1.Diffuse Panbronchiolitis: High-Resolution CT Findings and Correla:don with Pulmonary Function Test.

PURPOSE: Diffuse panbronchiolitis(DPB) is a chronic inflammatory airway disease of unknown causes mainly affecting the respiratory bronchioles and the more proximal bronchi. Findings on chest radiographs and high-resolution CT(HRCT) are well known and Akira classified HRCT findings of DPB into four types. Purpose of this study is to evaluate the relationship between findings of HRCT and PFT. METHODS AND MATERIALS: We retrospectively analyzed the chest radiographs and HRCT images of eleven patients with DPB and compared CT classification with pulmonary function test. RESULTS: Chest radiographs usually showed small nodular opacities throughout the both middle and lower lungs. The HRCT findings of DPB were centrilobularly distributed small nodular densities, branching linear densities contiguous with small round densities, dilated and thickened peripheral and central airways including bronchioles. More than one CT type by Akira's classification, usually two or three types, were found in nine patients. There was good correlation between CT types and FEV1%(p<0.05), CT types and FEV1/FVC (p<0.05) respectively. CONCLUSION: HRCT seems to be more useful for diagnosis and disease progression of DPB as compared with the chest radiographs or clinical stage such as pulmonary function test.

2.The Role of CT and MR in Diagnosis of Aortic Dissection.

PURPOSE: The purpose of this study was to determine the role of CT and MR imagings in the diagnosis aortic dissection and differentiation between the true and false lumen. MATERIALS AND METHODS: We retrospectively studied forty patients with aortic dissection(AD) diagnosed imagings or surgery. Of the forty patients, 19 were examined with only CT, 14 with CT and MR, and 7 with MI~: Our points of view were(1) the classification of AD according to configuration of intimal flap by cross-sectional imaging, (2) differentiation between the true and false lumens, (3) the course of the false lumen, and (4)! detectability of the origin of major branch vessels of the abdominal aorta. RESULTS: The classification by corss-sectional imaging were crescentic(65%), circumferential(15%), flat(12%), and irregular(8%) type, in which false negative diagnosis was made in 1 case of crescentic and circumferential type, respectively. In 2 case of flat type and 1 case of irregular type, the differentiation between the true and false lumen was impossible with CT. The course of the false lumen in descending thoracic aorta revealed countrclock wise rotation(66%), clockwise rotation(5%) or fixed(29%) apperance. MR imaging was superior to CT in the detection of the origin of major branch vessels of the abdominal aorta. The determination of the origin of major branches of abdominal aorta arising from the true and false lumen were impossible in 2 cases in which only CT was done. CONCLUSION: Diagnosis of crescentic and circumferential types of AD with narrow and thrombosed false lumen was problematic in both CT and MR with no difference of diagnostic accuracy between the two modalities. The differentiation between the true and false lumen was difficult in flat and irregular types with only CT. Therefore, when surgical treatment is considered as in type B aortic dissection, MR imaging is recommended in order to determine the origin of major branch vessels.

3.Alteration of Karyotype and Cell Cycle Induced by UVB Irradiation In Human Keratinocytes.

To investigate the change of cell cycle by genotoxic stress and rebound proliferation in human keratinocytes, the proportions of cell cycle phases were estimated with challenge of UVB irradiation (200 J/m2). With UVB irradiation cell cycle was estimated by Cell Fit program in Flowcytometer, and main change of the cell cycle was S-phase pro-longation. In karyotyping, near diploid number of chromosomes changed to hypoteraploid number. Cell cycle phase was estimated in two groups of cells, near diploid and hypoteraploid. In near dipoid cells, S-phase prolongation was specific phenomenon, while specific G0-G1 phase prolongation was shown in hypoteraploid cells which made transformed foci in culture. The new structural anomalies were del (5q21), 8p+, and t (5 : 8)(q21 : pter). Among them, del (5q21) was found in all transformed hypotetraploid cells. These data suggest that progress of cell cycle could be [G1-S-(G2-G1)-S] by UVB irradiation and deletion of 5q21 has a key role for anchorage independent growth, which is deletion of tumor suppressor gene APC locus. That is one of important mechanisms in keratinocyte transformation by UVB irradiation. With the changes of chromosome number and cell cycle, sizes of nuclei got to bigger by two times and growth rate was delayed.

