2.Fatigue Failure of Wire in Orthopaedic Use

Orthopaedic wire has been used widely as a good internal fixator in cases of fracture, the ligament reconstruction, and for the stabilization of cervical spine. But if it is used inappropriately, a wire can be easy to fail by elongation, untwisting, or breakage. Among them, the most common failure of wire is breakage. To clarify the mechanism and the causes of wire failure, the authors analysed serial roentgenograms and scanning electronmicroscopic findings of broken wire in total 13 cases, which were experienced at the department of orthopaedic surgery, St. Mary's Hospital, Catholic University Medical College From January 1981 to December 1987. The results obtained were as follows :1. The mechanism of wire failure were classified into the 4 types ; minor continual flexing motion in 6 cases, repeated minor back and forth torsion in 1 case, tension failure in 2 cases and combined type in 4 cass. 2. The causes of wire failure were in appropriate use and inadequte application, a lack of s ufficient mechanical strength, micromotion due to inadequate postoperative imm obilization, early mobilization before tissue healing as well as certain injuries to the wire during handling in operation.

3.Hydroxyapatite-Coated Primary Total Hip Arthroplasty: Minimum two year clinical and roentgenographic analysis

Plasma sprayed coating of the hydroxyapatite(HA) are biocompatible and may contribute to the early fixation of total joint prosthesis. We studied 64 hips(58 patients) who had hydroxyapatite coated primary total hip arthroplasty during the period from January 1992 to June 1993 about clinical and radiological result prospectively. The components have been followed for a minimum of 26 months and an average 31 months. Harris hip score averaged 92 at most recent follow up intervals. At the last follow up, the pain in 7 cases(11%) respectively and only slightly to mild in mostly. Although thigh pain has been occurred in 7 cases(11%), all patients had not trouble in daily activity. In radiological evaluation, there was a rapid and good endosteal bone formation of implants with bone apposition on the Gruen zone 2 and 6 of HA coating area. No radiolucent line formation was detected around HA coated area. In acetabular component, the endosteal bone formation was found in 12 cases and the radiolucent line was occurred in 4 cases. No revision were done for the femoral and acetabular component. The result of the study indicate that this hydroxyapatite coated component can provide satisfactory clinical and radiological result. Further prospective follow up is necessary to determine whether the favorable early result of this implant are maintained over longer period.

4.A Clinical Study on the Incompetent Internal Os of the Cervix.

The 68 cervical cerclage operations of incompetent internal os of the cervix(IIOC) were performed at the Taegu Catholic Medical Center from Jan. 1, 1990 to Dec. 31, 1995. The results of this study were as follows: 1. The incidence of operation for IIOC was 0.23% of total delivery. 2. Highest number of IIOC was observed in the age group of 25 to 29, and the average number of pregnancy before operation was 4.1. 3. The significant contributory factor was previous history of the induced or spontaneous abortion. 4. In all cases, cerclage method was McDonald operation. 5. The average fetal salvage rate after operation was 82.5% and from the 14th week to 16th week of gestation(43 cases), the success rate of operation was 85%. 6. The causes of operation failure were premature labor in 6 cases(55%), premature rupture of membrane in 3 cases(27%) and abortion in 2 cases(18%) in order.

5.MR imaging of dural sinus thrombosis: A case report.

We present a case of angiographically-confirmed transverse and sigmoid sinus thrombosis, image with MR, in a 20 year old male with a history of otitis media and maxillary sinusitis. T1-weighted image demonstrated an isosignal intensity mass with tubular-shaped low signal intensity in right transverse and sigmoid sinus. The thrombus had high signal intensity on T2-weighted image. The signal intensity of the thrombus on Gd-DTPA enhanced T1-weighted image was unusually high similar to that of transverse sinus. Although dural sinus thrombosis has a non-specific MR signal intensity, findings of MRI in this case may serve as an aid in future evaluation of venous thrombosis.

6.The Comparison Study of teh Effect of Ambroxol on Prevention of Infantile Respiratory Distress Syndrome in Preterm Delivery.

This study was conducted to evaluate the effect of ambroxol on preventing the infantile respiratory distress syndrome (IRDS) in preterm birth at the Dept. of Obstetrics and Gynecology of Taegu Catholic Medical Center during the period from Jan. 1996 to Dec. 1996. Total of 68 cases were evaluated including 16 ambroxol group and 52 control group. The result were as follows : 1. In the comparison of preventing IRDS, there was 0 case of IRDS in ambroxol group and 7 cases of IRDS in control group (13.46 %). There was a significant difference between two groups (p<0.05). 2. The side effects of ambroxol after administration were nausea in 5 cases, headache in 3 cases, and chest discomfort in 4 cases, but these were not serious and self controlled. 3. There was no significant difference in neonatal morbidity between two groups (p > 0.05).

