1.Two Cases of Dannheim's Anterior Chamber Lens Operation.

In this paper I report the two cases of Dannheim's anterior chamber lens operation in the monocular aphakic patient for the first time in Korea. The first case(19 years old male patient with traumatic cataract in his left eye) had been {)perated on July 10, 1960. After operation his corrected visual acuity was 0.8 wearing glass (Sph. +12.00D.). On Aug. 2S, 1960 he had taken another operation for the insertion of Dannheim's anterior chamber lens (power; +12.50D., size; 12.5mm.). On Oct. 8. 1960 the visual acuity was 0.9 with adding Sph. -5.50D. glass in front of his left eye. Through the slit lamp examination the loop of the anterior chamber lens was well fixed into the chamber angle and remained in situ. He has carried a very comfortable life without any remarkable trouble for 16 years untill nowadays. The second case (50 years old male patient with senile cataract complicated by secondary glaucoma in his left eye) had been operated intracapsullary on Jan. 15, 1960. After cataract operation he had used corneal contact lens and his corrected visual acuity for distant was 1.5 (Sph. +14.50D. Rad. 7.8) and I.O.P. was within normal limit. 9 years after cataract operation he refused to wear contact lens due to discomfortness and therefore he had reoperated to insert Dannheim's anterior chamber lens (same as case 1.) on Sept. 10. 1969. After operation his visual acuity was 0.8 for distant vision, but slight movement of the anterior chamber lens was noticed by slit lamp examination. On Jan. 12. 1970. he had had moderate iritis and slightly elevated I.O.P. (20.6mmHg.). Those symptoms had subsided after 7 day's treatment by local instillation of 2% T-pilocarpine sol. and oral acetazolamide and corticosteroid medication. On March 7, 1973. he revisited my clinic due to a severe ocular pain in his left eye. At that time the I.O.P. was 30mmHg. and the visual acuity was reduced to 0.04 due to severe anterior uveitis. And so I removed the anterior chamber lens on March 8. 1973. The Fixation of anterior chamber lens into the filtration angle is very important factor for the determination of the prognosis of operation. The continuous experiment and development of the method to fix the loop of the anterior chamber lens into the filtration angle is required for the sake of successful operation.

2.Clinical Experiences of Extended wear Soft Contact Lens: Permalens(Perfilcon-A): in Aphakic Patients.

Extended wear soft contact lens will be the fascinating idea among the contact lens wearers in near future. A contact lens that is to be left on the eye surface during periods of sleep must achieve a degree of ocular compatibilities not previously encountered in contact lens practice. In this paper I report the clinical studies of Permalens, that is made by Global Vision, England, in 6 aphakic patients. During the 4 months periods of follow-up studies, all patients ara using their lenses very comfortably and no specific complications are found until now.

3.A Survey of Post -Operative Daily Life Behavior and Degree of Satis faction in Patients Two or More Years after Cataract Surgery.

Recently, the number of cataract surgeries has rapidly increased. This study was devised to evaluate the post-operative daily life behavior and degree of satisfaction in the patients who underwent cataract surgery two or more years ago. Of 400 patients who underwent ECCE with IOL implantation, 125 patients[31%]replied to the mail questionnaire consisted of thirty questions. Hospital records were also reviewed. 84%of the patients were satisfied with outcomes of cataract surgery, which was significantly correlated with current visual acuity[p<0.01]. Degree of satisfaction was significantly higher in patients with bilateral surgery compared with those with unilateral surgery[p<0.05]. Age, sex, occupation, economic and general health status were not significantly associated with degree of satisfaction. As for life behavior, discomfort while driving an automobile was the most common complaint, especially in the patients who underwent unilateral surgery. Concerning ocular symptoms, many patients complained of foreign body sense, epiphora, glare, fatigue, and near vision discomfort. We reconfirmed that the decision to perform cataract surgery should be made with careful consideration of long term prognosis and above results.

4.Subluxation of Iris Supported Intraocular Lenses and their Management.

There are 2 subluxations of Binkhorst 2-loop lens among my 70 cases of intraocular lens implantation during 4 years duration. One Worst Medallion lens that was implanted by American doctor had subluxation 10 months after operation and managed with medical treatment by the author. One of the Binkhorst 2-loop lens was reposited by surgical method and another 2-loop lens was removed due to incomplete constriction of the pupil with vitreous prolapse during surgical manipulation. The two reposited caSeS had good vision but the removed case had very poor vision(20/200) due to Severe corneal endothelial dystrophy.

5.Successful Treatment of Trichophyton Rubrum Corneal Ulcer Following Posterior Chamber Lens Implantation.

A 55 year old female was admitted for cataract extraction in her right eye. On Jan. 12, 1986 she underwent an uncomplicated ECCE with insertion of Sinskey PC lens. On the 9th. PO day, slight thinning on the superior limbal area adhering with grayishwhite discharges and mild degrees of flares in the anterior chamber were found. On the next day the amout of hypopyon and thinning of wound margin were aggravated, so debridement of wound margin and resuturing of C/S wound with conjunctival flap covering were performed. The symptoms and signs of corneal ulcer worsened in spite of the massive antibiotic and corticosteroid therapy. On the 27th, post-reoperation day, Trichophyton rubrum was found on the culture specimen. After then antimycotic treatment with subconjuctival injection of Amphotericin-B, 5% Natamycin(pimaricin) eye drop, 1% Flucytosine eye drop and oral Nizoral(Ketaconazol) were started. The rethickening and slight healing signs of the corneal ulcer was noticed from 7 days after antimycotic treatment and complete healing with thick pannus formation was noticed 60 days after reoperation. On Jully 14 and July 21, 1986 Nd: YAG Laser iridotomy performed to break the inflammatory membranes. The final visual acuity is 20/(20/40 X Cyl. + 3.00D. A30 degrees) in the right eye.

