretinopathy) models allow identification of factor which may inhibit porliferation and contraction. In this study we evaluated the contraction of collagen matrix by choroidal fibroblast and the inhibition of contraction by antipoliferative drug. Each antiproliferative drug showed inhibition of collagen matrix contraction : Colchicine (0.1microgram/ml), Cytochalacin (0.05microgram/ml), Puromycin(10microgram/ml). Transmission electron microgram of collagen matrices containing colchicine, cytochalacin or puromycin showed no collagen fiber surrounding choroid fibroblast and showed cell destruction. Scanning electron microgram of collagen matrices containing colchicine, cytochalacin and puromycin showed that collagen fibers were well preserved without distortion. Colchicine, cytochalacin and puromycin are effective inhibitor of cell mediated contraction in additon to it`s potent antiproliferative effect wherease Interfereon has no anticontractile effect. The current study present a model to investigate the effect of antiproliferative drug on fibroblast mediated collagen matrix contraction.
