1.Treatment of Localized Vitiligo by Autologous Skin Graft and Systemic PUVA Therapy.

Vitiligo is a disease of unknown cause in which melanocytes are preferentially destroyed by unidentified mechanisms. Vitiligo does respond slowly to psoralens with ultraviolet light and less often to applications of topical steroids. Many investigators have developed therapeutic regimens for autografts of normal melanocyte in treatment for this disfiguring disease. We present herein a case of localized vitiligo in a 26-year-old female, who was successfully repigmented with autologous skin graft and systemic PUVA therapy. The achieved result demonstrate a possible therapeutic approach for vitiligo.

3.The Analysis of Epithelial Ingrowth after LASIK Using a Femtosecond Laser.

PURPOSE: To analyze the incidence and clinical course of epithelial ingrowth after laser in situ keratomileusis (LASIK) using a femtosecond laser. METHODS: We retrospectively evaluated the results of 1158 eyes of 581 patients who received LASIK with the flap created by a femtosecond laser from February 2006 to March 2009 at our institute. We investigated the incidence and clinical course of LASIK in which the flap was created by a femtosecond laser. RESULTS: Epithelial ingrowth was first detected one week after surgery in 12 (57%) of 21 eyes and within one month in 19 eyes (90%). Epithelial ingrowth was localized most commonly near the temporal and nasal flap edge in 12 eyes (57%). In two eyes (9.5%), an isolated ingrowth mass was located in the pupillary area. The development of more than 2.0 mm of epithelial ingrowth was observed in three eyes (0.25%). Among 28 eyes with loose epithelium or epithelial detachment during surgery, epithelial ingrowth was observed in six eyes (21.4%) compared with 15 (1.32%) of 1130 eyes without loose epithelium or epithelial detachment. Therefore, epithelial detachment during surgery was significantly associated with epithelial ingrowth (p = 0.00). CONCLUSIONS: In LASIK in which the flap is created by a femtosecond laser, damage to the epithelial layer increases the chances of epithelial ingrowth. Therefore, avoiding epithelial defects with a gentle maneuver is needed to reduce the chance of epithelial ingrowth during surgery.

5.The Efficacy of KR-1W Aberrometer in Assessing the Astigmatism after Toric Intraocular Lens Implantation.

PURPOSE: To evaluate the efficacy of the KR-1W aberrometer in assessing the astigmatism and the achieved axis after AcrySof Toric intraocular lens (IOL) implantation. METHODS: Thirty-nine patients (48 eyes) that had undergone phacoemulsification and AcrySof Toric IOL implantation were included in the present study. At postoperative 1 month, ocular aberrations of all the eyes were measured using the KR-1W aberrometer before mydriasis and the achieved axis of AcrySof Toric IOL was measured by slit lamp examination after mydriasis. The achieved axis measured by slit lamp examination and the astigmatism of the implanted AcrySof Toric IOL were compared with the KR-1W. RESULTS: The mean achieved lens axis after mydriasis measured by slit lamp examination and KR-1W was 86.43 +/- 47.49 degrees and 88.93 +/- 41.22 degrees, respectively. The correlation coefficient of the 2 methods was 0.992 (p < 0.001). The measured astigmatism according to the AcrySof Toric IOL model was 1.11 +/- 0.47D in SN60T3, 1.71 +/- 0.51D in SN60T4, and 3.32 +/- 0.78D in SN60T5. CONCLUSIONS: Without the need to directly evaluate the axis of implanted Toric IOL after mydriasis, the achieved lens axis can be measured by the KR-1W without mydriasis. The AcrySof Toric IOL model could be assessed by the KR-1W without information about the AcrySof Toric IOL model.

6.A Case of Tick Bite Caused by Ixodes Species.

The importance of the ticks in medical field has been emphasized because of their association with and transmission of various diseases. We report herein a case of tick bite in a 55-year-old male farmer, who visited our hospital on July 2, 1981 with a parasite attached on right lower flank and rice to small pea sized, pruritic erythematous papular skin eruptions on chest and right lower flank. The skin lesions disappeared completely with.in five days after removal of the parasite, which was identified with an adult female tick which belongs to Genus Ixodes.

8.Cutaneous leiomyoma-a clinical and histopathological study.

We reviewed 50 skin biopsies obtained from 40 patients with cutaneous leiomyoma during a 10 year period from January 1980 to December 1989. The result.s were summarized as follows: 1) Sex distribution showed a female preponderance with male.female ratio of 1: 1.9. 2) The age distribution was relatively even from childhood to old age. The mean age was 47,8 years, 3) The most common site of lesion was lower leg(34%) followed by hand(24%), foot(12% ), anterior chest(8% ), face(6% ), neck(4%), upper arm(4% ), labia major(2 %) and nipple(2%). 4) The lesions were relatively well circumscribed subcutaneous nodule, papule, plaque and nodule which measured between 0.3cm to 5cm in size and their colors were erythematous to light brown. 5) The most common clinical and histopathologic type was angioleiomyoma(75%) followed by solitary piloleiomyoma(12.5%), multiple piloleiomyoma(7,5%) and solitary genital leiomyoma(5% ). Angioleiomyomas were clincally associated with cavernous hemangioma, myoma of uterus and systemic lupus erythematosus. 7) The clinical diagnoses were mass(30% ), epidermal cyst(20%), dermatofibroma (12.5% ), leiomyoma(12,5%), neurilemmoma(5%), lipoma(5%), calcifying epithelioma(2.5%), metastatic carcinoma(2.5%), morphea(2.5% ), fibrocystic disease(2.5%), glomus tumor(2.5 % ) and Dupuytrens contracuture(2..5%).

9.Small Hepatic Cystic Lesions in Patients with Extrahepatic IVlalignancy: Incidence and Significance on CT.

PURPOSE: We evaluated the frequency of detection of small hepatic cystic lesion(SHC) on abdominal CT films and its significance in patients with extrahepatic malignancies. METHODS AND MATERIALS: We retrospectively reviewed the abdominal CT films of a total 1112 patients to determine the frequency of detection of SHC, and 62 patients who had both extrahepatic malignancies and SHC were selected. A total 106 SHC determined as benign or malignant in 40 patients was analyzed according to the size(< 1 cm, 1 2cm), the number(1, 2 4, >4), the margin(well-defined, iil-defined),and the location(central, peripheral) of individual cystic lesions. The findings of follow-up CT and ultrasonography and the results of biopsy were also reviewed. The frequency of combined renal cyst was also considered. RESULT: SHC of those 62 patients were classified as benign(27 patients, 43.6%), malignant(13 patients, 20. 9%), or undetermined(22 patients, 35.5%) by the follow-up CT, ultrasonography, and biopsy. Most benign SHC (74.4%) measured under 1 cm, but 75% of the malignant SHC were in 1 -2cm in size. The percentage of the malignancy in one and two to four SHC was 37.5% and 40%, respectively. The margin of SHC was ill-defined in 67.9% of the benign and 64.3% of the malignant lesions. SHCs were mainly located at the periphery of the liver(benign :62.7%, malignant: 64.3%). Coexisting renal cyst was rare. CONCLUSION: SHC is not uncommon finding(13.2%) in patients with extrahepatic malignancy and has a high probablity of metastasis when it is larger than 1 cm, which should be closely followed and intensively studied further.
