1.A Case of Primary Orbital Hemangiopericytoma.

Hemangiopericytoma is a rather uncommon neoplasm composed mainly of cells derived from the vascular pericytes of Zimmermann, which are normally found in close apposition to the endothelial cells of capillaries and postcapillary venules. Authors studied a case in which primary hemangiopericytoma located in the left orbit of a 54-year-old woman. Review of the literatures about incidence, clinical symptoms, histological features and biological behavior of the orbital hemangiopericytoma was presented.

2.Glomus Tumor in the Hand.

Glomus tumor arises from the subcutaneous glomus body, which is an arteriovenous anastomosis involved in the regulation of cutaneous blood circulation and are widely distributed in the body, especially the hand. Twelve patients (male 3, female 9) with glomus tumors of the hand, who were operated from Jan. 1986 to Dec. 1994 were selected. The lesions were distributed in the distal phalanges of the thumb (6 cases), the index (3 cases), the ring finger (2 cases) and the little finger (1 case). All patients had the classic triad of symtoms: pain, tenderness and temperature sensitivity. Palpable mass (4 cases) and bluish purple discoloration in the skin (6 cases) were also presented. They had no trauma history and on the roentgenography, bone change detected in 2 cases. A simple excision with partial nail extraction was done in all cases. The symtoms were disappeared in all cases and no recurrence was found. For a complete cure, meticulous complete excision is the only recommended treatment.

3.A Comparison of Physical Health, Anxiety and Depression between the Dan-Jeon Breathing Trained group and non-trained group.

The purpose of this study is to compare the physical health, anxiety and depression of the Dan-Jeon Breathing trained group with those of a non-trained group and to analyze the relationship between the training period of Dan-Jeon Bre athing method and physical health, anxiety, and depression. The number of people in the Dan-Jeon Breathing trained group over 3 months was 149 and in the non-trained group 142. Data were collected from August to October 1999. As tools, 35 physical symptom questions were chosen from The Cornell Medical Index modified by Nam(1965) and from the Symptom Checklist-90-Revision(SCL-90-R) by Kim(1984); 10 questions on anxiety and 13 questions on depression were used in the study. The statistics of the study were gathered by using SPSS Window; the analysis was made by applying x(2)-test, t-test, Pearson correlation, ANOVA and Scheffe tests. The results were as follows. 1. The Dan-Jeon Breathing trained group had lower physical symptom score and showed less anxiety and depression than the non-trained group, which supported the 1st, 2nd, 3rd hypotheses. 2. The longer the training period of Dan-Jeon Breathing, the lower the physical symptom score, which supports the 4th hypothesis, but the 5th and, 6th hypotheses were not supported because anxiety and depression did not get lower as the training period of Dan-Jeon Breathing got longer. 3. The physical symptom score, anxiety and depression that were made by the general characteristics of Dan-Jeon Breathing trained group were analyzed. As a result, the physical symptom score of women was greater than that of men. Physical symptoms score and the rate of anxiety and depression were different by education levels. The degree of anxiety was different by age. Those who have a religion have a higher physical symtoms score than people who have no religion. There was no difference in physical health score, anxiety and depression according to marital status, economic status and occupations. The results suggest that the physical and mental health status of Dan-Jeon Breathing trained group is better than that of the non-trained group. Physical symptoms scores gets lower as the training period of Dan-Jeon Breathing gets longer which results in the improvement of physical health status.

4.Expressions of MAGE-3, PCNA, p21, and p53 Proteins in the Hypopharyngeal Squamous Cell Carcinoma Cell Line (PNUH-12) Analysed by Bivariate Flow Cytometry.

MAGE (melanoma antigen gene) is a tumor specific shared antigen, presented by HLA class I molecules, which is recognized by cytotoxic T lymphocytes. MAGE proteins are expressed in malignant tumor cells, in contrast to no expression in normal or benign tissues except for testis and placenta. MAGE might be a potential target for immunotherapy of malignant tumors. However, its biological aspects associated with cell cycle are not yet described. The flow cytometry is a useful tool for objective and quantitative analyses of heterogenous tumor cell population. To understand the status of MAGE related to cell cycle and its relationship with p53 as the G1 checkpoint regulator, p21, and PCNA as a proliferative index, we investigated expression of MAGE-3 protein, mutant p53, p21, and PCNA by flow cytometry and immunohistochemical stain. In addition, double stains for MAGE-3/p53, p53/PCNA, and p53/p21 were analysed with bivariate flow cytometry. DNA histograms using MAGE-3/PI (DNA) and p53/PI (DNA) were also analysed. The cell line (PNUH- 12) used for this study originated from a hypopharyngeal squamous cell carcinoma, which has point mutation (exon 7, C-->G) of p53. The expression rate of MAGE-3 was 83%, PCNA 85%, and p53 81%. No expression for p21 was identified. MAGE-3 was expressed in cytoplasm, while both PCNA and p53 were expressed in nuclei of tumor cells. With bivariate analyses, coexpression rates of MAGE-3/p53 and p53/PCNA were 0.96 and 0.97, respectively. Both MAGE-3 and p53 showed constantly high level throughout the cell cycle. These results suggest that expression of MAGE-3 and mutant p53 is not dependent on the cell cycle. p21 seems to be inactivated.