7.A study on CT stage of cervix cancer

CT findings and comparison between clinical and CT staging were reviewed in 45 consecutive patients withbiopsy proven carcinoma of the cervix. CT was done for staging of primary untreted cercial cancer in 36 patientsand for diagnosis of recurrent cervical cancer or follow-up study 14 times in 9 patients . The results are asfollows; 1. As comparing with clinical staging in 36 cases, CT agreed with clinical stage in 19 cases (52.8%),upstaged in 10 cases(27.8%) and downstaged inn 7 cases (19.4%). 2. The main upstaged CT findings are detection ofunsuspected bladder or rectal invasion and paraaortic lymph node metastasis. Inadequate delineation ofintravaginal tumor was the leading causes of understage of CT than clinical one. 3. CT detected all 10 cases ofthe hydronephrosis shown by IVP and detected unsuspected bladder invasion in 3 cases. 4. CT was valuable in thediagnosis of recurrent tumor and follow-up study after treatment. 5. CT offeres distinct advantages over clinicalstaging and can be integrated into the present FIGO classification of uterine cervix carcinoma. CT can served asan initial radiological staging work-up of primary of recurrent cervical carcinoma.

8.Anthelmintic effects of single doses of fenbendazole and oxantel-pyrantel pamoate to the intestinal nematodes.

The present study was undertaken to evaluate the anthelmintic effects of fenbendazole against intestinal nematode; Ascaris lumbricoides, hookworm and Trichuris trichiura, and to compare the efficacy in fenbendazole, oxantel-pyrantel pamoate and placebo by means of double blind method. Out of 114 subjects harbouring Ascaris lumbricoides, hookworm and Trichuris trichiura, 36 cases were treated with single dose of fenbendazole, 38 cases with oxantel-pyrantel pamoate, and the remaining 40 cases had received the placebo. The results were as follows: In the group treated with fenbendazole (30-50 mg/kg), the cure rates were 83.9 percent in 31 subjects with Ascaris lumbricoides and 83.3 percent in 18 subjects with hookworm, and only 28.6 percent in 28 subjects with T. trichiura respectively. In the group treated with a single dose of oxantel-pyrantel pamoate (10 mg/mg), the cure rates were 96.7 percent in 30 subjects with A. lumbricoides, 95.2 percent in 21 subjects with hookworm, and 54.6 percent in 33 subjects with T. trichiura. Egg reduction rate was 85.7 percent in T. trichiura cases. On the other hand, the egg negative conversion rates in placebo group were 9.7, 8.3 and 33.3 percent in Ascaris, Trichuris and hookworm infections respectively. The above results showed that fenbendazole was highly effective against Ascaris and hookworm. However, incomparisom with oxantel-pyrantel pamoate, fenbendazole was less effective in regards of A. lumbricoides, hookworm and T. trichiura infections.

9.Concomitant Boost Radiotherapy for Stage 3 Non - Small Cell Lung Cancer.

PURPOSE: This study was undertaken to evaluate the treatment outcome and side effects of accelerated radiotherapy (RT) using concomitant boost for stage III non-small cell lung cancer (NSCLC). METHODS: Between April 1991 and December 1994, 102 patients with stage III NSCLC who had the favorable prognostic factors by CALGB criteria, were treated with concomitant boost radiotherapy. Patients were treated with standard large fields to 54 Gy in 6 weeks. The boost treatment was administered concomitantly during the last 2 weeks with a dose of 13 Gy in 10 fractions. The interfraction interval was at least 6 hours. The total tumor dose was 66-70 Gy, given over 6 weeks. RESULTS: With 30 months median follow-up period for survivors, median survival was 15 months with 2 and 3-year overall survival rates of 34% and 19%, respectively. Thirty patients (29%) who had achieved complete remission after RT showed significantly better 2-year survival rates than those without complete remission (58% vs 22%, p 0.001). Local failure and distant metastases as the first or only failure occurred in 40 (44%) and 13 (14%), respectively, and ultimate local and distant failure rates were 45% and 29%, respectively. Although Grade IV esophageal complication of T-E fistula was observed in one patient, most patients with pulmonary complication showed mild, transient radiation pneumonitis. CONCLUSION: This result suggests that the treatrnent of stage III NSCLC with concomitant boost RT may improve survival rates without enhanced radiation induced toxicity compared with conventional RT. Further investigation of dose escalation by conformal radiotherapy of combining chemotherapy and accelerated RT is warranted.

10.Epidermal Grafting after Removal of Recipient Epidermis by CO2 Laser Ablation in Vitiligo.

BACKGROUND: Epidermal grafting using cryotherapy for recipient sites is in widespread use. However the peripheral hypopigmented haloes that occur around the recipient sites require prolon gation of the treatment period. OBJECTIVE: We used a CO2 laser to remove the epidermis of the recipient sites for betteri results. METHODS: We treated lie localized vitiligo patients with CO2 laser to remove t.he epidermis and grafted suction blister rooves. We observed repigmentation and complications 1 month later. RESULTS & CONCLUSIONS : The superiority of this method is demonstrated by the fact 1) all prodedures can be completed on the day of operation 2) the incidence of hypertrophic scar and peripheral hypopigmented halos can be observed.The problems of this method are 1) uneven repigmentation of recipient. sites 2) hyperpigrnentation of recipient sites
