1.A Case of Reticulate Acropigmentation of Kitamura.

Reticulate acropigmentation of Kitamura, described first by Kitamura and Akam atsu in 1943, is a pigmentary disorder that may be determined by an autasomal dominant gene. Skin lesions are characterized clinically by depressed brownisL pigmented spots, in a reticulated pattern, on the extensor surface of the acral parts of the extremities. Most reported cases are confined to Japanese. We report herein a case of reticulate acropigmentation of Kitarnura in a 28- year-old woman, with a depressed brownish pigmented spots, in a reticulate pattern, on the dorsum of hands and left foot. The authors make the diagnosis of this case a reticulate acropigmentation of Kitamura for the clinical symptoms and histopathologic findings.

2.A Case of Carcinoma Erysipelatoides Associated with Carcinoma Telangiectaticum.

Carcinoma of the breast can produce cutaneous metastases which sbow highly diversified clinical pictures. The cutaneous metastases produce four definite clinical types: inflammatory carcinoma, telangiectatic carcinoma, nodular carcinoma, and carcinoma, en cuirasse. We present a case of 78-year-old nulliparous woman who had several painful dark purplish colored zosteriform papulovesicular eruptions, suggestive of herpes zoster, on the right breast, right axilla, scapular area, and upper arm for three month. Two month later, well dermacated linearly arranged erysipeloid eruptions appeared around the previous skin lesions and the right chest wall. Histopathological findings showed metastatic ductal carcinoma with massive vascular permeation but the primary focus was undetermined.

3.A Case of Tick Bite Caused by Ixodes Species.

The importance of the ticks in medical field has been emphasized because of their association with and transmission of various diseases. We report herein a case of tick bite in a 55-year-old male farmer, who visited our hospital on July 2, 1981 with a parasite attached on right lower flank and rice to small pea sized, pruritic erythematous papular skin eruptions on chest and right lower flank. The skin lesions disappeared completely with.in five days after removal of the parasite, which was identified with an adult female tick which belongs to Genus Ixodes.

4.Effect of Chitosan Oligosaccharide on Enzymes for Cancer Chemoprevention.

PURPOSE: Two types of chitosan oligosaccharides (COSs), COS I and COS II, were investigated for the effects on ascitic tumor and enzymes for cancer chemoprevention. MATERIALS AND METHODS: Chitosan oligosaccharides were administered once daily for 10 days after the tumor implantation. The change of body weight was observed for 20 days, and the survival rate of mice was determined after 21 days. Chitosan oligosaccharides were administered once daily for 10 days before the tumor implantation (1 106 cells). The number of ascitic tumor cells were measured at 6 days after tumor implantation. Chemopreventive potential of chitosan oligosaccharides was examined by the induction of quinone reductase and inhibition of cytochrome P450 1A1. RESULTS: Chitosan oligosaccharides exerted antitumor activity by inhibiting the growth of Ehrlich ascites tumor cells in vivo. Mice given Ehrlich cells and 10 or 100 mg/kg body weight of chitosan oligosaccharides had 33% survival after 21 days. Quinone reductase activity was increased with chitosan oligosaccharides. There were 26% and 33% inhibition in the activity of cytochrome P450 1A1 enzyme with the treatment of COS I and COS II, respectively. CONCLUSION: These results suggest that chitosan oligosaccharides has antitumor activity and cancer chemo preventive potential by inducing QR activity and inhibiting cytochrome P450 1A1.

5.Periodontal tissue response following different types of fixed retainers in young adult dogs.