4.Palliative Resection for Incurable Colorectal Cancer.

Despite a recent trend toward increased screening and public awareness for colorectal cancer, 30% of patients present with incurable disease. This study was designed to identify objective criteria that might help surgeons decide which patients with incurable colorectal cancer will benefit from palliative resection. Among 33 patients considered incurable colorectal cancer, twenty one patients underwent palliative resection and twelve patients had bypass surgery. Incidence of postoperative complication after palliative resection was 61.9%(13 cases), and after bypass surgery was 58.3%(7 cases). Among patients treated by palliative resection, one patient required reoperation for postoperative bleeding. The operative mortality after palliative resection was 19%(4 cases), and after bypass surgery was 25%(3 cases). The median survival was 11.4 months for patients treated by palliative resection, and was 9.7 months for patients treated by bypass surgery. These results show that palliative resection can be done safely and effectively in patients with incurable colorectal cancer. We believe this approach improved the quality of the remaining life in these patients.

5.A Case of Giant Keratoacanthoma.

A 63-year-old man had a huge verrucous protruding mass over the suprasternal area. The lesion enlarged rapidly over 3 mooths, and measured about 10×8 cm. The histologic finding of the biopsy specimen showed nests of squamous epithelium with central keratinization, infiltrating the dermis. The neoplasm was treated successfully with surgical excision.

7.Comparison between Glidescope and Lightwand for tracheal intubation in patients with a simulated difficult airway.

BACKGROUND: Although Lightwand and Glidescope have both shown high success rates for intubation, there has been no confirmation as to which device is most effective for difficult endotracheal intubation. We compared the Glidescope and Lightwand devices in terms of duration of intubation and success rate at the first attempt in a simulated difficult airway situation. METHODS: Fifty-eight patients were randomized to undergo tracheal intubation with either the Glidescope (Glidescope group, n = 29) or the Lightwand (Lightwand group, n = 29). All patients were fitted with a semi-hard cervical collar in order to simulate a difficult airway, and intubation was attempted with the assigned airway device. The data collected included the rate of successful endotracheal intubation, the number of attempts required, the duration of the intubation, as well as the interincisor distance, hemodynamic variables, and adverse effects. RESULTS: There was no difference between Glidescope group (92.6%) and Lightwand group (96.4%) in terms of success rate for the first attempt at intubation. The duration of successful intubation for the first tracheal intubation attempt was significantly longer in Glidescope group than in Lightwand group (46.9 sec vs 29.5 sec, P = 0.001). All intubations were completed successfully within two intubation attempts. The incidence of hypertension was significantly higher in Glidescope group than in Lightwand group (51.9% vs 17.9%, P = 0.008). CONCLUSIONS: In a simulated difficult airway situation, endotracheal intubation using Lightwand yielded a shorter duration of intubation and lower incidence of hypertension than when using Glidescope.

8.A Case of Multiple Keratoacanthoma with Intralesional 5-FU and Etretinate Therapy.

Keratoacanthoma is a benign epithelial growth tumor arising from the hair follicle predominantly in elderly patients on sun-exposed skin. Intralesional therapy with 5-fluorouracil is a safe treatment and may be the treatment of choice for multiple keratoacanthoma and solitary lesion which are difficult to remove surgically. We have experienced a case of multiple keratoacanthoma which involved the right nasolabial fold and the left ear. We treated them with oral etretinate and intralesional 5-fluorouracil(5-FU), therefore minimized the scar of the lesion and the possibility of recurrence.
