Peptic Ulcer*

The 68 cervical cerclage operations of incompetent internal os of the cervix(IIOC) were performed at the Taegu Catholic Medical Center from Jan. 1, 1990 to Dec. 31, 1995. The results of this study were as follows: 1. The incidence of operation for IIOC was 0.23% of total delivery. 2. Highest number of IIOC was observed in the age group of 25 to 29, and the average number of pregnancy before operation was 4.1. 3. The significant contributory factor was previous history of the induced or spontaneous abortion. 4. In all cases, cerclage method was McDonald operation. 5. The average fetal salvage rate after operation was 82.5% and from the 14th week to 16th week of gestation(43 cases), the success rate of operation was 85%. 6. The causes of operation failure were premature labor in 6 cases(55%), premature rupture of membrane in 3 cases(27%) and abortion in 2 cases(18%) in order.

This study was conducted to evaluate the effect of ambroxol on preventing the infantile respiratory distress syndrome (IRDS) in preterm birth at the Dept. of Obstetrics and Gynecology of Taegu Catholic Medical Center during the period from Jan. 1996 to Dec. 1996. Total of 68 cases were evaluated including 16 ambroxol group and 52 control group. The result were as follows : 1. In the comparison of preventing IRDS, there was 0 case of IRDS in ambroxol group and 7 cases of IRDS in control group (13.46 %). There was a significant difference between two groups (p<0.05). 2. The side effects of ambroxol after administration were nausea in 5 cases, headache in 3 cases, and chest discomfort in 4 cases, but these were not serious and self controlled. 3. There was no significant difference in neonatal morbidity between two groups (p > 0.05).

Three cases of angiosarcoma of the breast are described. One case in a 66 year old woman was a solid mass and histologically showed poor differentiation leading to a death 14 months after the diagnosis. Another two in 46 and 25 year old females were large vascular masses showing moderate differentiation with endothelial tufting and papillary projections, and abdominal and contralateral mammary, and liver metastasis occurred within postoperative 10 months and 6 months in each case. A radical mastectomy and adjuvant chemotherapy in case 2 were not lifesaving. Although histologic degree of differentiation seems prognosis-related, angiosarcoma is the most malignant tumor of the breast.

Fibrous hamartoma of infancy is a distinct clinicopathologic entity with unique microscopic findings and benign clinical course. This tumor is composed of intervening dense fibrocollagenous trabeculae, well defined mucoid areas and varying amounts of mature fat, which are arranged in organoid growth pattern. We report three typical cases of this tumor which were experienced during the recent two years at the Seoul National University Hospital. It is the first description on this tumor in Korean literature. Case 1 was a 10 month old boy who was admitted due to a mass in the right infra clavicular area for 6 months. The mass showed irregular and poorly circumscribed outer surface. Case 2 was a one month old girl who was presented with a diffuse ill defined hairy mass in the left buttock since birth. Case 3 was a 15 month old boy who was brought to the hospital due to well circumscribed scrotal mass fot 8 months. Microscopically all three cases showed very similar histological features. And all 3 cases are well after the removal of the tumors.

OBJECTIVE: In adhesive capsulitis, assessment of the mobility of the shoulder are likely to have intrinsic error because of the altering scapulohumeral rhythm and extrinsic error. For more objective and the precise assessment of shoulder mobility in adhesive capsulitis, we observed proper glenohumeral movement, the change of scapulohumeral angle, the scapulohumeral rhythm were evaluated with radiography and compared with clinical measurement of shoulder range of motion (ROM). METHOD: The subjects were 19 patients (21 shoulder) with adhesive capsulitis of the shoulder and undertook combination treatment of distention arthrography, per os medication of non-steroid anti- inflammatory drug, stellate ganglion block, physical therapy including exercise. We assessed change in visual analogue scale (VAS), joint space capacity, shoulder mobility by measurement of ROM, and scapulohumeral angle in active shoulder abduction under the fluroscopy. RESULTS: All the subjects experienced pain relief, increased shoulder mobility and restoration of scapulohumeral rhythm. The improvement of the shoulder extension and scapulohumeral angle is well correlated with the change of VAS and joint capacity. However, the improvement of the shoulder flexion, abduction, external rotation and internal rotation were poor correlated with them. CONCLUSION: We propose the measurement of scapulohumeral angle with active shoulder abduction in shoulder AP view as an assessment method for shoulder mobility in adhesive capsulitis.

PURPOSE: This study investigates the current situation and future trends in the field of cataract and refractive surgery in Korea. METHODS: Since 1995, the members of the Korean Society of Cataract and Refractive Surgery (KSCRS) have been surveyed annually on cataract and refractive surgery. The 2005 annual survey, consisting of 74 multiple-choice questions, was mailed in February 2005 to 364 KSCRS members. Seventy-six (20.8%) members answered. Current data were compared with those of previous annual surveys and with those of annual surveys of ASCRS (American Society of Cataract and Refractive Surgery) members. RESULTS: The duration of hospitalization has decreased annually and the use of topical anesthesia (49%) has hemained stationary. Self sealing wound construction was the main wound closure technique in phacoemulsification (66%). Acrylic intraocular lenses are the most preferred (75%) for small incisioncataract surgery. The implantation of phakic IOL was performed by 30% of respondents and interests in the value added by IOL has increased annually. CONCLUSIONS: This survey summarized current trends and changes in cataract and refractive surgery in Korea.

The 45 neonates with bronchopulmonary dysplasia among 355 neonates who were managed with mechanical ventilation and oxygen therapy in NICU of Seoul National University Chhildren's Hospital from January 1, 1968 to December 31, 1991, were analyzed for risk factors and outcome. The results were as follows: 1) The incidence of bronchopulmonary dysplasia was 12.7% 2) Respiratory Distress Syndrome was most common underlying problem. 3) The common radiologic findings of BPD were peripheral streaky density, emphysematous change, reticular or granular perihilar density, pulmonary interstitial emphysema, bubbly or small cystic change, and pneumothorax. 4) The survival rate of BPD showed 80% in the follow-up study. 5) Retinopathy of prematurity and intraventricular hemorrhage were more frequent significantly in the BPD group (p<0.05). 6) The risk factors of BPD were lower birth weight, shorter gestational age, longer duration of oxygen therapy an mechanical ventilation, and the presence of PDA.