The purpose of this study was to evaluate the material for fixed type retainer, allowing physiologic tooth movement. and proper remodeling of periodontal tissue during retention period. The present study was performed to observe the histologic changes of periodontal tissue after application of different types of fixed type retainer after orthodontic tooth movement in young adult dogs. For this study, 4 young adult dogs were used as a experimental animal and experimental group was divided into three groups ; experimental group 1 contained right side maxillary third incisors and canines, experimental group 2 contained contralateral teeth of same animals, and control group contained mandibular premolars. And each dogs were applied the 4 different types of fixed type retainer to experimental group 1. The experimental teeth were ligated on the Sentalloy closed coil spring(R)(Tomy Co., Japan) from maxillary third incisors and canines and applied orthodontic force at initial 200gm-forced during 1 week. All the experimental animals were sacrificed on the 3rd week after the orthodontic teeth movement and then the specimens were taken, fixed in formalin, embeded in parafin, sectioned 6-8micrometer in thickness and stained with Hematoxylin-Eosin staining, and Masson's trichrome staining method. Examined under the light microscopy. The following results were observed. 1. There were observed that decreased infiltration of giant cells in pressure side and increased the new bone forming in tension side on the specimen of 6-stranded 0.0195' Respond(R)(G&H Co., U.S.A.) group. Periodontal ligament fibers were much compressed or elongated in 3-stranded 0.018', 0.020' Dentaflex(R)(Dentarum Co., Germany), and Superbond C&B(R)(Sun Medical Co., Japan) groups. 2. In experimental group 1, necrotic bone inside the alveolar bone of pressure side, forming of the sharpey's fiber in osteoid tissue, and remodeling of the periodontal ligament were observed in all animals. 3. In experimental group 2, it was observed that the amount of bone resorption was equal or decreased in pressure side, and increased new bone forming and significantly decreased infiltration of giant cell than the experimental group 1. By this results, it considered that 6-stranded Respond(R)(G&H Co., U.S.A.) wire was the most useful material allowing early periodontal tissue remodeling.

6.Cervical Pedicle Screw Insertion Using the Technique with Direct Exposure of the Pedicle by Laminoforaminotomy.

OBJECTIVE: To present the accuracy and safety of cervical pedicle screw insertion using the technique with direct exposure of the pedicle by laminoforaminotomy. METHODS: We retrospectively reviewed 12 consecutive patients. A total of 104 subaxial cervical pedicle screws in 12 patients had been inserted. We also assessed the clinical and radiological outcomes and analyzed the direction and grade of pedicle perforation (grade 0: no perforation, 1: <25%, 2: 20% to 50%, 3: >50% of screw diameter) on the postoperative vascular-enhanced computed tomography scans. Grade 2 and 3 were considered as incorrect position. RESULTS: The correct position was found in 95 screws (91.3%); grade 0-75 screws, grade 1-20 screws and the incorrect position in 9 screws (8.7%); grade 2-6 screws, grade 3-3 screws. There was no neurovascular complication related with cervical pedicle screw insertion. CONCLUSION: This technique (technique with direct exposure of the pedicle by laminoforaminotomy) could be considered relatively safe and easy method to insert cervical pedicle screw.

7.Extensive Spinal Cord Infarction after Surgical Interruption of Thoracolumbar Dural Arteriovenous Fistula Presenting with Subarachnoid Hemorrhage.

Nontraumatic intracranial subarachnoid hemorrhage (SAH) attributable to the thoracolumbar dural arteriovenous fistulas (DAVFs) has been extremely rare. A 41-year-old male patient was admitted with severe acute headache, neck stiffness, and pronounced low-back pain radiating to both legs. The T2-weighted MR imaging showed irregular signal void and enlarged, varix like pouch formation with spinal cord compression at the T11-12 level. The angiogram revealed a DAVF.

8.A Gynecologic Aspects of Child Sexual Abuse.

OBJECTIVE: The purpose of this study was to evaluate victims of child sexual abuse and assess the similarities and differences between them. METHODS: The authors studied retrospectively 55 patients among 230 sexual abuse victims between the ages 2 and 13 who visited and were treated at the Department of Obstetrics and Gynecology, National Police Hospital between the dates Oct, 13, 2001 and Mar, 25, 2003 were followed and observed. RESULTS: Female infants and children composed 94.5% instances of sexual abuse were most common between 4 and 7 o'clock in the afternoon. Unlike adult cases, assailants were in most cases acquaintances (55.5%), and the crimes were committed most often at the homes of either victim or assailant (50.9%). 38.2% of victims had been directed to the National Police Hospital from police stations, 21.8% from other hospitals. Diagnosis resulted in 5.5% of cases with no observable symptoms, 18.2% with lacerations, among these cases 1 instance (1.8%) requiring surgical repair. 1 case each of Gonorrhea and genital herpes were found, with no instances of pregnancy. CONCLUSION: Child sexual abuse results in life-long bodily and psychological stress for both the victim and his/her family. Witnesses are in some cases unable to testify in legal proceedings, and the gathering of evidence is more difficult than in adult cases. This could lead to frequent social and legal negligence. Cooperation among such numerous and various professional institutions as hospitals, child guidance clinics, child psychiatrists, police, prosecutory offices and the courts is requisite to the settlement of these cases. Since the gynecologist plays the leading role in the identification and treatment of child and infant sexual abuse victims, and is often the first to come in contact with these cases, careful attention is required in the processes of inspection, inquiry, evidence collection, treatment and diagnosis.