6.Intraocular Lens (Binkhorst 2-Loop Lens) Implantation in 7 Cases.

7 cases of monocular senile cataract patients have received intraocular lens implant: operations. Among the 7 patients, 4 males and 3 females are included and the mean age was 63.7 years old. The operations were performed during the period between Aug, 1978 to Oct, 1979. The mean visual acuity after IOL implant was 0.62 and there is no difference from other repoters for postoperative visual improvements. Binocular visual functions are well maintained in all cases by the Titmus test. The final results with iridocapsular lens implantation after extracapsular cataract extraction reveaeld very encouraging facts although the observation periods were too short.

7.A Case of Optic Nerve Glioma.

Gliomas of the optic nerve are relatively uncommon. We have experienced one case of glioma of the optic nerve recently. The patient was 17 year old boy who had exophthalmos and visual disturbance in his left eye for three years. The vision was CF/30cm. in his left eye and the optic nerve head was extremely atrophic. The eyeball was displaced infero-temporally. The exophthalmic value was 14mm. in right and 18mm. in left eye. On July 31, 1986, the left eyeball was exposed through Kroenlein method and the tumor mass was excised totally but the eyeball remained intact. The gross appearance of the tumor was large fusiform shape(3.5 X 2.0 X 1.8cm.) with well encapsulated grayish-white surface. The cut-section of the tumor revealed grayish-white solid area and hemorrhagic cystic area. The histopathologic slides showed that the tumor was composed of multiple pilocytic astrocytic cells in the solid area and in microcystic area. The cytoplasm of the tumor cells were bipolar spindle shape and the nucleus was uniform in general and there was no sign of mitotic figure. Numerous dense and amorphous eosinophilic materials so called "Rosenthal fibers" were also seen in the microcystic areas. Finally we have diagnosed this tumor as the Optic nerve glioma.

8.The Evaluation of Fundus Findings of Hypertensive and Arteriosclerotic: Retinopathy Among 4143 Korean Adults(Part II).

The authors evaluated the fundus findings among the 4143 relatively healthy Koreans who visited the Health Clinic Center in Koryo General Hospital in Seoul for the routine physical check-up from June 1983 to June 1984. In this study, we investigated the relationships among the hypertensive arteriosclerotic retinopathies with the systemic blood pressure and serum cholesterol, neutral fat, beta-lipoprotein. The results are: 1. Among the 4143 cases, 2989 males and 1154 females are included, their age distribution was from 10 to 99 years old. 2. Of the 4143 cases, 101 cases had Hypertensive Retinopathy and of these cases K-W Grade I were most common. 3. 139 cases of the 4143 cases had Arteriosclerotic Retinopathy and G-I was most common. 4. The K-W Grades of the Hypertensive Retinopathy were positively related to the sistolic, the diastolic blood pressure. 5. There were no significant relationships between Hypertensive Retinopathy and serum cholesterol level, neutral fat, beta-lipoprotein. 6. In the A-V crossing phenomenon, the incidences of concealment and displacement were positively related to the Grades of the Arteriosclerotic Retinopathy. 7. To compare with the former evaluation(Part I) 5), the following results are obtained: (1) The incidence of hypertensive retinopathy in part I was 8% and 2.43 in part II. (2) The incidence of arteriosclerotic retinopathy in part I was 6.5% and 3% in part II.

9.The Progression of Myopia with Age.

The authors evaluated the 56 patients who received the diagnosis of myopia at the department of ophthalmology in Koryo General Hospital in Seoul from May 1975 to August 1985. In this study, we reviewed the statistics of the progression of myopia with age and with the number of visits to the hospital and also the differences between male and female and left eye and right eye. The results are as follows: 1. Myopia progresses with increase of age. 2. The mean diopter of myopia over 21 years is not so remarkable compared with below 20 years. But it is not absolutely true because the number of patients are so small. 3. On the average, males are slightly higher in the degree of myopia than female, but this seems to have no statistical meaning. 4. In terms of the degree of myopia, it is higher after the age of 12 than before 12.

10.Study of a Simplified Screening System for Cataract Patient.

Recently, the number of surgeries with age-related cataracts has increased remarkably high due to the safety of modern cataract surgery and reliable post-operative visual function has been widely recognized by patients. In spite of such an increase in the number of patients, the authors specualtes that there are still many cataract patients with highly deteriorated visual function who havehad no occasion to receive an ocular examinatio. In order to screen such patients during their general health examination, the authors` study group experienced and evaluated the simplified cataract screening system recently proposed by Sasaki et al, Kanazawa Medical University in Japan. The cataract screening system consists of two steps; primary screening system is 12-item questionnaires, and far vision. In order to evaluate the screening system in Korea, it was used to test 224 indivisual who had wanted to be examined ophthalmologically in two area, Yongsan, Seoul and Donghae, Gangwondo province. Initially, well-trained nurse did the screening system, and then the same patients were examined by one ophthalmologist. The agreement ratio between the screening test and the final judgement for cataract was 70.8% and the final cataract-screening rate was 95.1%. Based on these results, the authors propose the possibility of clinical use of this screening system in general health care units.