5.CT Findings of Tracheal Lipoma: A Case Report.

A patient with a history of asthma underwent computed tomography (CT). With a soft tissue window (width 330, level 30) the scan of the neck did not show any lesion within the airway lumen, but with a lung parenchymal window(width 1300, level -500), we were able to find a low attenuationg endotracheal mass lesion measuring -320HU. A bronchoscopy was performed and a polypoid mass approximately 2cm in diameter was found within the tracheal lumen. An endoscpic biopsy was not performed due to the risk of bleeding and the bronchoscopy was inconclusive for tissue diagnosis. CT, however, provided a definitive diagnosis by demonstrating fat within the tumor on the lung window.

7.Polymorphic Reticulosis.

Polymorphic reticulosis(PMR) is a unique clinicopathological entity of unknown etiology, which commonly present as an aggressive, necrotizing lesion of the upper respiratory tract. It is a separate nosologic entity from Wegener's granulomatosis and idiopathic mildine destructive disease. The origin of the cells composing polymorphic reticulosis has been controversial. We reviewed 15 cases of polymorphic reticulosis with respect to clinical and histologic bases, and immunohistochemical studies were done using UCHLI (CD45RO), as a T cell marker, CD-20 as a B cell marker and alpha- I -antichymotrypsin as a histiocytic marker. Almost all cases showed characteristic histologic and immunohistochemical features similar to those of peripheral T cell lymphoma. Thus, this study indidcated that polymorphic reticulosis is a type of malignant lymphoma of T cell lineage.
8.Procedure-Related Complications of Transjugular Intrahepatic Portosystemic Shunt (TIPS) and Its Clinical Significance.

PURPOSE: The purpose was to evaluate the procedure-related complications of the TIPS and its clinical significance. MATERIALS AND METHODS: The materials consisted of 52 patients who had 57 TIPS procedures for the management of variceal bleeding due to portal hypertension. To detect the occurrence of complications during transhepatic needle puncture(from hepatic vein to the portal vein), contrast material was injected with the withdrawl of the puncture needle. RESULTS: Procedure-related complications occurred in 28 patients (54%) among 52 patients. The corn lications were biliary tree puncture(15 cases), transperitoneal puncture(14 cases), stent malposition(3 cases) stent migration(1 case), hepatic arteries puncture(1 case), splenic vein perforation(1 case), and paroxysmal tricular tachycardia(1 case). Three patients had acute stent thrombosis with rebleeding immediately after procedure. The procedure was repeated in two patients. One patient was expired due to rebleeding. CONCLUSION: The procedure related complications were clinically insignificant in most instances, when stent thrombosis or proxysmal ventricular tachycardia was developed.

9.Treatment of Chronic Osteomyelitis by Transplantation of Autogenous Omentum with Microvascular Anastomosis

Omentum has the characteristic that it readily develops vascular anastomosis with adjacent tissues. It is suitable for eliminating a dead space regardless of the size, shape or site in the treatment of chronic osteomyelitis by use of technique of microvascular anastomoses. We report a case of chronic osteomyelitis treated by transplantation of autogenous omentum with microvascular anastomsis.

10.Vascularized Fibular Bone Graft for Tuberculous Spondylitis: Case Report

The tuberculous infection of the skeletal system especially, tuberculous spondylitis is frequently encountered with orthopedic fields. Up to date, Various methods of the treatment are present but each method has problems. Recently, as a development of the microsurgical instrumentation and surgical technique, we had been tried to the two patients of tuberculous spondylitis by vascularized fibular bone graft and anterior fusion. The advantages were as follows; 1. Shortened the bony union times by this operative method. 2. Earlier returning to the social activities compared to the ordinary method. 3. The children involved tuberculous spondylitis with severe kyphosis, who can be corrected the severe kyphosis by this operative method. 4. We can be obtained the rigid graft bone fixation with adequate length of bone by this operative method.
